CYP2E1 immunoglobulin G4 subclass antibodies after desflurane anesthesia

AIM: To investigate CYP2E1 IgG4 autoantibody levels and liver biochemical markers in adult patients after anesthesia with desflurane. METHODS: Forty patients who were > 18 years old and undergoing elective surgery under general anes-thesia with desflurane were studied. Alpha-glutathione-S-transferase(aGST) and IgG4 antibodies againstCYP2E1 were measured preoperatively and 96 h post-operatively, as well as complete blood count, prothrom-bin time(PT), activated partial thromboplastin time(aPTT), international normalized ratio(INR), aspartate aminotransferase(SGOT), alanine aminotransferase(SGPT), g-glutamyl-transpeptidase(gGT), alkaline phosphatase, total serum proteins, albumin and bili-rubin. A separate group of 8 patients who received re-gional anesthesia was also studied for calibration of the methodology used for CYP2E1 IgG4 and aGST measure-ments. Student's t-test and the Mann-Whitney U test were used for comparison of the continuous variables, and Fisher's exact test was used for the categorical variables. All tests were two-tailed, with statistical sig-nificance set as P re successfully discharged from the hospital. No statistically significant difference was observed regarding liver function tests(SGOT, SGPT, γGT, bilirubin, INR), aGST and CYP2E1 IgG4, before and after exposure to desflurane. After dividing patients into two subgroups based on whether or not they had received general anesthesia in the past, no significant difference in the levels of CYP2E1 IgG4 was observed at baseline or 96 h after desflurane administration(P = 0.099 and P = 0.051, respectively). Alpha-GST baseline levels and levels after the interven-tion also did not differ significantly between these two subgroups(P > 0.1). Aspects of the non-pharmacological treatment of irritable bowel syndrome

Irritable bowel syndrome(IBS) is one of the most commonly diagnosed gastrointestinal conditions. It represents a significant healthcare burden and remains a clinical challenge. Over the years IBS has been described from a variety of different perspectives; from a strict illness of the gastrointestinal tract(medical model) to a more complex multi-symptomatic disorder of the brain-gut axis(biopsychosocial/psychosomatic model). In this article we present aspects of the pathophysiology and the non-pharmacological treatment of IBS based on current knowledge. Effects of conditioned stress and/or traumatic influences on the emotional system(top-down) as well as effects on the intestine through stressors,infection,inflammation,food and dysbiosis(bottom-up) can affect braingut communication and result in dysregulation of the autonomic nervous system(ANS),playing an important role in the pathophysiology of IBS. Conditioned stress together with dysregulation of the autonomic nervous system and the emotional system may involve reactions in which the distress inside the body is not recognized due to low body awareness. This may explain why patients have difficulty identifying their symptoms despite dysfunction in muscle tension,movement patterns,and posture and biochemical functions in addition to gastrointestinal symptoms. IBS shares many features with other idiopathic conditions,such as fibromyalgia,chronic fatigue syndrome and somatoform disorders. The key to effective treatment is a thorough examination,including a gastroenterological examination to exclude other diseases along with an assessment of body awareness by a body-mind therapist. The literature suggests that early interdisciplinary diagnostic cooperation between gastroenterologists and body-mind therapists is necessary. re-establishing balance in the ANS is an important component of IBS treatment. Neurocognitive Disorders in Patients with HIV Infection with Virologic Suppression for More than 10 Years

