Diabetes and NAFLD:a high-risk cohort with definite therapeutic potential

Despite the fact that non-alcoholic fatty liver disease(NAFLD)and its severe clinical forms[non-alcoholic steatohepatitis(NASH)and NASH-cirrhosis]are highly prevalent in the general population,there are no licensed drugs for NAFLD,and lifestyle intervention remains the only treatment accepted by international *** is despite massive investments in research by pharmaceutical *** the presence of type 2 diabetes,novel anti-diabetic drugs offer an opportunity to reduce the burden of NAFLD,by adequate control of glucose and lipid metabolism,also reducing the risk of NASH progression,advanced fibrosis,and finally hepatocellular *** extensively reviewed the literature,based either on registration studies,ad hoc randomized studies or real-world data,to define the effectiveness of anti-diabetic drugs in the treatment of NAFLD and prevention of hepatocellular carcinoma(HCC).Metformin provides the best evidence for decreased risk of HCC,pioglitazone was associated with decreased progression to fibrosis,glucagon-like peptide-1 receptor agonists offer a possible opportunity to reduce NAFLD progression coupled with a definite protection for cardiovascular outcomes,and sodium-glucose cotransporter-2 inhibitors are likely to reduce lipid burden,simultaneously reducing the risk of progressive renal and heart *** the latter two drug classes,the effects on NAFLD might largely explained by decreased body weight,in keeping with the beneficial effects of intensive lifestyle intervention.

Undernutrition, risk of malnutrition and obesity in gastroenterological patients: A multicenter study

AIM: To investigate the prevalence of undernutrition, risk of malnutrition and obesity in the Italian gastroenterological population. METHODS: The Italian Hospital Gastroenterology Association conducted an observational, cross-sectional multicenter study. Weight, weight loss, and body mass index were evaluated. Undernutrition was defined as unintentional weight loss > 10% in the last threesix months. Values of Malnutrition Universal Screening Tool(MUST) > 2, NRS-2002 > 3, and Mini Nutritional Assessment(MNA) from 17 to 25 identified risk of malnutrition in outpatients, inpatients and elderly patients, respectively. A body mass index ≥ 30 indicated obesity. Gastrointestinal pathologies were categorized into acute, chronic and neoplastic diseases. RESULTS: A total of 513 patients participated in the study. The prevalence of undernutrition was 4.6% in outpatients and 19.6% in inpatients. Moreover, undernutrition was present in 4.3% of the gastrointestinal patients with chronic disease, 11.0% of those with acute disease, and 17.6% of those with cancer. The risk of malnutrition increased progressively and significantly in chronic, acute and neoplastic gastrointestinal diseases in inpatients and the elderly population. Logistical regression analysis confirmed that cancer was a risk factor for undernutrition(OR = 2.7; 95%CI: 1.2-6.44, P = 0.02). Obesity and overweight were more frequent in outpatients. CONCLUSION: More than 63% of outpatients and 80% of inpatients in gastroenterological centers suffered from significant changes in body composition and required specific nutritional competence and treatment.

Halogenated natural products(HNPs)are considered to be emerging contaminants whose environmental distribution and fate are only incompletely ***,several persistent and bioaccumulative HNP groups,together with manmade polychlorinated biphenyls(PCBs)and polybrominated diphenyl ethers(PBDEs),were quantified in the blubber of nine sperm whales(Physeter macrocephalus)stranded on the coast of the Mediterranean Sea in *** naturally occurring polybrominated hexahydroxanthene derivatives(PBHDs;sum of TetraBHD and TriBHD)were the most prominent substance class with up to 77,000 ng/g *** mean PBHD content(35,800 ng/g blubber)even exceeded the one of PCBs(28,400 ng/g blubber),although the region is known to be highly contaminated with manmade *** on mean values,Q1∼PBDEs>MeO-BDEs∼2,2′-diMeO-BB 80 and several other HNPs followed with decreasing *** blubber samples contained an abundant compound whose molecular formula(C_(16)H_(19)Br_(3)O_(2))was verified using high-resolution mass *** only plausible matching isomer was(2S,4′S,9R,9′S)-2,7-dibromo-4′-bromomethyl-1,1-dimethyl-2,3,4,4′,9,9′-9,9′-hexahydro-1H-xanthen-9-ol(OH-TriBHD),a hydroxylated secondary metabolite previously detected together with TriBHD and TetraBHD in a sponge known to be a natural producer of *** estimated mean amount of the presumed OH-TriBHD was 3000 ng/g blubber,which is unexpectedly high for hydroxylated compounds in the lipids of marine mammals.

