Feasibility of performing the 3-minute step test with remote supervision in children and adolescents with cystic fibrosis: A comparative study

Importance:The 3-min step test is a simple option to monitor submaximal exercise capacity,although its use via remote video monitoring has not been investigated in children with cystic fibrosis(CF).Objective:This study aimed to assess the feasibility and reproducibility of performing the 3-min step test with remote ***:A cross-sectional study including CF patients(6-18 years)from two CF services were ***,anthropometric,clinical,and lung function data were collected and two 3-min step tests were performed:(i)in-person supervision,and(ii)remotely supervised by video *** and after the tests,heart rate(HR),oxygen saturation(SpO_(2)),and the Borg score for dyspnea and lower limb fatigue were ***:Twenty-three patients(10.7±3.7 years)with a mean FEV1 of 89.5%±23.2%were *** were no significant differences between tests,with mean differences(95%confidence intervals)in final HR of-3.3(-8.9,2.4),change in HR of-1.9(-6.1,2.1),final SpO_(2) of 0.3(-0.4,1.0),and final dyspnea of 0.1(-0.8,0.9).The intraclass correlation coefficient was 0.852(final HR),0.762(final SpO_(2)),and 0.775(final lower limb fatigue).Significant and moderate correlations were found between tests for final HR(r=0.75),change in HR(r=0.61),and final SpO_(2)(r=0.61).The Bland-Altman analysis showed a mean difference in final SpO_(2) between tests of 0.3%(limit of agreement-3.0%,3.5%).Interpretation:Physiological responses between tests were similar,indicating it was feasible to perform the 3-min step test with remote supervision in CF children.

AIM: To investigate chromosome 8 numerical aberra- tions, C-MYC oncogene alterations and its expression in gastric cancer and to correlate these findings with histo- pathological characteristics of gastric tumors. METHODS: Specimens were collected surgically from seven patients with gastric adenocarcinomas. Immu- nostaining for C-MYC and dual-color fluorescence in situ hybridization (FISH) for C-MYC gene and chromosome 8 centromere were performed. RESULTS: All the cases showed chromosome 8 aneu- ploidy and C-MYC amplification, in both the diffuse and intestinal histopathological types of Lauren. No significant difference (P denocarcinomas. C-MYC high amplification, like homogeneously stained regions (HSRs) and double minutes (DMs), was observed only in the intestinal-type. Structural rearrangement of C-MYC, like translocation, was observed only in the diffuse type. Regarding C-MYC gene, a significant difference (P < 0.05) was observed between the two histological types. The C-MYC protein was expressed in all the studied cases. In the intestinal- type the C-MYC immunoreactivity was localized only in the nucleus and in the diffuse type in the nucleus and cytoplasm. CONCLUSION: Distinct patterns of alterations between intestinal and diffuse types of gastric tumors support the hypothesis that these types follow different genetic path- ways.

Dyslipidemias and Related Factors in Brazilian Adolescents from Rural and Urban Areas

This study aimed to evaluate the prevalence of dyslipidemias and related factors, in Brazilian adolescents from rural and urban areas. This is a cross-sectional study with 182 adolescents aged 10 to 13 years, students, residing in the rural and urban area of Brazil;grouped according to age, gender and socioeconomic class. Sexual maturation, physical activity, blood pressure, nutritional status and body composition were evaluated. Total cholesterol, LDL, HDL and serum triglycerides were dosed. The food frequency questionnaire was applied, quantifying energy, carbohydrates, proteins, lipids, full fat and fibers. Bivariate and multiple analyses were carried out, by logistic regression. It was verified that 84.6% (n = 154) adolescents presented some lipid profile alteration. There was no difference in the lipid profile between adolescents for residence place. However, as for gender, it was noticed that girls presented higher levels of total cholesterol and HDL (p des values compared with eutrophic and with those with appropriate body fat percentage (p der and signs of sexual maturation were associated with total cholesterol;fiber intake remained associated with HDL and signs of sexual maturation with LDL (p des the related factors were gender, nutritional status and protein intake (p demias found between adolescents from rural and urban areas reinforces the importance of monitoring these alterations and information as for the related risk factors, with programs for correction of changeable factors.

