Seminomas are most commonly diagnosed in clinical stage I(CSI).After orchiectomy,approximately 15%of patients in this stage have subclinical *** radiotherapy(arT)delivered to the retroperitoneum and ipsilateral pelvic lymph nodes has been the mainstay of treatment for many *** highly efﬁcient,with long-term cancer-speciﬁc survival(CSS)rates approaching almost 100%,arT is associated with considerable long-term consequences,particularly cardiovascular toxicity and increased risk of secondary malignancies(SMN).Therefore,active surveillance(AS)and adjuvant chemotherapy(ACT)were developed as alternative treatment *** AS prevents patient overtreatment,it is associated with strict follow-up regimens and increased radiation exposure due to repeated *** to equivalent CSS rates to arT,and lower toxicity,one course of adjuvant carboplatin presents the cornerstone of chemotherapy for CSI *** is almost 100%for patients with CSI seminoma,regardless of the chosen treatment ***,a personalized approach in treatment selection is ***,routine radiotherapy for CSI seminoma patients is no longer ***,it should be reserved for patients who are unﬁt or unwilling for AS or ***ﬁcation of prognostic factors for disease relapse allowed for the development of risk-adapted treatment strategy and stratiﬁcation of patients in low-risk and high-risk *** risk-adapted policy needs further validation,surveillance is currently recommended in low-risk patients,while ACT is reserved for patients with a higher risk of relapse.

Investigating short- and long-term transfer effects of a Taiji beginner course in participants' daily life

in recent years research investigating various health benefits of Taiji practice has markedly increased. Despite this growing scientific interest, essential questions such as to what extent a Taiji course may exert noticeable effects in participants＇ everyday life, what these effects are, and how and where potential transfer effects occur, have hardly been considered. The aim of our study was to explore transfer effects from a Taiji course into participants＇ daily lives. METHODS： We conducted a longitudinal observational study in 45 healthy participants at the end of their three-month Taiji beginner course （tpl） and at two months （tp2） as well as one year after course completion （tp3）. Participants were asked to report their Taiji practice behavior at all time points, as well as to rate and describe perceived transfer effects of Taiji course contents on their daily life at tpl and tp3. RESULTS： Transfer effects were reported by 91.1% of all respondents after course completion （tpl） and persisted in 73.3% at the one-year follow-up assessment （tp3）, counting ＂increase of self-efficacy＂, ＂improvement of stress management＂, and ＂increase of body awareness＂ as the most frequently mentioned effects. Transfer effects predominantly occurred in participants＇ work and social environments, as well as during everyday activities in public areas. While self- reliant Taiji practice frequency significantly decreased from 82.2% at tpl to 55.6% at tp3 （P 〈 0.001）, the magnitude of self-reported transfer effects did not （P = 0.35）. As explorative analyses revealed, regular Taiji course attendance was highly correlated with stronger transfer effects at tpl （r = 0.51; P 〈 0.001） and tp3 （r = 0.35; P = 0.020）. Participants reporting high self-reliant Taiji practice frequency at tp2 were likely to maintain a regular practice routine at tp3 （r = 0.42; P 〈 0.004）, whereas self-reliant practice frequency and transfer effects at tpl were positively correlated with self-reliant practice freque

Stable gastric pentadecapeptide BPC 157 in the treatment of colitis and ischemia and reperfusion in rats: New insights

