Erectile dysfunction(ED),a condition closely related to cardiovascular morbidity and mortality,is frequently associated with obesity.In this study,we aimed to determine the prevalence of ED and evaluate the associated risk factors in a cohort of 254 young(18–49 years)nondiabetic obese(body mass index[BMI]≥30 kg m−2)men from primary care.Erectile function(International Index of Erectile Function[IIEF-5]questionnaire),quality of life(Aging Males'Symptoms[AMS scale]),and body composition analysis(Tanita MC-180MA)were determined.Total testosterone was determined using high-performance liquid chromatography–mass spectrometry.Multivariate logistic regression analysis was used to study the factors associated with ED.ED prevalence was 42.1%.Subjects with ED presented higher BMI,waist circumference,number of components of the metabolic syndrome,AMS score,insulin resistance,and a more unfavorable body composition than those without ED.Multivariate logistic regression analysis showed that a pathological AMS score(odds ratio[OR]:4.238,Pmary care-based cohort of nondiabetic young obese men,ED affected>40%of subjects.A pathological AMS score,the degree of obesity,and age were positively associated with ED,while elevated HDL-cholesterol levels were inversely associated with the odds of presenting ED.Further prospective studies are needed to evaluate the long-term consequences of ED in this population.

Recent reports suggest that aging is not solely a physiological process in living beings;instead, it should be considered a pathological process or disease(Amorim et al., 2022). Consequently, this process involves a wide range of factors, spanning from genetic to environmental factors, and even includes the gut microbiome(GM)(Mayer et al., 2022). All these processes coincide at some point in the inflammatory process, oxidative stress, and apoptosis, at different degrees in various organs and systems that constitute a living organism(Mayer et al., 2022;Aguilarhernández et al., 2023).

The application of machine learning(ML)algorithms in various fields of hepatology is an issue of interest.However,we must be cautious with the results.In this letter,based on a published ML prediction model for acute kidney injury after liver surgery,we discuss some limitations of ML models and how they may be addressed in the future.Although the future faces significant challenges,it also holds a great potential.

Background Schistosomiasis is highly endemic in sub-Saharan Africa and frequently imported to Europe. Male urogenital manifestations are often neglected. We aimed to ascertain the prevalence of genitourinary clinical signs and symptoms among long-term African migrants in a non-endemic European country using a serology test.Methods We carried out a prospective, community-based cross-sectional study of adult male migrants from sub-Saharan Africa living in Spain.Schistosoma serology tests and microscopic urine examinations were carried out, and clinical data were obtained from an electronic medical record search and a structured questionnaire.Results We included 388 adult males, mean age 43.5 years [Standard Deviation (SD) = 12.0, range: 18–76]. The median time since migration to the European Union was 17 [Interquartile range (IQR): 11–21] years. The most frequent country of origin was Senegal (N = 179, 46.1%). Of the 338, 147 (37.6%) tested positive forSchistosoma. Parasite eggs were present in the urine of only 1.3%. Nine genitourinary clinical items were significantly associated with positiveSchistosoma serology results: pelvic pain (45.2%;OR = 1.57, 95%CI: 1.0–2.4), pain on ejaculation (14.5%;OR = 1.85, 95%CI: 1.0–3.5), dyspareunia (12.4%;OR = 2.45, 95%CI: 1.2–5.2), erectile dysfunction (9.5%;OR = 3.10, 95%CI: 1.3–7.6), self-reported episodes of infertility (32.1%;OR = 1.69, 95%CI: 1.0–2.8), haematuria (55.2%;OR = 2.37, 95%CI: 1.5–3.6), dysuria (52.1%;OR = 2.01, 95%CI: 1.3–3.1), undiagnosed syndromic STIs (5.4%), and orchitis (20.7%;OR = 1.81, 95%CI: 1.0–3.1). Clinical signs tended to cluster.Conclusions Urogenital clinical signs and symptoms are prevalent among male African long-term migrants with a positiveSchistosoma serology results. Genital involvement can be frequent even among those with long periods of non-residence in their sub-Saharan African countries of origin. Further research is needed to develop diagnostic tools and validate therapeutic approaches to chronic schistosomiasis.

