Unfavourable TB Outcomes in PLHIV with Delayed TB Treatment Initiation—A Retrospective Analysis

Background & Objectives: Timely initiation of anti-tubercular treatment (ATT) is key to obtain favourable outcomes among people living with HIV (PLHIV) co-infected with tuberculosis (TB). There is little evidence to predict treatment outcome and the delay in initiation of ATT among co-infected. The analysis explores the relationship between different ATT initiation delay times and TB outcomes. Methods: Retrospective analysis of secondary data of HIV-TB co-infected patients attending two antiretroviral therapy (ART) clinics in Pune district of Maharashtra, India. Logistic regression models were used to calculate risk. Results: Of the total 457 co-infected PLHIV initiated on ATT from December 2016 to June 2018, there were 298 who had completed TB treatment. Outcomes of ATT were available for 243 patients. Of the 243 patient’s records analysed, 61% were males and ≤11% were less than 18 years. 20% were previously treated. The risk of unfavourable outcomes increased monotonically from OR = 1.1, to OR = 1.73 for >2 days to >7 days delay in ATT initiation respectively. Males (OR = 2.11), newly initiated patients (OR = 1.54) are exposed to higher risk of unfavourable TB outcomes when ATT initiation is delayed beyond seven days. Conclusion: Risk of unfavourable outcomes increase in delay time for ATT initiation. Patient characters should be considered for prioritised referral and availability of test results on the same day of diagnosis to foster same day ATT initiation. More analysis on larger data sets with richer clinical information will generate confirmatory evidence.

Impact of microbiota on female fertility and gynecological problems

The uterus was previously considered a sterile environment for embryo implantation and fetal growth; however, evidence showed that different microorganisms in the female reproductive tract may regulate human fertility. TheLactobacillus family is among the most prevalent bacteria in the vagina and uterus of the female reproductive system. Interestingly, aLactobacillus-dominated (LD) uterine environment is associated with better pregnancy outcomes. Nevertheless, the mechanism by which an LD environment improves pregnancy outcomes is unknown. In the uterus, many commensal bacteria (eg,Bifidobacterium, Prevotella,Enterobacter,Streptococcus, andStaphylococcus) produce short-chain fatty acids (SCFAs), including acetate, butyrate, and propionate. SCFAs are crucial in modulating cytokine production (eg, IL-6 and IL-10) and immune cell populations (eg, T cells and macrophages) during embryo implantation and gynecological diseases. This minireview provides an overview of the roles of lactobacilli and SCFAs in female fertility and related diseases.

Substrate temperature dependent studies on properties of chemical spray pyrolysis deposited CdS thin films for solar cell applications

Thin films of CdS have been prepared by chemical spray pyrolysis by spraying precursor solution directly onto soda lime glass（SLG） substrates. Influence of substrate temperature on structural, optical, morphological and electrical properties have been investigated by using various techniques such as low angle X-ray diffraction（XRD）, Raman spectroscopy, X-ray photoelectron spectroscopy（XPS）, field emission scanning electron microscopy（FESEM）, atomic force microscopy（AFM）, transmission electron microscopy（TEM）, UV–visible spectroscopy photoluminescence（PL） spectroscopy etc. Formation of CdS has been confirmed by low angle XRD,Raman spectroscopy and XPS analysis. XRD pattern showed that CdS films are polycrystalline, have hexagonal structure and prefer orientation of crystallites shifts from（101） to（002） with increase in substrate *** spectroscopy revealed that exciton-phonon coupling depends on substrate temperature and hence on crystallite size. Optical band gap increased from 2.43 to 2.99 eV when substrate temperature increased from 325 to 475 ℃. Transmittance of the film also showed an increasing trend from 52% to 80% with increase in substrate temperature. Such high band gap and transmittance values of CdS films prepared at 475℃ make it a useful window material in CdS/CdTe and CdS/Cu_2S heterojunction solar cells.

An Unusual Presentation of Leptospirosis: A Case Report

Leptospirosis is a life-threatening zoonotic disease of global distribution. It has variable presentation ranging from mild febrile illness to life-threatening complications like acute renal failure, acute hepatic failure, pulmonary hemorrhages and cardiac arrhythmias. Cardiac manifestations of leptospirosis are not uncommon but are underdiagnosed. Cardiac manifestations may vary from minor rhythm abnormalities to fatal ventricular arrhythmias. We report an unusual presentation of leptospirosis in a patient who presented to the Emergency Department with epigastric pain and congestive cardiac failure with rhythm disturbances and selective myocardial involvement mimicking as Non ST elevation Inferior wall myocardial infarction (NSTEMI). The Cardiac manifestations of Leptospirosis need meticulous intensive care management as the mortality rate is very high in this subset of patients. Index suspicion and early treatment remains the mainstay of this life threatening zoonotic disease.