Prolonged exposure of colon cancer cells to the epidermal growth factor receptor inhibitor gefitinib(Iressa^(TM))and to the antiangiogenic agent ZD6474:Cytotoxic and biomolecular effects

AIM： To analyze the biological effects of prolonged in vitro exposure of HT-29 and LoVo colon cancer cell lines to gefitinib （Iressa^TM）, an inhibitor of epidermal growth factor receptor （EGFR） activity, and ZD6474, an inhibitor of both KDR and EGFR activities. METHODS： Cells were treated with each drug for up to 2 wk using either a continuous or an intermittent （4 d of drug exposure followed by 3 d of washout each week） schedule. RESULTS： In both cell types, prolonged exposure （up to 14 d） to gefitinib or ZD6474 produced a similar inhibition of cell growth that was persistent and independent of the treatment schedule. The effects on cell growth were associated with a pronounced inhibition of p-EGFR and/ or p-KDR expression. Treatment with gefitinib or ZD6474 also inhibited the expression of EGFR downstream signal molecules, p-Erkl/2 and p-Akt, although the magnitude of these effects varied between treatments and cell lines. Furthermore, expression of the drug resistance-related protein ABCG2 was shown to significantly increase after 14 d of continuous exposure to the two drugs. CONCLUSION： We conclude that long-term exposure of colon cancer cells to gefitinib and ZD6474 does not modify their cytotoxic effects but it might have an effect on sensitivity to classical cytotoxic drugs.

Benefits of Biochars and NPK Fertilizers for Soil Quality and Growth of Cowpea (Vigna unguiculata L. Walp.) in an Acid Arenosol

Fertilization is required for optimum plant growth, particularly in unfertile soils, while optimizing nutrient use effciency is an alternative to reduce inorganic fertilizer needs and reduce environmental problems caused by nutrient leaching. This study investigated soil properties and cowpea yield responses to biochars(BCs) made from different feedstocks, baby corn peel biochar(BC1), branches of mango tree biochar(BC2), and rice husk biochar(BC3), applied in combination with nitrogen-phosphorus-potassium(NPK) *** experiment was conducted in a greenhouse, using an acid sandy soil(Arenosol) that was submitted for 70 d to the following eight treatments: i) control;ii) full dose of NPK(a commercial compound fertilizer(12-24-12 of N-P2O5-K2O)+ urea(46% N));iii) BC1+ half dose of NPK;iv) BC1 + full dose of NPK;v) BC2 + half dose of NPK;vi) BC2 + full dose of NPK;vii) BC3 + half dose of NPK;and viii) BC3 + full dose of NPK. All biochars were applied at a rate of 0.9%(weight/weight), and each type of biochar was combined with half and full doses of NPK fertilizers. Soil pH increased significantly(P < 0.05) in treatments with BC1 and BC2,while cation exchange capacity(CEC) and available P were higher in the treatments with BC1;BC1 and BC2 also induced higher activity of enzymes related to the P cycle and higher cowpea yield. Similar soil properties and cowpea yield parameters were obtained with the full and half doses of NPK fertilizers for each type of biochar used. In conclusion, biochars in the combination with NPK fertilizers improved soil chemistry and enzymatic activities, allowing reduced fertilizer application and food production costs in the acid soil studied.