Alexander disease is a rare neurodegenerative disorder caused by mutations in the glial fibrillary acidic protein,a type III intermediate filament protein expressed in astrocytes.Both early(infantile or juvenile)and adult onsets of the disease are known and,in both cases,astrocytes present characteristic aggregates,named Rosenthal fibers.Mutations are spread along the glial fibrillary acidic protein sequence disrupting the typical filament network in a dominant manner.Although the presence of aggregates suggests a proteostasis problem of the mutant forms,this behavior is also observed when the expression of wild-type glial fibrillary acidic protein is increased.Additionally,several isoforms of glial fibrillary acidic protein have been described to date,while the impact of the mutations on their expression and proportion has not been exhaustively studied.Moreover,the posttranslational modification patterns and/or the protein-protein interaction networks of the glial fibrillary acidic protein mutants may be altered,leading to functional changes that may modify the morphology,positioning,and/or the function of several organelles,in turn,impairing astrocyte normal function and subsequently affecting neurons.In particular,mitochondrial function,redox balance and susceptibility to oxidative stress may contribute to the derangement of glial fibrillary acidic protein mutant-expressing astrocytes.To study the disease and to develop putative therapeutic strategies,several experimental models have been developed,a collection that is in constant growth.The fact that most cases of Alexander disease can be related to glial fibrillary acidic protein mutations,together with the availability of new and more relevant experimental models,holds promise for the design and assay of novel therapeutic strategies.

There are documentary records referring to paclobutrazol(PBZ)as a growth bioregulator that inhibits the gibberellin synthesis and its application increases yields in fruit and vegetable crop productions.Its agronomic management includes it as an emerging technology to reduce vigour,promote flower induction and flower development in fruit trees with increased economic returns.Its use is banned in some countries because of concerns about residues that can cause harmful effects on the environment.Therefore,the aim of this article was to collect,analyse and summarise relevant information on the use of PBZ in fruit tree production and its possible risks to the environment.The results obtained indicated that the application of PBZ can be effective in solving some problems related to flowering if it is applied in the right amount and at the right time.However,it is necessary to elucidate the physiological processes with which it is associated and its response to be taken into account to increase yield.PBZ is currently used in fruit trees such as mango,lime,apple and guava,increasing their productivity.However,some studies have shown its residual effect on the environment.Therefore,PBZ is a viable strategy,because it presents a series of advantages in the production of fruit trees.However,it is vital to generate protocols that seek its regulation with a rational and sustainable approach.

Characterization and QTL identification in eggplant introgression lines under two N fertilization levels

Lowering nitrogen inputs is a major goal for sustainable agriculture.In the present study,a set of 10 Solanum melongena introgression lines(ILs)developed using Solanum incanum as the exotic donor parent were grown under two nitrogen fertilization doses supplied with the irrigation system:1)8.25 mmol·L-1NH4NO3,corresponding to the high nitrogen treatment(HN),and 2)no external nitrogen supply,corresponding to the low nitrogen treatment(LN).Twenty traits,including plant growth and yield parameters,fruit size and morphology,nitrogen and carbon content in leaf and fruit,and phenolics content in fruit,were evaluated.The aim was to select of potential materials for eggplant breeding under low N inputs,as well as to identify and locate putative QTLs associated with the traits evaluated.No significant differences were observed between the soil characteristics of the HN and LN treatments,except for nitrogen and iron content,which was slightly lower in the HN,probably as a consequence of higher nutrient removal from soil by plants in the latter group.Analysis of variance showed that lowering nitrogen inputs did not significantly affect the final yield,fruit morphology,size and phenolics content.Most agronomic traits were highly and positively correlated with each other under both treatments,as well as total phenolics with chlorogenic acid content.The assessment of the differences between each IL and the recipient parent resulted in the identification of 36 QTLs associated with most of the traits—12 were specific to the HN,17 specific to the LN,and 7 were stable across treatments.The introgressed fragment of S.incanum generally had a negative effect on the trait,except for QTLs for fruit dry matter,for fruit length on chromosome 10 under the HN,and for fruit pedicel length on chromosome 9 under the LN.The increase over AN-S-26 of the allele of S.incanum for the QTLs detected ranged between-73.98% and 26.03% in HN and-73.67%and 34.43% in LN.These findings provide useful tools for the utilization of S.incanum in eggplant breed

Management of betablocked patients after sustained virological response in hepatitis C cirrhosis

