Colorectal carcinoma(CRC) is one of the most frequent cancers. Along the surface of the large bowel, several foci of CRC may appear simultaneously or over the time. The development of at least two different tumours has been defined as multiple primary CRC(MPCRC):When more than one tumour is diagnosed at the same time, it is known as synchronous CRC(SCRC), while when a second neoplasm is diagnosed some time after the resection and/or diagnosis of the first lesion, it is called metachronous CRC(MCRC). Multiple issues can promote the development of MPCRC, ranging from different personal factors, such as environmental exposure, to familial predisposition due to hereditary factors. However, most studies do not distinguish this dichotomy. High- and low-pentrance genetic variants are involved in MPCRC. An increased risk for MPCRC has been described in Lynch syndrome, familial adenomatous polyposis, and serrated polyposis. Non-syndromic familial CRCs should also be considered as risk factors for MPCRC. Environmental factors can promote damage to colon mucosae that enable the concurrence of MPCRC. Epigenetics are thought to play a major role in the carcinogenesis of sporadic MPCRC. The methylation state of the DNA depends on multiple environmental factors(e.g., smoking and eating foods cooked at high temperatures), and this can contribute to increasing the MPCRC rate. Certain clinical features may also suggest individual predisposition for MPCRC. Different etiopathogenic factors are suspected to be involved in SCRC and MCRC, and different familial vs individual factors may be implicated. MCRC seems to follow a familial pattern, whereas individual factors are more important in SCRC. Further studies must be carried out to know the molecular basis of risks for MPCRC in order to modify, if necessary, its clinical management, especially from a preventive point of view.

Ionospheric delay modeling is not only important for Global Navigation Satellite System(GNSS)based space weather study and monitoring,but also an efficient tool to speed up the convergence time of Precise Point Positioning(PPP).In this study,a novel model,denoted as Quasi-4-Dimension Ionospheric Modeling(Q4DIM)is proposed for wide-area high precision ionospheric delay *** Q4DIM,the Line Of Sight(LOS)ionospheric delays from a GNSS station network are divided into different clusters according to not only the location of latitude and longitude,but also satellite elevation and *** Global Ionosphere Map(GIM)and Slant Ionospheric Delay(SID)models that are traditionally used for wide-area and regional ionospheric delay modeling,respectively,can be regarded as the special cases of Q4DIM by defining proper grids in latitude,longitude,elevation,and ***,Q4DIM presents a resilient model that is capable for both wide-area coverage and high *** different sets of clusters are defined to illustrate the properties of Q4DIM based on 200 EUREF Permanent Network(EPN)*** results indicate that Q4DIM is compatible with the GIM ***,it is proved that by inducting the elevation and azimuth angle dependent residuals,the precision of the 2-dimensional GIM-like model,i.e.,Q4DIM 2-Dimensional(Q4DIM-2D),is improved from around 1.5 Total Electron Content Units(TECU)to better than 0.5 *** addition,treating Q4DIM as a 4-dimensional matrix in latitude,longitude,elevation,and azimuth,whose sparsity is less than 5%,can result in its feasibility in a bandwidth-sensitive applications,e.g.,satellite-based Precising Point Positioning Real-Time Kinematic(PPP-RTK)***,the advantages of Q4DIM in PPP over the 2-dimensional models are demonstrated with the one month's data from 30 EPN stations in both high solar activity year 2014 and low solar activity year 2020.

As a new Ionosphere Associate Analysis Center(IAAC)of the International GNSS Service(IGS),Chinese Academy of Sciences(CAS)started the routine computation of the real-time,rapid,and final Global Ionospheric Maps(GIMs)in *** method for the generation of CAS rapid and final GIMs and recent updates are presented in the *** quality of CAS post-processed GIMs is assessed during 2015-2018 after the maximum of solar cycle *** perform an independent and fair assessment,Jason-2/3 Vertical Total Electron Contents(VTEC)are first used as the references over the *** differential Slant TECs(dSTEC)generated from 55 Multi-GNSS Experimental(MGEX)stations of the IGS are also employed,which provides a complementing way to evaluate the ability of electron content models to reproduce the spatial and temporal gradients in the *** the test period,Jet Propulsion Laboratory(JPL)GIMs present significantly positive deviations compared to the Jason VTEC and GPS *** University of Catalonia(UPC)rapid GIM UQRG exhibits the best performance in both Jason VTEC and GPS dSTEC *** CAS GIMs show comparable performance with the results of the first four IAACs of the *** expected,the poor performance of all GIMs is in equatorial regions and the high latitudes of the southern *** consideration of generating multi-layer or three-dimensional ionospheric maps is emphasized to mitigate the inadequacy of ionospheric single-layer assumption in the presence of pronounced latitudinal *** use of ionospheric observations from the new GNSS constellations and other space-or ground-based observation techniques is also suggested in the generation of future GIMs,given the sparse GPS/GLONASS stations in the southern hemisphere.

