Synchronous and Asynchronous Blastomere Cleavage at Cryopreservation: Effect on Subsequent Embryo Survival, Pregnancy and Live Birth Rates

Capsule: Although embryos with synchronous blastomere cleavage showed higher post-thaw survival rates, pregnancy rates did not differ. Thus, embryos with all cleavage patterns may be safely cryopreserved. Objective: To compare post-thaw embryo survival, pregnancy and live birth rates of embryos with synchronous vs asynchronous blastomere cleavage in frozen embryo transfer (FET) cycles. Design: Retrospective study. Setting: University-affiliated IVF unit. Patients: One thousand and sixty FET cycles performed from 2004-2006. Interventions: Cycles were divided into 3 groups: 1: cycles in which only embryos with synchronous blastomere cleavage were frozen;2: cycles in which only embryos with asynchronous blastomere cleavage were frozen;3: cycles in which both embryos with synchronous and asynchronous blastomere cleavage were frozen. Clinical and laboratory data were recorded and analyzed. Main Outcome Measures: Post-thaw embryo survival, morphologic grading, pregnancy and live birth rates. Results: A total of 1863 embryos were analyzed. Synchronous embryos had higher blastomere survival rates and morphological grading at thawing. Pregnancy and birth rates did not differ among groups. In a multivariant logistic regression analysis, a number of transferred embryos and embryo morphological grading at thawing were the only parameters that affected pregnancy and live birth rates. Conclusions: Embryos with both synchronous and asynchronous blastomere cleavage can be selected by classical embryo grading and safely cryopreserved.

LTT switch unit for capacitive energy storages

When the capacitor cell is discharged in the short-circuit mode, the current pulse amplitude and duration are maximal. Therefore, this mode is the most severe for discharge switches of capacitive energy storage. The characteristics of the transient process of the discharge capacitive energy storage and the current loads acting in the facility discharge circuit have been defined for this mode. The test bench for definition of the LTT(Light Triggered Thyristors) loading capacity is described. The limiting characteristics have been experimentally obtained for LTT, at which there emerges the thermo-generation *** process of OFF-ON switching of LTTs has been investigated, the necessity is shown to use the speedup R-C circuits to ensure fast and stable transition of LTT into the conducting state. The design of the switch unit for the capacitive energy storage comprising LTTs and crowbar diodes is described, and the transient processes of current switching in crowbar diodes are considered. The tests carried out during switching of pul se current up to 100 kA at a voltage of 6 kV have confirmed the workability of the switch unit.

The Introduction of Pharmaceutical Care in Psychiatric Outpatients in Community Pharmacy of the Northeast Region of Argentina

The purpose of this paper is to implement a pharmaceutical care program in psychiatric outpatients in a community pharmacy. Outpatients （536） with psychiatric treatment requiring the dispensing of medication prescribed by a psychiatrist were followed up in a community pharmacy, where different medicines were prescribed as PS （pharmaceutical specialties）, PC （pharmaceutical compounding） or both PS and PC. Each prescription was registered with details on a patient level. Also, three reporting sheets were designed： patients profile, patients monitoring and patients counseling. The total study population in the community pharmacy consisted of 536 outpatients： 357 （66.6%） females and 179 （33.4%） males. Most of the outpatients （78.5%） have health insurance, 50% correspond to public and 28.5% to private institution. The other patients （21.5%） do not have medical insurance. We also observed that the education level of these patients was： primary school 19.1%; high school 45.9%; college 15.3% and university 20.7%. Many patients had more than one psychiatric diagnosis, to whom were prescribed different medicines. All the medication studies on the charts were screened for prescriptions with antidepressants and other psychotropic drugs, starting on the date of first diagnosis made by a psychiatrist. The counseling to the patients was also registered. The possibility of the follow-up of these outpatients in the community pharmacy promoted the development of the psychiatric pharmacy and all advances in care for patients with mental health needs, working in closer collaboration with psychiatrists.

Conceptual design of 2MJ capacitive energy storage

The conceptual design of the capacitive energy storage intended for operation in laboratory conditions is considered. This capacitive energy storage includes the capacitor cells of 200 kJ stored energy, each incorporating one self-healing high-energy-density capacitor, one semiconducting switch unit on the basis of Light Triggered Thyristors(LTT) and the pulse diodes, as well as the toroid inductor. Power supply of the capacitive energy storage is provided by high voltage(HV) 3 x 10 kV, 50 Hz industrial electrical network, using the HV power transformer and phase-controlled rectifier. Special design for the capacitor cell and the module of capacitor cells is proposed, which provides easy access to all components of the capacitor cells during operation and maintenance. Control system of the capacitive energy system has a hierarchic structure and includes programmable logic controller(PLC), programmer for non-synchronous triggering of capacitor cells discharges and data acquisition system.

Ocular syphilis resurgence in an urban underserved community in the United States

AIM:To evaluate the clinical characteristics,demographics,and visual outcomes of patients with ocular syphilis at an urban hospital to increase awareness and assist in earlier diagnosis and treatment of the resurgent ***:A retrospective chart review was performed on patients with ICD-9 and ICD-10 diagnosis codes correlating with syphilis or syphilis-related ocular diseases between 2010 and *** evaluated included age,gender,race,vision,ocular findings,human immunodeficiency virus(HIV)status and men who have sex with men status,recreational drug and alcohol ***:Ocular syphilis was diagnosed in 40 patients(53 eyes)of a total of 229 patients who tested positive for syphilis via serum and/or cerebrospinal fluid treponemal testing from *** patients with ocular syphilis,most patients were males,aged 45 or above and Black,and had no diagnosed HIV *** 50%patients had 20/40 vision or *** 50%had nongranulomatous anterior uveitis as their initial presentation,and 49%of patients had involvement of the posterior *** glaucoma(5.7%),papillitis(7.5%),vasculitis(5.7%),and retinal detachment(5.7%)were rarer presentations of the disease and were associated with a poorer visual ***:Given the increased prevalence and protean manifestations of syphilis,early diagnosis and treatment are *** studies on ocular syphilis are warranted to understand this resurging disease.

