We read with considerable interest the paper from Passeri et al.[1]questioning how the IIIb preoperative determination of a Klatskin tumor could be considered a risk factor for unresectability,and we could not refrain from making some critical recommendations about it.Although the paper was nicely written,in our opinion,no meaningful conclusion should however be drawn from it,due to the small sample size of the reported series.

To the Editor：Associating liver partition and portal vein ligation for stagedhepatectomy （ALPPS） was introduced in 2007 [1]. This techniqueis now used worldwide and has a wide spectrum of indications.However, scientific debate and un-regulated surgical experimentationconcerning the application of this technique led the surgeons to organize the first Consensus Meeting [2] to build a scientificconsensus. One of the most controversial indications to ALPPS isits application for Klatskin tumors. The aim of this letter is to analyzethe critical aspects of laparoscopic microwave ALPPS for radicalsurgical treatment of Klatskin tumors.

The Authors summarize problems,criticisms but also advantages and indications regarding the recent surgical proposal of associating liver partition and portal vein ligation(PVL)for staged hepatectomy(ALPPS)for the surgical management of colorectal liver metastases.Looking at published data,the technique,when compared with other traditional and well established methods such as PVL/portal vein embolisation(PVE),seems to give real advantages in terms of volumetric gain of future liver remnant.However,major concerns are raised in the literature and some questions remain unanswered,preliminary experiences seem to be promising.The method has been adopted all over the world over the last 2 years,even if oncological long-term results remain unknown,and benefit for patients is questionable.No prospective studies comparing traditional methods(PVE,PVL or classical 2 staged hepatectomy)with ALPPS are available to date.Technical reinterpretations of the original method were also proposed in order to enhance feasability and increase safety of the technique.More data about morbidity and mortality are also expected.The real role of ALPPS is,to date,still to be established.Large clinical studies,even if,for ethical reasons,in well selected cohorts of patients,are expected to better define the indications for this new surgical strategy.

Preclinical and clinical studies indicate that psychostimulants,in addition to having abuse potential,may elicit brain dysfunctions and/or neurotoxic effects.Central toxicity induced by psychostimulants may pose serious health risks since the recreational use of these substances is on the rise among young people and adults.The present review provides an overview of recent research,conducted between 2018 and 2023,focusing on brain dysfunctions and neurotoxic effects elicited in experimental models and humans by amphetamine,cocaine,methamphetamine,3,4-methylenedioxymethamphetamine,methylphenidate,caffeine,and nicotine.Detailed elucidation of factors and mechanisms that underlie psychostimulant-induced brain dysfunction and neurotoxicity is crucial for understanding the acute and enduring noxious brain effects that may occur in individuals who use psychostimulants for recreational and/or therapeutic purposes.

We present a short retrospective review of the existing literature about the dynamics of(dry)granular matter under the effect of vibrations.The main objective is the development of an integrated resource where vital information about past findings and recent discoveries is provided in a single treatment.Special attention is paid to those works where successful synthetic routes to as-yet unknown phenomena were identified.Such landmark results are analyzed,while smoothly blending them with a history of the field and introducing possible categorizations of the prevalent dynamics.Although no classification is perfect,and it is hard to distillate general properties out of specific observations or realizations,two possible ways to interpret the existing results are defined according to the type of forcing or the emerging(ensuing)regime of motion.In particular,first results concerning the case where vibrations and gravity are concurrent(vertical shaking)are examined,then the companion situation with vibrations perpendicular to gravity(horizontal shaking)is described.Universality classes are introduced as follows:(1)Regimes where sand self-organizes leading to highly regular geometrical“pulsating”patterns(thin layer case);(2)Regimes where the material undergoes“fluidization”and develops an internal multicellular convective state(tick layers case);(3)Regimes where the free interface separating the sand from the overlying gas changes inclination or develops a kind a patterned configuration consisting of stable valleys and mountains or travelling waves;(4)Regimes where segregation is produced,i.e.,particles of a given size tend to be separated from the other grains(deep containers).Where possible,an analogy or parallelism is drawn with respect to the companion field of fluid-dynamics for which the assumption of“continuum”can be applied.

BACKGROUND For compensated advanced chronic liver disease(cACLD)patients,the first decompensation represents a dramatically worsening prognostic event.Based on the first decompensation event(DE),the transition to decompensated advanced chronic liver disease(dACLD)can occur through two modalities referred to as acute decompensation(AD)and non-AD(NAD),respectively.Clinically Significant Portal Hypertension(CSPH)is considered the strongest predictor of decompensation in these patients.However,due to its invasiveness and costs,CSPH is almost never evaluated in clinical practice.Therefore,recognizing noninvasively predicting tools still have more appeal across healthcare systems.The red cell distribution width to platelet ratio(RPR)has been reported to be an indicator of hepatic fibrosis in Metabolic Dysfunction-Associated Steatotic Liver Disease(MASLD).However,its predictive role for the decompensation has never been explored.AIM In this observational study,we investigated the clinical usage of RPR in predicting DEs in MASLD-related cACLD patients.METHODS Fourty controls and 150 MASLD-cACLD patients were consecutively enrolled and followed up(FUP)semiannually for 3 years.At baseline,biochemical,clinical,and Liver Stiffness Measurement(LSM),Child-Pugh(CP),Model for End-Stage Liver Disease(MELD),aspartate aminotransferase/platelet count ratio index(APRI),Fibrosis-4(FIB-4),Albumin-Bilirubin(ALBI),ALBI-FIB-4,and RPR were collected.During FUP,DEs(timing and modaities)were recorded.CSPH was assessed at the baseline and on DE occurrence according to the available Clinical Practice Guidelines.RESULTS Of 150 MASLD-related cACLD patients,43(28.6%)progressed to dACLD at a median time of 28.9 months(29 NAD and 14 AD).Baseline RPR values were significantly higher in cACLD in comparison to controls,as well as MELD,CP,APRI,FIB-4,ALBI,ALBI-FIB-4,and LSM in dACLD-progressing compared to cACLD individuals[all P0.472 and>0.894 as the best

Innovative Services for Electric Mobility Based on Virtual Sensors and Petri Nets

About 60%of emissions into the earth’s atmosphere are produced by the transport sector,caused by exhaust gases from conventional internal combustion engines.An effective solution to this problem is electric mobility,which significantly reduces the rate of urban pollution.The use of electric vehicles(EVs)has to be encouraged and facilitated by new information and communication technology(ICT)tools.To help achieve this goal,this paper proposes innovative services for electric vehicle users aimed at improving travel and charging experience.The goal is to provide a smart service to allow drivers to find the most appropriate charging solutions during a trip based on information such as the vehicle’s current position,battery type,state of charge,nearby charge point availability,and compatibility.In particular,the drivers are supported so that they can find and book the preferred charge option according to time availability and the final cost of the charge points(CPs).To this purpose,two virtual sensors(VSs)are designed,modeled and simulated in order to provide the users with an innovative service for smart CP searching and booking.In particular,the first VS is devoted to locate and find available CPs in a preferred area,whereas the second VS calculates the charging cost for the EV and supports the driver in the booking phase.A UML activity diagram describes VSs operations and cooperation,while a UML sequence diagram highlights data exchange between the VSs and other electromobility ecosystem actors(CP operator,EV manufacturer,etc.).Furthermore,two timed Petri Nets(TPNs)are designed to model the proposed VSs,functioning and interactions as discrete event systems.The Petri Nets are synchronized by a single larger TPN that is simulated in different use cases and scenarios to demonstrate the effectiveness of the proposed VSs.
