Bacteriological Profile of Infections Encountered in a Pneumology Service in a Moderate-Income Country [Pneumology Department of ChU Cocody (Côte d’Ivoire)]

Introduction: Infections represent a real public health problem aggravating the morbidity and mortality of hospitalized patients. Methods: This was a retrospective study with descriptive purposes over a period of 05 years, in the Pneumology Department of the University hospital of Cocody. Results: The average age in our population was 42 years. We observed a male predo- minance of 64.5%, with a sex ratio of 1.8. Medical history was dominated by hIV infection (23.5%), followed by tuberculosis (15.6%). Concerning lifestyle, smoking was found in 38% of cases. Symptoms progressed chronically in 80% of cases. An infectious syndrome was found in 75% of cases. Microbial culture was positive in 42% of cases. Pseudomonas aeruginosa and Klebsiella pneumoniae were encountered in 26.4% of cases each, followed by Echerichia coli in 10.4% of cases. Klebsiella pneumoniae in the first three years topped the list, but gradually. Pseudomonas aeruginosa maintained its leadership over the last three years. Pseudomonas aeruginosa isolates expressed a resistance rate of 9.8% to ceftazidime and 8.1% to imipenem;to aztreonam (36%), ticarcillin (33.3%) and levofloxacin. These strains were susceptible to fosfomycin (100%), mero- penem (96.6%) and amikacin (96%). For isolated strains of Entero- bacteria- ceae, resistance was observed about ticarcillin (83.3%) and amoxicillin clavula- nic acid (71.2%). Streptococcaceae showed resistance to tetracycline (69.2%) and erythromycin (50%). Over the years there has been an increase in re- sistance to amoxicillin-clavulanic acid ceftriaxone. The death rate was 14%. Conclusion: The bacterial profile of infections is dominated by germs respon- sible for nosocomial infection with significant mortality.

Ecogenotoxicology in earthworms： A review

Pollutant dynamics and bioavailability greatly differ in soil and aquatic systems. Therefore, specific approaches and models are needed to assess the impact of soil contamination to terrestrial ecosystems. Earthworms among other soil invertebrates have received more attention because of their ecological importance. They represent a dominant part of the soil biomass and are soil engineers regulating important soil processes, notably fertilization. The release in soils of pollutants known for their persistence and/or their toxicity is a concern. Exposure of terrestrial species to pollutants that may alter genomic function has become an increasing topic of research in the last decade. Indeed, genome disturbances due to genetic and epigenetic mechanisms may impair growth, as well as reproduction and population dynamics in the long term. Despite their importance in gene expres- sion, epigenetic mechanisms are not yet understood in soil invertebrates. Until now, pollutant-induced changes in genome expression in natural biota are still being studied through structural alteration of DNA. The first biomarker relating to genotoxicant exposure in earthworms from multi-contaminated soils reported is DNA adducts measurements. It has been replaced by DNA breakage measured by the Comet assay, now more commonly used. Functional genomic changes are now being explored owing to molecular ＂omic＂ technologies. Approaches, objectives and results are overviewed herein. The focus is on studies dealing with genotoxicity and populational effects established from environmentally-relevant experiments and in situ studies [Current Zoology 60 （2）： 255-272, 2014].

BACKGROUND Splanchnic vein thrombosis(SVT)is a major complication of moderate and severe acute *** is no consensus on whether therapeutic anticoagulation should be started in patients with acute pancreatitis and *** To gain insight into current opinions and clinical decision making of pancreatologists regarding SVT in acute *** A total of 139 pancreatologists of the Dutch Pancreatitis Study Group and Dutch Pancreatic Cancer Group were approached to complete an online survey and case vignette *** threshold to assume group agreement was set at 75%.RESULTS The response rate was 67%(n=93).Seventy-one pancreatologists(77%)regularly prescribed therapeutic anticoagulation in case of SVT,and 12 pancreatologists(13%)for narrowing of splanchnic vein *** most common reason to treat SVT was to avoid complications(87%).Acute thrombosis was the most important factor to prescribe therapeutic anticoagulation(90%).Portal vein thrombosis was chosen as the most preferred location to initiate therapeutic anticoagulation(76%)and splenic vein thrombosis as the least preferred location(86%).The preferred initial agent was low molecular weight heparin(LMWh;87%).In the case vignettes,therapeutic anticoagulation was prescribed for acute portal vein thrombosis,with or without suspected infected necrosis(82%and 90%),and thrombus progression(88%).Agreement was lacking regarding the selection and duration of long-term anticoagulation,the indication for thrombophilia testing and upper endoscopy,and about whether risk of bleeding is a major barrier for therapeutic *** In this national survey,the pancreatologists seemed to agree on the use of therapeutic anticoagulation,using LMWh in the acute phase,for acute portal thrombosis and in the case of thrombus progression,irrespective of the presence of infected necrosis.

Melanoma is the deadliest skin cancer and is responsible for over 7000 deaths in the US *** spread of cancer,or metastasis,is responsible for these deaths,as secondary tumors interrupt normal organ *** tumor cells,or those cells that spread throughout the body from the primary tumor,are thought to be responsible for *** developed an optical method,photoacoustic flow cytometry,in order to detect and enumerate circulating melanoma cells(CMCs)from blood samples of *** tested the blood of StageⅣmelanoma patients to show the ability of the photoacoustic flow cytometer to detect these rare cells in *** then tested the system on archived blood samples from StageⅢmelanoma patients with known outcomes to determine if detection of CMCs can predict future *** detected between 0 and 66 CMCs in StageⅣ*** the StageⅢstudy,we found that of those samples with CMCs,two remained disease free and five developed *** those without CMCs,six remained disease free and one developed *** believe that photoacoustic detection of CMCs provides valuable information for the prediction of metastasis and we postulate a system for more accurate prognosis.
