Correction of low corneal astigmatism in cataract surgery

· AIM: To evaluate and compare aspheric toric intraocular lens(IOL) implantation and aspheric monofocal IOL implantation with limbal relaxing incisions(LRI) to manage low corneal astigmatism(1.0-2.0 D) in cataract surgery.· METHODS: A prospective randomized comparative clinical study was performed. There were randomly recruited 102 eyes(102 patients) with cataracts associated with corneal astigmatism and divided into two groups. The first group received toric IOL implantation and the second one monofocal IOL implantation with peripheral corneal relaxing incisions. Outcomes considered were: visual acuity, postoperative residual astigmatism, endothelial cell count, the need for spectacles, and patient satisfaction. To determine the postoperative toric axis, all patients who underwent the toric IOL implantation were further evaluated using an OPD Scan III(Nidek Co, Japan). Follow-up lasted 6mo.· RESULTS: The mean uncorrected distance visual acuity(UCVA) and the best corrected visual acuity(BCVA) demonstrated statistically significant improvement after surgery in both groups. At the end of the follow-up the UCVA was statistically better in the patients with toric IOL implants compared to those patients who underwent implantation of monofocal IOL plus LRI. The mean residual refractive astigmatism was of 0.4 D for the toric IOL group and 1.1 D for the LRI group(P <0.01). No difference was observed in the postoperative endothelial cell count between the two groups.· CONCLUSION: The two surgical procedures demonstrated a significant decrease in refractive astigmatism. Toric IOL implantation was more effective and predictable compared to the limbal relaxing incision.

Spontaneous or secondary to intravitreal injections of anti-angiogenic agents retinal pigment epithelial tears in age-related macular degeneration

·AIM:Toevaluatethevisualfunctionevolutionofretinal pigment epithelial(RPE) tears in patients with age-related macular degeneration(AMD) according to type of occurrence [spontaneous or secondary to anti-vascular endothelial growth factor(anti-VEGF) injection] and the topographic location of the tear after a two-year followup period.·METHODS: A total of 15 eyes of 14 patients with RPE tears in exudative AMD were analyzed retrospectively at the University Eye Clinic of Trieste. Inclusion criteria were: patient age of 50 or older with AMD and RPE tears both spontaneous occurring or post anti-VEGF treatment. Screening included: careful medical history,complete ophthalmological examination, fluorescein angiography(FA), indocyanine green angiography(ICG),autofluorescence and infrared imaging and optical coherence tomography(OCT). Patients were evaluated every month for visual acuity(VA), fundus examination and OCT. Other data reported were: presence of PED,number of injections before the tear, location of the lesion.·RESULTS:Meanfollow-up was24wk(SD±4wk). Atotal of 15 eyes were studied for RPE tear. In 6 cases(40%),the RPE tears occurred within two years of anti-VEGF injections the others occurred spontaneously. In 13cases(86.6%), the RPE tear was associated with pigment epithelial detachment(PED). In 7 cases(46.6%), the RPE tear occurred in the central area of the retina and involved the fovea. Two lesions were found in the parafoveal region, six in the extra-macular area. In all cases visual acuity decreased at the end of the follow-up period(P <0.01) independently of the type or the topographical location of the lesion.·CONCLUSION: RPE tear occurs in exudative AMD as a spontaneous complication or in relation to anti-VEGF injections. Visual acuity decreased significantly and gradually in the follow-up period in all cases. No correlation was found between visual loss and the type of onset or the topographic location of the tears.

Thrombotic microangiopathy involving the gallbladder as an unusual manifestation of systemic lupus erythematosus and antiphospholipid syndrome: Case report and review of the literature

Gallbladder disease is no more common in patients with systemic lupus erythematosus (SLE) than in the general population. We describe a 17-year-old patient with SLE, who developed nephritis that was well controlled with medications. Initial treatment consisted of azathioprine, aspirin and prednisone with stable control of her symptoms. Two years later she developed a right quadrant abdominal pain, and an abdominal ultrasound revealed microlithiasic cholecystitis. Open cholecystectomy was performed and the histopathological findings revealed vasculitis with thrombotic microangiopathy in the gallbladder. This case presentation illustrates that calculous or acalculous cholecystitis should be considered as a manifestation of active SLE and APS.

