Management of obstructed colorectal carcinoma in an emergency setting:An update

Colorectal carcinoma is common,particularly on the left *** 20%of patients,obstruction and ileus may be the first clinical manifestations of a carcinoma that has advanced(stage II,III or even IV).Diagnosis is based on clinical presentation,plain abdominal radiogram,computed tomography(CT),CT colonography and positron emission tomography/*** best management strategy in terms of short-term operative or interventional and long-term oncological outcomes re-mains *** the most common left-sided obstruction,the first choice should be either emergency surgery or endoscopic decompression by self-expen-dable metal stents or *** operative plan should be either one-stage or two-stage ***-stage resection with on-table bowel decompression and irrigation can be accompanied or not accompanied by proximal defunctioning stoma(colostomy or ileostomy).Primary anastomosis is more convenient but has increased risks of anastomotic leakage and ***-stage resection(Hart-mann’s procedure)is safer and the most widely used despite temporally affecting quality of *** control surgery in high-risk frail patients is less frequently performed since it can be successfully substituted with endoscopic stenting or *** the less common right-sided obstruction,one-stage surgical resection is more beneficial than endoscopic *** role of minimally invasive surgery(laparoscopic or robotic)is a subject of *** laparoscopic-assisted management is advantageous to some extent but requires much expertise due to inherent difficulties in dissecting the distended colon and the risk of rup-ture and subsequent septic *** decompressing stent as a bridge to elective surgery more substantially decreases the risks of morbidity and mortality than emergency surgery for decompression and has equivalent medium-term overall survival and disease-free survival *** combination with neoadjuvant chemotherapy or radiation may have a p

Enhanced heavy and extra heavy oil recovery: Current status and new trends

Due to the increased demand for energy resources these days, especially due to the Russian-Ukrainian war, the focus of the major countries is turning strongly towards improving oil production, especially heavy and extra heavy oil, which represents 40% of the world oil reserve. Steam-based and thermal (EOR) procedures are promising techniques for recovering heavy oil reservoirs, but they suffer from a sequence of problems and complications that arise after long-term application. These complications comprise steam breakthrough, steam overlap, and steam/rock interactions. This research presents the currently applied techniques to maximize the productivity of heavy oil, such as steam injection, cyclic steam stimulation, in-situ combustion, and steam-assisted gravity drainage. Thermal technologies face numerous obstacles, as they are energy and water-intensive processes that are not environmentally friendly. The research also presents future trends in energy-saving and environmentally friendly techniques that enhance heavy oil recovery through vapor extraction (VAPEX) steam-solvent hybrid techniques, electromagnetic energy, sonication, and nanotechnology. The findings of this review reported that all the presented techniques focus on how to reduce the oil viscosity and in-situ upgrade the crude oil properties. In turn, these enhance both the productivity rate and oil recovery and minimize the production cost. This article can be considered a comprehensive review of thermal recovery methods in heavy and extra-heavy oil, in addition to screening criteria used for each method. Tuning structural,electrical,dielectric and magnetic properties of Mg-Cu-Co ferrites via dysprosium(Dy^(3+))doping

In current research work,Dy3+substituted Mg0.5Cu0.25Co0.25Fe2-xDyxO4(0.0≤x≤0.04 with the step interval of 0.01)soft ferrites were synthesized by the sol-gel auto combustion *** prepared samples were characterized by the techniques using X-ray diffraction(XRD),scanning electron microscopy(SEM),energy dispersive X-ray spectroscopy(EDS),Fourier transform infrared(FTIR)spectroscopy,curre nt-voltage(Ⅰ-Ⅴ)measurement,LCR meter,vibrating sample magnetometer(VSM)and *** data reveal that the average crystallite size is 49.71 nm and the lattice constant is 0.83703 nm for sample x=*** non-uniform grain growth was demonstrated by micrographs and impurity-free elemental composition was observed from EDX *** DC resistivity has an increasing and decreasing trend in ferromagnetic and paramagnetic regions with an increase in ***,the high resistivity is observed with the order of 1010Ω·cm,and the activation energy is 0.944 eV for samples x=*** dielectric parameters including dielectric constant,dielectric losses,and impedance gradually decrease with the increase in frequency from 8 Hz to 8 *** minimum dielectric loss at high frequency is found for sample x=*** coercivity(Hc)and saturation magnetization(Ms)are found in the ranges of 520.82-544.02 Oe and 20.5841-21.1473 emu/g,*** observations confirm that dysprosium(x=0.03)doped MCC-soft ferrites may be applicable in transformer cores,microwave absorbance,and telecommunication devices.

