Pro-atherosclerotic markers and cardiovascular risk factors one year after liver transplantation

AIM:To investigate pro-atherosclerotic markers(endothelial dysfunction and inflammation)in patients one year after liver ***:Forty-four consecutive liver transplant(LT)outpatients who were admitted between August2009 and July 2010,were followed-up by for 1 year,exhibited no evidences of infection or rejection,all of them underwent tacrolimus-based immunosuppressive regimens were consecutively *** cytokines(TNFα,IFNγ,IL-8,and IL-10),endothelial biomarkers(sVCAM-1,sICAM-1,MPO,adiponectin,PAI-1,SAP,SAA,E-selectin,and MMP-9),high sensitive C-reactive protein,and Framingham risk score(FRS)were *** anthropometric data,aminotransferases,metabolic syndrome features,glucose and lipid profiles,and insulin resistance data were also *** LT recipients were compared to 22 biopsy-proven non-alcoholic steatohepatitis(NASH)patients and 20healthy controls(non-obese,non-diabetics,and nondyslipidemic).RESULTS:The LT recipients had significantly younger ages and lower body mass indices,aminotransferases,fasting glucose and insulin levels,glucose homeostasis model and metabolic syndrome features than the NASH *** cardiovascular risk markers,such as Hs-CRP and FRS[2.0(1.0-8.75)],were lower in the LT patients compared to those observed in the NASH patients(P=0.009).In contrast,the LT recipients and NASH patients had similar inflammatory and endothelial serum markers compared to the controls(pg/mL):lower IL-10 levels(32.3 and 32.3 vs 62.5,respectively,P=0.019)and higher IFNγ(626.1 and 411.9 vs 67.9,respectively,P<0.001),E-selectin(48.5 and 90.03vs 35.7,respectively,P<0.001),sVCAM-1(1820.6and 1692.4 vs 1167.2,respectively,P<0.001),and sICAM-1(230.3 and 259.7 vs 152.9,respectively,P=0.015)***:Non-obese LT recipients have similar pro-atherosclerotic serum profiles after a short 1-year follow-up period compared to NASH patients,suggesting a high risk of atherosclerosis in this population.

Obesity has become a major global health challenge and it is a risk factor for the development of several comorbid conditions. Additionally, obesity has considerable economic consequences. Obesity is a multifactorial condition that arises from independent influences of genetic and social-environmental factors on food intake and physical activity. It has been difficult to establish clear associations between weight status and the intake of single foods or food groups. In most people, the predisposition to obesity has a polygenic basis, which means that obesity will develop if an individual has several polygenic variants that increase body weight. The FTO gene was the first GWAS-identified obesity-susceptibility gene and since then other polygenic variants that are associated with BMI and dietary intake have also been identified. However, this is still an active area of research as more polygenetic variants await discovery.

Gastric Leiomyosarcoma Associated with Hypoglycemia in Dog—A Case Report

Neoplasms of the digestive system are uncommon in dogs, and may be of epithelial, neuroendocrine, hematopoietic and mesenchymal origin. The leiomyosarcoma is a tumor of mesenchymal origin of smooth, malignant, slow-growing muscles. Paraneoplastic syndromes can affect different systems, being the most commonly found: hypercalcemia, hypoglycemia, cachexia and anemia. There are rare cases of mesenchymal tumors of the gastrointestinal tract in dogs that result in hypoglycaemia with clinical signs. The objective of this study is to report a case of gastric leiomyosarcoma associated with clinical hypoglycemia. The patient had neurological signs such as ataxia and seizures associated with hypoglycemia, which did not recur after surgical excision of the tumor. The definitive diagnosis was based on the histopathological and immunohistochemical examination of the tumor.

Cholelithiasis is a common disease in the general population. One of its risk factors is the type 2 diabetes mellitus, related to metabolic abnormalities associated to overweight, obesity, insulin resistance, hypertriglyceridemia and dietary habits. The main risk factors to cholelithiasis are genetic susceptibility, ileum resection, diabetes mellitus, obesity, hemolytic anemia, age, estrogens and cirrhosis. This study aims to make a correlation between diabetes mellitus and obesity versus prevalence of cholelithiasis in these patients.

