Hyperplastic polyps have traditionally been considered not to have malignant potential.New pathological classification of serrated polyps and recent discoveries about the serrated pathway of carcinogenesis have revolutionized the concepts and revitalized the research in this area.Until recently,it has been thought that most colorectal cancers arise from conventional adenomas via the traditional tumor suppressor pathway initiated by a mutation of the APC gene,but it has been found thatthis pathway accounts for only approximately 70%-80% of colorectal cancer(CRC)cases.The majority of the remaining colorectal cancer cases follow an alternative pathway leading to CpG island methylator phenotype carcinoma with BRAF mutation and with or without microsatellite instability.The mechanism of carcinomas arising from this alternative pathway seems to begin with an activating mutation of the BRAF oncogene.Serrated polyposis syndrome is a relatively rare condition characterized by multiple and/or large serrated polyps of the colon.Clinical characteristics,etiology and relationship of serrated polyposis syndrome to CRC have not been clarified yet.Patients with this syndrome show a high risk of CRC and both sporadic and hereditary cases have been described.Clinical criteria have been used for diagnosis and frequent colonoscopy surveillance should be performed in order to prevent colorectal cancer.In this review,we try to gather new insights into the molecular pathogenesis of serrated polyps in order to understand their possible clinical implications and to make an approach to the management of this syndrome.

AIM: To investigate and compare the hepatogenic transdifferentiation of adipose tissue-derived stem cells (ADSC) and bone marrow-derived mesenchymal stem cells (BMSC) in vitro. Transdifferentiation of BMSC into hepatic cells in vivo has been described. Adipose tissue represents an accessible source of ADSC, with similar characteristics to BMSC.METHODS: BMSCs were obtained from patients under-going total hip arthroplasty and ADSC from human adi-pose tissue obtained from lipectomy. Cells were grown in medium containing 15% human serum. Cultures were serum deprived for 2 d before cultivating under similar pro-hepatogenic conditions to those of liver develop-ment using a 2-step protocol with sequential addition of growth factors, cytokines and hormones. Hepatic differ-entiation was RT-PCR-assessed and liver-marker genes were immunohistochemically analysed. RESULTS: BMSC and ADSC exhibited a fibroblastic mor-phology that changed to a polygonal shape when cells differentiated. Expression of stem cell marker Thy1 de-creased in differentiated ADSC and BMSC. However, the expression of the hepatic markers, albumin and CYPs increased to a similar extent in differentiated BMSC and ADSC. Hepatic gene activation could be attributed to in-creased liver-enriched transcription factors (C/EBPβ and HNF4α), as demonstrated by adenoviral expression vec-tors.CONCLUSION: Mesenchymal stem cells can be induced to hepatogenic transdifferentiation in vitro. ADSCs have a similar hepatogenic differentiation potential to BMSC, but a longer culture period and higher proliferation ca-pacity. Therefore, adipose tissue may be an ideal source of large amounts of autologous stem cells, and may become an alternative for hepatocyte regeneration, liver cell transplantation or preclinical drug testing.

A risk scoring system for seafood supply chain breaches and examination of freshwater fish imported to Australia

Legislative changes have altered the way imported edible seafood is inspected in Australia.Greater onus of responsibility has been placed on exporting countries to provide documentary evidence of adherence to internally recognized food safety standards.According to global trade agreements,any additional safety tests applied to freshwater fish imported into Australia must be justified.Therefore,the aim of this study was to develop a risk scoring method to provide justification for identifying countries as'Freshwater fish high risk'and to examine the seafood they export to Australia for seafood supply chain breaches.Scori ng was con ducted using six predictor variables,identified in the literature as important contributors to seafood supply chain breaches,to achieve the outcome variable,Country considered'Freshwater fish high risk.Sixty-seven fish fillets(9.55 kg)of the same species were examined from the third highest scoring country(Country 20)and 562(5.6 kg)whole fish from the sixth highest scoring country(Country 22).Country 20 had supply chain breaches of 28 macroscopic yellow cysts in one fillet.Two hundred and thirteen parasites and other supply chain breaches were identified in fish from Country 22,in eluding retai ned liver(91 per cent),visible mud(11 per cent),a variety of debris(16 per cent)and,depending on the commodity code,these fish were imported to Australia under full intestine(90 per cent),retained gills(89 per cent),and partial intestine(9 per cent).Three serious physical hazards were recovered from the edible portion of three'consumer-ready' fish and snails of Genus Lymnaea and Indoplanorbis were recovered from gill mud also from'consumer-readyz fish.The study showed variable results from the scoring system and vast differences in seafood supply chain breaches between the third and sixth highest scoring countries.

