Background:A national drug resistance survey(DRS)was implemented for the first time in Timor-Leste(TL)in *** primary objective of the survey was to assess the prevalence of drug resistance among new and previously treated pulmonary TB patients in the ***:This nation-wide cross-sectional survey was conducted in 2019 targeting all new and previously treated sputum smear-positive pulmonary TB *** samples were submitted to the National TB Reference Laboratory for confirmation of TB and to determine resistance to rifampicin by Xpert MTB/*** was performed on solid media,and culture isolates of confirmed TB cases were shipped to the WHO Supranational TB Reference Laboratory in Chennai,India for whole genome sequencing(WGS).Survey summary statistics,data cross-tabulations and analysis of potential risk factors of rifampicin-resistant TB(RR-TB)were conducted using R statistical software(version 3.5.2).Results:A total of 953 sputum-smear positive patients were enrolled,of which 917 were confirmed as positive for TB by either Xpert MTB/RIF or *** electronic web-based system was used for entry and storage of the *** resistance was detected among 0.6%(95%CI 0.2-1.3)of new cases and 2.7%(95%CI 0.5-8.2)of previously treated *** was conducted for validation purposes on 65 randomly selected isolates(29%of RR-TB(2/7)and 7%of RS-TB(63/910)by Xpert MTB/RIF or pDST).The original test results agreed with the WGS validation results for 62/64 isolates(97%).Conclusion:The prevalence of RR-TB in Timor-Leste is relatively low compared to the estimated proportions of RR-TB in the WHO South-East Asia Region(2.5%[95%CI 1.9-3.3]among new cases and 14%[95%CI 7.7-21]among previously treated cases).The rapid sputum collection and transportation mechanism implemented in the survey demonstrates its feasibility in low resource settings and should be replicated for routinely transporting TB specimens from microscopy labs to GeneXpert ***

Evaluation of Reinforced Concrete Structures with Views for Rehabilitation

The increasing number of pathological occurrences is found in reinforced concrete structures, so does the demand for inspection and maintenance services in residential communities in the state of Pernambuco, which aim to detect damage to the structure in a timely manner to minimize repair costs, thus applying preventive maintenance in order to guarantee the estimated useful life of the project for the building. However, it is observed that the practice of periodic inspections is not yet part of the culture of a certain segment of the population that does not have economic means and knowledge of the importance of maintenance to ensure the integrity of the structure. It was used in this work an inspection script with use of tests and methodology to quantify damage and to check the state of degradation of two residential buildings with more than 20 years and built near the seafront where environmental aggression is strong. The deterioration found in each of the buildings has been differentiated due to different corrective maintenance performed at every building. The last maintenance, performed in Building A, was performed by corrective hand of unskilled labour guided by the community itself, which has no expertise in the field of civil engineering. In Building B, the situation is similar, compounded by the opinion of some tenants that there is no need for this routine, even the structure being in critical condition, as was confirmed with the application of the methodology employ. There should be a legislation that governs the execution of inspections and maintenance programs in addition to the existing technical standards. The methodologies presented suitable for checking the degree of degradation of the structures and performed tests confirmed the diagnosis obtained by visual inspection and quantification of damage.

A Study Protocol on the Evaluation of Referral Strategies for Inflammatory Arthritis in Primary Care Patients at the Level of Healthcare Organization, Patient Relevant Outcomes and Costs

Background: Considering the importance of getting the right patient at the right location to maintain and optimize quality of life of inflammatory arthritis patients, appropriate referral by general practitioners is essential. This study aims to assess the effect and cost effectiveness of different referral strategies for inflammatory arthritis in primary care patients. Methods: This study follows a cluster randomized controlled trial design. General practitioners from primary care centers in Southwest-The Netherlands are randomly assigned to either one of the two strategic interventions for referring adult patients who are in the opinion of the general practitioner suspected of inflammatory arthritis: 1) Standardized digital referral algorithm based on existing referral models PEST, CaFaSpA and CARE;2) Triage by a rheumatologist in the local primary care center. These interventions will be compared to a control group, e.g. usual care. The primary outcome is the percentage of patients diagnosed with inflammatory arthritis by the rheumatologist. Secondary outcomes are quality of life as a patient reported outcome, work participation and healthcare costs. These data, including demographic and clinical parameters, are prospectively collected at baseline, three, six, and twelve months. Discussion: If this study can demonstrate improvements in appropriate referrals to the rheumatologist, thereby improving cost-effectiveness, there is sufficient supporting evidence to implement one of the referral strategies as a standard of care. Finally, with these optimization strategies a higher quality of care can be achieved, that might be of value for all patients with arthralgia. Syzygium aromaticum L.(Clove)Essential Oil as a Reducing Agent for the Green Synthesis of Silver Nanoparticles

The present investigation details a green synthesis of silver nanoparticles (AgNP) using the essential oil of Syzygium aromaticum L. (clove) as reducing agent, which is a matrix with a high content of eugenol, an important compound for the reducing action of silver nitrate. The synthesis of AgNP was performed at different pH conditions (pH 7, 8, 9 and 10), and was monitored by UV-Vis Spectroscopy, Dynamic Light Scattering (DLS) and Transmission Electron Microscopy. The synthesized nanoparticles presented characteristic Surface Plasmon Resonance bands with maximum absorbance between 405 and 460 nm. The DLS analysis revealed particle sizes from 31 to 72 nm and zeta potential between -30.1 and -50.8 mV indicating good stability against the agglomeration of the particles in solution. The micrographs obtained by TEM showed different particle shapes and a predominance of spherical-shaped nanoparticles, and average size ranging from 27 to 94 nm. The clove-based silver nanoparticles were efficient in controlling the growth of Escherichia coli and Staphylococcus aureus bacteria, and the minimum inhibitory concentration ranged from 60 to 100 μL/mL. This study highlights the feasibility of clove essential oil as an alternative for the synthesis of silver nanoparticles by a simple, inexpensive and eco-friendly method.

Cognitive Ability in Elderly in a City in Northeastern Brazil: An Analytical Study

To analyze the cognitive ability of elderly people assisted by the Family Health Strategy (FHS) in a northeastern Brazilian city, a cross-sectional, analytical study was conducted with 406 elderly assisted by the FHS in Mossoro-RN, Brazil, from March to May 2014. The data were collected through two validated questionnaires. For the data analysis, the Alpha Cronbach’s, Kruskal-Wallis test, chi-square and Pearson correlation statistical tests were applied. There was a predominance of women (67.98%) in the survey. Among the respondents, 50% had an index of up to 27 points in Pfeffer Instrument, identifying greater degree of dependence in performing instrumental daily activities related to mobility in the community (39.7%) and financial management (57.1%). On Mini-Mental State Examination (MMSE), 75% of seniors had an index above 27 points, with the scores classification as standard (50.99%), mild (25.12%), moderate (22.66%) and severe (1.23%). Although changes have been identified in cognitive performance and independence of elderly people in some areas, these difficulties do not completely affect the lives and daily activities of the elderly.
