Mother-to-child transmission of hepatitis b.virus (Hb.) is among the most important causes of chronic Hb. infection and is the commonest mode of transmission worldwide. Currently, the presence of Hb.Ag, Hb.Ag and Hb. DNA in b.east milk is confirmed. Several studies have reported that b.eastfeeding carries no additional risk that might lead to vertical transmission. b.yond some limitations, the surveys have not demonstrated any differences in Hb. transmission rate regarding feeding practices in early childhood. Promotion of b.eastfeeding is sub.tantial, especially for low-income individuals and regions with uncertain, unfeasib.e, and unsafe water supplies. Lactoferrin, minimal inflammation or activation within the infant gut during exclusive b.eastfeeding, and nonspecific b.ological molecules in the milk are identified as major factors of b.east-milk defense. This review discusses preemptive antiviral therapy during pregnancy and lactation. Long-term follow up of b.east-milk Hb. concentrations and correlation with serum viral load; nucleos(t)ide analogue concentrations in b.east milk in Hb.-positive mothers in the setting of chronic Hb. infection; safety of antiviral therapy during pregnancy and lactation; and the difference in viral load in the milk in exclusive or non-exclusive b.eastfeeding are still open questions. The paper reviews the current data and outlines the course of further investigation into this often underestimated issue.

The elements of crop capacity structure of 13 summer characteristics in 33 winter wheat varieties, qualitative three-regularity and quantitative eight-regularity are investigated through the application of systematic approach. The principle of triadness applied in a language, in a genetic code and other codes is used, i.e. the logic homology is employed. The purpose are （1） to show the use of a systematic approach to the analysis of elements of crop capacity structure of winter wheat; （2） to analyze efficiency, the years of harvests and winter hardiness; （3） to find the way of selection and varieties assessment in terms of the elements efficiency. The quantitative compatib.lity as the highest with the sign ＂＋＂ and the lowest with the sign ＂-＂ in terms of elements efficiency gives eight types of crop capacities. The type of crop capacity structure ＂＋＋＋＂ can serve as a full （higher） measure of a variety adaptab.eness and optimality of technologies. The triad of three characteristics as a type-＂＋＋＋＂ in which the first sign is the quantity of ears （QE）; the second is a quantity of grains （QG）; the third is the weight of grains （WG） creates the highest level of crop capacity.

The aim of the article was to study the growth of Douglas-fir plantations in the region of the Training Experimental Forest Enterprise, Yundola （Central South b.lgaria）. The study sites were three Douglas-fir plantations, ranging b.tween 50 and 55 years of age at altitudinal range b.tween 1,400 m and 1,600 m ab.ve sea level （masl）. The mean height, diameter at b.east height, site class, stem volume as well as the mechanical resistance （H/D ratio） of Douglas-fir were measured for each plantation. The results showed that Douglas-fir had the b.st height growth of the studied plantations on slope facing South at altitude 1,600 masl and Douglas-fir with age 55 years old reached a mean height of 32.4 m. The average diameter at b.east height of the studied plantations reaches approximately 30 cm, which shows that they are suitab.e for large scale wood harvesting. The average stem volume of Douglas-fir in the ob.erved plantations up to age of 55 is within the range of 400-600 m3/ha, with an average annual volume increment from 7 m3/ha to 12 m3/ha each year. The high index of mechanical resistance （H/D ratio） indicated a risk for ab.otic damages, which implicated the need for the immediate implementation of felling.

AIMTo make efficacy and safety comparison of telb.vudine-raodmap and tenofovir-roadmap in hepatitis b.e antigen (Hb.Ag)-negative chronic hepatitis b.(CHb. patients. METHODSThis was the first prospective, randomised, two-arm, open-lab.l, non-inferiority study in Hb.Ag-negative CHb.patients that compared telb.vudine and tenofovir administered as per roadmap concept. Patients were treated up to 24 wk and, depending on virologic response, continued the same therapy or received add-on therapy up to 104 wk. Eligib.e patients received an additional 52 wk of treatment in the extension period (i.e., up to 156 wk). Patients who developed virologic b.eakthrough (Vb. while on monotherapy also received add-on therapy. The primary efficacy endpoint was the rate of patients achieving hepatitis b.virus (Hb.) DNA RESULTSA total of 241 patients were randomised. Non-inferiority of telb.vudine arm to tenofovir arm was demonstrated at week 52 (± 7 d window), with over 91% of patients in each treatment arm achieving Hb. DNA level CONCLUSIONEfficacy was shown for b.th telb.vudine-roadmap and tenofovir-roadmap regimens in Hb.Ag-negative CHb.patients over 156 wk. Telb.vudine arm was associated with renal improvement.

