Is there a window of opportunity to optimize trastuzumab cardiac monitoring?

BACKGROUND It remains unclear whether the current arbitrary screening recommendations of trastuzumab-related cardiotoxicity provides an adequate balance between preventing heart damage and curtailing a curative *** To determine the incidence rate and consequences of trastuzumab-induced cardiotoxicity as adjuvant treatment in a real-world *** We present a retrospective analysis of cardiac function measured by echocardiogram at baseline and every 3 mo during trastuzumab *** was defined as a drop in left ventricular ejection fraction(LVEF)≥10%from baseline and/or any drop<50%.RESULTS Between January 2011 and December 2014,407 patients were ***(93.6%)were treated with an anthracycline followed by a taxane-based regimen and trastuzumab for 12 *** patients(9.8%)had *** of them were symptomatic,and 28(72.5%)completely recovered *** happened early as shown by LVEF measured on echocardiogram 2 to 4 as compared to 5 to 7(odds ratio=2.47,95%confidence interval:1.09,5.63,P=0.024).There were 54 deaths(13.3%)during the 70-mo follow-up period;1(0.2%)was attributed to late cardiotoxicity(4 years after treatment).The absence of symptomatic cardiotoxicity during trastuzumab treatment and moreover the early occurrence on the treatment period may translate into a strategy to evaluate less *** We observed a 10%rate of asymptomatic cardiotoxicity,which mirrors the results from the large adjuvant *** being transient,an LVEF drop led to frequent treatment delays and *** remains unclear whether LVEF decline is predictive of late cardiotoxicity,and treatment efficacy is compromised.

Mortality Trends for Neglected Tropical Diseases in the State of Sergipe,Brazil,1980-2013

Background:Neglected Tropical Diseases are a set of communicable diseases that affect the population so low socioeconomic status,particularly 1.4 billion people who are living below the poverty *** study has investigated the magnitude and mortality time trends for these diseases in the state of Sergipe,Northeast Region of ***:We conducted an ecological study of time series,based on secondary data derived from the Mortality Information System of the Ministry of *** mortality rates(crude,age-standardized rates and proportional ratio)were calculated from the deaths due to Neglected Tropical Diseases in the state of Sergipe,from 1980 to *** time trends were obtained using the Joinpoint regression ***:Three hundred six thousand and eight hundred seventy-two deaths were certified in the state and Neglected Tropical Diseases were mentioned as the underlying cause in 1,203 certificates(0.39%).Mean number of deaths was 35.38 per year,and crude and age-standardized mortality rates were,respectively:2.16 per 100000 inhabitants(95%CI:1.45-2.87)and 2.87 per 100000 inhabitants(95%CI:1.93-3.82);the proportional mortality ratio was 0.41%(95%CI:0.27-0.54).In that period,Schistosomiasis caused 654 deaths(54.36%),followed by Chagas disease,with 211(17.54%),and by Leishmaniases,with 142(11.80%)*** other diseases totalized 196 deaths(16.30%).There were increasing mortality trends for Neglected Tropical Diseases,Schistosomiasis and Chagas disease in the last 15 years,according to the age-standardized rates,and stability of the mortality trends for ***:The Neglected Tropical Diseases show increasing trends and are a real public health problem in the state of Sergipe,since they are responsible for significant mortality *** following diseases call attention for showing greater number of deaths in the period of study:Schistosomiasis,Chagas disease and *** finally suggest that public managers take appropri

Renin-angiotensin system in the pathogenesis of liver fibrosis

Hepatic fibrosis is considered a common response to many chronic hepatic injuries. It is a multifunctional process that involves several cell types, cytokines, chemokines and growth factors leading to a disruption of homeostatic mechanisms that maintain the liver ecosystem. In spite of many studies regarding the development of fibrosis, the understanding of the pathogenesis remains obscure. The hepatic tissue remodeling process is highly complex, resulting from the balance between collagen degradation and synthesis. Among the many mediators that take part in this process, the components of the Renin angiotensin system （RAS） have progressively assumed an important role. Angiotensin （Ang） II acts as a profibrotic mediator and Ang-（1-7）, the newly recognized RAS component, appears to exert a counter-regulatory role in liver tissue. We briefly review the liver fibrosis process and current aspects of the RAS. This review also aims to discuss some experimental evidence regarding the participation of RAS mediators in the pathogenesis of liver fibrosis, focusing on the putative role of the ACE2-Ang-（1-7）- Mas receptor axis.

To evaluate the therapeutic effects of bone marrow-derived CD11b+CD14+ monocytes in a murine model of chronic liver *** liver damage was induced in C57BL/6 mice by administration of carbon tetrachloride and ethanol for 6 mo. Bone marrow-derived monocytes isolated by immunomagnetic separation were used for therapy. The cell transplantation effects were evaluated by morphometry, biochemical assessment, immunohistochemistry and enzyme-linked immunosorbent ***11b+CD14+ monocyte therapy significantly reduced liver fibrosis and increased hepatic glutathione levels. Levels of pro-inflammatory cytokines, including tumor necrosis factor-α, interleukin (IL)-6 and IL-1β, in addition to pro-fibrotic factors, such as IL-13, transforming growth factor-β1 and tissue inhibitor of metalloproteinase-1 also decreased, while IL-10 and matrix metalloproteinase-9 increased in the monocyte-treated group. CD11b+CD14+ monocyte transplantation caused significant changes in the hepatic expression of α-smooth muscle actin and *** therapy is capable of bringing about improvement of liver fibrosis by reducing oxidative stress and inflammation, as well as increasing anti-fibrogenic factors.