Scoring and Consequential Validity Evidence of Computer-and Paper-Based Writing Tests in Times of Change

Little is known about how the assessment modality,i.e.,computer-based(CB)and paper-based(PB)tests,affects language teachers’scorings,perceptions,and preferences and,therefore,the validity and fairness of classroom writing *** present mixed-methods study used Shaw and Weir’s(2007)sociocognitive writing test validation framework to examine the scoring and consequential validity evidence of CB and PB writing tests in EFL classroom assessment in higher *** handwritten and word-processed texts of 38 EFL university students were transcribed to their opposite format and assessed by three language lecturers(n=456 texts,152 per teacher)to examine the scoring validity of CB and PB *** teachers’perceptions of text quality and preferences for assessment modality accounted for the consequential validity evidence of both *** revealed that the assessment modality impacted teachers’scorings,perceptions,and *** teachers awarded higher scores to original and transcribed handwritten texts,particularly text organization and language *** teachers’perceptions of text quality differed from their ratings,and physical,psychological,and experiential characteristics influenced their preferences for assessment *** results have implications for the validity and fairness of CB and PB writing tests and teachers’assessment practices.

This study investigates the development of novel montmorillonite/castor oil blends to formulate sustainable lubricating greases to promote the replacement of petrochemicals industry-derived materials by substances obtained from renewable ***,the effect of the thickener concentration on the rheological,chemical,thermal,tribological properties,and atomic force microscopy(AFM)microstructure of these systems were *** results showed that the C20A nanoclay content could be used to modulate the viscosity values,the linear viscoelastic functions,and tribological properties of these montmorillonite *** general,these gel-like dispersions exhibited remarkable lubricant properties;the samples showed values of the friction coefficient and wear scars similar or lower than those obtained with model bentonite grease.

Early response of understory vegetation to the mass dieback of norway spruce in the European lowland temperate forest

Spruce-dominated forests are commonly exposed to disturbances associated with mass occurrences of bark *** dieback of trees triggers many physical and chemical processes in the ecosystem resulting in rapid changes in the vegetation of the lower forest *** aimed to determine the response of non-tree understory vegetation to the mass dieback of norway spruce(Picea abies)in the first years after the disturbance caused by the European spruce bark beetle(Ips typographus)*** study area was the Białowieża Biosphere Reserve covering the Polish part of the emblematic Białowieża Forest,in total 597km^(2).The main data source comprised 3,900 phytosociological relevés(combined spring and summer campaigns)collected from 1,300 systematically distributed forest sites in 2016–2018–the peak years of the bark beetle *** found that the understory responded immediately to mass spruce dieback,with the most pronounced changes observed in the year of the disturbance and the subsequent ***-tolerant forest species declined in the initial years following the mass spruce dieback,while hemicryptophytes,therophytes,light-demanding species associated with non-forest seminatural communities,as well as water-demanding forest species,*** acetosella,the most common understory species in the Białowieża Forest,showed a distinct fluctuation pattern,with strong short-term expansion right after spruce dieback,followed by a gradual decline over the next 3–4 years to a cover level 5 percentage points lower than before the ***,our study revealed that mass spruce dieback selectively affects individual herb species,and their responses can be directional and non-directional(fluctuation).Furthermore,we demonstrated that the mass dieback of spruce temporarily increases plant species diversity(α-diversity).

An intelligent active probing and trace-back scheme for IoT anomaly detection

Due to their simple hardware,sensor nodes in IoT are vulnerable to attack,leading to data routing blockages or malicious tampering,which significantly disrupts secure data *** Intelligent Active Probing and Trace-back Scheme for IoT Anomaly Detection(APTAD)is proposed to collect integrated IoT data by recruiting Mobile Edge Users(MEUs).(a)An intelligent unsupervised learning approach is used to identify anomalous data from the collected data by MEUs and help to identify anomalous nodes.(b)Recruit MEUs to trace back and propose a series of trust calculation methods to determine the trust of nodes.(c)The last,the number of active detection packets and detection paths are designed,so as to accurately identify the trust of nodes in IoT at the minimum cost of the network.A large number of experimental results show that the recruiting cost and average anomaly detection time are reduced by 6.5 times and 34.33%respectively,while the accuracy of trust identification is improved by 20%.

In recent years,the notion of resilience has been developed and applied in many technical areas,becoming exceptionally pertinent to disaster risk *** a disaster situation,accurate sensing information is the key to efficient recovery *** general,resilience aims to minimize the impact of disruptions to systems through the fast recovery of critical functionality,but resilient design may require redundancy and could increase *** this article,we describe a method based on binary linear programming for sensor network design balancing efficiency with *** application of the developed framework is demonstrated for the case of interior building surveillance utilizing infrared sensors in both twoand three-dimensional *** method provides optimal sensor placement,taking into account critical functionality and a desired level of resilience and considering sensor type and *** problem formulation,resilience requirements,and application of the optimization algorithm are described in *** of sensor locations with and without resilience requirements shows that resilient configuration requires redundancy in number of sensors and their intelligent *** tasks are successfully solved by the described method,which can be applied to strengthen the resilience of sensor networks by *** proposed methodology is suitable for large-scale optimization problems with many sensors and extensive coverage areas.