AIM To provide new insights in treatment of colitis and ischemia and reperfusion in rats using stable gastric pentadecapeptide BPC 157. METHODS Medication [BPC 157,L-NAME,L-arginine(alone/combined),saline] was bath at the blood deprived colon segment. During reperfusion,medication was BPC 157 or saline. We recorded(USB microscope camera) vessel presentation through next 15 min of ischemic colitis(ICrats) or reperfusion(removed ligations)(IC + RL-rats);oxidative stress as MDA(increased(IC-and IC + RLrats)) and NO levels(decreased(IC-rats);increased(IC + RL-rats)) in colon tissue. IC + OB-rats [IC-rats had additional colon obstruction(OB)] for 3 d(IC + OBrats),then received BPC 157 bath. RESULTS Commonly,in colon segment(25 mm,2 ligations on left colic artery and vein,3 arcade vessels within ligated segment),in IC-,IC + RL-,IC + OB-rats,BPC 157(10 μg/kg) bath(1 m L/rat) increased vessel presentation,inside/outside arcade interconnections quickly reappeared,mucosal folds were preserved and the pale areas were small and markedly reduced. BPC 157 counteracted worsening effects induced by L-NAME(5 mg) and L-arginine(100 mg). MDA-and NO-levels were normal in BPC 157 treated IC-rats and IC + RLrats. In addition,on day 10,BPC 157-treated IC + OBrats presented almost completely spared mucosa with very small pale areas and no gross mucosal defects;the treated colon segment was of normal diameter,and only small adhesions were *** BPC 157 is a fundamental treatment that quickly restores blood supply to the ischemically injured area and rapidly activates collaterals. This effect involves the NO system.

Relativistic Supernova Blast Waves Exhibit Properties of Gravitational Lenses and the Hubble Constant

Simulations based on Supernova (SN) observations predict several galactic SN explosions (SNe) can occur every century. Unlike SNes within the Interstellar Medium (ISM) where ambient gas generally absorbs blast waves within a million years, SNes occurring in a rarified environment outside of the ISM generate blast waves which remain in a relativistic free expansion phase for more extended periods. The SN blast wave forms an expanding spherical shell and when multiple blast waves intersect, the overlapping region naturally takes the form of a ring, an arc, or an Einstein Cross structure. The analysis shows the relativistic plasma establishes a medium with permeability which drives the index of refraction greater than 1. As a result, when a shock discontinuity forms in the overlapping region, light is reflected from the host galaxy which exposes the intersecting blast wave regions. The expanding shells are shown to induce an achromatic redshift to the reflected light consistent with those measured for gravitational lenses. Further, it is shown that a Hubble equation for a blast wave around the Milky Way Galaxy can be parameterized to approximate measured redshifts over a wide range of distances.

Effects of Additional Rifle-Carriage Training on Physiological Markers and Roller-Skiing Performance in Well-Trained Biathletes

Suspicious brush cytology is an indication for liver transplantation evaluation in primary sclerosing cholangitis

AIM To investigate markers for high-grade dysplasia for the optimal timing of liver transplantation in patients with primary sclerosing cholangitis(PSC).METHODS Earlier data support a dysplasia-carcinoma sequence,even low-to high-grade dysplasia,in PSC-associated cholangiocarcinoma(CCA). Surveillance using endoscopic retrograde cholangiography(ERC) and brush cytology aims to detect cases of biliary dysplasia,and liver transplantation is an option in cases with suspicion of malignancy in brushing. This study investigated markers to identify patients with highgrade biliary dysplasia for optimal timing in early liver transplantation. Patients undergoing surveillance using ERC and brush cytology during 2008-2014 and who were diagnosed with biliary dysplasia in explanted liver or CCA until February 2016 were included in the study. Demographic data,cholangiography findings,laboratory values,cytological morphology and DNA ploidy were *** Thirty PSC patients had biliary neoplasia in the explanted liver during the study period. Sixteen of these patients had low-grade dysplasia,10 patients had high-grade dysplasia,and 4 patients had CCA. Fifteen PSC patients diagnosed with CCA were not transplanted. Patients with low-grade dysplasia were younger. Alkaline phosphatase or carcinoembryonic antigen values did not differ between groups during surveillance,but carbohydrate antigen 19-9 was higher in CCA patients. No difference in PSC duration,ERC scores,suspicious cytology,or ploidy analysis was found between groups. No difference was observed between fibrosis stage in explanted livers. Low-and high-grade dysplasia could not be differentiated before liver transplantation based on liver enzymes,tumour markers,ERC scores,brush cytology or DNA *** Repeated suspicion of neoplasia in brush cytology should be an indication for evaluations of liver transplantation prior to the development of CCA.