In the Amazon region,several residues that have been misused can serve as feedstocks for biochar production with the aim of recovering soils contaminated by heavy ***,these biochars need to be firstly tested for their adsorption capacity as well as their physicochemical attributes prior to field ***,this study aimed to characterize and evaluate the adsorption capacities of Cd^(2+) and Cu^(2+) of biochars produced from acai(BA),Brazil nut(BN),and palm kernel cake(BK)*** were produced by slow pyrolysis at four different temperatures(400,500,600,and 700℃).The physicochemical properties of the biochars,such as cation exchange capacity,ash,recalcitrance index,and aromaticity were enhanced with increased pyrolysis *** adsorption capacities of Cd^(2+) and Cu^(2+) showed high correlations with the physicochemical properties of biochar,indicating the importance of these characteristics in the adsorption ***,the adsorption of Cd^(2+) and Cu^(2+) also increased with the increase in the pyrolysis *** a competitive system,Cd^(2+)exhibited higher adsorption capacity than Cu^(2+) for all *** general,BN showed the highest adsorption capacity,followed by BK and *** produced from the Amazonian residues have the potential to improve soil quality when used as amendments in the recovery of soils contaminated with Cd and Cu,representing an environmentally sound technology for the reuse of these residues.

Approach to Pain in Endometriosis Patients: Systematic Literature Review

Background: Endometriosis is a gynecological, multifactorial condition that affects women’s life quality, especially due to the pain inherent to it. Pain is caused by physical and psychological aspects;therefore, several factors must be analyzed, aiming to treat the whole. Conventional, drug or surgical treatments are the most used;however, the search for alternative therapeutic methods is growing. Objective: To analyze the approach to pain in women with endometriosis, seeking a better understanding of the pathophysiological mechanisms of this disease, its impact on life quality, as well as the means of treatments in use and innovations in this field. Methods: Systematic literature review, conducted in the Virtual Health Library databases between 2010 and 2021, restricted to articles in English and Portuguese with the descriptors: Endometriosis, Pain, and Treatment. After reading the abstracts, 197 articles were found, and, after reading them, 59 articles were selected. Results: This review analyzed 59 scientific studies that rigorously met the previously established characteristics in the sample selection. The synthesis included the following aspects: author/year of publication, article title, objective, type of study and database. Related factors such as acute or chronic pain, late diagnosis, discrediting symptomatic complaints and trivialization by health professionals have a profound impact on the physical and mental health of women with endometriosis. On the other hand, there are several classes of drugs, surgical procedures, and alternative therapies available for pain management. Conclusion: Given the context addressed, a certain need is suggested for health professionals to better understand the disease and adopt new behaviors or improve existing ones, such as reception, active listening, early diagnosis and comprehensive and individualized treatment. This study aimed to identify the chemical composition of essential oil from fruits(EOAF)and the hexanic crude extract from aerial parts(At-Hex)of atemoya(Annona cherimola x Annona squamosa),a hybrid belonging to the Annonaceae *** and antimicrobial activity was also *** was obtained by hydrodistillation using a Clevenger apparatus,and their composition was analyzed by gas chromatography-mass spectrometry(GC-MS)*** was tested against human tumor cell lines HCT-116(colon carcinoma),SF-295(glioblastoma),OVCAR-8(ovarian carcinoma)and HL60(leukemia)using 3-(4,5-dimethyl-2-thiazolyl)-2,5-diphenyl-2H-tetrazolium bromide(MTT)assay,while antimicrobial activity was conducted by bioauthography method against eleven microorganisms ***-four compounds were identified in the EOAF and twenty-nine in *** monoterpenes linalool(25.70%),α-pinene(10.38%),β-pinene(9.12%),transocimene(7.43%),and the sesquiterpene bicyclogermacrene(12.58%)were the major constituents of EOAF,whereas the sesquiterpene spathulenol(13.91%)was the main compound of ***-Hex showed a high cytotoxicity against SF-295(glioblastoma).These findings show an important chemotaxonomic contribution for the Annonaceae family,mainly for the Annona *** proved to be a promising source of substances with potential cytotoxic activity.

Behavioral Risk Factors for Chronic Non-Communicable Diseases in Adults and Elderly People in the City of Ribeirão Preto—SP

Aims: to analyze the presence and simultaneity of behavioral risk factors for chronic non-communicable disease (NCD) in adults and the elderly. Methods: quantitative, cross-sectional and observational study. Healthy and unhealthy eating are classified according to the frequency of food consumption. Smoking is expressed by the percentage of smokers. Harmful alcohol consumption by the percentage of individuals who consumed alcoholic beverages at least once in the last 30 days. Classification of the level of physical inactivity required to practice at least 150 minutes of physical activity. Results: the sample constituted 719 people, 535 adults and 184 elderly. The behavioral risk factor reported by most adults 499 (93.3%) and elderly 156 (84.8%) was unhealthy eating. Frequency 72% higher of smoking, four times higher of harmful alcohol consumption and 10% higher of unhealthy food among adults when compared to the elderly. The simultaneity between risk factors was 39% greater in adults than in the elderly. Discussion: risk factors evaluated in this study are key aspects of the development of NCDs. Adults have 2.43 times the chance of presenting two risk factors and 7.73 times the chance of presenting three in relation to the elderly. Conclusion: To achieve more effective and differentiated results, knowing and directing measures to control behavioral risk factors, whether isolated or concurrent, requires specific knowledge.
