Successful use of emergent endovascular aneurysm repair for a ruptured hypotensive abdominal aortic aneurysm

Ruptured aortic aneurysm has a surgical mortality ranging from 50%-70%, one of the highest rates of all vascular emergencies [1-5]. Less invasive approaches to repair have been developed that could potentially improve these statistics [1,5-7]. Endovascular aneurysm repair (EVAR) is a minimally invasive approach that may be an alternative to open surgical repair for select cases of ruptured aortic aneurysms [1,5]. Unfortunately, the role of EVAR in patients with acute rupture of an abdominal aneurysm is not clear, especially for patients that are hemodynamically unstable [1,8-10]. The literature is limited regarding use of EVAR in this population of patients. We present a case of the successful use of EVAR for an emergent repair in a hypotensive hemodynamically unstable patient with an acutely ruptured abdominal aortic aneurysm (AAA).

Sustainable Discoloration of Textile Chromo-Baths by Spent Mushroom Substrate from the Industrial Cultivation of Pleurotus ostreatus

Synthetic dyes are recalcitrant to degradation and toxic to different organisms. Physical-chemical treatments of textile wastewaters are not sustainable in terms of costs. Biological treatments can be more convenient and the lig-nin-degrading extracellular enzymatic battery of basidiomycetes are capable to discolor synthetic dyes. Many basidi-omycetes are edible mushrooms whose industrial production generates significant amount of spent mushroom substrate (SMS) with residual high levels of lignin-degrading extracellular enzymatic activities. We have demon-strated that the low cost organic substrate, the SMS deriving from the cultivation of the basidiomycetes Pleurotus ostreatus, is able to discolor anthraquinonic, diazo and monoazo-dyes when incubated in dying chromo-reactive and chromo-acid baths containing surfactants and anti-foams, where the concentrations of the different dyes are exceeding the one recovered in the corresponding wastewaters. Laccase was the lignin-degrading extracellular enzyme involved in the discolouring process. The exploitation of the low cost SMS in the treatment of textile wastewaters is proposed. Accordingly, a toxicological assessment, based on a cyto-toxicity test on a human amnion epithelial cell line (WISH) and the estimation of the germination index (GI%) of Lactuca sativa, Cucumis sativus and Sorghum bicolor, has been performed, showing the loss of toxicity of the chromo-baths after being discoloured by the SMS.

Factors Contributing to Contamination of Street Foods in Bamako, Mali

The World Health Organization states that foodborne diseases are a worldwide public health issue. Although street foods can provide nutritious and affordable ready-to-eat meals for city dwellers, their health risks can outweigh the benefits. A cross-sectional study was conducted in the Bamako district, focusing on street food vendors near schools, universities, extensive markets, administrative centers, and major roads. We aimed to sample fifty (50) sellers per municipality, making 300 sellers for the Bamako district. We developed a survey sheet to collect data, and six teams rotated between the municipalities each month. Before starting the collection, the teams were provided administrative papers approved by the municipal authority. The survey revealed three types of sales sites: fixed (65%), semi-fixed (30%), and mobile (4.40%). The proportion of sellers was 26.8%, 23.2%, 19.7%, and 4.2% in municipalities III, IV, and I. In municipalities I, II, III, IV, and VI, respectively, 92%, 95.70%, 93%, 87.2%, and 100% of the sellers were female. The age distribution of sellers was 65.63%, 46.81%, 40.82%, 38.30%, 36.17%, 36%, and 32% in the 25-34 and 35 - 44 age groups. Illiteracy rates were 59.20%, 61.70%, 55.30%, 75%, and 56% in municipalities I, II, III, IV, and VI, respectively. The study identified two categories of sellers: 48.3% in type 1 and 51.7% in type 2. The first category comprised 154 sellers, and the second 165 sellers. The survey found that 66.00%, 56.00%, 48.90%, 44.90%, 38.30%, and 34.40% of municipal V, VI, III, I, II, and IV sales sites were open-air. In municipality I, 63.30% of the sites were under hangars, while in municipalities II and IV, the corresponding percentages were 51.10% and 59.40%, respectively. Moreover, 46.00%, 31.90%, 31.30%, 30.60%, and 27.70% of the sites in municipalities VI, II, IV, I, and III were located next to gutters. We look at the link between climate change and vector-borne diseases in low- and middle-income countries in Africa. The large endemicity and escalating threat of diseases such as malaria and arboviral diseases, intensified by climate change, disproportionately affects vulnerable communities globally. We highlight the urgency of prioritizing research and development, advocating for robust scientific inquiry to promote adaptation strategies, and the vital role that the next generation of African research leaders will play in addressing these challenges. Despite significant challenges such as funding shortages within countries, various pan-African-oriented funding bodies such as the African Academy of Sciences, the Africa Research Excellence Fund, the Wellcome Trust, the U.S. National Institutes of Health, and the Bill and Melinda Gates Foundation as well as initiatives such as the African Research Initiative for Scientific Excellence and the Pan-African Mosquito Control Association, have empowered (or are empowering) these researchers by supporting capacity building activities, including continental and global networking, skill development, mentoring, and African-led research. This article underscores the urgency of increased national investment in research, proposing the establishment of research government agencies to drive evidence-based interventions. Collaboration between governments and scientific communities, sustained by pan-African funding bodies, is crucial. Through these efforts, African nations are likely to enhance the resilience and adaptive capacity of their systems and communities by navigating these challenges effectively, fostering scientific excellence and implementing transformative solutions against climate-sensitive vector-borne diseases.

