Chemical characterisation of particulate matter in urban transport modes

Traffic is a main source of air pollutants in urban areas and consequently daily peak exposures tend to occur during *** exposure to particulate matter(PM)was monitored while cycling and travelling by bus,car and metro along an assigned route in Lisbon(Portugal),focusing on PM_(2.5) and PM_(1)0(PM with aerodynamic diameterspectively)mass concentrations and their chemical *** vehicles,the indoor-outdoor interplay was also *** PM_(2.5) mean concentrations were 28±5,31±9,34±9 and 38±21μg/m 3 for bus,bicycle,car and metro modes,*** carbon concentrations when travelling by car were 1.4 to 2.0 times higher than in the other transport modes due to the closer proximity to exhaust *** are marked differences in PM chemical composition depending on transport *** particular,Fe was the most abundant component of metro PM,derived from abrasion of rail-wheel-brake *** concentrations of Zn and Cu in cars and buses were related with brake and tyre wear particles,which can penetrate into the *** the motorised transport modes,Fe,Zn,Cu,Ni and K were correlated,evidencing their common traffic-related *** average,the highest inhaled dose of PM_(2.5) was observed while cycling(55μg),and the lowest in car travels(17μg).Cyclists inhaled higher doses of PM_(2.5) due to both higher inhalation rates and longer journey times,with a clear enrichment in mineral *** presented results evidence the importance of considering the transport mode in exposure assessment studies.

Experimental assessment and prediction of temporal scour depth around a spur dike

spur dikes are river training structures that have been extensively used worldwide for towards enhancing flood control and the stability of embankments and ***,scour around spur dikes can be a major problem affecting their stability and hydraulic *** precise computation of temporal scour depth at spur dikes is very important for the design of economical and safe spur *** study focuses on experimentally assessing the temporal variation of scour depth around a vertical wall spur dike and identifying the parameters,which mostly influence spur dike performance for a channel bed surface comprised of sand-gravel *** the current study,the authors did physical experiments in a flume based study to obtain new data,aimed at deriving a new predictive model for spur dike scour and comparing its performance to others found in the *** was found that the dimensionless temporal scour depth variation increases with an increase in(i) the threshold velocity ratio,(ii) the densimetric Froude number of the bed surface sediment mixture,(iii) the flow shallowness(defined as the ratio of the approach flow depth,y,to the spur dike’s transverse length,l),and(iv) the flow depth-particle size *** is also concluded that the temporal scour depth variation in the sediment mixture is influenced by the non-uniformity of sediment and decreases with an increase in the non-uniformity of the sediment mixture.A new mathematical model is derived for the estimation of temporal scour depths in sand-gravel sediment *** proposed equation has been calibrated and validated with the experimental data,demonstrating a good predictive capacity for the estimation of temporal scour depth evolution.

subclinical peritonitis due to perforated sigmoid diverticulitis 14 years after heart-lung transplantation

Acute complicated diverticulitis, particularly with colon perforation, is a rare but serious condition in transplant recipients with high morbidity and mortality. Neither acute diverticulitis nor colon perforation has been reported in young heart-lung grafted patients. A case of subclinical peritonitis due to perforated acute sigmoid diverticulitis 14 years after heart-lung transplantation is reported. A 26-year-old woman, who received heart-lung transplantation 14 years ago, presented with vague abdominal pain. Physical examination was normal. Blood tests revealed leukocytosis. Abdominal X-ray showed air-fluid levels while CT demonstrated peritonitis due to perforated sigmoid diverticulitis. sigmoidectomy and end-colostomy (Hartmann's procedure) were performed. Histopathology confirmed perforated acute sigmoid diverticulitis. The patient was discharged on the 8th postoperative day after an uneventful postoperative course. This is the first report of acute diverticulitis resulting in colon perforation in a young heart-lung transplanted patient. Clinical presentation, even in peritonitis, may be atypical due to the masking effects of immunosuppression. A high index of suspicion, urgent aggressive diagnostic investigationof even vague abdominal symptoms, adjustment of immunosuppression, broad-spectrum antibiotics, and immediate surgical treatment are critical. Moreover, strategies to reduce the risk of this complication should be implemented. Pretransplantation colon screening, prophylactic pretransplantation sigmoid resection in patients with diverticulosis, and elective surgical intervention in patients with nonoperatively treated acute diverticulitis after transplantation deserve consideration and further studies.