BACKGROUND Current guidelines do not address the post–sustained virological response management of patients with baseline hepatitis C virus (HCV) cirrhosis and oesophageal varices taking betablockers as primary or secondary prophylaxis of variceal bleeding. We hypothesized that in some of these patients portal hypertension drops below the bleeding threshold after sustained virological response, making definitive discontinuation of the betablockers a safe option. AIM To assess the evolution of portal hypertension, associated factors, non-invasive assessment, and risk of stopping betablockers in this population. METHODS Inclusion criteria were age > 18 years, HCV cirrhosis (diagnosed by liver biopsy or transient elastography > 14 kPa), sustained virological response after directacting antivirals, and baseline oesophageal varices under stable, long-term treatment with betablockers as primary or secondary bleeding prophylaxis. Main exclusion criteria were prehepatic portal hypertension, isolated gastric varices, and concomitant liver disease. Blood tests, transient elastography, and upper gastrointestinal endoscopy were performed. Hepatic venous pressure gradient (HVPG) was measured five days after stopping betablockers. Betablockers could be stopped permanently if gradient was mary prophylaxis. Median time from end of antivirals to gradient was 67 wk. Venous pressure gradient was < 12 mmHg in 13 patients (39.4%). In univariate analysis the only associated factor was a MELD score decrease from baseline. Fast-track protocols in laparoscopic liver surgery:Applicability and correlation with difficulty scoring systems

BACKGROUND Few series have reported the utility of fast-track protocols(FTP)in minimally invasive liver surgery.AIM To report the applicability of FTP in minimally invasive liver surgery and to correlate with difficulty scores.METHODS The series of patients undergoing minimally invasive liver surgery from 2014 was analyzed.Iwate,Southampton and Gayet’s scores were compared as predictors of FTP adherence.Accomplishment of FTP was considered within 24-h,48-h and 72-h.Multivariate models were performed to define discharge<24 h,<72 h,complications and readmissions.RESULTS From 160 cases,78 were candidates for FTP,of which 22(28.2%),19(24.4%)and 14(17.9%)were discharged in<24-h,48-h and 72-h,respectively(total=71.5%).Iwate,Southampton and Gayet’s scores achieved area under the receiver operating characteristic values for<24-h stay of 0.780,0.687 and 0.698,respectively.Sensitivity and specificity values for the best score(Iwate)were 87.7%and 66.7%,respectively(cutoff=5.5).In multivariate models,<72 h stay and complications revealed body mass index as a risk factor independent from difficulty scores.CONCLUSION The development of aggressive FTP is feasible and<24-h stay can be achieved even in moderate and advanced complexity cases.Difficulty scores,including body mass index value,may be useful to predict which cases may adhere to these protocols.

AIM:To compare the visual perception(color and chromatic-achromatic contrast vision)of a small cohort of COVID-19 patients at the time of infection and after 6mo with that of a healthy population matched for sex and age.METHODS:A total of 25 patients(9 females,16 males,mean age:54±10y)with COVID-19 hospitalized in the COVID-19 Unit of the University Clinical Hospital of Valladolid were recruited for this preliminary study.Visual perception,as determined by monocular measurement of contrast sensitivity function(CSF)and color vision was assessed in each patient using the Optopad test.The results obtained were then compared with those of a sample of 16 age-and sex-matched healthy controls(5 females,11 males,mean age:50±6y)in which the same measurement procedure was repeated.Statistically significant differences between groups were assessed using the Mann-Whitney U test.Measurements were repeated after a minimum follow-up period of 6mo and statistically significant differences between the two time points in each group were assessed using the Wilcoxon signed rank test.RESULTS:Discrimination thresholds(color and chromatic-achromatic contrast vision)and their corresponding sensitivity,calculated as the inverse of the discrimination threshold,were evaluated.Analysis of the data revealed higher contrast threshold results(i.e.,worse contrast sensitivity)in the COVID-19 group than in the control group for all spatial frequencies studied in the Optopad-CSF achromatic test and most of the spatial frequencies studied in the Optopad-CSF chromatic test for the red-green and blue-yellow mechanisms.In addition,color threshold results in the COVID-19 group were also significantly higher(i.e.,worse color sensitivity)for almost all color mechanisms studied in the Optopad-Color test.At 6mo,most of the differences found between the groups were maintained despite COVID-19 recovery.CONCLUSION:The present results provide preliminary evidence that visual perception may be impaired in COVID-19,even when the infection has passed.Although fur
