In minimally invasive surgery, one of the main objectives is to ensure safety and target reaching accuracy during needle steering inside the target organ. In this research work, the needle steering approach is determined using a robust control algorithm namely the integral sliding mode control(ISMC) strategy to eliminate the chattering problem associated with the general clinical scenario. In general, the discontinuity component of feedback control input is not appropriate for the needle steering methodology due to the practical limitations of the driving actuators. Thus in ISMC, we have incorporated the replacement of the discontinuous component using a super twisting control(STC)input due to its unique features of chattering elimination and disturbance observation characteristics. In our study, the kinematic model of an asymmetric flexible bevel-tip needle in a soft-tissue phantom is used to evaluate stability analysis. A comparative study based on the analysis of chattering elimination is executed to determine the performance of the proposed control strategy in real-time needle steering with conventional sliding mode control using vision feedback through simulation and experimental results. This validates the efficacy of the proposed control strategy for clinical needle steering.

Lithium-sulphur(Li-S)batteries are currently considered as next-generation battery technology.Sulphur is an attractive positive electrode for lithium metal batteries,mainly due to its high capacity(1675 m Ah g^(-1))and high specific energy(2600 Wh kg^(-1)).The electrochemical reaction of lithium with sulphur in non-aqueous electrolytes results in the formation of electrolyte soluble intermediate lithium-polysulphides.The dissolved polysulphides shuttle to the anode and get reduced at the anode resulting in Li metal corrosion.The solubility of polysulphide gradually reduces the amount of sulphur in the cathode,thereby limiting the cycle life of Li-S batteries.Several strategies have been proposed to improve the cycling stability of Li-S batteries.A unique approach to eliminate the polysulphide shuttle is to use ultramicroporous carbon(UMC)as a host for sulphur.The pore size of UMC which is below 7A,is the bottleneck for carbonate solvents to access sulphur/polysulphides confined in the pores,thereby preventing the polysulphide dissolution.This perspective article will emphasise the role of UMC host in directing the lithiation mechanism of sulphur and in inhibiting polysulphide dissolution,including the resulting parasitic reaction on the lithium anode.Further,the challenges that need to be addressed by UMC-S based Li-S batteries,and the strategies to realise high power density,high Coulombic efficiency,and resilient Li-S batteries will be discussed.

Background There is increasing epidemiological evidence supporting the association between onchocerciasis and seizures,reinforcing the concept of onchocerciasis-associated epilepsy(OAE).The aim of this paper is to provide an update on the new knowledge about OAE and to propose recommendations to the World Health Organization how to address this public health problem.Main text During the 2nd International Workshop on OAE held on 19-21 September,2023,in Antwerp,Belgium,participants recognised OAE as a substantial yet neglected public health problem,particularly in areas of sub-Saharan Africa where onchocerciasis remains hyperendemic.Evidence from prospective population-based studies suggest that strengthening onchocerciasis elimination efforts leads to a significant reduction of OAE incidence.There is a need to validate an OAE case definition to estimate the burden of disease and identify onchocerciasis-endemic areas requiring intensification of onchocerciasis elimination programmes and integration of epilepsy care t is expected that raising awareness about OAE will boost the population uptake of ivermectin.The implementation of a commu-nity-based epilepsy treatment programme offering free anti-seizure medications(ASMs)has shown high effectiveness in reducing the frequency of seizures and improving the overall quality of life of people with epilepsy.Conclusions To reduce OAE burden,enhanced collaboration between onchocerciasis and mental health pro-grammes at community,national,and international levels is required.Urgent efforts are needed to ensure the uninter-rupted provision of free ASMs in onchocerciasis-endemic areas.Furthermore,OAE should be included in the quantif-cation of the onchocerciasis disease burden.

Potential for Increasing Soil Nutrient Availability via Soil Organic Matter Improvement Using Pseudo Panel Data

Fixed and random effect models were applied to a pseudo-panel data built of soil analysis reports from tobacco farms to analyze relationships between soil characteristics like soil organic matter (SOM) and soil nitrogen (N), phosphorous (P) and potassium (K) and to explore the potential for improving nutrients availability by increasing SOM content. These econometric models may account for unobserved specific characteristics such as location-specific characteristics, management strategies, farmers’ skills and preferences and environmental heterogeneity. Positive relationships were found between N, P and K availability and SOM. The random effect model reports a highly significant elasticity of N with respect to SOM of 0.75, meaning that an increase of 1% of SOM will increase soil N by 0.75%. Using this elasticity, the required SOM improvement of green manure was calculated at which costs of green manure would exactly equal benefits in terms of reduced N fertilizer use. Costs and benefits are equal if the SOM increases from 1.55% to 3.61%, which is barely achieved according to the literature. Hence, growing green manure crops to increase SOM and thereby N availability is not economically attractive. However, additional benefits may arise from SOM improvement and growing green manure crops.

