Neurolymphomatosis: A Rare Cause of Multiple Mononeuropathy

Neurolymphomatosis, defined as invasion of cranial nerves and peripheral nerve roots, plexus or nerves by Non Hodgkin’s Lymphoma is a very rare clinical entity. We describe a case of 69 years old gentleman, who presented to us with asymmetric, painful sensorimotor polyneuropathy. He was admitted with 2 months history of dry cough, constitutional symptoms, paraesthesias on right side of face along with painful asymmetrical quadriparesis. Nerve conduction studies were suggestive of asymmetrical sensorimotor axonal and demyelinating neuropathy. Cerebrospinal fluid analysis revealed mild pleocytosis with raised protein. FDG-PET showed intense uptake in both adrenals, abdominal lymph nodes, sacral nerve roots and brachial plexus. Fine needle aspiration cytology of adrenal mass revealed evidence of diffuse large B cell Non Hodgkin’s Lymphoma. As patient succumbed to illness, an autopsy was done, which revealed diffuse large B cell lymphoma involving adrenals, brachial and lumbosacral plexuses. Our report provides important insights into a rare cause of painful demyelinating multiple mononeuropathy and emphasises on increasing role and diagnostic utility of PET imaging in evaluation of patients presenting with multiple mononeuropathy, especially with regards to paraneoplastic or neoplastic causes such as lymphoma.

在许多精神障碍中(包括强迫症和精神分裂症),拒食是常见的表现。当突然出现拒食时,这可能需要急诊处理;一旦变成慢性,它就成为一种难以改变的根深蒂固的行为。对拒食患者的诊断可能有难度,尤其是对有共病问题、智力受损或对自身状况缺乏自知力的患者。鼻饲是一种有效的短期干预方法,当患者重新进食时即可中止这一干预。然而,在某些情况下,患者可能变得依赖于鼻饲。我们介绍一个病例,报告一名患有精神分裂症、强迫症、边缘智力以及癫痫的患者由于拒绝进食而由其家庭成员用鼻饲管喂食超过三年。

Successful implantation requires concerted interactions during the apposition,adhesion,and invasion of the embryo into a receptive ***,the embryo implantation rate for assisted reproduction remains low despite the transfer of good quality *** in endometrial transcriptomics,proteomics,lipidomics,and even microbiota all play important roles in embryo ***,the expression of steroid hormone-regulated adhesive and anti-adhesive molecules during the embryo implantation window is becoming an area of increasingly intense *** review(a)summarizes the different molecules expressed in the receptive endometrium and(b)proposes the use of surface protein markers to predict pregnancy outcomes from assisted reproduction.

Triple drug combination of telmisartan, amlodipine and hydrochlorothiazide in the treatment of essential hypertension

Background: Triple drug therapy comprising angiotensin receptor blocker (ARB), calcium channel blocker (CCB) and hydrochlorothiazide (HCT) effectively controls essential hypertension as evident from the literature. This study was undertaken to assess the efficacy and safety of triple combination compared to the dual drug therapy. Methodologies: A total of 220 male and female patients with essential hypertension were enrolled in the study. The patients were divided into two groups. Group A received a bilayer tablet of FDC of Telmisartan + Amlodipine + HCT and group B received FDC tablet of Telmisartan + HCT. Both the treatments were administered once daily for twelve weeks. The patients were asked to follow-up on week 1, 2, 4, 6, 8 and 10 for periodic efficacy and safety evaluations. Effect on systolic blood pressure (SBP), diastolic blood pressure (DBP) and quality of life (QOL) were recorded during the course of the trial. Results: Blood pressure reduction (BP) to the desired goals was observed with both the treatments. The SBP and DBP reductions were superior in triple combination therapy than double combination. Both treatments improved QOL of patients. Conclusion: Triple drug combination of telmisartan, amlodipine and HCT may serve a potential role in achieving desired BP goals, in patients with essential hypertension, which are otherwise poorly managed by either monotherapy or dual drug therapy.

背景尽管静脉置管时的疼痛经常让人备受困扰，但通常都是在没有任何镇痛措施下进行静脉置管的。临床上曾尝试用一些药物和非药物方法来减轻静脉置管的疼痛，但结果各异。本研究拟评价一次闪光对于减轻静脉置管痛的效果。方法90例拟择期行腹腔镜胆囊切除术的ASAⅠ～Ⅱ级的成年患者（15～60岁），随机分为3组，每组30例，组1为对照组；组2为分散注意力组，即在非优势手的手背静脉进行置管前给患者照相而不使用闪光；组3为闪光组，静脉置管前照相并且使用闪光；在照相后立即使用18G的穿刺针进行静脉置管。结果每组均有2例患者首次静脉穿刺失败，在数据分析中即予以剔除，闪光组疼痛的发生率为50％（28例中14例发生），其他2组为100％（28例全部发生），通过视觉模拟评分评估的疼痛严重程度分散注意力组和闪光组显著低于对照组，而闪光组也显著低于分散注意力组。结论静脉置管前使用闪光是一种减轻疼痛的安全而简单有效的方法。