The global burden of illegal fishing is estimated to be extensive.Intricately constructed trade routes introduce illegally caught fish products into the global commercial market,including Australia.To date,no studies have investigated the potential for illegally caught fish to harbor zoonotic parasites.Any tests applied to fish imported to Australia must be scientifically justified according to international trade agreements.The primary aim of this study was to develop a risk scoring method that provides a scientific basis for the development of protocols to examine fish imported to Australia for zoonotic parasites.The secondary aim was to estimate and prioritize the provenance of fish,which may be high-risk areas for illegal,unreported,and unregulated(IUU)1 fishing.The third aim was to calculate the amount of unreported catch from each of the ten highest-risk countries.Scoring was conducted using seven predictor variables,which were identified in the published literature as important,within the forensics of IUU fishing,for identifying the“IUU or unreported catch risk”of each provenance.The unreported catch(UC)2 for the highest scoring provenances(1–10)was calculated after risk scoring.The highest and second highest scoring provenances,30 and 67,had 39.8%and 41.55%UC,respectively;Provenance 79,which had the tenth highest risk score,had 6.9%UC.Linear regression analysis showed a non-significant association between the size of the exclusive economic zone and UC.Number of commercial spp.was the greatest indicator of UC.The analysis showed that for every unit increase in the number of different commercial spp.available,there was an increase of 5.28 units in the percentage of UC.Mean provenance risk scores and percentage of UC were linearly related.There was a 79.4%decrease in the mean risk scores between provenances 1–5 and 6–10;a decrease was also observed in the UC between the two groups(33.7%and 15.5%,respectively).The proposed scoring method appears to be a good predictor of UC,with a clear association between th

Background: Both competitive and recreational sports are considered a positive practice that is good for human health and quality of life, so it is difficult to understand how young, energetic and apparently healthy people can die while playing their usual sports. When these events occur in elite athletes during monitored events, they normally receive considerable media attention. Objective: To describe the case of sudden death in a young professional athlete who was successfully aided and resuscitated outside our hospital, and to review the literature about sudden death in sports. Materials and Method: We searched for Sudden Death (according to the International Classification of Diseases (ICD-9 and ICD-10)) in our hospital but found no other relevant cases. We performed a literature search on the state of the art in sudden death (Google Scholar, PubMed, LILACS, Scielo and the Revista Española de Cardiología). Results: Although we observed that the prevalence of sudden death is not very high, it still represents a potentially avoidable event. An adequate medical examination prior to regular sporting practice can decrease the incidence of sudden death in athletes under the age of 35. Data available in the literature support medical check-ups that include history-taking, physical examination and electrocardiogram. The findings of this evaluation may also prompt further assessment (electrocardiograph, ergometry). Conclusions: We could not identify standard, obligatory guidelines for screening possible heart diseases causing cardiorespiratory failure and sudden death. As a measure for primary prevention, we propose a protocol for reducing the risk of sudden death in athletes.

Caregiver Burden in the Management of Frail Elderly Patients with Diabetes in Internal Medicine

Aim: Advanced age and fragility often lead to dependence, making caregiver assistance necessary for performing the activities of daily living. Patients with diabetes are characterized by an elevated presence of comorbidities and subsequent frailty—a relevant consideration when making decisions regarding treatment goals. Health professionals may overlook the caregiver experience when assessing the circumstances surrounding frail elderly patients with diabetes, but this factor can have a large impact on relevant caregiver and patient outcomes. Methods: An observational, retrospective study was carried out to assess the impact of improved management in diabetes on caregiver burden. We assessed patients’ performance of activities of daily living using the Barthel scale. Our primary outcome was caregiver burden, as assessed using the Zarit scale. Results: At baseline, 81.4% of the caregivers reported severe overburden (>55 points on the Zarit scale). At three months, the proportion of caregivers reporting this level of burden had dropped to 76.7% and remained stable at six months (77.1%). In contrast, 8.5% of the caregivers reported moderate overburden (47 to 55 points) at baseline, compared to 13.3% at three months and 12.5% at six. The proportion of caregivers reporting no overburden (Conclusions: Caregiver burden remained persistently high, decreasing only slightly after six months.

AIM:To evaluate changes in colorectal cancer(CRC) survival over the last 20 years.METHODS:We compared two groups of consecutive CRC patients that were prospectively recruited:Group Ⅰincluded 1990 patients diagnosed between 1980 and 1994.GroupⅡincluded 871 patients diagnosed in 2001.RESULTS:The average follow up time was 21 mo(1-229)for GroupⅠand 50 mo(1-73.4)for GroupⅡ.Overall median survival was significantly longer in Group Ⅱthan in GroupⅠ(73 mo vs 25 mo,P<0.001)and the difference was significant for all tumor stages.Post surgical mortality was 8% for GroupⅠand 2% for Group Ⅱ(P<0.001).Only 17% of GroupⅠpatients received chemotherapy compared with 50% of GroupⅡpatients(P<0.001).CONCLUSION:Survival in colorectal cancer patients has doubled over the past 20 years.This increase seems to be partly due to the generalization in the administration of chemotherapy and to the decrease of post surgical mortality.
