EUS-guided Gall Bladder Drainage in Severe Liver Disease: A Single-center Experience in Critically Ill Cirrhotics

Background and Aims:Acute calculous cholecystitis with impending gall bladder perforation in severe liver diseases including decompensated cirrhosis and acute-on-chronic liver failure(ACLF)is difficult to manage,due to the procedures such as cholecystectomy and percutaneous cholecystostomy being associated with high risk and complications in these ***:Four cases of severe liver disease with acute calculous cholecystitis who presented to the Institute of Liver and Biliary Sciences(New Delhi,India)for further management were included in the study if they underwent endoscopic ultrasound-guided gall bladder drainage(EUS-GBD).The patients were followed up for a minimum of 3 months and outcomes were ***:Four cases of severe liver disease(three ACLF and one decompensated cirrhosis),with model for end-stage liver disease scores of 24,26,23 and 25 respectively,presented with acute calculous cholecystitis(Tokyo grade III)and systemic sepsis(high total leukocyte counts).Their international normalized ratios were 2.3,2.6,2.2 and 2.9 respectively,and two were in shock,requiring inotropes at *** of the abdomen confirmed hugely distended gall bladder with stone impacted at the neck and moderate *** these cases underwent EUS-GBD by linear echo endoscope,and had the gastric wall punctured in the antrum using a 19G access needle followed by dilatation of the tract using controlled radial expansion balloon and Sohendra *** three cases,the plastic stents were *** the fourth case,a Nagi stent was *** the patients recovered and were discharged within a ***:EUS-GBD is challenging in severe liver disease but represents a life-saving procedure,and hence can be attempted in such critically ill patients with utmost care and precaution.

Performance of Photovoltaic-double skin façade(Photovoltaic-DSF)system in summer has been *** to high solar ingress,cooling requirement of a building significantly ***-DSF system provides a shield and controls the heat gain through fenestration in the interior *** the present article,mathematical correlations are developed for energy characterization of forced-ventilated Photovoltaic-DSF system in India’s hot summer zone *** Photovoltaic-DSF system has been installed and monitored for Jaipur’s summer months(May to July).L25 Orthogonal array of design parameters(air cavity thickness,air velocity,and PV panel’s transparency)and their respective levels have been developed using Taguchi design to perform *** on experimental results,multiple linear regression has been used to forecast solar heat gain coefficient,PVs electrical power and daylighting illuminance indoors as function of design *** statistical significance of mathematical relationships is sorted by variance analysis,which is found to be in good accord with field measurements(R2>0.90).The proposed correlations are pragmatic in designing Photovoltaic-DSF systems for hot summer *** Photovoltaic-DSF system with 30%transmittance and air velocity of 5 metres per second in 200 mm air cavity thickness achieved maximum energy performance in hot summers.

Rapid urbanisation and quest for better livelihood, push-pull factor of occupations education, policy changes attract large scale rural population to urban areas. It is well documented that in spite of better public health facilities including tertiary care hospitals which are available in the urban areas but the services are underutilised by the urban poor. Aim: Hence, in this paper, it is attempted to comprehend the reasons for underutilisation of available public health facilities and to compare the difference with non-slum areas of the major metropolitan cities of India. Methods: A secondary data from National Family Health Survey-III for five major metropolitan cities namely, Delhi, Hyderabad, Mumbai, Kolkata and Chennai is used for the analysis. Slum data which are classified by both the agencies, that is census of India and NFHS-III as slum households only considered for analysis. Results: In Mumbai slums about 90% of the households are having water sources from public tap or piped to yard followed by Hyderabad having better water supply and Chennai slum dwellers having minimum access to good water sources. About 11.4% of the households do not know where their toilet drainage is connected. There is a significant (P 0.05) 75% to 79%, Kolkata and Chennai have parallel high proportion, that is more than 95% (P > 0.05) and Mumbai stands as median percent age as 89.6. Apart from Delhi, about 40% to 45% of the slum population is in the opinion of “long waiting time” in the government hospitals, and the same trend of proportions is observed for “poor quality of service”.

