Psychiatrists' approach to vascular risk assessment in Latin America

AIM: To explore the way in which Latin American psychiatrists approach the screening of vascular risk factors in patients receiving antipsychotic ***: This was a descriptive, cross sectional study that surveyed Latin-American physicians to evaluate differences between groups divided in three main sections. The first section included demographic and professional data. The second section asked about the available medical resources: weighing scales, sphygmomanometer and measuring tape. Finally, the third section aimed at looking into the attitudes towards car-diovascular prevention. The latter was also divided into two subsections. In the first one, the questions were about weight, blood pressure and waist perimeter. In the second subsection the questions asked about the proportion of patients:(1) that suffered from overweight and/or obesity;(2) whose lipids and glycemia were controlled by the physician;(3) that were questioned by, and received information from the physician about smoking; and(4) that received recommendations from the physician to engage in regular physical activity. The participants were physicians, users of the medical website Intramed. The visitors were recruited by a banner that invited them to voluntarily access an online self-reported structured questionnaire with multiple options. RESULTS: We surveyed 1185 general physicians and 792 psychiatrists. Regarding basic medical resources, a significantly higher proportion of general physicians claimed to have weighing scales(χ2 = 404.9; P < 0.001), sphygmomanometers(χ2 = 419.3; P < 0.001), and measuring tapes(χ2 = 336.5; P < 0.001). While general physicians measured overweight and metabolic indexes in the general population in a higher proportion than in patients treated with antipsychotics(Z =-11.91; P < 0.001), psychiatrists claimed to measure them in patients medicated with antipsychotics in a higher proportion than in the general population(Z =-3.26; P < 0.001). Modelling of tropospheric delays in geosynchronous synthetic aperture radar

As a direct consequence of the orbital height, the integration time in geosynchronous synthetic aperture radar(GEO SAR) with metric or decimetric azimuth resolutions is in the order of several hundreds or even thousands of seconds. With such long integration time, the compensation of residual tropospheric propagation terms poses one of the fundamental challenges associated with GEO SAR missions. In order to better characterise the impact of the propagation errors on GEO SAR imaging, we put forward a model for the simulation of the tropospheric delay appropriate for the accurate simulation of GEO SAR surveys. The suggested model, with a deterministic background component and a random turbulent one, incorporates some of the most recent meteorological data for the characterization of the troposphere. To illustrate the relevance of the derivation, the suggested model is used for performance estimation and raw data simulation of GEO SAR raw data. Substantial conclusion on the system impulse response and the associated calibration requirements is also drawn from the analysis.

Turbulent boundary layer control(TBLC) for skin-friction drag reduction is a relatively new technology made possible through the advances in computational-simulation capabilities,which have improved the understanding of the flow structures of *** in micro-electronic technology have enabled the fabrication of active device systems able to manipulating these *** combination of simulation,understanding and micro-actuation technologies offers new opportunities to significantly decrease drag,and by doing so,to increase fuel efficiency of future *** literature review that follows shows that the application of active control turbulent skin-friction drag reduction is considered of prime importance by industry,even though it is still at a low technology readiness level(TRL).This review presents the state of the art of different technologies oriented to the active and passive control for turbulent skin-friction drag reduction and contributes to the improvement of these technologies.

Cross-Over Assessment of the AmbuAuraGain, LMA Supreme New Cuff and Intersurgical I-Gel in Fresh Cadavers

Background: The AmbuAuraGain is a new single-use supraglottic airway device with gastric channel designed to facilitate intubation. The aim of the study was to assess the anatomic position and the performance of the AuraGain in fresh cadavers compared to that of the Intersurgical i-gel and LMA Supreme New Cuff. Methods: The 3 devices were inserted in random order in 7 fresh cadavers without difficult airway criteria. The assessed items were: Insertion time, number of attempts and ease of insertion, airway seal pressure, ease of gastric tube insertion, endoscopic view of vocal cords, efficacy of guided tracheal intubation through the AuraGain and i-gel, and anatomic fit by lateral X-ray and neck dissections. Results: All devices were successfully inserted within 3 attempts, except for one case of the LMA Supreme. Adjusting manoeuvres were often required to accomplish correct insertion. A 16 G gastric tube was easily advanced through all AuraGain and LMA Supreme devices. Fiberoptic tracheal intubation was effectively achieved through all AuraGain and i-gel devices in less than 60 s. Lateral X-ray and neck dissections confirmed optimal alignment of all devices with the respiratory and digestive tracts. Conclusions: Insertion of the new AmbuAuraGain required adjusting manoeuvres in some cases, as observed with the other two devices, and achieved similar airway seal pressures. Passage of a large bore gastric tube was as fast as with the LMA Supreme and ease of guided intubation was similar to that of the i-gel.

Human skeletal muscle perilipin 2 and 3 expression varies with insulin sensitivity

Background: Impaired insulin sensitivity may partly arise from a dysregulated lipid metabolism in human skeletal muscle. This study investigates the expression levels of perilipin 2, 3, and 5, and four key lipases in human skeletal muscle from the subjects that exhibit a range from normal to very low insulin sensitivity. Methods: 25 middle aged male participants were matched for lean body mass and recruited into three groups;type 2 diabetes patients (T2D), impaired glucose tolerance (IGT), and healthy sedentary controls (CON) according to their glucose tolerance and VO2peak. A muscle biopsy was obtained from vastus lateralis, and a two-step sequential euglycaemic-hyperinsulinaemic clamp was performed. Muscle samples were analyzed by Western blot for expression of perilipin 2, 3, 5, adipose triglyceride lipase (ATGL), hormone-sensitive lipase (HSL), endothelial lipase (EL) and lipoprotein lipase (LPL). Results: Perilipin 3 expression was higher in T2D compared to CON. Perilipin 2 expression was higher in CON than T2D. We observed no difference in expression of perili pin 5, ATGL, HSL, EL or LPL between the groups. Conclusions: In the present study the muscle perilipin 3 expression and perilipin 2 expression varied markedly with insulin sensitivity. This difference in perilipin expression may indicate that the lipid droplet function and thus storage and release of fatty acid-vary with insulin sensitivity.

Internal ribosome entry site-based vectors for combined gene therapy

Gene therapy appears as a promising strategy to treatincurable diseases. In particular, combined gene therapy has shown improved therapeutic efficiency. Internal ribosome entry sites(IRESs), RNA elements naturally present in the 5' untranslated regions of a few m RNAs, constitute a powerful tool to co-express several genes of interest. IRESs are translational enhancers allowing the translational machinery to start protein synthesis by internal initiation. This feature allowed the design of multi-cistronic vectors expressing several genes from a single m RNA. IRESs exhibit tissue specificity, and drive translation in stress conditions when the global cell translation is blocked, which renders them useful for gene transfer in hypoxic conditions occurring in ischemic diseases and cancer. IRES-based viral and non viral vectors have been used successfully in preclinical and clinical assays of combined gene therapy and resulted in therapeutic benefits for various pathologies including cancers, cardiovascular diseases and degenerative diseases.
