We aim to evaluate prostate health index as an additional risk-stratification tool in patients with Prostate Imaging Reporting and Data System score 3 lesions on multiparametric magnetic resonance *** with biochemical or clinical suspicion of having prostate cancer who underwent multiparametric magnetic resonance imaging in two tertiary centers(Queen Mary Hospital and Princess margaret Hospital,Hong Kong,China)between January 2017 and June 2022 were ***-magnetic resonance imaging fusion biopsies were performed after prostate health index *** who only had Prostate Imaging Reporting and Data System score 3 lesions were further stratified into four prostate health index risk groups and the cancer detection rates were *** of the 747 patients,47.3%had Prostate Imaging Reporting and Data System score 3 lesions *** detection rate of clinically significant prostate cancer in this group was 15.0%.The cancer detection rates of clinically significant prostate cancer had statistically significant differences:5.3%in prostate health indexng the patients,26.9%could have avoided a biopsy with a prostate health indexng clinically significant prostate *** health index could be used as an additional risk stratification tool for patients with Prostate Imaging Reporting and Data System score 3 *** could be avoided in patients with low prostate health index,with a small risk of missing clinically significant prostate cancer.

BACKGROUND Chylous ascites is a rare complication in colorectal surgery with limited *** To systematically review all available evidence to describe the incidence,clinical presentation,risk factors and management *** The systematic review was performed through PubMed,MEDLINE,EMBASE and Cochrane and cross-checked up to November *** data collated included:Demographics,indications(benign vs malignant),site of disease,surgical approach,extent of lymphadenectomy,day to and method of diagnosis of chylous ascites and management *** A total of 28 studies were included in the final analysis(426 cases).Patient age ranged from 31 to 89 *** except one case were performed for *** the 426 cases,195 were right-colonic,121 left-colonic,103 pelvic surgeries and 7 *** majority were diagnosed during the same inpatient stay by recognition of typical drain appearance and increased *** cases were diagnosed during outpatient visits with increased abdominal distention and subsequently underwent *** cases were managed successfully non-operatively(fasting with prolonged drainage,total parenteral nutrition,somatostatin analogues or a combination of these).Only three cases required surgical intervention after failing conservative management and subsequently resolved *** factors identified include:Right-colonic surgery/tumour location,extent of lymphadenectomy and number of lymph nodes *** Chylous ascites after colorectal surgery is a relatively rare *** the majority of cases resolved without surgical intervention,preventative measures should be undertaken such as meticulous dissection and clipping of lymphatics during lymphadenectomy to prevent morbidity.

The Soil and Water Assessment Tool(SWAT)is widely used to relate farm management practices to their impacts on surface waters at the watershed scale,yet its smallest spatial unit is not generally defined by physically meaningful *** hydrologic response unit(HRU)is the smallest spatial unit of the model,and the standard HRU definition approach lumps all similar land uses,soils,and slopes within a subbasin based upon user-defined *** standard method provides an efficient way to discretize large watersheds where simulation at the field scale may not be computationally *** relatively smaller watersheds,however,defining HRUs to specific spatial locations bounded by property lines or field borders would often be advantageous,yet this is not currently possible within the ArcSWAT *** this study,a simple approach is demonstrated that defines HRUs by field boundaries through addition of uniquely named soils to the SWAT user soil database and creation of a field boundary layer with majority land use and soil *** of nitrogen,phosphorus,and sediment losses were compared in a case study watershed where SWAT was set up using both the standard HRU definition and field boundary ***-scale results were reasonable and similar for both methods,but aggregating fields by majority soil type masked extremely high soil erosion predicted for a few *** from field-based HRU delineation may be quite different from the standard approach due to choosing a majority soil type in each farm *** approach is flexible such that any land use and soil data prepared for SWAT can be used and any shapefile boundary can divide HRUs.

The ubiquitous occurrence of perfluorinated compounds(PFCs) in environmental samples has drawn much *** human exposure studies found relatively high perfluorooctane sulfonate(PFOS) concentrations in blood samples from several cities in China when compared with other *** objectives of the present study were:(1) to measure PFC concentrations and compositions in chicken egg samples from local markets in China;and(2) to conduct a preliminary human health risk assessment of egg *** pooled egg samples from eight locations were analyzed for 11 *** results showed that close to 100% of the PFOS in the egg was distributed in egg yolk and PFOS was not detected in egg white(ng/g wet weight,w/w).Of the perfluoroalkylsulfonates,only PFOS was detected in all egg samples,while of the perfluoroalkylcarboxylates,perfluoroundecanoic acid(PFUnDA) was detected in all samples,followed by perfluorooctanoate(PFOA)(75% occurrence) and perfluorodecanoic acid(PFDA)(50% occurrence).PFOS concentrations in egg ranged from 45.0 to 86.9 ng/g w/*** results suggested that current concentrations of PFOS in domestic chicken eggs are unlikely to cause immediate harm to Chinese populations.