Computational Study of the Alkylation Reaction of the Nitrogen Mustard Mechlorethamine Using NBO Model and the QTAIM Theory

Substances known as nitrogen mustards turn into aziridinium ion through the intramolecular cyclization SN1. This ion reacts with the DNA preferably at the N7 position of the guanine, and because of this, it is an important antineoplastic agent. Based on this, the objective of this study is to quantify the interaction between the nitrogen mustard mechlorethamine and the guanine, using the NBO analysis and the QTAIM theory. The results of the NBO analysis showed that when the triangular cycle C4-N1-C5 is formed, there is some resonance among these atoms. This analysis also showed that the electronic transition at the sigma antibondingorbital σ* N1-C4 presents higher perturbation energy of second order, indicating that this bond is broken at the nucleophilic attack of the N7 nitrogen of guanine. The analysis that refers to the electron density obtained by the QTAIM theory indicates that the guanine proximity enables an electron density polarization of the BCPs aziridinium ion of mechlorethamine making that the frontal part of the ion becomes electron deficient. Finally, the relative results to the Laplacian of the electron density obtained by the QTAIM theory showed that the guanine approximation increases the “hole” factor at the C4, proving that the nucleophilic attack based on the “lump-hole” concept causes the region of that atom is the site of alkylation reaction.

The Optimization of Aspergillus ***4 Tannase Production under Submerged Fermentation

Tannase is a hydrolytic enzyme that is involved in the biodegradation of tannins and it has biotechnological potential in the pharmaceutical, chemical, food and beverage industries. Microorganisms, especially filamentous fungi, are important tannase producers. The aims of this work were to find a potential tannase producer and to improve the cultivation conditions. Three Aspergillus species (A. japonicus 246A, A. tamarii 3 and Aspergillus sp. GM4) were investigated in different culture media (Adams, Czapeck, Khanna, M5 and Vogel) and inducers (1% and 2% tannic acid;1% green tea;1% methyl gallate;1% gallic acid). Aspergillus sp. GM4 and Adams medium were selected. The tannase production by Aspergillus sp. GM4 in Adams medium was induced in the presence of 2% (w/v) tannic acid and gallic acid as carbon sources, while green tea was not able to induce tannase production. The Plackett-Burman screening design was performed with the variables MgSO4, KH2PO4, yeast extract, tannic acid, agitation rate and salt solution. The variables MgSO4 and agitation rate were selected for the optimization of tannase production using a Central Composite Rotatable design. Under optimized conditions, a 2.66-fold increase in the enzyme production was observed with small modifications in the medium composition.

Coronavirus disease-19(COVID-19)has become a pandemic,being a global health concern since december 2019 when the first cases were *** acute respiratory syndrome coronavirus 2,the COVID-19 causal agent,is aβ-coronavirus that has on its surface the spike protein,which helps in its virulence and pathogenicity towards the ***,effective and applicable diagnostic methods to this disease come as an important tool for the management of the *** use of the molecular technique PCR,which allows the detection of the viral RNA through nasopharyngeal swabs,is considered the gold standard test for the diagnosis of ***,serological methods,such as enzyme-linked immunosorbent assays and rapid tests,are able to detect severe acute respiratory syndrome coronavirus 2-specific immunoglobulin A,immunoglobulin M,and immunoglobulin G in positive patients,being important alternative techniques for the diagnostic establishment and epidemiological *** the other hand,reverse transcription loop-mediated isothermal amplification also proved to be a useful diagnostic method for the infection,mainly because it does not require a sophisticated laboratory apparatus and has similar specificity and sensitivity to ***,imaging exams provide findings of typical pneumonia,such as the ground-glass opacity radiological pattern on chest computed tomography scanning,which along with laboratory tests assist in the diagnosis of COVID-19.