Neuronal activity controls the development of interneurons in the somatosensory cortex

BACKGROUND： Neuronal activity in cortical areas regulates neurodevelopment by interacting with defined genetic programs to shape the mature central nervous system. Electrical activity is conveyed to sensory cortical areas via intracortical and thalamocortical neurons, and includes oscillatory patterns that have been measured across cortical regions. OBJECTIVE： In this work, we review the most recent findings about how electrical activity shapes the developmental assembly of functional circuitry in the somatosensory cortex, with an emphasis on intemeuron maturation and integration. We include studies on the effect of various neurotransmitters and on the influence of thalamocortical afferent activity on circuit development. We additionally reviewed studies describing network activity patterns. METHODS： We conducted an extensive literature search using both the PubMed and Google Scholar search engines. The following keywords were used in various iterations：＂intemeuron＂, ＂somatosensory＂, ＂development＂, ＂activity＂, ＂network patterns＂, ＂thalamocortical＂, ＂NMDA receptor＂, ＂plasticity＂. We additionally selected papers known to us from past reading, and those recommended to us by reviewers and members of our lab. RESULTS： We reviewed a total of 132 articles that focused on the role of activity in interneuronal migration, maturation, and circuit development, as well as the source of electrical inputs and pattems of cortical activity in the somatosensory cortex. 79 of these papers included in this timely review were written between 2007 and 2016. CONCLUSIONS： Neuronal activity shapes the developmental assembly of functional circuitry in the somatosensory cortical interneurons. This activity impacts nearly every aspect of development and acquisition of mature neuronal characteristics, and may contribute to changing phenotypes, altered transmitter expression, and plasticity in the adult. BACKGROUND Alpha-defensin has been widely studied for the diagnosis of periprosthetic joint infection(PJI).However,there is a lack of detailed information regarding the proper laboratory technique of the enzyme-linked immunosorbent assay(ELISA)method,such as sample *** To assess the influence of dilution in the synovial fluid during ELISA for the diagnosis of knee PJI;and determine which dilution presents a better *** Forty samples of synovial fluid from arthroplasty knees were included,17 in the infected group and 23 in the aseptic group,according to Musculoskeletal Infection Society ***,five synovial fluid samples from each group were assessed for quantitative analysis of alpha-defensin using *** dilution ratios(1:10,1:100,1:500,1:1000 and 1:5000)were tested based on the predetermined cutoff value of 5.2 mg/*** dilutions that performed better were used to compare the results of all *** For infected cases,a gradual increase in the dilution of synovial fluid samples led to an equivalent increase in alpha-defensin *** same was not observed in the aseptic *** 1:1000 and 1:5000 dilutions presented satisfactory results to differentiate infected and aseptic *** analyses were performed using 1:1000 and 1:5000 for all 40 *** 1:1000 dilution resulted in a sensitivity of 88.2%(95%CI,66%-98%)and specificity of 95.7%(95%CI,79%-99%),whereas the 1:5000 dilution presented a sensitivity of 94.1%(95%CI,73%-99%)and a specificity of 100%(95%CI,86%-100%).CONCLUSION The synovial fluid dilution had an important influence on the alpha-defensin ELISA *** of 1:5000 showed the best performance for the diagnosis of knee *** results of this study set the basis for a more reliable and reproducible alpha-defensin ELISA during the investigation of PJI,contributing to the expansion of this technique in different treatment centers worldwide.

Performance of alpha-defensin lateral flow test after synovial fluid centrifugation for diagnosis of periprosthetic knee infection

BACKGROUND The quantitative alpha-defensin enzyme-linked immunosorbent assay(ELISA)demands a prior synovial fluid centrifugation,whereas this processing is not routinely required prior to the alpha-defensin lateral flow *** To evaluate whether a prior synovial fluid centrifugation could lead the lateral flow performance to achieve comparable results to ELISA during periprosthetic joint infection(PJI)*** Fifty-three cases were included in this study:22 classified as PJI and 31 classified as aseptic cases,according to Musculoskeletal Infection Society 2013 *** fluid samples were submitted to centrifugation,and the supernatant was evaluated by ELISA and lateral flow *** sensitivity(SE),specificity(SP)and accuracy of each method were calculated as well as the agreement between those two *** In all of the 31 samples from aseptic patients,alpha-defensin ELISA and lateral flow tests showed negative results for *** the 22 infected patients,the lateral flow test was positive in 19 cases(86.4%)and the ELISA was positive in 21(95.5%).Sensibility,SP and accuracy were,respectively,86.4%(95%CI:65.1%-97.1%),100%(95%CI:88.8%-100%)and 93.2%(95%CI:82.8%-98.3%)for the lateral flow test and 95.5%(95%CI:77.2%-99.9%),100%(95%CI:88.8%-100%)and 98.1%(95%CI:89.9%-100%)for *** agreement of 96.2%between those methods were *** statistical difference was found between them(P=0.48).CONCLUSION Alpha-defensin lateral flow test showed high SE,SP and accuracy after a prior synovial fluid centrifugation,achieving comparable results to *** the lower complexity of the lateral flow and its equivalent performance obtained in this condition,a prior centrifugation might be added as a valuable step to enhance the PJI diagnosis.

Effects of Aerobic Conditioning on Individuals Post-Stroke: What Is the Evidence?