Phosphorus(P)deficiency is one of the main constraints on crop production in Arenosols(acid sandy soil).The high cost of P fertilizers may represent an insurmountable obstacle in many poor countries,leaving the exploitation of their own calcareous and phosphate rocks as the only low-cost and long-term *** is suggested to have positive effects on soil properties;however,there is no published research on the synergistic effects of biochar and rocky materials in modifying soil *** aim of this study was to investigate the chemical and biochemical responses of an acid Arenosol treated with phosphate rock(PR),calcareous rock(CR),and biochar(BC),and the implications for corn yield.A soil from Marracuene District,Mozambique was used,where corn was grown for 90 d with the soil treated with:no addition(control),water-soluble zinc phosphite fertilizer(WSP),PR,WSP+CR,WSP+BC,WSP+CR+BC,PR+BC,and PR+CR+*** was produced by pyrolysis of babycorn peels for 4 h at 450?C and applied at 11 g *** soil pHH2 Oincreased from about 4.54 in the control to 7.38 in the PR+CR+BC *** oxidizable organic carbon,cation exchange capacity,and available P were higher in the treatments containing BC than in the *** treatments containing CR and/or BC led to the highest activities of alkaline phosphomonoesterase,phosphodiesterase,andα-glucosidase,which increased P availability and gave the greatest biomass and *** suggest that biochar provides additional soluble P and supplies adsorption sites for phosphate,preventing its evolution to unavailable ***,PR applied together with BC contributed to an 840%yield increase compared to the *** treatments containing WSP and BC facilitated phosphite oxidation to phosphate and increased crop yield.

Ticagrelor therapy and atrioventricular block:Do we need to worry?

Ticagrelor is a potent,direct P2Y12 antagonist with rapid onset of action and intense platelet inhibition,indicated in patients with acute coronary syndromes(ACS).This drug is usually well tolerated,but some patients experience serious adverse effects:Major bleeding;gastrointestinal disturbances;dyspnoea;ventricular pauses > 3 *** the unexpected high incidence of bradyarrhythmias,a PLATO substudy monitored this side effect,showing that ticagrelor was associated with an increase in the rate of sinus bradycardia and sinus arrest compared to *** side effect was usually transient,asymptomatic and not associated with higher incidence of severe atrioventricular(AV) block or pacemaker needs.A panel of experts from Food and Drug Administration did not consider bradyarrhythmias a serious problem in clinical practice and,accordingly,current labeling of the drug does not give any precaution or contraindication regarding this ***,recently some articles have described ACS patients with high-degree,life-threatening,AV block requiring drug discontinuation and,in some cases,pacemaker *** this paper,we describe and discuss five published case reports of severe AV block following ticagrelor therapy and two other cases managed in our *** analysis of literature suggests that,although rarely,ticagrelor can be associated with lifethreatening AV *** and careful monitoring are required especially in patients with already compromised conduction system and/or treated with AV blocking *** studies,with long-term rhythm monitoring,would help to define the outcome of patients at higher risk of developing this complication.

How to select elderly colorectal cancer patients for surgery: a pilot study in an Italian academic medical center

Objective: Cancer is one of the most common diagnoses in elderly patients. Of all types of abdominal cancer, colorectal cancer(CRC) is undoubtedly the most frequent. Median age at diagnosis is approximately 70 years old worldwide. Due to the multiple comorbidities affecting elderly people, frailty evaluation is very important in order to avoid over- or undertreatment. This pilot study was designed to investigate the variables capable of predicting the long-term risk of mortality and living situation after surgery for ***: Patients with 70 years old and older undergoing elective surgery for CRC were prospectively enrolled in the study. The patients were preoperatively screened using 11 internationally-validated-frailty-assessment tests. The endpoints of the study were long-term mortality and living situation. The data were analyzed using univariate Cox proportional-hazard regression analysis to verify the predictive value of score indices in order to identify possible risk ***: Forty-six patients were studied. The median follow-up time after surgery was 4.6 years(range, 2.9-5.7 years) and no patients were lost to follow-up. The overall mortality rate was 39%. Four of the patients who survived(4/28, 14%) lost their functional autonomy. The preoperative impaired Timed Up and Go(TUG), Eastern Cooperative Group Performance Status(ECOG PS), Instrumental Activities of Daily Living(IADLs), Vulnerable Elders Survey(VES-13) scoring systems were significantly associated with increased long term mortality ***: Simplified frailty-assessing tools should be routinely used in elderly cancer patients before treatment in order to stratify patient risk. The TUG, ECOG-PS, IADLs and VES-13 scoring systems are potentially able to predict long-term mortality and disability. Additional studies will be needed to confirm the preliminary data in order to improve management strategies for oncogeriatric surgical patients.