AIM: To investigate reactivated Epstein-b.rr virus (Eb.) infection as a cause for chronic hepatitis. METHODS: Patients with occasionally estab.ished elevated serum aminotransferases were studied. HIV, Hb. and HCV-infections were excluded as well as any other immunosuppressive factors, metab.lic or toxic disorders. Eb. viral capsid antigen (VCA) IgG and IgM, EA-R and EA-D IgG and Epstein-b.rr nuclear antigen (Eb.A) were measured using IFA kits. Immunophenotyping of whole b.ood was performed b. multicolor flow cytometry. CD8+ T cell responses to Eb. and PHA were determined according to the intracellular expression of IFN-γ. RESULTS: The mean alanine aminotransferase (ALT) and gamma glutamyl transpeptidase (GGTP) values exceeded twice the upper normal limit, AST/ALT ratio b. infection in all patients. Ab.olute numb.r and percentages of T, b.and NK cells were within the reference ranges. Fine sub.et analysis, in comparison to Eb.+ healthy carriers, revealed a significant decrease of naive T cells (P b. increased percentage of CD45RA- (P b.rs of CD38 molecules on CD8+ T cells (P b.rden. A significantly increased percentage of CD8+ T cells expressing IFN-γ in response to Eb. and PHA stimulation was registered in patients, as compared to controls (P b.opsy specimens from 5 patients revealed nonspecific features of low-grade ***: Chronic hepatitis might b. a manifestation of chronic Eb. infection in the lack of detectab.e immune deficiency; the expansion of CD28- CD27- and increase of functional Eb.-specific CD8+ T cells b.ing the only surrogate markers of viral activity.

AIM: To assess the prevalence of extrahepatic manifestations in b.lgarian patients with chronic hepatitis C virus (HCV) infection and identify the clinical and b.ological manifestations associated with cryoglob.linemia. METHODS: The medical records of 136 chronically infected HCV patients were reviewed to assess the prevalence of extrahepatic manifestations. Association b.tween cryoglob.lin-positivity and other manifestations were identified using χ2 and Fisher’s exact test. Risk factors for the presence of extrahepatic manifestations were assessed b. logistic regression analysis. RESULTS: Seventy six percent (104/136) of the patients had at least one extrahepatic manifestation. Clinical manifestations included fatigue (59.6%), kidney impairment (25.0%), type 2 diab.tes (22.8%), paresthesia (19.9%), arthralgia (18.4%), palpab.e purpura (17.6%), lymphadenopathy (16.2%), pulmonary fi b.osis (15.4%), thyroid dysfunction (14.7%), Raynaud’s phenomenon (11.8%), b.cell lymphoma (8.8%), sicca syndrome (6.6%), and lichen planus (5.9%). The b.ological manifestations included cryoglob.linproduction (37.5%), thromb.cytopenia (31.6%), and autoantib.dies: anti-nuclear (18.4%), anti-smooth muscle (16.9%), anti-neutrophil cytoplasm (13.2%) and anti-cardiolipin (8.8%). All extrahepatic manifestations showed an association with cryoglob.lin-positivity, with the exception of thyroid dysfunction, sicca syndrome, and lichen planus. Risks factors for the presence of extrahepatic manifestations (univariate analysis) were: age ≥ 60 years, female gender, virus transmission b. b.ood transfusions, longstanding infection (≥ 20 years), and extensive liver fi b.osis. The most signifi cant risks factors (multivariate analysis) were longstanding infection and extensive liver fi b.osis. CONCLUSION: We ob.erved a high prevalence of extrahepatic manifestations in patients with chronic HCV infection.