Background: Abstinence and anger control are promoted as ways to improve quality of life. Trait anger has been seen as one casual factor for alcohol related aggression and poor quality of life. The present study assesses anger control, quality of life and its relation with trait anger among alcohol dependents, abstainers & social drinkers. Material & Method: 150 subjects (50 subjects in each group) in the age range of 20 - 45 years were taken for the study. Semi-structured interview schedule, State-Trait Anger Expression Inventory-2, WHOQOL-BREF were administered in individual setting. Results: Dependent alcohol users and abstainers reported the significant impact of alcohol use on family & occupational functioning. Quality of life has correlation with anger control (in/out), trait anger, temperament & reaction in the control & abstainers group, whereas quality of life has negative correlation with trait anger expression among the alcohol group. Conclusions: Alcohol use has been associated with the trait anger expression and poor quality of life. It has implications for promoting alcohol abstinence, in improving quality of life and in anger control.

BACKGROUND Internal hernia(IH)is a rare culprit of small bowel obstruction(SBO)with an incidence of<1%.It poses a considerable diagnostic challenge requiring a high index of suspicion to prevent misdiagnosis,improper treatment,and subsequent morbidity and *** To determine the clinico-demographic profile,radiological and operative findings,and postoperative course of patients with IH and its association with *** Medical records of 586 patients with features of SBO presenting at a tertiary care centre at Lucknow,India between September 2010 and August 2023 were *** Out of 586 patients,7(1.2%)were diagnosed with *** these,4 had congenital IH and 3 had acquired *** male-to-female ratio was 4:*** median age at presentation was 32 ***-enhanced computed tomography(CECT)was the most reliable investigation for preoperative identification,demonstrating mesenteric whirling and clumped-up bowel *** paraduodenal hernia and transmesenteric hernia occurred with an equal frequency(approximately 43%each).Intraoperatively,one patient was found to have bowel ischemia and one had associated malrotation of *** follow-up,no recurrences were *** IH,being a rare cause,must be considered as a differential diagnosis for SBO,especially in young patients in their 30s or with unexplained abdominal pain or discomfort post-surgery.A rapid imaging evaluation,preferably with CECT,is necessary to aid in an early diagnosis and prompt intervention,thereby reducing financial burden related to unnecessary investigations and preventing the morbidity and mortality associated with closed-loop obstruction and strangulation of the bowel.

Friction stir processing(FSP)is an emerging solid-state technique for preparing surface composites using various *** and metallic particles are easily reinforced in a matrix by this *** composites made from an LM24 alloy reinforced with graphite and tin,with good wettability and material flow owing to the low melting point of tin,were fabricated by FSP at two rotational speeds of 1,400 and 1,000 *** its low hardness,the graphite/LM24 surface composite fabricated at a higher rotational speed of 1,400 rpm exhibited better wear ***,its frictional behavior was not significantly affected by the *** fabricated surface composites with graphite and tin reinforcement as well as graphite-only reinforcement exhibited contradicting behaviors under sliding wear *** post wear analysis indicated that abrasion,adhesion,layer formation,and delamination occurred on the composite surfaces.

Hot pressed silicon carbide(SiC) composites prepared with 0, 10, 30 or 50 wt% tungsten carbide(WC) are subjected to dry sliding wear against WC-Co and steel ball. In particular an attempt has been made to answer the following important questions:(i) How does the load(from 5 to 20 N) effect sliding wear behaviour of SiC-ceramics against WC-Co and steel counterbodies?(ii) Is there any effect of WC content on friction and wear characteristics of SiC ceramics?(iii) Does the dominant material removal mechanism of SiC ceramics change with the addition of WC or counterbody?(iv) What is the influence of mechanical properties on the sliding wear? Experimental results indicated that coefficient of friction(COF) for the SiC ceramics varied between 0.66 and 0.33 with change in load and counterbodies. Wear volume for SiC ceramics found approximately 6-10 times more against WC-Co as compared against steel. Wear volume changes from 2.0 × 10^(–3)mm^3 to 1.2 × 10^(–2)mm^3 with change in counterbodies for SiC -(10, 30 or 50 wt%) WC composite at 20 N. SiC ceramics indicated abrasion and composites reveal tribochemical wear as major material removal mechanisms. Wear is influenced by the hardness of counterbody and fracture toughness of SiC -WC composites.
