Background: Due to improvements in diagnosis, the better outcomes of oncological patients and the increase in the average age, the incidence of synchronous tumours is likely to increase. Aim: To reflect on the challenges of a case with multiple integrated diagnostic and therapeutic approaches and to bring to consideration the increasing prevalence of similar situations. Case Presentation: In this clinical case, the authors describe the evolution of an asymptomatic patient with several synchronous tumours (a GEJ primary adenocarcinoma, a low grade urothelial carcinoma in situ, a localized squamous cell lung carcinoma and 2 IPMNs). Conclusion: The challenge of this case lies in the difficult diagnostic approach, the assembly of a multidisciplinary and time-sensitive treatment plan and the individualized follow-up, due to lack of guidelines. More research is needed in this area.

The majority of nanoparticles tend to agglomerate in bacterial growth media. Thus, nanoparticle-specific characteristics can get lost. To investigate the influence of nanoparticles on bacteria, these particles should remain in their nanoparticulate state. The present study demonstrates the stabilization of commercially available zinc oxide (ZnO) with sodiumhexametaphosphate (SHMP) in bacterial growth medium (LB) to avoid agglomeration of these particles after the addition to LB. This established method is appropriate to stabilize ZnO agglomerates as small as 43 nm. The method of fractionated centrifugation was used to obtain stable agglomerates (also stable in the presence of bacteria) with different mean diameters. The SHMP-stabilized ZnO inhibits the growth of Pseudomonas putida with increasing concentration (up to 500 mg/L) and decreasing agglomerate size (43 - 450 nm).

Case Report: A Long Survivor Patient with Stage IV Gastric Adenocarcinoma

Gastric cancer (GC) is the fifth most common malignancy of the world and third leading cause of cancer death. At diagnosis, 35% of GC patients have distant metastases and in these cases the survival rate is very poor with a median overall survival (OS) inferior to 1 year. We report a case of a 67-year-old woman with gastric carcinoma initially deemed limited stage on diagnosis (cT2N0M0), treated surgically with radical subtotal gastrectomy with Billroth II reconstruction. In the staging CT scan, the patient presented a liver image that was considered benign. Three months later, due to abdominal pain, the patient performed another CT scan and the diagnostic of a large single liver metastasis was made;retrospectively it was observed that the lesion was present at diagnosis and that it had increased. A biopsy was performed which confirmed the metastatic origin. In a multidisciplinary team, the lesion was considered unresectable. She was proposed for first line (1st L) palliative chemotherapy (ChT) with FOLFIRI, with partial response as best response. After 30 cycles of FOLFIRI, bone metastases were diagnosed. The patient was submitted to a cementoplasty of D11-12 and L1-2. Afterwards, she started 2nd L ChT with mFOLFOX6 and at the same time she started zoledronic acid every 28 days. The best response to mFOLFOX6 was stable disease. Since November 2018, the patient has been treated with zoledronic acid every 28 days alone and maintains stable disease without ChT.

利用可见-近红外激光漫反射光谱图像法及化学计量学方法检测了苹果的可溶性固形物含量和硬度。为了消除苹果形状对品质检测的影响,分析了5个不同波长的激光(680,780,880,940和980 nm)照射在富士苹果果面形成的光斑图像,并通过一系列图像处理方法得出了的苹果形状修正算法。分别利用修正前后的光谱图像参数建立苹果可溶性固形物含量和硬度的偏最小二乘回归(PLSR)模型,模型预测值和真实值之间的相关系数从0.78和0.80分别提高到0.87和0.89。为了消除果面镜面反射对苹果内部品质检测的影响,文章提出了象素强度频率检测法,并比较了利用果面光斑图像中光强均值和不同强度对应的象素强度频率建立的苹果硬度和可溶性固形物含量的PLSR模型效果,得出象素强度频率的模型效果更好。苹果可溶性固形物含量和硬度PLSR模型中预测均方根误差(RMSEP)分别从1.28°Brix和8.23 ***-2降低到0.84°Brix和6.17 ***-2。

Right ventricle dysfunction does not predict mortality in patients with SARS-CoV-2-related acute respiratory distress syndrome on extracorporeal membrane oxygenation support