The emergence of antiretroviral resistance mutations represents a major threat to the achievement of national and global goals for the elimination of HIV-1 infection. The global strategy in 2019 in Cte d'Ivoire is a new national policy for the management of people living with HIV with the administration of dolutegravir (DTG)-based fixed-dose combination. The aim of our study was to evaluate HIV-1 resistance to antiretrovirals (ARVs) in infected adult subjects in Cte d’Ivoire in the context of a systematic switch to a DTG-based combination. Between February 2022 and October 2023, a cross-sectional survey with random sampling was conducted in 06 services caring for people living with HIV. A total of 139 participants were included in the study. Adults with a viral load ≥ 1000 copies/mL were tested for HIV-1 ARV resistance mutations. Molecular analyses were performed using protocol of ANRS-MIE (National Agency for Research on AIDS and emerging infectious diseases). The interpretation is performed by HIVGRAD (https://***/cms/grade/). The frequencies of HIV-1 resistance to non-nucleotide reverse transcriptase inhibitors (NNRTIs), nucleotide reverse transcriptase inhibitors (NRTIs), integrase inhibitors (IINTs) and protease inhibitors (PIs) were 82%, 73%, 19% and 11% respectively. The main mutations observed in the different classes were K103N (45%), M184V (64%), E157Q (19%) and L10V/M46I/A71V/I54V (6%) respectively. This study reveals the emergence of resistance to DTG-based fixed-dose combinations, favored by high rates of resistance to NRTIs and NNRTIs. This finding underlines the need for enhanced viral load monitoring and HIV-1 genotyping tests to guide the choice of NRTIs for combination therapy. In addition, monitoring for mutations to second-generation NRTIs is essential, given the scale-up of DTG-based regimens currently underway in Cte d’Ivoire.

Biomarkers in colorectal cancer: Current clinical utility and future perspectives

Colorectal cancer(CRC) is a major cause of cancer death worldwide. CRC has poor prognosis and there is a crucial need for new diagnostic and prognostic biomarkers to avoid CRC-related deaths. CRC can be considered a sporadic disease in most cases(75%-80%), but it has been suggested that crosstalk between gene mutations(i.e., mutations of BRAF, KRAS, and p53 as well as microsatellite instability) and epigenetic alterations(i.e., DNA methylation of Cp G island promoter regions) could play a pivotal role in cancer development. A number of studies have focused on molecular testing to guide targeted and conventional treatments for patients with CRC, sometimes with contrasting results. Some of the most useful innovations in the management of CRC include the possibility to detect the absence of KRAS, BRAF, NRAS and PIK3 CA gene mutations with the subsequent choice to administer targeted adjuvant therapy with anti-epidermal growth factor receptor antibodies. Moreover, CRC patients can benefit from tests for microsatellite instability and for the detection of loss of heterozygosity of chromosome 18 q that can be helpful in guiding therapeutic decisions as regards the administration of 5-FU. The aim of this review was to summarize the most recent evidence on the possible use of genetic or epigenetic biomarkers for diagnosis, prognosis and response to therapy in CRC patients.

Robot-assisted oncologic pelvic surgery with Hugo™robot-assisted surgery system:A single-center experience

Carbon concentrations and carbon storage capacity of three old-growth forests in the Sila National Park,Southern Italy

Old-growth forests play a key-role in reducing atmospheric carbon dioxide(CO_(2)) concentrations by storing large CO_(2)amounts in biomass and soil over *** quantifies the carbon pool into different forest compartments in three Mediterranean old-growth forests of Southern Italy populated by Pinus laricio,Fagus sylvatica and Abies *** carbon pools have been assessed per compartment,i.e.,living trees,dead wood,litterfall(foliar and woody),roots and 0-20 cm topsoil,combining the whole old-growth forest mass,(i.e.,using tree allometric relationships,deadwood factor conversions,root-to-shoot ratios,litterfall and soil samplings) by the respective organic carbon *** results show the considerable capacity of these forest ecosystems in storing CO_(2)in biomass and soil,with carbon pool values ranging from 532.2to 596.5 Mg C *** trees and 0-20 cm topsoil had larger carbon pool,contributing 53.0 and 22.1%,*** most cases,organic carbon concentration was higher(more than 60%) than the average carbon conversion rate of 50%,especially in living trees,deadwood,and woody *** study contributes further scientific evidence of the capacity of old-growth forests in storing CO_(2)in their different compartments,with special evidence on tree biomass,litterfall and mineral soil,thereby highlighting the key role of old-growth forests within the challenge of climate change mitigation.

Background: Gradients in local environmental characteristics may favour the abundance of species with particular traits, while other species decline, or favour species with different traits at the same time, without an increase in average species abundances. Therefore, we asked: do variations in species and traits differ along gradients of deadwood variables? Do species abundance and trait occurrence change with species richness within or between functional groups? Thus, we analysed the beetle assemblages of five forest sites located in Italy, along the Apennines ***: From 2012 to 2018 we sampled beetles and five deadwood types in 193 plots to characterise the deadwood gradient: standing dead trees, snags, dead downed trees, coarse woody debris, and stumps. We modelled beetle species relative abundances and trophic traits occurrences against the deadwood variables using joint species distribution ***: Out of 462 species, only 77 showed significant responses to at least one deadwood type, with a weak mean response across species. Trophic groups showed mostly negative responses to deadwood variables. Species abundance increased with species richness among sites only for phytophagous and saproxylophagous. Trait occurrence did not increase with species richness among sites, except for phytophagous and ***, trait occurrence changed significantly with species richness of several trophic groups within some *** found that increases in species richness do not result in decreases in species abundance of a given trophic group, but rather null or positive relationships were found suggesting low interspecific ***: Our findings suggest that in Mediterranean mountain forests there is still room for increasing the level of naturalness, at least for what concerns deadwood management.