Background: Stroke has been considered the major cause of chronic disability in the world and ranks among the leading causes of death. Despite the fact that new vascular events often happen and risk factors are the same that Cardiovascular Diseases, secondary prevention through non-pharmacological measures, are not part of the routine physical rehabilitation after stroke. Objective: The aim is to answer the following clinical question: Should aerobic fitness be part of the physical rehabilitation of individuals post-stroke? Methodological Design: The methodology used in the study was an integrative literature review. Inclusion Criteria: Articles in English are published in secondary databases: systematic reviews with or without meta-analysis, which address the question PICO: patient (post-stroke), intervention (aerobic exercise), comparison (with or without aerobic conditioning) the outcome (volume of oxygen consumed peak (VO2peak), quality of life (QoL), morbidity and mortality). Outcomes: VO2peak, QoL, complications and mortality. Analysis of Studies: The methodological quality of included studies is assessed using the tools: PRISMA and R-AMSTAR. Results: There was “good” evidence for the inclusion of aerobic exercise on physical rehabilitation of post-stroke individuals to improve their physical capacity (VO2peak), facilitating the completion of activities of daily living and gait. There was weak evidence on the association of aerobic fitness with positive results on QoL in stroke victims and insufficient results to evaluate the effect of aerobic fitness on the risk of stroke recurrence and mortality. Conclusion: The aerobic conditioning should be included in the rehabilitation of stroke victims.

In vitro Evaluation with 5SBF of Alloy ASTM F75 Thermally Treated with Wollastonite

Samples of a cobalt-based alloy that underwent a surface treatment were evaluated. The samples, which were obtained by casting alloy ASTM F 75, were ground and polished on one side until a mirror finish was obtained. The samples were encapsulated in wollastonite （W） using uniaxial pressure, treated at 1 220 ~C for 1 h and subsequently tempered in water. The characterisation of the sample indicated that part of the ceramic encapsulating material was mechanically incorporated on the metallic surface by growth of the oxide layer of the alloy. After thermal treatment, a series of specimens were submerged in a solution with 5-fold simulated body fluid （5SBF） for 3, 5 and 21 days. Characterisation by scanning electron microscopy （SEM）, energy dispersive spectroscopy （EDS） and X-ray diffraction （XRD） indicated nucleation and growth of a homogenous layer of apatite, beginning on the third day when the sample was submerged.

Sweet Potato and Cassava Can Modify Cholesterol Profile in Humans with Moderately Raised Serum Cholesterol Levels

Sweet potato (kamote) and cassava are good sources of dietary fiber and resistant starch and are staple foods in the Philippines. The objective of the study is to determine changes in glucose and lipid profile after consumption of sweet potato and cassava in humans with moderately raised serum glucose and lipid profile. Sweet potato (Ipomoea batatas, Super Taiwan variety), and cassava (Manihot esculenta) were used as test foods while white bread was used as the control food. Fifty-nine apparently healthy human adults were fed with the test foods for 90 days and grouped into three: Control, Sweet potato and Cassava. After an overnight fast, 10 mL blood samples were taken from the study participants for total cholesterol, HDL-C, LDL-C, triglycerides, glucose and hemoglobin A1C measurements. Height/weight and percent fat mass were also determined. The groups given sweet potato and cassava significantly increased HDL-C (P 0.05). There was a significant decrease in LDL-C in the cassava group (P 0.05). The group given slice bread significantly increased serum triglycerides (P 0.05). No significant results were observed with regards to BMI, percent fat, glucose, HbA1C, and total cholesterol. In conclusion, sweet potato and cassava increased HDL-C and decreased LDL-C in humans with moderately raised serum glucose and cholesterol levels. Sustainable intake of sweet potato and cassava may be promising in the prevention for risk of cardiovascular diseases as well as obesity and type 2 diabetes mellitus.

What is the prognosis of emphysematous pyelonephritis associated with extended-spectrum beta-lactamases producing microorganisms?

Objective:To describe the microbiological characteristics in emphysematous pyelonephritis(EPN),demonstrate the frequency of extended-spectrum beta-lactamase(ESBL)microorganisms,and determine if these microorganisms are associated with the prognosis of patients with ***:We conducted a retrospective study in patients with a diagnosis of EPN in a tertiary care hospital of the northeast region of Mexico during the period from January 2011 to January *** variables were analyzed to determine association with the presence of ESBL-producing *** significance was set with p<***:A total of 63 patients were included;55(87.3%)of them were females,with a median age of 55(interquartile range:45-65)*** management was indicated in 38.1%;42.9%were treated with ureteral stent;12.7%with open or percutaneous drainage;15.8%with early nephrectomy;and 9.5%with delayed *** mortality was 13(20.6%)cases;23(36.5%)cases required admission to the intensive care *** most frequent microorganism isolated was Escherichia coli(n=34,53.9%).ESBL microorganisms were found in 31.7%of the *** significant association of ESBL was found with admission to the intensive care unit,or with increased ***:To our knowledge,this is the first study that evaluates ESBL microorganisms as a prognostic factor in *** factors associated with a poor prognosis in patients with EPN have been *** microbiological factors,specifically ESBL-producing bacteria,do not seem to influence in the prognosis of these patients.
