Movement behaviors and their association with depressive symptoms in Brazilian adolescents:A cross-sectional study

Introduction: At the beginning of the COVID-19 pandemic, a drop in the number of patients treated for cardiac emergencies raised concern about cardiovascular mortality in that period. An increase in care delay for patients with ST-segment elevation myocardial infarction (STEMI) may have affected clinical outcomes. Objectives: To analyze delay times and clinical outcomes of patients with STEMI undergoing primary percutaneous coronary intervention (PPCI), before and during the COVID-19 pandemic. Methods: Retrospective observational study that included patients with STEMI undergoing PPCI from December 2018 to July 2021. The COVID-19 pandemic cases were divided into two groups: pandemic I—from March to August 2020;and pandemic II—from September 2020 to July 2021. Patients were compared according to the period of hospitalization. Primary outcomes were delay times in assistance and clinical outcomes (acute kidney injury [AKI], post-procedural vascular complications and in-hospital mortality). Results: 108 patients were included, 39 (36.1%) in the pre-pandemic period, 13 (12.1%) in pandemic I and 56 (51.8%) in pandemic II. Time from onset of symptoms to arrival at the service and door-to-balloon time did not differ significantly among groups. Vascular complications were more frequent during the pandemic (I and II) than in the pre-pandemic period (2.5% pre-pandemic vs 15.4% pandemic vs 12.5% pandemic II;p = 0.03). AKI incidence was similar in all three periods. There was a non-significant increase in in-hospital mortality during the COVID-19 pandemic. Conclusion: In patients with STEMI, there was an increase in vascular complications and a trend toward increased mortality during the COVID-19 pandemic. Delay times to admission and reperfusion did not differ significantly between before and during the pandemic.

巴西目前有37个转基因品种获准商业化,转基因性状包括除草剂抗性、昆虫和病毒抗性。《巴西生物安全法》对科研及市场产品研发过程中如何安全运用遗传工程技术建立了指导准则,该法设立了新的机构并让高校与政府机构参与共同制定生物安全指导方针、进行风险评估和行政监督。根据该法,巴西设立了"国家生物安全理事会"(CNBS),并重组了"国家生物安全技术委员会"(CTNBio)。CTNBio负责建立生物安全指导准则,并向所有从事转基因生物相关机构颁发"生物安全许可证"(CQB)。《生物安全法》将所有违反其准则的行为或不作为均视为违法。在注册与监督机构(OERF)确定制裁标准、规定罚金数额并提交给联邦政府后,即可对违法行为实施制裁。除此之外,巴西有关转基因生物的重要法规还有第8078号法,该条款赋予国内所有消费者知情权。依据该法律,巴西司法部建立了食品标识体系,规定人体食用或饲料用食品或食品成分若含有超过1%的转基因生物成分,必须在商品标签上注明并附转基因标志(正中间含有字母T的黄色三角形)。不可否认,在这部新的《生物安全法》颁布实施后,遗传工程技术在巴西农业中的应用呈现快速增长。毫无疑问,在各机构的协作参与下,生物安全立法框架中的法律、规定和指导准则联合作用,让投资者、科研工作者、公私营机构及其他所有巴西农业的利益相关者重拾信心。

Gastric Gist as Cause of High Digestive Hemorrhage

Introduction: Gastrointestinal stromal tumor (GIST) occurrence is uncommon and usually manifested by nonspecific signs and symptoms, which hinders its reasoning as a diagnostic hypothesis. The objective is to report a case of female GIST with curative treatment through video-laparoscopy with partial gastrectomy. Case report: A 61-year-old female patient attended the Luiz Gioseffi Jannuzzi School Hospital (HELGJ) emergency room, with a sudden onset of upper gastrointestinal bleeding with hemodynamic instability. After stabilization, she was admitted for diagnostic investigation, showing sub-epithelial lesion of antrum in upper digestive endoscopy (UDE) and solid-cystic mass in the gastric antrum to computed tomography (CT). The conduct was the video laparoscopy surgical approach with partial gastrectomy. The histopathological and immunohistochemical aspects of the surgical specimen showed a gastrointestinal stromal tumor (GIST) in a gastric antrum of low grade of malignancy, positive e-Kit and mycotic index (50 cga) Discussion: Compared to the scarce reports in literature, the case described presented typical epidemiology, but the clinical picture was not the most incident. Conclusions: Given the rarity of this pathology cases, it is essential to report these in order to elucidate the specificities and also to discuss diagnostic and therapeutic methods.