To the Editor： To date, epilepsy is one of the most common neurological disorders globally, affecting more than 50 million people worldwide. It is debilitating and represents a large health-care and economic burden, especially when treatment may often be expensive and not necessarily effective. It has been suggested that the burden of lifetime epilepsy has been significantly increasing in China in recent decades, The International League Against Epilepsy has defined epilepsy in 2014 as a neurological disorder with at least two unprovoked seizures occurring more than 24 h apart.

Testicular germ cell tumors(TGCTs)represent the most common malignancies in young males but they also show the highest cure rate in solid tumors with more than 95%newly diagnosed TGCTs being ultimately cured[1].Singh et al.[2]recently provided precious insight into the genomic aberrations underpinning the molecular characteristics of these tumors and a clear presentation of the mechanisms underlying sensitivity/resistance to *** importantly,the authors identified pre-target,on-target and post-target factors that can explain,the two sides of the same coin:the exquisite sensitivity of these cancers to platinum-based chemotherapy and at the same time,drug resistance when these factors are lacking.

Attention Deficit/Hyperactivity Disorder (ADHD) is characterised by developmentally inappropriate levels of inattention, impulsivity and hyperactivity. As might be expected of a disorder in which inhibitory deficits form part of the diagnostic criteria, deficits in response inhibition in ADHD have been evidenced in a number of studies. To date, the tasks used in such studies have required participants to inhibit the learned stimulus-response associations that result in unwanted behavior. However, no research has examined the inhibition of stimulus-stimulus associations, formally ‘conditioned inhibition’. The present study used video game style conditioned inhibition procedures, developed for children and adolescents with a clinical diagnosis of ADHD and suitable for typically developing matched controls. Two computer-based tasks (‘Mission to Mars’ and ‘Weapon-X’) required participants to predict the occurrence of an outcome based on the stimuli presented. We selected 12 male participants with ADHD on medication (methylphenidate), but without comorbid Tourette Syndrome, pervasive developmental disorder, learning disability or psychosis. This group showed overall normal inhibition of stimulus-stimulus associations, measured repeatedly over trials and with two task variants. There was no correlation between inhibitory learning and symptom severity ratings. However, participants with ADHD on higher dosages of methylphenidate, or longer duration of treatment with methylphenidate, showed improved ability to anticipate outcomes following the different stimulus presentations on non-inhibited versus inhibited trials. This effect was most clearly demonstrated on the Weapon-X task. Thus, methylphenidate doserelatedly improved the expression of associative learning. This action may contribute to its therapeutic effects in improving cognitive function in ADHD.

【目的】优化稳定期慢性阻塞性肺疾病(chronic obstructive pulmonary diseases,COPD)患者支气管肺泡灌洗液(bronchoalveolar lavage fluids,BALF)细菌宏基因组DNA的提取方法,以便于高效提取微量的细菌DNA进行后续的PCR反应和测序。【方法】取稳定期COPD患者的BALF 5mL,离心收集细胞。为了有效提取样品中革兰氏阳性菌的基因组,对QIAGEN的DNA提取试剂盒的操作步骤进行优化:加入裂解缓冲液ATL后首先运用研磨珠和多功能生物样品匀质器破碎菌壁,再加入蛋白酶K孵育,然后加入裂解缓冲液AL振荡混匀。无水乙醇沉淀DNA后,将全部溶液过柱,用洗液AW1和AW2各洗柱一次,最后加50μL洗脱液洗脱DNA。提取的DNA定量后,运用PCR方法检测样本中的细菌16S rDNA量,并按照测序要求构建DNA文库进一步验证。【结果】试剂盒优化法提取的BALF的DNA总量为467.5(135.0-1697.5)ng,明显高于按照传统酚-氯仿法提取的DNA总量95.0(0-612.5)ng,并且所提取的DNA可以很好的扩增细菌的16S rDNA以及构建DNA文库,改良后的扩增产物明显增多(P=0.002)。【结论】使用DNA提取试剂盒结合研磨珠和多功能生物样品匀质器破菌壁的方法能够更高效的提取BALF中微量的宏基因组DNA,为进一步的测序和菌群分析打下基础。
