Risk scores for allograft failure: Are they still useful in liver recipients from donation after circulatory death?

BACKGROUND Liver grafts from donation after circulatory death(DCD)are associated with a higher risk of early graft dysfunction,determined by the warm ischemia and cold ischemia *** is essential to have precise criteria to identify this complication in order to guide therapeutic *** To validate different graft and recipient survival scores in patients undergoing liver transplantation(LT)with DCD *** A retrospective and observational unicentric study was conducted on 65 LT patients with grafts obtained from controlled DCD donors from November 2013 to November *** United Kingdom(UK)risk score,early allograft dysfunction(EAD)Olthoff score,and model for early allograft function(MEAF)score were used to evaluate the risk of graft and recipient survival *** survival analysis purposes,we used the Kaplan-Meier method,and the differences between subgroups were compared using the log-rank(Mantel-Cox)*** Sixty-five patients were included in the *** UK risk score did not demonstrate predictive capacity for recipient or graft ***,in donors aged over 70 years old(18.4%),it significantly predicted graft survival(P<0.05).According to Kaplan-Meier survival curves,graft survival rates at 6 months,2 years,and 5 years in the futility group dramatically decreased to 50%compared to the other groups(log-rank 8.806,P<0.05).The EAD Olthoff and MEAF scores did not demonstrate predictive capacity for recipient or graft *** on Kaplan-Meier survival curves,patients with a MEAF score≥7 had a lower graft survival rate at 6 months,2 years,and 5 years compared to patients with a lower MEAF score(log-rank 4.667,P<0.05).CONCLUSION In our series,both UK DCD risk score and MEAF score showed predictive capability for graft survival.

Autophagy is a lysosomal-mediated degradation pro-cess that controls the turnover of organelles and long-lived *** of its role in maintaining cellu-lar homeostasis,autophagy is frequently stimulated in response to stress conditions such as those imposed by chemotherapy,nutrient deprivation,and microenviron-mental *** this capacity,autophagy serves as a mechanism to generate alternative sources of metabolic fuel that promote survival when preferred energy gener-ating pathways are impaired[1].Over the past decade,stress-induced autophagy has emerged as an important driver of malignant progression and anticancer drug *** all classes of clinically relevant anti-cancer agents,as well as radiotherapy,have been dem-onstrated to stimulate autophagy in preclinical studies[2].In the overwhelming majority of cases,genetically or pharmacologically impairing autophagy has been shown to yield therapeutic benefit in a manner that significantly improves the efficacy of the relevant agents or modalities[3].

Autoimmune hepatitis (AIH) is an important disorder that predominantly results in inflammatory liver disease in genetically predisposed *** clinicopathological picture is characterized by symptoms associated with both systemic inflammation and hepatic dysfunction,and with increased serum aminotransferases,elevated IgG,autoantibodies,and interface hepatitis on liver *** usually results in liver injury as a consequence of chronic hepatitis and ***,rarely,patients may present with fulminant liver *** diagnosis is important in all instances because the disease can be highly responsive to immunosuppressive therapeutic *** untreated,the disease is associated with high morbidity and *** we provide an overview of the current state of knowledge on AIH and summarize the treatment options for this serious condition in *** also discuss the pathogenesis of the disease as a possible consequence of autoimmunity and the breakdown of hepatic *** focus on regulatory T cell impairments as a consequence of changes in CD39 ectonucleotidase expression and altered purinergic *** understanding of hepatic tolerance may aid in the development of specific and well-tolerated therapies for AIH.

Every year billions of chickens are shipped thousands of miles around the globe in order to meet the ever increasing demands for this cheap and nutritious protein ***,transporting chickens internationally can also increase the chance for introducing zoonotic viruses,such as highly pathogenic avian influenza A(H5N1)to new *** study used a retrospective analysis of poultry trading data from 2003 through 2011 to assess the risk of H5N1 poultry infection in an importing *** found that the risk of infection in an importing country increased by a factor of 1.3(95%CI:1.1e1.5)for every 10-fold increase in live chickens imported from countries experiencing at least one H5N1 poultry case during that *** results suggest that the risk in a particular country can be significantly reduced if imports from countries experiencing an outbreak are decreased during the year of infection or if biosecurity measures such as screening,vaccination,and infection control practices are *** findings show that limiting trade of live chickens or increasing infection control practices during contagious periods may be an important step in reducing the spread of H5N1 and other emerging avian influenza viruses.

AIM： To investigate renin expression in pericytes dur-ing normal kidney development and after deletion of angiotensinogen, the precursor for all ***： We examined the distribution of renin ex-pressing cells by immunoshistochemistry in the intersti-tial compartment of wild type （WT） and angiotensino-gen deficient （AGT -/-） mice at different developmental stages from embryonic day 18 （E18： WT, n = 4; AGT -/-, n = 5） and at day 1 （P1： WT, n = 5; AGT -/-, n = 5）, 5 （P5： WT, n = 7; AGT -/-, n = 8）, 10 （P10： WT, n = 3; AGT -/-, n = 5）, 21 （P21： WT, n = 7; AGT -/-, n =5）, 45 （P45： WT, n = 3; AGT -/-, n = 3）, and 70 （P70： WT, n = 2; AGT -/-, n = 2） of postnatal life. We quanti-fied the number of pericytes positive for renin at all the developmental stages mentioned above and compared the results of AGT -/- mice to their WT ***： In WT mice, renal interstitial pericytes syn-thesize renin in early life supporting a lineage relation-ship with renin cells in the vasculature. The number of pericytes positive for renin per area of 0.32 mm2 （density） in WT mice was maintained from fetal life till weaning age （E18 = 4.25 ± 0.63, P1 = 3.75 ± 0.48, P5 = 3.75 ± 0.48, P10 = 4 ± 0.71, P21 = 3.8 ± 0.58） and markedly decreased in adult life （P45 = 1.2 ± 0.37, P70 = 0.8 ± 0.20）. Mediastinal Liposarcoma: Case Report and Literature Review

Liposarcomas (LS) are the second most common type of soft tissue malignancies in adults;they arise from mesenchymal cells and account for 1% of all adult cancers and 15% of all pediatric cancers. The site of origin can be from anywhere there is fat in the human body. LS are classified based on the primary site of origin, and mediastinal LS are extremely rare. When mediastinal neoplasms are stratified based on histology, they represent less than 1% of all mediastinal tumors. To date, less than 150 cases have been reported in the English literature. This article aims to present an unusual case of an extremely rare malignancy and perform a systematic review of the latest literature. In this report, our group is documenting the presentation, management, and outcome of a 65-year-old male patient with a massive anterior mediastinal primary LS. The tumor was displacing the mediastinum into the right chest, occupying most of the left chest, and pushing the diaphragm into the abdomen. Mediastinal liposarcomas are extremely rare malignancies and can prove to be challenging to diagnose and treat. Aggressive surgical treatment with R0 resection is the gold standard, however, tumor biology in many cases is associated with variable growth rates and encroachment of adjacent vital structures and blood vessels. When complex anatomical structures preclude an R0 resection, there is a high incidence of local recurrence. In cases where there is a high risk of recurrence, radiotherapy is indicated and chemotherapy has a more limited role.