Renal Function after Major Uro-Oncologic Surgery and Dexmedetomidine Infusion

Objective: Acute kidney injury in major surgery is associated with increased postoperative mortality. This study aimed to evaluate renal function after major urologic surgery and intraoperative dexmedetomidine infusion. Methods: Thirty oncologic patients with normal renal function scheduled for prostatectomy or nephrectomy, anesthetized with combined epidural and general anesthesia, were randomized to receive either intraoperative blind infusion of dexmedetomidine (Dexmedetomidine Group, n = 15, 0.5 μg/kg load dose plus 0.7 μg/kg/h) or 0.9% saline (Control Group, n = 15) until the end of surgery. Intraoperative and cumulative 24-hour diuresis, serum creatinine (SCr), calculated creatinine clearance (ClCr) and serum cystatin C (SCys) at postoperative days 1, 2 and 3 and 2 weeks after surgery were evaluated. Results: Mean ± standard deviation values for intraoperative diuresis in Dexmedetomidine and Control Groups were 566 ± 396 mL and 298 ± 153 mL, respectively (p = 0.014). Cumulative 24-hour diuresis in Dexmedetomidine and Control Groups was 1947 ± 266 mL and 1748 ± 237 mL, respectively (p = 0.91). Mean values of SCr, ClCr and SCys were not significantly different from their baseline values in both groups and no significant differences were seen between groups at any moment for two weeks (p > 0.05). Conclusion: According to the doses used in this study, despite an intraoperative increase in diuresis, intraoperative infusion of dexmedetomidine did not influence renal performance up to two weeks after major uro-oncologic surgery, as evaluated by SCr, ClCr and SCys.

The study area is the Urucu Oil Province, Municipality of Coari, State of Amazonas, Brazil. This research represents a contribution to the hydrogeological knowledge in the northern region of Brazil, particularly in the central part of the Amazon rainforest, where researches on isotopic are still incipient. The primary goal was to determine, by stable isotopes 18O and 2H measurements, interrelationships between surface water and groundwater, in order to understand the origin and mechanisms of groundwater recharge and discharge. For this, samples of rainwater, superficial water and groundwater were collected between June 2008 and May 2009 for stable isotopic analyzes. This understanding is important in cases of eventual contaminations of the area, which could degrade the water resources. The results show that the superficial waters are typically light waters and have meteoric origin, and the groundwater recharge is by direct rainfall infiltration with primary evaporation before reaching the groundwater table in the Icá-Solimoes Aquifer System. The isotopic signatures similarities between groundwater and superficial waters indicate both waters’ contributions in the streams and, therefore, in the Urucu river.

Profile of Patients with Stroke and Disorders of the Vocal Tract

Background: The present work aims to characterize the profile of patients with stroke treat at a hospital located in the Region of the Mata of Minas Gerais, Brazil, considering the findings of the clinical vocal tract, kind of stroke, age and gender of such patients. Methodology: To obtain data, the clinical profile of 133 patients with a clinical or tomography diagnosis of stroke was analyzed, and the results were presented in percentage. For quantitative data average and analysis the tests were done with associations that held χ2 test, and for significance it was considered p Results: From the total of patients, 63 were women, accounting for 47.4% and the other 52.6% were males. Clinically, they were characterized with the highest percentage for ischemic stroke (89.4%) compared to the hemorrhagic type (10.6%). Most of them were referred for computed tomography (86.5%) and remained hospitalized for an average of 6.496 ± 7.372 days. Similar percentages were obtained in the analysis of the population in question, when considering if they had (54.1%) or not (49.6%) any damage in their speech, language skills or swallowing. There were different types of disabilities in patients with stroke. Men with an average age of 69.8 ± 13.9 presents mostly ischemic stroke, and the majority of patients with stroke had hemiplegia and abnormalities of the vocal tract, dysphasia, and aphasia. While older patients had an ischemic stroke and were presented with left hemiplegia, the younger ones suffered from hemorrhagic strokes that caused a disability characterized as right hemiplegic. Conclusion: Our results show important conclusions regarding the clinical evolution of the vocal tract of patients who suffered strokes during the period of the analysis, being useful for better comprehension of how the vocal tract from these patients evolved according to the kind of stroke, sex and age also allowing a contraposition with other future statistics periods available in literature. Analysis of PTEN Methylation Pattern by Pyrosequencing in Renal Cell Carcinoma

