Use of Initial Modified RECIST Tumor Response Evaluation Criteria for Predicting Survival in Patients with Hepatocellular Carcinoma Undergoing Transarterial Chemoembolization with Drug-Eluting Beads

Introduction: Transarterial chemoembolization (TACE) reduces tumor growth and increases survival in patients with hepatocellular carcinoma (HCC). Drug-eluting beads (deB) deliver slow-release chemotherapy and reduce systemic toxicity during TACE. This study correlated initial tumor response according to modified RECIST (mRECIST) criteria and 1-year survival in patients with HCC treated with TACE-deB, and identified predictors of tumor response. Methods: Fifty-two patients with HCC received TACE-deB loaded with doxorubicin 75 mg during a 6-month period. Tumor response was evaluated 1 month after the procedure according to mRECIST criteria. Results: Most patients were cirrhotic and etiology of liver disease was hepatitis C in 26/52 (50%). Similar numbers of patients had Barcelona Clinic Liver Cancer (BCLC) A and BCLC B disease. Most patients had one nodule (66%). Complete response (CR) was achieved in 12/52 (23%), partial response in 19/52 (37%), stable disease in 4/52 (8%) and progressive disease in 17/52 (32%). Largest HCC ≤58 mm and BCLC stage A were associated with CR. The 1-year survival was 74%, with survival rates of 95% and 56% in the BCLC A and B groups, respectively. Variables reflecting tumor extension were associated with better survival. CR according to mRECIST criteria was a predictor of better 1-year survival (100% vs. 64%, P deB. Further studies are needed to evaluate other predictors of survival and to determine if tumor response predicts long-term survival.

Objective: Obesity has become a worldwide epidemic that is directly related to chronic tissue inflammation and impaired fibrinolysis, both of which lead to an increased risk of thrombosis. The management of complications after bariatric surgery is still a challenge. The study aims to report the incidence of portomesenteric venous thrombosis (PMVT) in patients undergoing bariatric surgery. Introduction: Bariatric surgery is the procedure of choice for the treatment of obesity type III. despite the safe technique, the surgeon must be aware of the risks linked to laparoscopy such as the PMVT. Computed tomography (CT) with intravenous contrast was the imaging method of choice for diagnosis. Materials and Methods: We performed a retrospective analysis of 4790 patients from August 1999 to June 2020 was observed the incidence of PMVT in this group. Of all patients, 72.8% were female and 27.2% male. The mean age was 46.3 years (34 - 72) and the mean BMI before bariatric surgery was 41.4 kg/m2. Results: The study showed an incidence of PMVT of 0.12% (N = 6), portal vein thrombosis was of 0.10% (n = 5) and 0.03% of MVT (n = 1). Of these, 72.5% (n = 3473 patients) underwent laparoscopic gastric bypass, 23.6% (n = 1131 patients) were submitted to the laparoscopic sleeve gastrectomy and 3.8% (n = 186 patients) to other procedures. The mean postoperative hospital stay for bariatric surgery was 3.3 days. The average time of hospital stay of patients complicated with PMVT range from 5 to 14 days. No patient died of complications related to PMVT. Conclusion: Portomesenteric vein thrombosis is a rare presentation after laparoscopic bariatric surgery. However, it is a serious complication that has a high index of suspicion for making its diagnosis. Conservative treatment using anticoagulants and thrombolytics is effective, it should always be considered the main treatment option. Cotton,a hydrofilic textile fiber,has unstable characteristics and,for this reason,it varies its properties according to the environment *** and temperature are the two most important factors that lead to a cotton spinning sector and influence its *** two properties can change the entire spinning *** this,moisture and temperature must be kept under control when used during the spinning process,once the environment is hot and dry,the cotton yarns absorb moisture and lose the minimal *** to this information,this paper was developed testing four types of cotton yarns,one kind of cotton from Brazil and the others from *** yarns were exposed to different temperatures and moisture in five different tests and in each test,six samples were examined through physical and mechanical tests:resistance,strength,tenacity,yarn’s hairness,yarn’s evenness and yarn’s *** the analyses were accomplished at Laboratório de Mecânica dos Fluidos and at COATS Corrente S.A.,where,it was possible to use the equipments which were fundamental to develop this paper,such as the STATIMAT ME that measures strength,tenacity,Zweigler G566 that measure hairiness in the yarn,a skein machine and a twisting *** analysis revealed alterations in the yarn’s characteristics in a direct way,for example,as moisture and temperature were increased,the yarn’s strength,tenacity and hairness were increased as *** the results of all analysis,it is possible to say that with a relatively low temperature and a high humidity,cotton yarns have the best performance.

