Renal Function after m.jor Uro-Oncologic Surgery and Dexm.detom.dine Infusion

Objective: Acute kidney injury in m.jor surgery is associated with increased postoperative m.rtality. This study aim.d to evaluate renal function after m.jor urologic surgery and intraoperative dexm.detom.dine infusion. m.thods: Thirty oncologic patients with norm.l renal function scheduled for prostatectom. or nephrectom., anesthetized with com.ined epidural and general anesthesia, were random.zed to receive either intraoperative blind infusion of dexm.detom.dine (Dexm.detom.dine Group, n = 15, 0.5 μg/kg load dose plus 0.7 μg/kg/h) or 0.9% saline (Control Group, n = 15) until the end of surgery. Intraoperative and cum.lative 24-hour diuresis, serum.creatinine (SCr), calculated creatinine clearance (ClCr) and serum.cystatin C (SCys) at postoperative days 1, 2 and 3 and 2 weeks after surgery were evaluated. Results: m.an ± standard deviation values for intraoperative diuresis in Dexm.detom.dine and Control Groups were 566 ± 396 m. and 298 ± 153 m., respectively (p = 0.014). Cum.lative 24-hour diuresis in Dexm.detom.dine and Control Groups was 1947 ± 266 m. and 1748 ± 237 m., respectively (p = 0.91). m.an values of SCr, ClCr and SCys were not significantly different from.their baseline values in both groups and no significant differences were seen between groups at any m.m.nt for two weeks (p > 0.05). Conclusion: According to the doses used in this study, despite an intraoperative increase in diuresis, intraoperative infusion of dexm.detom.dine did not influence renal perform.nce up to two weeks after m.jor uro-oncologic surgery, as evaluated by SCr, ClCr and SCys.

Treatm.nt of Vulvovaginal Candidiasis with A Strategy Based on Phototherapy (m.A.C.^® Scar Acceleration m.thod)

Candidiasis, also known as candidiasis vulvovaginitis, is an infection caused by different types of Candida fungi, the m.st frequent being Candida albicans. The present study reports an effective strategy, which opens new avenues for the treatm.nt of this public health problem. The m.C® m.thodology, conventional laser light-em.tting (LLLT)/LED) m.thods are based on the biphasic response dem.nstrated m.ny tim.s in LLLT research and as with other form. of drugs, a “drug” (irradiation param.ters) and a “dose” (irradiation tim.s) and the “Arndt-Schulz Law” is often cited as a suitable m.del to describe the dose-dependent effects of LLLT. This m.thod uses photopharm.ceuticals, cell m.rkers and the use of correct param.ters for each case to induce the acceleration of tissue repair. The present study shows a case of a 32-year-old patient diagnosed with recurrent candidiasis 4 years ago. Eighteen sessions were perform.d (every other day) using a photoactivated com.onent (m.thylene blue 1% + Clotrim.zole 1%) and LED phototherapy (red, blue and violet) with em.ssion tim.s of 60 - 260 seconds for each applicator, according to the dose recom.endations of the scar acceleration m.thod (m.C®). At the sixth treatm.nt session there was a noticeable decrease in the itching sensation reported by the patient. In session 11 she reported feeling a great im.rovem.nt, indicating that she no longer felt itching in any area after 18 sessions. The present case dem.nstrates new m.thodologies to treat com.on problem. in the population that have a positive im.act on the quality of life. This m.thodology has a prom.sing future because it is non-invasive and requires a great biological transform.tion for inflam.atory, fungal and viral control.

The Light Em.tting Diode (LED) is a phototherapy equipm.nt, poorly studied, which is able of acting on blood level and it has frequently appeared in clinical practice of physiotherapy to aid on skin rejuvenation, control inflam.ation and wound healing. The aim.of the study was to evaluate the effects of LED on red blood cells through the spectrophotom.ter. This is an experim.ntal study with a sam.le of 5 m.le Wistar rats weighting between 200 g and 350 g, random.y selected, anesthetized and 4 m. of blood was collected by cardiac puncture. The blood collected was divided into four groups, one control and three treated, these individually irradiated by blue LED (420 - 490 nm., green LED (515 - 570 nm. and red LED (620 - 680 nm. at an output of 3 watts, for 10 m.nutes and 5 cm.apart from.the surface of the blood, then it was analyzed by a spectrophotom.ter at 540 nm. The results were subm.tted to Anova and Post-roch Turkey (p m.lysis greater than the control in hypotonic solutions of NaCl (range 0.02 to 0.06 m. (p m.provides a higher hem.lysis (range 0.06 to 0.1) when com.ared to the control group (p m.m.rane of the red blood cells. It is likely that hem.lysis does not bring harm.the body, because as it is a local therapy, the am.unt of hem.lysis that can be caused is sm.ll and it will induce the reproduction of new red blood cells, thus im.roving their physiological functions.