Objectives: We aimed to determine the frequency of PTEN (phosphatase with tensin homology deleted in chromosome 10) hypermethylation in renal cell carcinoma (RCC) and its impact on overall (OS) and disease-free survival (DFS). The association between PTEN hypermethylation and clinic and pathologic factors was also analyzed. Materials and Methods: The authors analyzed 137 patients who had undergone a radical or partial nephrectomy between 1997 and 2011. The methylation pattern was quantified individually at multiple CpG islands that are located on the exon 1 of PTEN gene by pyrosequencing. Results: Mean follow-up was 32.3 months. PTEN hypermethylation was detected with a low frequency (3.6%). There were no deaths or disease recurrence among patients with PTEN hypermethylation. Determining the association between PTEN hypermethylation and clinical and pathological factors was not possible due to the low frequency observed. Conclusion: The results show that the methylation of promoter CpG sites in PTEN is relatively infrequent in renal carcinogenesis and has no prognostic impact on survival.

Surgical Outcomes of a Small Incision Limbus-Based Revision for Failed Trabeculectomies

Background: Trabeculectomy does not always work as long as necessary to control glaucoma. This study aimed to describe the initial outcomes of a surgical technique for restoring failed trabeculectomies. Methods: We reviewed patients with failed trabeculectomy with mitomycin (MMC) who underwent limbus-based revision. After the dissection of the subconjunctival area through a smaller incision (3 mm), MMC was applied and episcleral fibrosis was excised. Each criterion was classified as complete or qualified for glaucoma medication. Results: Nineteen patients (19 eyes) were included in this study. The mean follow-up was 9.4 ± 8.6 months. At the last follow-up visit, the mean intraocular pressure was reduced from 20.9 ± 8.4 (range, 12 - 44 mmHg) to 11.6 ± 3.6 mm Hg (range, 6 - 19 mmHg) (p Conclusions: Our initial results suggest that limbus-based revision is an effective alternative to restoring failed trabeculectomies with minor postoperative complications.

Background: The prostate is the site of problems that have great clinical relevance. Beside this, the alterations in their volume can be divided into benign or malignant pathologies, but of multifactorial cause and there is no examination that alone is reliable for the diagnosis. Despite the evolution of the various methods of diagnostic imaging in the evaluation of pelvic diseases, the diagnosis of prostate cancer still requires histological confirmation obtained by transrectal ultrasound guided biopsy, a procedure that is generally safe and well tolerated by the patients. The most common histological type found in prostate biopsies is adenocarcinoma. Methodology: The objective of this study was to study the values of density of the prostate-specific antigen (DPSA) compared to histopathological results of prostate biopsy, patients attended at the Urology Outpatient Clinic of the Luiz Gioseffi Jannuzzi School Hospital from 1999 to 2007, using Exploratory, retrospective and documentary descriptive study. The sample was divided into groups of patients with adenocarcinoma and patients without adenocarcinoma, who presented only benign prostatic hyperplasia. Results: Of the 251 patients undergoing prostate biopsy, 124 were diagnosed with adenocarcinoma and 127 without adenocarcinoma. It was observed in this study that the DPSA was higher in adenocarcinoma than in the cases of patients with prostatic hyperplasia alone, and could therefore be used as an auxiliary tool in both diagnosis and follow-up of patients with adenocarcinoma. Conclusion: In this study, it can be observed that DPSA was higher in patients presenting with adenocarcinoma than in patients presenting only Benign Prostatic Hyperplasia (BPH), reinforcing its promising role to assist in the diagnosis and follow-up of patients with adenocarcinoma.