Background: HIV-associated neurocognitive disorder (HAND) may appear in patients with viral and immunological response to treatment and remain unnoticed during the initial stage of the infection. The goal of this study is to evaluate the development and/or progression of HAND in patients with undetectable viral load for more than ten years. Methods: We included adult HIV-infected patients who were under antiretroviral treatment and had undetectable plasma viral load for more than ten years (blips were included). These patients had already been subjected to neurocognitive evaluation five years previously. Demographic, clinical and analytical data were analysed. For the neurocognitive evaluation, the WAIS-III subtests (digit symbol coding and symbol search), trail making test (TMT) A and B, Stroop test and categorical verbal fluency (animals) tests were used. SPSS??version 22.0 for Windows was used for statistical analysis. results: In this re-evaluation, performed 4.76 (±1.82) years after the first one, 9 (36%) patients showed deficits in processing speed (WAIS-III and TMT A), 8 (32%) executive function (TMT B and Stroop) and 12 (48%) verbal fluency. There were significant statistical differences between the past and current executive function tests (p = 0.029 and p = 0.01), highlighting worsening of deficits. No differences were found for the other tests. No association was found between deficit progression and the studied variables. Conclusions: Although not generally noticed on regular appointments, in this small population, worsening of executive function deficits (mental flexibility and divided attention) was found. Classical risk factors for HAND did not appear to interfere in its progression. Speed of information processing and categorical verbal fluency remained stable.

Dear Editor,Pancreatic ductal adenocarcinoma(PDAC)remains a significant health problem with an increase in the incidence and a five-year survival rate of only 10%[1].The Phosphoinositide 3-kinase-protein kinase-B-mechanistic target of rapamycin(PI3K-AKT-mTOR)pathway is a driver pathway in PDAC and an important therapeutic target[2].We[3]and others[4–6]have demonstrated that the mTOR kinase is a therapeutic target in PDAC,and rationally designed mTOR inhibitor(mTORi)-based combination therapies are emerging[2].

Augev Method and an Innovative Use of Vocal Spectroscopy in Evaluating and Monitoring the rehabilitation Path of Subjects Showing Severe Communication Pathologies

A strongly connotative element of developmental disorders (DS) is the total or partial impairment of verbal communication and, more generally, of social interaction. The method of Vocal-verb self-management (Augev) is a systemic organicistic method able to intervene in problems regarding verbal, spoken and written language development successfully. This study intends to demonstrate that it is possible to objectify these progresses through a spectrographic examination of vocal signals, which detects voice phonetic-acoustic parameters. This survey allows an objective evaluation of how effective an educational-rehabilitation intervention is. This study was performed on a population of 40 subjects (34 males and 6 females) diagnosed with developmental disorders (DS), specifically with a diagnosis of the autism spectrum disorders according to the DSM-5. The 40 subjects were treated in “la Comunicazione” centers, whose headquarters are near Bari, Brindisi and Rome. The results demonstrate a statistical significance in a correlation among the observed variables: supervisory status, attention, general dynamic coordination, understanding and execution of orders, performing simple unshielded rhythmic beats, word rhythm, oral praxies, phono-articulatory praxies, pronunciation of vowels, execution of graphemes, visual perception, acoustic perception, proprioceptive sensitivity, selective attention, short-term memory, segmental coordination, performance of simple rhythmic beatings, word rhythm, voice setting, intonation of sounds within a fifth, vowel pronunciation, consonant pronunciation, graphematic decoding, syllabic decoding, pronunciation of caudate syllables, coding of final syllable consonant, lexical decoding, phoneme-grapheme conversion, homographic grapheme decoding, homogeneous grapheme decoding, graphic stroke.

Objective： Being overweight or obese comprises a significant risk factor for atherosclerosis. Fat tissue also generates factors stimulating angiogenesis, the process by which new blood vessels form. The purpose of this paper is to assess concentrations of the vascular endothelial growth factor A （VEGF-A） and its soluble type-1 and type-2 re- ceptors （sVEGFR-1 and sVEGFR-2） in plasma of patients with peripheral arterial disease （PAD） depending on the level of nutrition according to body mass index （BMI）. Methods： The study group included patients suffering from symptomatic PAD （n=46） in Fontaine classes Ila-lV without any history of neoplastic disease and who have a normal BMI （n=15）, are overweight （n=21） or are obese （n=10）. The control group （n=30） consisted of healthy non-smoking volunteers who were neither overweight nor obese. Venous blood plasma samples were collected from both groups at rest in the morning to determine plasma concentrations of VEGF-A, sVEGFR-1, and sVEGFR-2 using the enzyme- linked immunosorbent assay （ELISA） method. results： The group of patients with PAD co-existent with being over- weight or obese tended to have higher mean concentration levels of VEGF-A and sVEGFR-2 when compared with patients suffering from PAD with normal BMIo A statistically significant positive correlation was obtained between BMI and average plasma concentrations of sVEGFR-2 （R=0.37, P=-0.0103）. However, no significant correlation was no- ticed between BMI and VEGF-A or sVEGFR-1 concentrations. Conclusions： A positive correlation determined be- tween the level of antiangiogenic factor and BMI value may be indicative of the linearly growing prevalence of some antiangiogenic factors in patients with metabolic disorders, which may be one of numerous factors contributing to incomplete efficiency of collateral circulation development in patients with PAD.