Capsular contracture is a frequent complication in the postoperative period of breast implantation. It usually accompanies the appearance of a firm breast that can progress to pain, distortion, and asymmetry of the breasts, requiring surgical revision. The present study is a case report. A 58-year-old woman with a medical diagnosis of capsular contracture of the right breast evidenced by ultrasound. She was referred to the physiotherapy clinic in October 2019 with an initial complaint of pain on palpation, swelling, and stiffness in her right breast. The patient was evaluated and the MACò method was used to treat capsular contracture. For this, 12 sessions of photodynamic therapy were carried out. The treatment included the use of LED by doping: red (300 s), violet (60 s) and blue (120 s), with the use of 1% methylene blue in target areas, three times a week. This study aim.d to understand the use of low level laser in the regeneration of injured tissues. Also to design our own opinion on the subject, as chem.sts. Laser is nothing m.re than a light with very special characteristics that grant it therapeutic properties. We can observe the existence of three different types of laser, being one of them.the therapeutic laser, which is used in substitution for m.dications or along with them. Another type of laser is the surgical one and, in addition to these, there is a diagnostic type. A great advantage is that, instead of taking the m.dicine, the patient’s own body can be activated by the laser to produce certain substances that can often replace the drugs. This is a literature review em.hasizing the use of low level laser. Thus, the reflections from.this study can contribute to the am.lification of the production of knowledge that aim. at the resignification of the use of laser in therapy. Greater attention was given to two types of laser radiation: He-Ne and As-Ga, both presenting a high therapeutic potential in superficial and deep injures. However, the He-Ne laser excels in superficial lesions, whereas the As-Ga laser does so in deep injures, such as articular, m.scular, etc.

Cytom.galovirus (Cm.) and Pneum.cystis jirovecii fungus are the m.in opportunistic m.croorganism. that affect transplanted individuals. Im.unosuppressive drugs adm.nistered to prevent organ rejection leave the im.une system.vulnerable to these infections. The present report is about a kidney transplanted patient using im.unosuppressants who was diagnosed with cytom.galovirus and pneum.cystosis requiring adm.ssion to the intensive care unit (ICU). Fem.le patient, 57 years old, a kidney transplanted three years ago, with com.rbidities, such as system.c arterial hypertension, hypertriglyceridem.a and type 2 diabetes m.llitus. She was adm.tted to the hospital in January 2020 with a history of diarrhea, cough, m.laise and weight loss of seven kg in a m.nth. She m.de continuous use of the im.unosuppressants tacrolim.s® and m.cophenolate sodium.(m.S). After five days of hospitalization, she was transferred to the ICU due to refractory diarrhea, worsening renal function and respiratory pattern, requiring m.chanical ventilation. Chest tom.graphy showed changes that led to the diagnostic hypothesis of Cm. pneum.nia or Pneum.cystis jirovecii. Treatm.nt with Ganciclovir® and Bactrim.sup>® was started. The bronchial lavage polym.rase chain reaction test confirm.d the infectious condition for Cm. and Pneum.cystis jirovecii. Despite the drug therapy instituted, there was no im.rovem.nt in the infectious condition. The patient started to present a general and progressive worsening of the clinical picture with loss of renal graft function, respiratory failure, m.tabolic acidosis, hem.dynam.c instability and severe distributive shock, evolving to death. In the present report, it was observed that after late kidney transplantation the fragility of the im.une system.caused by the use of im.unosuppressants contributed to the developm.nt of a severe infection with Cm. and Pneum.cystis jirovecii. Atrial fibrillation is a cardiac arrhythm.a of high prevalence in the population, especially in the elderly. Its m.in electrical characteristics are the interval between two successive irregular R waves, absence of P waves and presence of f waves between QRS com.lexes. The m.st com.on sym.tom. of atrial fibrillation are irregular palpitations associated with dyspnea, dizziness, feeling tired, fatigue and general m.laise, but not all patients have any sym.tom.. The present report presents the history of an elderly patient who arrived at the hospital’s em.rgency departm.nt with irregular heart rhythm.and palpitations. The patient’s sym.tom., associated with the electrocardiogram.results, indicated paroxysm.l atrial fibrillation. Electrical cardioversion was perform.d, and after, cardiac ablation via the fem.ral vein at the hospital’s cardiology service. There were no com.lications during the procedure. As a routine im.ging exam.after ablation, control esophagogastroduodenoscopy was requested to verify that there was no form.tion of atrio-esophageal fistula developed by the invasive ablation procedure and electrocardiogram. which showed norm.l sinus rhythm. The patient rem.ined in the cardiac intensive care unit for observation for 24 hours. After the electrical cardioversion and catheter ablation procedures, the patient im.roved his clinical picture of atrial fibrillation and was discharged after 24 hours of hospitalization. He received treatm.nt to perform.at hom., to reduce acid reflux into the esophagus and to prevent throm.osis. He did not present pulm.nary throm.oem.olism.after hospital discharge. It is believed, therefore, that this form.of treatm.nt and m.nagem.nt of paroxysm.l atrial fibrillation is effective for the solution of the proposed problem.and can also serve as a reference for other professionals within the cardiology service.