Knowledge about Diabetes Mellitus and Self-Care Activities before and after an Educational Program: A Pilot Study

Background: Health education has proven to be an effective strategy to enable people with diabetes mellitus to manage this condition. However, few studies in Brazilian population samples have been conducted to evaluate the effects of educational programs on diabetes knowledge and self-care. Aim: To evaluate the knowledge about diabetes mellitus and the performance of self-care activities, before and after participation in an educational program. Methods: This is an intervention study, with a quantitative approach, in a single comparison group, for the analysis of “before and after” results related to an educational program focused on self-care and concurrent physical training. The study interventions consisted of 42 exercise sessions, as well as individual educational meetings, according to the needs of each participant and through nursing consultations, using educational material prepared from the literature. The sample was initially composed of 33 adults with diabetes mellitus, but 18 completed the study. For the assessment of knowledge and self-care activities, the revised Brazilian versions of the Diabetes Knowledge Scale and the Diabetes Self-Care Activity Questionnaire were used, respectively, in the two moments of the study, that is, prior to the first educational meeting and immediately before the first physical training session, and after the last educational meeting, which ran parallel to the 42nd physical training session, making a six-month interval between the two assessment moments for each participant. Results: The analysis of knowledge about diabetes showed significant improvement after the educational program and, as for self-care, there was clinical improvement in all dimensions, but only the dimension “general diet” obtained statistically significant improvement. Artemisia annua: A New Version of a Traditional Tea under Randomized, Controlled Clinical Trial for the Treatment of Malaria

Introduction: The traditional antimalarial tea Artemisia annua, indicated for centuries in China to treat fevers, is again arousing interest for the treatment of malaria due to improvements attained in the plant composition by a few Institutions throughout the world, including the State University of Campinas (UNICAMP), Brazil, increasing its principal component by more than 100 times as from standard varieties, giving 1% in artemisinin and an expressive biomass yield such as 2 tons of dried leaves/hectare. Clinical trials carried out with this material in African countries have proven its therapeutic potential for a new generation of Artemisia tea in the treatment of falciparum malaria. In addition to artemisinin, recent studies have identified and quantified other compounds present in the crude extract and characterized their contributions to the anti-malarial efficacy, including their action against chloroquine-resistant strains. The majority of the clinical trials carried out with Artemisia tea in African countries have shown that the control of the parasitaemia is efficient in the initial treatment period, but few trials have followed the patients up to the 28th day. This first clinical trial carried out in Brazil with the A. annua infusion, after toxicological trials that defined the safety of this form of medication. Methods: The therapeutic efficacy of the tea was measured in patients with falciparum malaria over 28 days, comparing it with the current first-line treatment namely artemether-lumefantrine (Coartem?). The trial was carried out in controlled groups according to official protocol approved by the National Ethics in Research Committee (CONEP: 77/ 2011) and a rigorous control of the 17 patients with non-serious cases of falciparum malaria, recruited in the following three municipalities of the State of Pará, Brazil: Tucuruí, Goianésia do Pará and Anajás. Surgical Technique Used for Portal Vein Thrombosis when Thrombectomy Is Not Possible During Liver Transplantation

Portal vein thrombosis (PVT) was for a long time considered a barrier to liver transplantation. The aim of this study is to demonstrate the surgical technical options for portal vein reconstruction during liver transplantation in patients with PVT in which thrombectomy was not possible. Between September 1991 and March 2009, 420 liver transplanted patients were retrospectively analyzed, identifying 29 cases with PVT (6.9%). Preoperative diagnosis, preoperative risk factors, surgical technique options to treat various forms (grades) of PVT, postoperative recurrence and actuarial survival rates were studied. In three cases of PVT grade II and in one case PVT grade III the thrombectomy was insufficient, requiring some surgical technique options (13.79%). In two cases placement of iliac vein graft was performed, in one an anastomosis of the portal vein with collateral splenorenal vein and in the other with left gastric varicose. The actuarial survival rate for patients without PVT and patients with PVT and effective thrombectomy was 73.8% while those with PVT who needed some type of surgical option was 75%. Our results suggest that actuarial survival rates were similar among patients with PVT or PVT with effective thrombectomy when compared with PVT that required some surgical options.

Climate Characterization of the Machado-Mg Region through Geotechnology Techniques and Tools

The characteristics of a region and its environmental changes are directly associated with the characteristics of the climate and the changes that occur in it. The importance of the study of the climate of a region is widely known in agriculture, by the size of the impact that the climate has on the productivity and final quality of the products. From the studies of these spatial and temporal changes in climatic and environmental information, excellent results have been obtained in understanding the variability of atmospheric phenomena that occur throughout. The objective of the present work was to perform the climatic characterization of the Machado-MG region, analyzing data collected on the elements of the climate, applying methodologies that allow its spatialization, from geotechnology tools, more precisely, the techniques of interpolation and manipulation of spatial data in the Geographic Information Systems (GIS) environment. For this purpose, it has been used the climatic data obtained from meteorological stations of the National Institute of Meteorology-INMET, located in the state of Minas Gerais, referring to the monthly and annual averages of a historical series of 55 years (1961-2015). The results obtained allowed to characterize the climate of the municipality, which presented an average temperature around 15°C to 25°C and with average rainfall between 100 - 200 mm in the rainiest periods. The relative humidity of the air presents throughout the year values in the average of 60% to 80%. From the spatialization maps of temperature, precipitation and relative humidity of the air, it becomes possible to analyze the spatial behavior of the climatic elements and thus to have the perception of the climate dynamics in the region.
