Mitral and tricuspid valve surgery is increasingly being performed through a right mini-thoracotomy approach. Although valve repair is preferred over replacement whenever possible, valve replacement may be required in certain patients. In situations where the mitral or tricuspid anatomy are unfavorable and exposure is difficult, seating a prosthetic valve in the annulus through a right mini-thoracotomy approach may be difficult, compared to conventional sternotomy approach. This is complicated by limited tactile feedback in the minimally invasive approach. Herein, we describe several simple maneuvers that facilitate proper prosthetic valve seating and visualization of the posterior annulus during minimally invasive valve operations via right mini-thoracotomy.

CSES(China Seismo-Electromagnetic Satellite) is a mission developed by CNSA(Chinese National Space Administration) and ASI(Italian Space Agency), to investigate the near-Earth electromagnetic, plasma and particle environment, for studying the seismo-associated disturbances in the ionosphere-magnetosphere transition zone. The anthropogenic and electromagnetic noise,as well as the natural non-seismic electromagnetic emissions is mainly due to tropospheric activity. In particular, the mission aims to confirming the existence of possible temporal correlations between the occurrence of earthquakes for medium and strong magnitude and the observation in space of electromagnetic perturbations, plasma variations and precipitation of bursts with highenergy charged particles from the inner Van Allen belt. In this framework, the high energy particle detector(HEPD) of the CSES mission has been developed by the Italian LIMADOU Collaboration. HEPD is an advanced detector based on a tower of scintillators and a silicon tracker that provides good energy and angular resolution and a wide angular acceptance, for electrons of 3–100 Me V, protons of 30–200 Me V and light nuclei up to the oxygen. CSES satellite has been launched on February 2^(nd), 2018 from the Jiuquan Satellite Launch Center(China).

BACKGROUND Mean age of patients with valves diseases is significantly increasing,and,in the near future,cardiac surgeons will have to deal with a considerable number of patients aged more than 80 *** remarkable results gained by the minimally invasive approach have encouraged its application in more complex and fragile patients,such as older *** study aimed to identify the rate of early mortality and major complications,and independent predictors for mid-term mortality in octogenarians undergoing minimally invasive valve *** Octogenarian patients undergoing right mini-thoracotomy mitral and/or tricuspid valve surgery between 2006 and2020 were *** endpoint was to identify independent predictors for mid-term mortality,and secondary endpoints were operative morality,stroke,independent predictors for early composite outcome,and quality of life at *** Analysis was performed on 130 *** occurred in one patient(0.8%),while operative mortality was 6%(eight patients).One-year and five-year survival were 86% and 64%,*** regression identified age and creatinine level as independent predictors of mid-term mortality,survival analysis showed that age ≥ 84 years and creatinine level ≥ 1.22 mg/dL were the cut-off points for worst *** gender and hypertension were found to be independent predictors of early composite *** Results of the present study show that age alone should not be considered a contraindication for minimally invasive valve *** patients who are most likely to have survival and functional benefits after surgery is decisive to achieve optimal health outcomes and prevent futile procedures.

AIM: To reviewing genetic and epigenetic make-up of metastatic colorectal cancers (mCRCs) addicted to epidermal growth factor receptor (EGFR) ***: The present study summarizes the potential value of prognostic and predictive biomarkers in selecting mCRC patients treated with anti-EGFR therapy. A meta-analysis was performed using a systematic search of PubMed, Medline and Web of Science to identify eligible papers until March 21st, 2016 using these following terms: ‘‘colorectal cancer’’, “predictive biomarkers’’, “anti-EGFR therapy”, “KRAS”, “NRAS’’, “PIK3CA”, “TP53”, “PTEN”, ‘‘EGFR”, “MET”, “HER2”, “epiregulin”, “amphiregulin”, “prognostic biomarkers”, “BRAF”, “miRNA” and “antibody-dependent cell-mediated cytotoxicity (ADCC) activity”. Two investigators independently evaluated and extracted data from each identified studies based on selected criteria of inclusion and ***: The introduction of agents targeting EGFR such as cetuximab and panitumumab increased overall survival of mCRCs. Nevertheless, it has firstly became evident that response rates to cetuximab regimens in unselected patient populations were typically lower than 30%. Clinical data confirmed the predictive value of RAS mutations for resistance to cetuximab and panitumumab leading to the license of these monoclonal antibodies exclusively for the management of patients with RAS-wild type colorectal cancers. Nested stromal-epithelial tumor(NSET) is a nonhepatocytic and non-biliary tumor of the liver consisting of nests of epithelial and spindled cells with associated myofibroblastic stroma and variable intra-lesional calcification and ossification, which represents a very rare and challenging disease. Most of the reported cases have been treated with surgery, obtaining a long survival outcome. Here, we report the case of a 31-year-old Caucasian man who underwent surgery at our institution for a large, lobulated, multinodular mass of the right hemi-liver. The histological exam confirmed the diagnosis of NSET. After 6 mo from surgery, a liver recurrence was described and a chemoembolization was performed. After a further disease progression, based on the correlation between the histological features of the disease and those of the hepatoblastoma, a similar chemotherapy regimen(with cisplatin and ifosfamide/mesna chemotherapy, omitting doxorubicin due to liver impairment) was administered. However, infection of the biliary catheter required a dose modification of the treatment. No benefit was noted and a progression of disease was radiologically assessed after only four cycles. The worsening of the clinical status prevented further treatments, and the patient died a few months later. This case report documents how the NSET might have an aggressive and non-preventable behavior. No chemotherapy schedules with a proved efficacy are available, and new data are needed to shed light on this rare neoplasm.