AIM:To analyze the role of rs12979860 and rs8099917polymorphisms in hepatitis C virus(HCV)genotype 1infection of ***:A total of 145 adult patients diagnosed with genotype 1 chronic hepatitis C(CHC)who had completed a 48-wk regimen of pegylated-interferonα-2a or-2b plus ribavirin combination therapy were recruited from six large urban healthcare centers and199 healthy blood donors(controls)from a single site between January 2010 and January *** on the patients’response to treatment was *** chain reaction-restriction fragment length polymorphism genotyping of the interleukin(IL)28B gene fragment encompassing the single nucleotide polymorphisms(SNPs)rs12979860(C/T)and rs8099917(T/G)was carried out for 79 of the CHC patients and 199 of the ***-directional amplicon sequencing of the two SNPs was carried out for the remaining 66 CHC ***:SNP rs12979860 genotyping was successful in 99.5%of the controls and 97.2%of the CHC patients,whereas the SNP rs8099917 genotyping was successful in 95.5%of the controls and 100%of the CHC *** genotype and allele distributions for both rs12979860 and rs8099917 were significantly different between the control and CHC patient groups,with significantly higher genotype frequencies of CC and TT in the controls(P=0.037 and 0.046,respectively)and of TT and GG in the CHC patients(P=0.0009and 0.0001,respectively).Analysis of the CHC patients who achieved sustained virological response(SVR)to treatment(n=55)indicated that the rs12979860 C allele and CC genotype were predictors of SVR(P=0.02).No significant correlation was found between rs8099917 genotypes and treatment response,but carriers of the T allele showed significantly higher rates of SVR(P=0.02).Linkage disequilibrium analysis of the group that achieved SVR showed a significant association between rs12979860 and rs8099917(P=0.07).CONCLUSION:The higher allele frequency of rs12979860 C and rs8099917 T observed in non-HCVinfec

Objectives: The purpose of this study was to evaluate and compare the use of propinox hydrochloride and scopolamine hydrochloride in patients presenting abdominal colic (abdominal pain), in terms of treatment efficacy and tolerability. Material & Methods: This was an analytical, retrospective, comparative study based on hospital records of outpatients treated at Serviço de Clínica Médica do Hospital das Clínicas Costantino Otaviano (HCTCO) and at Santa Casa de Misericórdia do Rio de Janeiro, from 1988-1998. Subjects were divided into two groups: patients from Group 1 were treated with propinox hydrochloride, while patients from Group 2 were treated with scopolamine hydrochloride. Statistical analysis was performed using GraphPad Prism version 5.0. For comparison of categorical variables, we used the chi-squared or Fisher’s test, while continuous variables were analyzed using ANOVA or the Student’s T test. Results: A total of 1042 subjects were included, of which 525 were allocated to Group 1 and 517 to Group 2. Mean treatment duration was 9.166 days (±4.208) in Group 1 and 8.795 days (±5.052) in Group 2, with no statistically significant difference in treatment duration between the two groups (p = 0.198). All subjects in Group 1 were treated with propinox 10 mg (2 coated tablets, three times per day) while all subjects in Group 2 were treated with scopolamine hydrochloride 10 mg (2 coated tablets, three times per day). There were no statistically significant between-group differences in weight, BMI, heart rate, and respiratory rate at pre- and post-treatment;with the exception of higher post-treatment systolic blood pressure in Group 1, blood pressure measures also remained homogenous. Adverse events were reported among both treatment groups with no significant between-group difference in incidence (p = 0566). This study consists in the investigation of the application of ornamental rock tailings applied in remineralization in the Brazilian soil through the rocking *** to the Brazilian dependence on the importation of agricultural inputs,the use of the method aims beyond improving the physical and chemical properties of the soil,supplying the demand for such data,and in return to give an adequate end to pollutant *** tailings have chemical characteristics that are of great importance for soil *** methodology employed was divided into three stages,the first one was the physicochemical characterization of the rock dust,followed by the characterization of the soil experimented and the cultivation of the plant species Phaseolus *** results obtained so far showed favorable results since the characteristics obtained through the physicochemical analyses were compatible with those suitable for the soil,after the application of the powder,which had its high pH and which proved that the tailings corrected the soil acidity of the sample in question,placing it at an ideal level for the cultivation of new species.

depending on the treatment and weight of the breasts or abdomen, they may exceed volumes considered giant and morbidly obese. In these cases, and when the patient’s BMI is high above 40 kg/m2, the weight of the breasts or abdomen produces what we consider suffocation when the patient is placed in horizontal position on surgical tables, decreasing his respiratory capacity and increasing the difficulty in treating respiratory or embolic risks. An 8-kg breast on the patient’s chest prevents normal breathing. An abdomen with a volume of 30 kg causes difficulties in all senses, making the physiological expansion of the lungs impossible and even preventing surgical assistance to patients. These patients are almost always customers who sleep in the sitting position to breathe better. The gigantic extirpation of the surgical parts facilitates a better respiratory expansion reducing by a large percentage the risk of death, what we call body relief. This relief does not free the patient from bariatric surgery for a possible weight loss, which is vital for the proper functioning of the organs and decreasing arterial hypertension and diabetes.

Since 1978, Brazil has invested into bio-fuels alternatives, especially ethanol from sugar-cane processing and that has increased the bagasse production, which requires proper destination and a potential use of such biomass is the gasification process. In the present study, a conceptual design of a gasification system to convert sugar cane bagasse into syngas is presented, which considers air as gasification agent in a flexible configuration of bubbling fluidized bed reactor operating from atmospheric pressure up to 2 MPa, providing a net power output （referred at gas cold conditions） of 3 MWt and 66 MWt, respectively. In this last case, the gas may be used not just as a fuel for gas turbines and internal combustion engines for power generation but also to feed Fischer-Tropsch processes. The optimized conceptual design of the gasifier is described here and was achieved using the CSFMB （Comprehensive Simulator of Fluidized and Moving Bed Equipment）. Simulations predicted the production of gas with high heating value as well stable operations at both conditions. The conceptual design would be followed by the detailed one and construction. Tests would be carried in the near future and would allow direct comparison between the calculated and experimental results.