Central and peripheral neurological involvement in monoclonal gammopathies of undetermined significance

Several studies have suggested a pathogenetic role of paraproteinaemias in PNS damage. Over the few last years, the presence of symptomatic or subclinical PNS lesions in CNS diseases like multiple sclerosis has been described. On the other hand, CNS demyelinating lesions and cervical atrophy have been re- ported in patients affected by chronic inflammatory demyelinating polyradiculoneuropathy (CIDP). Very few cases of MGUS associated with CNS disease alone or with both CNS and PNS disease have been re- ported. Since 1999, we have been studying 16 patients (8 M, 8 F), with a mean age 60.2 ± 13.4, affected by MGUS associated with symptomatic neurological central and/or peripheral diseases. Patients affected with lymphomas, lupus erithematosus and other immunological diseases were excluded. Involvement of both PNS and CNS was not associated to a particular type of paraproteinemia: monoclonal IgM were found in 8 patients;monoclonal IgG in 6 patients and mono- clonal IgA in 1 patient and Igl in 1 patient. High anti- nervous system autoantibodies were found in 10/16 patients and antiMAG antibodies were detected in patients with paraproteinemic demyelinating neuropathy (PDN). High reactivity anti-nervous system might support the hypothesis of a pathogenetic role of MGUS in these neurological diseases. Nevertheless, at present, we cannot exclude that there is only a circumstantial association between MGUS and neurological damages, particularly concerning CNS.

Evaluation of enterochromaffin cells and melatonin secretion exponents in ulcerative colitis

AIM: To study an assessment of the number of enterochromaffin cells and expression of hydroxyindole-Omethyltransferase in colonic mucosa and urine excretion of 6-sulfatoxymelatonin in patients with ulcerative colitis. METHODS: The study included 30 healthy subjects (groupⅠ-C), 30 patients with ulcerative proctitis [group Ⅱ-ulcerative proctitis (UP)] and 30 patients with ulcerative colitis [group Ⅲ-ulcerative colitis (UC)] in acute phases of these diseases. The number of enterochromaffin cells (EC) was estimated in rectal and colonic mucosa. Bioptates were assembled from many different parts of the large intestine. Immunorective cells collected from various parts of the colon were counted according to the Eurovision DAKO (Dako A/S, Copenhagen, Denmark) System in the range of 10 fields in each bioptate at × 200 magnification. The level of mRNA expression of hydroxyindole-O-methyltransferase (HIOMT) in colonic mucosa was estimated with RT-PCR. Urine 6-sulfatoxymelatonin (6-HMS) excretion was determined immunoenzymatically using an IBL (IBL International GmbH, Hamburg, Germany) kit (re 54031). reSULTS: The number of EC cells in healthy subjects (C) was 132.40 ± 31.26. In patients of group Ⅱ (UP) and group Ⅲ (UC) the number of these cells was higher 225.40 ± 37.35 (P respectively. Similar differences were related to HIOMT expression, which was 1.04 ± 0.36 in group C, 1.56 ± 0.56 (P retion was as follows: C 16.32 ± 4.95 μg/24 h, UP 26.30 ± 7.29 μg/24 h (P relation between number of EC cells and 6-HMS excretion was noted in all groups: r = 0.766 in patients with UP, r = 0.703 with UC and r = 0.8551 in the control group; the correlation between the results is statistically significant.