Sometimes more than one expert advises the court. They use different notional frameworks in doing so, and may report different opinions about the case they examined. In this case, the authors discuss the relation between the use of a SSRI (serotonin reuptake inhibitor) and a fatal tragedy in a family where the mother was accused of killing her husband and daughter a few days after use of the SSRI. No fewer than seven experts were heard (four behavioral experts, one behavioral toxicologist, one pharmacist and one pharmacologist) at the ensuing trial, and various possible diagnoses were dealt with in the experts" reports and at trial. More attention must be paid to the mentally debilitating influence of SSRI medication on certain psychological functions shortly after its intake. Although a mono-causal relationship between that influence and the accused"s intention is necessary to exculpate the accused from guilt, the authors believe that a singular connection is never happened in the case.

BACKGROUND Glomerulopathy with fibrillary deposits is not uncommon in routine nephropathology practice,with amyloidosis and fibrillary glomerulonephritis being the two most frequently encountered entities.Renal amyloid heavy and light chain(AHL)is relatively uncommon and its biopsy diagnosis is usually limited to cases that show strong equivalent staining for a single immunoglobulin(Ig)heavy chain and a single light chain,further supported by mass spectrometry(MS)and serum studies for monoclonal protein.But polyclonal light chain staining can pose a challenge.CASE SUMMARY Herein we present a challenging case of renal AHL with polyclonal and polytypic Ig gamma(IgG)staining pattern by immunofluorescence.The patient is a 62-yearold Caucasian male who presented to an outside institution with a serum creatinine of up to 8.1 mg/dL and nephrotic range proteinuria.Despite the finding of a polyclonal and polytypic staining pattern on immunofluorescence,ultrastructural study of the renal biopsy demonstrated the presence of fibrils with a mean diameter of 10 nm.Congo red was positive while DNAJB9 was negative.MS suggested a diagnosis of amyloid AHL type with IgG and lambda,but kappa light chains were also present supporting the immunofluorescence staining results.Serum immunofixation studies demonstrated IgG lambda monoclonal spike.The patient was started on chemotherapy.The chronic renal injury however was quite advanced and he ended up needing dialysis shortly after.CONCLUSION Tissue diagnosis of AHL amyloid can be tricky.Thorough confirmation using other available diagnostic techniques is recommended in such cases.

The global prevalence of obesity is increasing rapidly with an exponential rise in incidence of type 2 diabetes mellitus in recent years.‘Diabesity’,the term coined to show the strong interlink between obesity and diabetes,is the direct cons-equence of the obesity pandemic,and poses significant challenges in the management of the disease.Without addressing the clinical and mechanistic complications of obesity such as metabolic-associated fatty liver disease and obstructive sleep apnoea,a rational management algorithm for diabesity cannot be developed.Several classes of anti-diabetic medications including insulins,sulphonylureas,thiazolidinediones and meglitinides are associated with the risk of weight gain and may potentially worsen diabesity.Therefore,appropriate selection of antidiabetic drug regimen is crucial in the medical management of diabesity.The role of non-pharmacological measures such as dietary adjustments,exercise interventions and bariatric procedures should also be emphasised.Unfortunately,the importance of appropriate and optimal management of diabesity is often overlooked by medical professionals when achieving adequate glycemic control which results in inappropriate management of the disease and its complications.This review provides a narrative clinical update on the evidence behind the management of diabesity.

BACKGROUND Xanthogranulomatous inflammation(XGI)is an uncommon process involving an accumulation of inflammatory cells,commonly lipid-laden macrophages.XGI has been described to occur throughout the body but only rarely in the lower gastrointestinal tract.We describe a case of XGI contributing to chronic obstructive symptoms in the terminal ileum,in which the patient had an initial diagnostic laparoscopy,continued to have symptoms,then proceeded to have the definitive treatment.To our knowledge,this is the first report of XGI associated with a prior small bowel anastomosis.CASE SUMMARY We report the case of a 42-year-old female who presented with intermittent epigastric pain and subjective fevers.She had undergone a laparoscopic small bowel resection for Meckel’s diverticulum five years prior.Her workup was notable for computed tomography scan demonstrating mild inflammation and surrounding stranding at the level of the prior anastomosis.She underwent a laparotomy,resection of the prior anastomosis and re-anastomosis,with final histopathological examination findings consistent with mural XGI.CONCLUSION XGI can occur at the site of a prior bowel anastomosis and cause chronic obstructive symptoms.

Automated metadata annotation is only as good as training dataset,or rules that are available for the domain.It's important to learn what type of data content a pre-trained machine learning algorithm has been trained on to understand its limitations and potential biases.Consider what type of content is readily available to train an algorithm-what's popular and what's available.However,scholarly and historical content is often not available in consumable,homogenized,and interoperable formats at the large volume that is required for machine learning.There are exceptions such as science and medicine,where large,well documented collections are available.This paper presents the current state of automated metadata annotation in cultural heritage and research data,discusses challenges identified from use cases,and proposes solutions.