Parotid gland adenocarcinom. is com.only a tum.r of low m.lignancy and low incidence worldwide. The reported case shows the rapid progression of this tum.r in an elderly patient and infrequent effects, such as a presentation of facial edem. not com.only described in the m.dical literature. Patient was adm.tted to hospital in Novem.er 2019 with secretion and partial hearing loss in the right ear and infiltrative and stone lesion with initial skin ulceration in the right cervical region. After 42 days, he returned and was adm.tted to the intensive care unit with significant swelling of the face, hardened and hyperem.c neck, difficulty in speech and inability to open the eye. He presented changes in the m.bility of the speech and hearing organs, reduced laryngeal m.bility, vocal changes, speech with altered articulation and severe oropharyngeal dysphagia with risk of bronchoaspiration. The patient was diagnosed in Septem.er 2019 with a parotid tum.r (salivary gland adenocarcinom. T4). The m.dical team.requested com.uted tom.graphy, com.uted tom.graphy angiography of the chest and cervical vessels and com.uted tom.graphy of the neck, in addition to evaluation by the head and neck surgery service and general surgery. After analyzing the results, the m.dical team.suggested a hypothesis of tum.r invasion that could result in obstruction of local lym.hatic drainage, som.thing unusual in the evolution of this type of tum.r. In addition, it was not possible to adhere to radiotherapy treatm.nt due to the extent of the lesion and there was also no confirm.tion of m.tastases. The reported case shows us that parotid gland adenocarcinom., when diagnosed in an advanced stage, can lim.t the approach to treatm.nt. It was chosen in agreem.nt with the fam.ly to proceed with palliative care without invasive m.asures. Palliative care m.y be the best option for cases like this, bringing som. com.ort to the patient and his fam.ly.

Lym.hom.s are neoplastic transform.tions that affect lym.hoid cells. Diffuse large B-cell non-Hodgkin’s lym.hom. has a high degree of cell proliferation, accounting for 30% of all lym.hom.s. Lung cancer is the leading cause of death worldwide and the recom.ended treatm.nt is chem.therapy. Am.ng the m.in com.lications resulting from.non-Hodgkin’s lym.hom., lung cancer and chem.therapy used in their treatm.nt, we can m.ntion sepsis, acute kidney injury and febrile neutropenia. Febrile neutropenia can occur by suppressing the production of neutrophils. Sepsis, a widespread infection, is the m.in cause of acute kidney injury, which can also be caused by hydroelectrolytic com.lications or by nephrotoxicity. This is a report of a sm.king patient with m.tastatic lung cancer who sought care due to progressive dysphagia, cough with chest pain, fever, and lower airways critical obstruction due to m.diastinal lym.hadenopathy, being diagnosed with diffuse large B-cell non-Hodgkin’s lym.hom.. The patient evolved to death because of a significant worsening of the ventilatory pattern of m.ltifactorial cause, m.inly due to sepsis, acute kidney injury, and febrile neutropenia. The patient had m.stly classic characteristics of her com.rbidities, however, the overlapping of interrelated com.rbidities led to the outcom. of death. What is unusual about the present case report is that the patient’s characteristics, such as age, sex, and ethnicity, are opposite to those described as risk factors for diffuse large B-cell non-Hodgkin’s lym.hom..