Purpose Common peroneal nerve palsy is quite disabling and every effort should be made to prevent its injury during the *** We retrospectively reviewed the prospectively collected data of 7 cases of tibial plateau fractures in association with proximal fibula fracture from January 2019 to September 2019 who presented to emergency room of our *** In addition to fibular neck fracture,the first case had type 6 tibial plateau displaced fracture and the second case had displaced acetabular fracture with instability of knee with tibial tuberosity *** peroneal nerve palsy developed following application of distal tibial skeletal traction in both the *** 6 such cases remained neurologically intact as traction was not applied to *** Such iatrogenic complication could have been prevented if the injury pattern of"concomitant medial and lateral columns"of the proximal leg is kept in mind by the treating surgeon before applying skeletal traction.

Rectangular Microstrip Antenna Using Inductive Septums for Dual Band Operation with a New Resonant Mode

A novel rectangular microstrip antenna on conventional dielectric substrate with centrally loaded inductive septums are proposed for dual band operation. The proposed antenna has been thoroughly investigated and the results are presented in the paper. The conventional patch antenna fabricated on common substrates always resonates at its dominant TM10 mode which produces the radiation field along its broadside direction. In the present investigation, the same microstrip antenna is designed on conventional substrate with centrally located inductive septums beneath the patch, with a view to develop a new resonant mode which will produce radiation like dominant TM10 mode. The speciality of the proposed antenna is to excite a new resonant mode with good impedance bandwidth while the traditional dominant TM10 mode is kept unaltered. A thorough quantitative analysis is presented to justify the reason of generation of new resonant mode along with the traditional dominant TM10 mode. An easy and handful formulation has been established for calculation of the frequency for new resonant mode as a function of antenna parameters and the gap between the septums. The proposed idea has been verified through a commercial software package for a patch operating in C band and an excellent agreement is revealed.

Tacrolimus confers lower acute rejection rates and better renal allograft survival compared to cyclosporine

AIM To compare the impact of tacrolimus(FK) and cyclosporine(CYA) on acute rejection and graft survival and to assess the predominant causes of graft loss between patients receiving these two calcineurin inhibitors(CNIs).METHODS Retrospective review of 1835 patients who received a kidney transplant(KTX) between 1999-2012. Patients were grouped based on initial CNI utilized: 1195 in FK group, 640 in CYA group. Data on baseline characteristics, clinical outcomes, and causes of graft loss in both groups were analyzed. RESULTS Cumulative acute rejection rates were 14% in the FK vs 24% in the CYA group. Despite more marginal donor characteristics in the FK group, these patients had better graft survival rates compared to the CYA group. Three and five year graft survival rates were 88% and 84% respectively in the FK group compared to 79% and 70% respectively in the CYA group(P < 0.001). After multivariate analysis, which controlled for confounders, FK use was a strong predictor for lower acute rejectionrates [odds ratio(OR) 0.60, 95%CI: 0.45-0.79] and better renal allograft survival(OR 0.740, 95%CI: 0.58-0.94). Death with a functioning graft was the most common cause of graft loss in both groups. Common causes of death included cardiovascular disease, infections, and malignancies. Chronic allograft nephropathy was also found to be an important cause of graft loss, being more prevalent in the CYA group. CONCLUSION The use of FK-based maintenance immunosuppression therapy is associated with a significantly lower rate of acute rejection and better graft survival compared to CYA-based regimen. Individualizing immunosuppression through risk-stratified CNI choice may lead to improved outcomes across all spectra of KTX patients.

BACKGROUND Remdesivir is a broad-spectrum antiviral drug having in vitro activity against severe acute respiratory syndrome coronavirus 2 and is currently being used on a compassionate basis outside of clinical *** To analyze the efficacy and safety of remdesivir compared with other interventions in coronavirus disease 2019(COVID-19)*** We searched online databases to include randomized controlled trials evaluating the efficacy and safety of remdesivir compared with other interventions in COVID-19 *** summarized efficacy and safety data as risk ratios(RRs)with 95%confidence interval(CI)and used Mantel-Haenszel fixed or randomeffect *** estimated the number needed to treat(NNT)to cause one additional *** used the GRADE approach to assess the quality of the evidence for all outcome *** We included four randomized controlled *** observed no significant difference in mortality(RR:0.83;95%CI:0.57–1.20;I2=59%)and rate of ventilation(RR:0.69;95%CI:0.41-1.18;I2=77%)between remdesivir-and placebo-treated *** showed higher rates of clinical recovery than placebo(RR:1.10;95%CI:1.04–1.16;I2=0%;NNT:14.3).We observed no difference in overall adverse events between remdesivir-and placebo-treated patients(RR:1.05;95%CI:0.86–1.27;I2=77%).We observed less risk of serious adverse events(RR:0.75;95%CI:0.63–0.89;I2=0%)in remdesivir-than placebo-treated *** GRADE approach suggested moderate quality of evidence for all efficacy and safety *** We observed limited clinical benefit of remdesivir over placebo in the treatment of *** findings could be biased because of the small number of trials.