The treatment of patients with inflammatory bowel disease(IBD),especially those with severe or refractory disease,represents an important challenge for the clinical *** seems to be no exaggeration to say that in these patients,not only the scientific background of the gastroenterologist is tested,but also the abundance of“gifts”that he should possess(insight,intuition,determ-ination,ability to take initiative,etc.)for the successful outcome of the *** daily clinical practice,depending on the severity of the attack,IBD is treated with one or a combination of two or more pharmaceutical *** combin-ations include not only the first-line drugs(e.g.,mesalazine,corticosteroids,antibiotics,etc)but also second-and third-line drugs(immunosuppressants and biologic agents).It is a fact that despite the significant therapeutic advances there is still a significant percentage of patients who do not satisfactorily respond to the treatment ***,a part of these patients are going to *** recent years,several small-size clinical studies,reviews,and case reports have been published combining not only biological agents with other drugs(e.g.,immunosuppressants or corticosteroids)but also the combination of two biologi-cal agents simultaneously,especially in severe *** our opinion,it is at least a strange(and largely unexplained)fact that we often use combinations of drugs in a given patient although studies comparing the simultaneous administration of two or more drugs with monotherapy are very *** mentioned above,there is a timid tendency in the literature to combine two biological agents in severe cases unresponsive to the applied treatment or patients with severe extraintestinal *** appropriate dosage,the duration of the administration,the suitable timing for checking the clinical and laboratory outcome,as well as the treatment side-effects,should be the subject of intense clinical research *** this

Molecular markers(PECAM-1,ICAM-3,HLA-DR)determine prognosis in primary non-Hodgkin's gastric lymphoma patients

AIM： To investigate the prognostic significance of PECAM-1, ICAM-3 and HLA-DR antigens in patients with primary non-Hodgkin＇s gastric lymphoma. METHODs： We immunohistochemically studied PECAM-1, ICAM-3 and HLA-DR antigen expression in 36 B-cell MALT-type primary gastric lymphoma patients. Ten non-malignant and ten healthy gastric tissue specimens were used as controls. Clinicopathological and survival data were correlated with the staining results. REsULTs： HLA-DR antigen expression was detected in 33 gastric lymphoma patients （91.7%） and 6 nonmalignant patients （54.5%）. PECAM-1 stained tumor cells of 10 patients （27.8%）, endothelial cells of 9 patients （25%） and inflammatory infiltrate of 4 patients （40%） with benign gastric disease. ICAM-3 expression was observed on the tumor cells of 17 patients （47.2%）, while 5 non-malignant patients （50%） were stained positive as well. None of the healthy controls was stained for any of the genes studied. In the multivariate analysis, HLA-DR antigen and PECAM-1 were proved to be statistically significant independent prognostic factors associated with a favourable and an unfavourable prognosis respectively （P= 0.009 and P= 0.003）. In the univariate analysis, PECAM-1（＋）/ICAM-3（-） and HLA-DR（-）/ICAM-3（-） patients exhibited a significantly decreased overall survival compared to those with the exactly opposite gene expression patterns （P=0.0041 and P= 0.0091, respectively）. Those patients who were HLA-DR（＋）/ICAM-3（＋）/PECAM-I（-）（n = 8） had a significantly higher survival rate compared to the rest of the group （n = 24）（P= 0.0289）. CONCLUsION： PECAM-1, ICAM-3 and HLA-DR are representative markers of tumor expansion potential and host immune surveillance respectively. Their combined use may help us to identify high-risk patients who could benefit from more aggressive therapeutic protocols.

Effects of albumin/glutaraldehyde glue on healing of colonic anastomosis in rats

To evaluate the effect of local surgical adhesive glue (albumin/glutaraldehyde-Bioglue) on the healing of colonic anastomoses in rats. METHODsForty Albino-Wistar male rats were randomly divided into two groups, with two subgroups of ten animals each. In the control group, an end-to-end colonic anastomosis was performed after segmental resection. In the Bioglue group, the anastomosis was protected with extraluminar application of adhesive glue containing albumin and glutaraldehyde. Half of the rats were sacrificed on the fourth and the rest on the eighth postoperative day. Anastomoses were resected and macroscopically examined. Bursting pressures were calculated and histological features were graded. Other parameters of healing, such as hydroxyproline and collagenase concentrations, were evaluated. The experimental data were summarized and computed from the results of a one-way ANOVA. Fisher’s exact test was applied to compare percentages. REsULTsBursting pressures, adhesion formation, inflammatory cell infiltration, and collagen deposition were significantly higher on the fourth postoperative day in the albumin/glutaraldehyde group than in the control group. Furthermore, albumin/glutaraldehyde significantly increased adhesion formation, inflammatory cell infiltration, neoangiogenesis, and collagen deposition on the eighth postoperative day. There was no difference in fibroblast activity or hydroxyproline and collagenase concentrations. CONCLUsIONAlbumin/glutaraldehyde, when applied on colonic anastomoses, promotes their healing in rats. Therefore, the application of protective local agents in colonic anastomoses leads to better outcomes.