Non-Specific Factors Involved in the Treatment： Its Influence on Treatment Adherence

Introduction： The effectiveness of treatment depends on the efficacy of the therapy and the level of compliance of the patient. NF （non-specific factors） involved in treatment are all those effects that do not depend on the pharmacological properties of the drug. Materials and Methods： The job lasted a year. The sample consisted of patients with mental health disorders and was divided into two groups. Treatment compliance was measured with the Morisky-Green Test. Results were compared using the chi square test and relative risk. Results and Discussion： Group A had 23 patients ending 91.3% of them and group B of 22 patients, ending the 77.27%. At the beginning of the study, we found in the group A 0.86 NF/patients while in the group B 0.82 NF/patient. At the end of the study there was a 54.65% decrease in group A while in group B the proportion remained. At the beginning of the study both groups had approximately 40% of compliant patients. Data that remained in the control group rose to 80.95% in group A at the end of the study. Conclusions： The work demonstrates the negative influence of these factors on adherence to treatment.

Mapping aboveground biomass and its prediction uncertainty using LiDAR and field data, accounting for tree-level allometric and LiDAR model errors

Background: The increasing availability of remotely sensed data has recently challenged the traditional way of performing forest inventories, and induced an interest in model-based inference. Like traditional design-based inference, model-based inference allows for regional estimates of totals and means, but in addition for wall-to-wall mapping of forest characteristics. Recently Light Detection and Ranging(LiDAR)-based maps of forest attributes have been developed in many countries and been well received by users due to their accurate spatial representation of forest resources. However, the correspondence between such mapping and model-based inference is seldom appreciated. In this study we applied hierarchical model-based inference to produce aboveground biomass maps as well as maps of the corresponding prediction uncertainties with the same spatial resolution. Further, an estimator of mean biomass at regional level, and its uncertainty, was developed to demonstrate how mapping and regional level assessment can be combined within the framework of model-based ***: Through a new version of hierarchical model-based estimation, allowing models to be nonlinear, we accounted for uncertainties in both the individual tree-level biomass models and the models linking plot level biomass predictions with LiDAR metrics. In a 5005 km2 large study area in south-central Sweden the predicted aboveground biomass at the level of 18 m×18 m map units was found to range between 9 and 447 Mg·ha^-1. The corresponding root mean square errors ranged between 10 and 162 Mg·ha^-1. For the entire study region, the mean aboveground biomass was 55 Mg·ha^-1 and the corresponding relative root mean square error 8%. At this level 75%of the mean square error was due to the uncertainty associated with tree-level ***: Through the proposed method it is possible to link mapping and estimation within the framework of model-based inference. Natural course of ovarian torsion after untwisting: A case report with radiographic imaging

Background: Radiographic studies largely contribute to prompt diagnosis of ovarian torsion, though post-operative changes are not described and might contribute to unnecessary intervention. Case: We report a case of ovarian torsion diagnosed based on clinical presentation and radiographic findings. The detorsion was successfully performed through laparosopic access. Subsequently patient presented for elective ultrasonography and MRI which diagnosed multicystic ovarian mass, attributed to hematoma which resolved spontaneously with no intervention in 3 weeks. Conclusion: Ovarian changes after detorsion consistent with development of hematoma may be benign and resolve spontaneously.

BACKGROUND Type 1 diabetes(T1D)contributes to altered lipid profiles and increases the risk of cardiovascular disease(CVD).Youth with T1D may have additional CVD risk factors within the first decade of *** To examine risk factors for dyslipidemia in young subjects with *** Longitudinal and cross-sectional retrospective study of 170 young subjects with T1D(86 males;baseline mean age 12.2±5.6 years and hemoglobin A1c 8.4%±1.4%)were followed in a single tertiary diabetes center for a median duration of 15 *** for outcomes of lipid profiles at last visit(total cholesterol[TC],triglycerides[TGs],low-density lipoprotein-cholesterol[LDL-c],and highdensity lipoprotein-cholesterol[HDL-c])were analyzed by stepwise linear regression *** At baseline,79.5%of the patients had at least one additional CVD risk factor(borderline dyslipidemia/dyslipidemia[37.5%],pre-hypertension/hypertension[27.6%],and overweight/obesity[16.5%])and 41.6%had multiple(≥2)CVD risk factors.A positive family history of at least one CVD risk factor in a first-degree relative was reported in 54.1%of the *** of elevated TC:family history of CVD(β[SE]=23.1[8.3],P=0.006);of elevated LDL-c:baseline diastolic blood pressure(DBP)(β[SE]=11.4[4.7],P=0.003)and family history of CVD(β[SE]=20.7[6.8],P=0.017);of elevated TGs:baseline DBP(β[SE]=23.8[9.1],P=0.010)and family history of CVD(β[SE]=31.0[13.1],P=0.020);and of low HDL-c levels:baseline DBP(β[SE]=4.8[2.1],P=0.022]).CONCLUSION Our findings suggest that elevated lipid profiles are associated with DBP and a positive family history of *** is of utmost importance to prevent and control modifiable risk factors such as these,as early as childhood,given that inadequate glycemic control and elevation in blood pressure intensify the risk of dyslipidemia.