BACKGROUND The prognostic role of right ventricle dilatation and dysfunction(RVDD)has not been elucidated in patients with coronavirus disease(COVID)-related respiratory failure refractory to standard treatment needing extracorporeal membrane oxygenation(ECMO)*** To assess whether pre veno-venous(VV)ECMO RVDD were related to inintensive care unit(ICU)*** We enrolled 61 patients with COVID-related acute respiratory distress syndrome refractory to conventional treatment submitted to VV ECMO and consecutively admitted to our ICU(an ECMO referral center)from 31th March 2020 to 31th August *** echocardiographic exam was performed immediately before VV ECMO *** Males were prevalent(73.8%)and patients with a body mass index>30 kg/m^(2) were the majority(46/61,75%).The overall in-ICU mortality rate was 54.1%(33/61).RVDD was detectable in more than half of the population(34/61,55.7%)and associated with higher simplified organ functional assessment(SOFA)values(P=0.029)and a longer mechanical ventilation duration prior to ECMO support(P=0.046).Renal replacement therapy was more frequently needed in RVDD patients(P=0.002).A higher in-ICU mortality(P=0.024)was observed in RVDD *** echo variables were independent predictors of in-ICU *** In patients with COVID-related respiratory failure on ECMO support,RVDD(dilatation and dysfunction)is a common finding and identifies a subset of patients characterized by a more severe disease(as indicated by higher SOFA values and need of renal replacement therapy)and by a higher in-ICU ***(also when considered separately)did not result independently associated with in-ICU mortality in these patients.

BACKGROUND In brain death donors(BDDs),donor management is the key in the complex donation *** management goals,which are standards of care or clinical parameters,have been considered an acceptable barometer of successful donor *** To test the hypothesis that aetiology of brain death could influence haemodynamic management in *** Haemodynamic data(blood pressure,heart rate,central venous pressure,lactate,urine output,and vasoactive drugs)of BDDs were recorded on intensive care unit(ICU)admission and during the 6-h observation period(Time 1 at the beginning;Time 2 at the end).RESULTS The study population was divided into three groups according to the aetiology of brain death:Stroke(n=71),traumatic brain injury(n=48),and postanoxic encephalopathy(n=19).On ICU admission,BDDs with postanoxic encephalopathy showed the lowest values of systolic and diastolic blood pressure associated with higher values of heart rate and lactate and a higher need of norepinephrine and other vasoactive *** the beginning of the 6-h period(Time 1),BDDs with postanoxic encephalopathy showed higher values of heart rate,lactate,and central venous pressure together with a higher need of other vasoactive *** According to our data,haemodynamic management of BDDs is affected by the aetiology of brain *** with postanoxic encephalopathy have higher requirements for norepinephrine and other vasoactive drugs.

The hepatitis C virus(HCV) infection is an important public health problem and it is associated with hepatic and extrahepatic manifestations. Autoimmune thyroid diseases are common in HCV infected patients and the standard interferon-based treatment is associated with an increase of the immune-mediated thyroid damage. Recent evidence in the literature analyzed critical points of the mechanisms of thyroid damage, focusing on the balance between the two sides of the interaction: The environment(virus infection with potential crossreaction) and the host(susceptibility genes with consistent immune response). The spectrum of antiviral treatment for chronic HCV infection is rapidly expanding for the development of dual o triple therapy. The availability of interferon-free combined treatment with direct antiviral agents for HCV is very promising, in order to ameliorate the patient compliance and to reduce the development of thyroid autoimmunity.

Objectives:In recent years,there has been a substantial increase in the global consumption of honey,driven by the high demand for natural products that offer health ***,consumers show a preference for honey,recognised for its superior quality,namely honey with a Protected Designation of Origin(PDO)or honey produced in protected areas,as it is associated with superior attributes and *** is one of the leading countries in the production of PDO honey,with Montesinho Natural Park(MNP)being an excellent example of high-quality Portuguese honey,known for its distinctive ***,environmental threats pose a double challenge,leading to a decline in honey production and compromising its overall *** study aimed to assess the specifc physicochemical parameters and nutritional characteristics of MNP honey and investigate their correlation with the different locations of *** and Methods:MNP honey samples(n=13)were obtained from local producers or purchased at *** physicochemical parameters,such as 5-hydroxymethylfurfural,diastase activity,moisture and ash content,free acidity,electrical conductivity and pH,were determined according to honey ***,colour parameters,protein,low-molecular-weight carbohydrates(monoand disaccharides),and mineral content were ***:The results obtained were consistent with the requirements outlined in the legislation and those described in the *** results suggest that geographical factors within the park and boundaries do not contribute to variations in the analysed ***:A signifcant level of homogeneity was evident in all parameters evaluated among the MNP honey *** is the frst comprehensive study of the physicochemical properties of honey from various apiaries within the MNP.
