Marfan’s syndrome (MFS) is an autosomal dominant disorder characterized by aortic root dilation, mitral valve prolapse and aortic regurgitation. The typical aneurysm location involves the thoracic aorta (60%). Thoracoabdominal aortic dissections are quite infrequent (10%). A type A aortic dissection can have fatal complications, such as aortic rupture and cardiac tamponade, and a prompt diagnosis is crucial. The present case involves a 46-year-old patient with Marfan’s syndrome who was admitted to the emergency department for dyspnea. The patient presented congestive signs and manifestations suggesting acute heart failure. An initial transthoracic echocardiogram (TTE) showed ventricular hypertrophy and diastolic dysfunction. Additionally, TTE showed plurivalvular regurgitation, pulmonary hypertension (42 mmHg), aortic dilatation, and a dissection flap. Coronary computed tomography angiography (CCTA) was performed, confirming the suspicion of ascending aortic dissection. The patient underwent a three-stage intervention for the repair of all the defects, with adequate recovery. This was a rare presentation of aortic dissection in MFS, which was a diagnostic challenge for the heart team.

Determination of Mercuric Ion in Water Samples with a LED Exciting and CCD Based Portable Spectrofluorimeter

The fluorescent characteristics of a fluorimetric chemosensor for mercuric ion, Hg2+, employing a synthesized Rhodamine 6G derivative, have been analyzed. For that, a portable spectrofluorimeter composed of a 515 nm LED as excitation source, two fiber-optics and a CCD camera as detector, has been used, intended for “in situ” analysis. A highly selective Rhodamine based probe for Hg2+, that is water soluble and gives a positive response upon analyte binding, is reported. The reagent is bearing a monothiospirolactone group in a Rhodamine 6G architecture and the thiol atom served for the direct attack of thiophilic Hg2+. The fluorescence enhancement is attributed to the spirolactone ring opening and the coordination of two sulphur atoms to Hg2+ giving a 2:1 reagent: Hg2+ stoichiometry complex.

Hydrometeorology of Anthropogenic Erosion in the Metropolitan Region of São Paulo

Erosive processes play an important role in environmental degradation. Rain is the main erosive agent in the Metropolitan Region of Sao Paulo. This study characterized the erosion events caused by precipitation leveraging the accumulated daily precipitation estimate generated by the Climate Prediction Center Morphing Method (CMORPH) and integrating the surface telemetric network using the Statistical Objective Analysis method (SOAS). From the Civil Defense database, 400 events were identified in the Metropolitan region of Sao Paulo (MRSP) area between 2000 and 2013 and, of these, 3 were chosen to carry out meteorological and climatological analyses. In an initial observation, 58% of them were found to occur in summer. Two regions with the highest number of erosion events were observed, in the Serra do Mar and Serra da Cantareira. In the Serra do Mar core, the municipality of São Bernardo do Campo was the one with the greatest amount of erosion. Precipitation volumes were estimated for accumulations of 30 minutes, 1 day, 1 month, and 1 year. The results, from the 3 events, indicate accumulated precipitation in 30 minutes from 10 mm to 19.8 mm, daily from 30.8 mm to 69.5 mm, and 1 month from 369.7 mm to 742.5 mm, and 1 year (2010) from 1712.9 mm to 1961.8 mm. In these events, it was noted that there were heavy rains in December 2009 and January 2010. It was also noted that the CMORPH and SOAS identify the rain events found by the São Paulo meteorological radar. The meteorological analyzes of the events based on images from the São Paulo meteorological radar and the Meteosat-9 satellite indicate that the active precipitation systems are associated with cold fronts, instability lines, and isolated convection.

The southern region of the state of Chihuahua is mining by origin, as this has been proved ghost towns of Minas Nuevas, it is also called Villa Escobedo, Emerald and Blue. Minerals are nonrenewable resources. There are only reserves and waste deposited in tailings ponds of low-rade precious metals （gold and silver）. A conventional method for recovering gold and silver ore is low-grade cyanidation and the solvent consider toxic and highly dangerous for the flora, fauna, humans and the environment. The mechanism of cyanide poisoning is inhibition of cytochrome oxidase--an enzyme needed for cell respiration. Without compromising efficiency, sensitivity and cost of the process, the toxic solvent is replaced by a less aggressive one, which helps the environment, public and occupational health. （NH4）2S203 has no appreciable damage to conservative amounts, is a selective aqueous solvent for leaching gold and silver, easy to get and manipulate. This work is done in the Technological Institute of Parral in a low-grade ore from the southern region of the state of Chihuahua. This document has been enriched presented in the IMEC 2014, with the increase of series Ⅴ and Ⅵ.