The aim of this review is to compare the effectiveness of percutaneous vertebroplasty and kyphoplasty to treat pain and improve functional outcome from ver-tebral fractures secondary to osteoporosis and tumor conditions. In 2009, two open randomized controlled trials published in the New England Journal of Medicine questioned the value of vertebroplasty in treating ver-tebral compression fractures. Nevertheless, the prac-tice of physicians treating these conditions has barely changed. The objective of this review is to try to clarify the most important issues, based on our own experi-ence and the reported evidence about both techniques, and to guide towards the most appropriate choice of treatment of vertebral fractures, although many ques-tions still remain unanswered.

Driving Skills in HIV-Infected Patients Well Controlled with Antiretroviral Therapy

A study was made to determine whether HIV-infected patients with prolonged virological control suffer subclinical neurocognitive problems capable of interfering with driving skills, compared with the general population, and to explore the possible existence of differences between those treated with and without efavirenz. Material and Methods: We included 40 patients without history of neoplasm, psychiatric disorders or infections of the central nervous system associated or not to HIV, with stable and effective antiretroviral therapy during at least 48 months. Use was made of the ASDE DRIVER TEST N-845 standardized by the Spanish traffic authorities, and for which data corresponding to the Spanish general population were obtained from the manufacturer of the test battery. The Student t-test was used to compare the different variables with the population standards, and the comparison of proportions Z-statistic was used to determine the proportion of subjects above the accepted limit of normality cutoff point. These analyses were replicated for the two sub-samples (with or without efavirenz therapy), with a 95% confidence level. The SPSS version 15 statistical package and Epidat 3.1 program were used. Results: The scores obtained in the HIV group were significantly poorer in the anticipation speed tests and in one of the multiple reactions test, though better results were obtained in the bimanual visual-motor coordination test. There were no differences in the percentages of patients with scores below the recommended limits. On comparing the treatment subgroups (efavirenz versus protease inhibitor), no differences were recorded in any of the study variables, and the differences with respect to the general population were the same as those described for the global group. OBJECTIVES: In recent years, the percentage of heart transplantation (HT) with short/medium-term assistance devices has increased. This study aims at analyzing primary graft failure and in-hospital mortality according to the type of care. MATERIAL AND METHODS: From January 2013 to December?2017 all patients undergoing urgent HT with circulatory/ventricular assistance?were retrospectively and consecutively recruited. Combined transplants, retransplantations and pediatric transplants were excluded. The sample was divided in 10 groups according to the type of shot/medium term assistance devices. RESULTS: A total of 53 patients were recruited, 79% men, average age 49 ± 13 years. 26 patients (51%) had mechanical ventilation at the time of the HT. Primary graft failure occurred in 20 patients (38.5%), whilst it was more frequent in patients assisted with ECMO (8 patients in group 1 (45%) and 7 in group 2 (54%), p 0.5). 14 deaths (27%) were registered, whereas a higher mortality was observed in the group assisted with ECMO (6 patients in group 1 (34%) and 4 in group 2 (31%), p 0.6). Mortality was 17% in the group of non-urgent Levitronix. CONCLUSIONS: Despite not showing statistical significance due to the low number of patients in some subgroups, hospital?mortality was high in HT patients with circulatory assistance. A trend towards a higher incidence of primary graft failure and mortality in patients assisted with ECMO was observed. The direct implantation of a centrifugal pump as a bridge to urgent transplantation could identify a subgroup with a better prognosis.