Contamination of Water for Domestic Use and Human Consumption by Heavy Metals Derived from Mining Activity in Parral, Chihuahua, Mexico

Research was done with the objective of evaluating the quality of water supplied to the potable water system in Hidalgo del Parral, which comes from two main supply sources： wells from ＂Valle del Verano＂, which provide water to the east side of the city, and the Water Treatment Plant, which receives water from different mines and supplies the west side of the city. When this plant is overcome on its capacity of treatment, a part of the water from the mines is fed directly to the northwest part of the city. The metals lead （Pb）, chromium （Cr）, cadmium （Cd）, zinc （Zn）, and arsenic （As） were analyzed by atomic absorption （AA） technique, based on the United States Environmental Protection Agency （EPA） Method 7000B （Revision 2, February 2007）. The digestion process was done according to the guidelines established in Method NMX-AA-051-SCFI-2001, Analysis of Water-Determination of Metals by Atomic Absorption, developed by the National Standardization Technical Committee for Environmental Protection （Mexico）. The samples were collected from the following points： 13 wells and 2 re-pumping stations of ＂Valle del Verano＂, the inlet and outlet of the Water Treatment Plant, and domestic faucets. The east side of the city, supplied by the wells of ＂Valle del Verano＂, is metal-pollution free. The water that comes from the mines contains high concentrations of As, Zn, Cd and Pb, with levels that can get to 0.12, 32.6, 0.26 and 3.5 mg/L, respectively. This water pollution generated by mining can be concluded from the high levels found in the domestic samples in which, for several points, the concentrations exceed the parameters set by the current guideline. According to the results, it is vital to set a plan of remediation for the water that comes from the mines, because there is a great risk for health in the consumotion of this water.

Aggressive Angiomyxoma: An Unusual Female Pelvic Tumour. Report of Three Cases and Review of the Literature

Aggressive angiomyxoma was first described in 1983 by Steeper and Rosai, and fewer than 150 cases have been reported in the world medical literature. It is a soft-tissue tumour of the pelvis and perineum. The recurrence rate is high, and often extensive resections are performed with considerable morbidity. These tumours are benign, locally infiltrative mesenchymal neoplasms with a predilection for the female pelvis and perineum and they usually tend to recur. Furthermore, these tumours often reach too large dimensions before becoming clinically symptomatic;their incidence is higher in women of the reproductive age group;however a few cases of its occurrence outside the pelvis have also been reported. In this study, we reported three cases with aggressive pelvic angiomyxoma treated with surgical methods and used an approach that described by Kraske in order to get access to lower rectal cancers. Accurate preoperative diagnosis should alert the surgeon to the need for wide excision, which is essential for prevention of local recurrence.

Effective prophylaxis with rFVIIa in young haemophiliacs with inhibitors using a schedule similar to FVIII prophylaxis in non-inhibitor patients

Objective: To assess the role of early prophylaxis with recombinant activated factor VII (rFVIIa) in young haemophiliacs with inhibitors and to determine whether it can reduce bleeding episodes and prevent joint damage. Patients and Methods: Ten severe haemophiliacs, less of three years old, with up to four joint bleeds and inhibitors to FVIII who started early prophylaxis with rFVIIa, were included. A number of haemorrhages/year/patient and haemarthroses/ year/patient were compared before the start of prophylaxis, which include both the time before (initial period) and after inhibitor diagnosis (inhibitor period), with those during prophylaxis (prophylaxis period). Results: The mean time of inhibitor diagnosis was 15.6 months (range: 2.3-34.1). The mean time between inhibitor diagnosis and the start of prophylaxis was 7.1 months (range: 0-23.2), shorter than the time of rFVIIa prophylaxis (mean: 10.3 months;range: 4.1-32.0). Bleeding episodes for the three time periods were 45, 36 and 17, respectively, or 0.29 and 0.51 haemorrhages/year/patient in the two periods prior to prophylaxis compared to 0.17 during prophylaxis. Total haemarthroses for the three-time periods were 7, 5 and 5, respectively. The haemarthroses/year/patient in the pre-prophylaxis period were 0.032 and 0.070, compared to0.049 inthe prophylaxis period. rFVIIa schedules were either 90 μg/kg three times weekly or 90 μg/kg daily. Conclusions: Early prophylaxis with rFVIIa may be efficacious in young haemophiliacs with inhibitors, reducing joint bleeds. After the risky period of inhibitor presence, they are able to continue rFVIII prophylaxis with success assured to prevent arthropathy.

Satisfaction of the Service Provider with the Organization of the Timely Cervical Cancer Detection Program

Cervical cancer is a priority health problem in Mexico. The objective of this work was to know the degree of satisfaction of the service provider with the organization of the program of Timely Detection of Cervical Cancer, specifically in taking of the cervical cytology. The study design was observational, transversal and prospective conducted from January to May 2015 in Jurisdiction 1 of the Health Services of San Luis Potosí, Mexico, in 20 health centers. Participants were 87 service providers whose function is to take cervical cytology (Pap smears). A 32-question questionnaire was applied, with Likert scale with 5 levels of response: 1 (dissatisfied), 2 (little satisfied), 3 (indifferent), 4 (satisfied) and 5 (very satisfied). The score range was 32 points (minimum job satisfaction) up to 160 points (Maximum job satisfaction). We analyzed 2 dimensions: 1) intrinsic factors that have to do with motivation and professional recognition, and 2) extrinsic factors related to infrastructure, materials, remuneration and labor policies. Each factor consisted of four categories. Descriptive statistics and linear correlation of Pearson were applied for the analysis of data. Participants signed an informed consent letter. Of 4 categories, in the intrinsic dimension, the one with the highest percentage of satisfaction was “delegation of activities” with 28.8%. In the extrinsic dimension, the category with the highest percentage of satisfaction was “infrastructure and materials” with 49.4%;while satisfaction increase in intrinsic dimensions also does in extrinsic dimensions, such correlations are significant (p delegation of activities and remuneration (r = 0.074 and p = 0.497). The higher hierarchical level, the men, the bachelor academic level and older people showed the highest level of satisfaction.
