Objective:Male breast cancer(MBC) represents her-2/neu status are established prognostic factors in female breast cancer but not yet studied enough in male breast *** aim of the study was to verify the clinico-pathologic profile of male patients with breast cancer including analysis of hormone receptors and her-2/neu state and its impact on survival ***:This is a retrospective study carried on a total of 123 male patients presented to the National Cancer Institute(NCI),Cairo University,Egypt-with breast carcinoma from January 1999 to December ***:The patients had a median age of 58(ranged from 29-92) *** 39.8% of the patients presented with T4 *** presentation,12.2% of the cases had metastasis with bone represented 86.7% of metastatic *** 92.7% of patients were subjected to modified radical mastectomy and radical *** duct carcinoma reported in 91.1%,and 76.4% of the patients had grade II *** profile was reported in 74% of the patients with 71.4% positive ER and 69.2% positive *** 57 cases tested for her-2/neu,10.5% were *** A was the most common subtype detected in male breast carcinoma patients constituting 66.7%.Comparison with female patients with breast carcinoma revealed some differences regarding stage,hormone profile,her-2/neu status and breast cancer *** as adjuvant,neo-adjuvant and metastatic was given for 73.1%,17.3% and 9.6% of the cases,with 83% of them had an anthracyclin-containing ***-nine patients received radiation treatment,65.2% and 34.8% of them with adjuvant and palliative aim,*** of adjuvant radiotherapy had a median value of 4410(3400-5000) *** hormonal treatment(Tamoxafin) was given for 47 patients all of them with ER and/or PR positive for a median period of 33.5(4-60) *** 5-year overall survival(OS),loco-regional control(LRC),metastasis-free

Skeletal events of Anastrozole versus Tamoxifen on bone mineral density and bone biomarker osteocalcin in postmenopausal women with early breast cancer

Objective： Postmenopausal women with breast cancer are at increased risk of bone loss because of age related estrogen deficiency face which accelerated with the use of aromatase inhibitors （AIs）. We aimed to study the effect on bone mineral density （BMD） and bone formation biomarker osteocalcin level in postmenopausal breast cancer patients, for the first three years of adjuvant hormonal treatment of both groups Tamoxifen versus Anastrozol. Methods： One-hundered postmenopausal breast cancers were prospectively randomized to receive either Tamoxifen 20 rag/day （n = 50） or Anastrozole 10 mg （n = 50）. Both BMD and osteocalcin were assessed initially before treatment and then at regular intervals for both groups. Results： Use of Tamoxifen was associated with significant annual decrease in osteocalcin （P = 0.001）, whereas Anastrozole group had gradual increase of the annual levels （P 〈 0.01）. BMD decreased significantly in Anastrozole versus Tamoxifen groups （2.6% vs. 0.4%, P 〈 0.001）. Osteoporosis T 〈 -2.5 was reported significantly higher in Anastrozole group （P 〈 0.01）. Women with initial osteopenia in Anastrozole group showed significant decrease in BMD （P 〈 0.05）. The addition of bisphosphonate for patients with early osteoporosis markedly improved both osteocalcin level and BMD. Conclusion： Tamoxifen preserves BMD in postmenopausal breast cancer patients, whereas Anastrozole accelerates age associated fall in BMD especially in the first year of therapy, moreover, the addition of bisphosphonate can help to decrease the skeletal related events associated with treatment to ensure better quality of life with treatment.

Background: Although non-specific, transabdominal bowel ultrasonography, in real time or in color Doppler;has been beneficial in evaluating the location, extent and activity of inflammatory bowel disease (IBD). Aim: To assess the potential role of bowel transabdominal bowel ultrasonography in the diagnosis and follow-up of IBD. Patients and Methods: A cohort of 30 adults histologically-proven active IBD patients;ulcerative colitis (UC) (n = 20), Crohn’s disease (CD) (n = 10) were prospectively assessed in addition to 20 non-IBD participants representing the control group. All participants underwent the full colonoscopy in addition to real time and color Doppler ultrasonography. Bowel wall thickness, peri-enteric changes, and hemodynamic changes in the portal vein and mesenteric arteries were recorded at initial enrollment and after complete remission in 10 IBD patients. 背景:肝脏缺血再灌注损伤后肝细胞受到增殖和凋亡的双重调控。肝大部切除后,余肝经历缺血再灌注损伤,其肝再生受到明显抑制,缺血预处理可减轻此损伤,促进肝再生,其机制是否也与这有关,目前未见相关报道。目的:探讨缺血预处理对大鼠自体肝移植后余肝肝细胞凋亡和增殖的影响。设计、时间及地点:随机对照动物实验,于2006-09/2007-07在中南大学湘雅医学院实验动物中心完成。材料:雄性SD大鼠144只,随机分为3组:单纯肝叶切除组、自体肝移植组、缺血预处理组,48只/组。方法:单纯肝叶切除组大鼠只行肝左、中叶切除,不阻断肝右、尾叶血流。自体肝移植组大鼠结扎切断肝后与食管腹段之间的静脉交通支,游离liberate尾状叶,游离第一肝门、肝上及肝下下腔静脉后,阻断并经门静脉持续低温灌注保存液,同时进行无血肝切除(切除肝左、中叶),肝脏在体持续低温灌洗15min。解除肝门阻断,完全恢复肝脏血流。快速复温肝脏表面,并冲洗腹腔,缝合关腹。缺血预处理组大鼠在门静脉灌注前先阻断肝右、尾叶血流10min,然后开放血流10min,余步骤同自体肝移植组。各组大鼠分别于肝叶切除后0,1,3,6,12,24,48,72h时取材。主要观察指标:生化分析仪测定血清丙氨酸氨基转移酶及天冬氨酸氨基转移酶的水平,流式细胞仪检测肝细胞凋亡指数,通过Ki-67抗原的表达检测肝细胞增殖情况。结果:与自体肝移植组比较,除肝叶切除后0h外,其余各时间点单纯肝叶切除组、缺血预处理组血清丙氨酸氨基转移酶和天冬氨酸氨基转移酶水平均明显降低(Ph(即正常肝组织)基本不存在凋亡细胞;单纯肝叶切除组肝大部切除后余肝肝细胞凋亡指数稍有升高;自体肝移植组复灌注后肝细胞凋亡指数急剧升高,约在12h达到高峰,而后逐渐下降;与自体肝移植组比较,缺血预处理组肝细胞凋亡指数明显降低(Ph达高峰,而后逐渐下降。与单纯肝叶切除组比较,自体肝移植组Ki-67表达率明显降低(P<0.05);与自体肝移植组比较,缺血预处理组Ki-67表达率明显增高(P<0.05)。结论:缺血预处理可减轻自体肝移植后肝缺血再灌注损伤所致肝细胞凋亡,促进肝细胞增殖,这可能是其促进肝再生的机制之一。

Diabetes is a major health global problem that has reached alarming levels. The present study aims for studying the effect of date palm pollen (Phoenix dactylifera L.) El-Hayani cultivar on serum glucose and lipids profile in induced diabetic male albino rats. Palm pollen chemically analyzed besides chemical constituents, mineral contents, polyphenols and flavonoids. Male Albino rats (36 rats weight 170-190 gm) were divided into 6 groups. 1: Normal control (-), 2: Alloxanized diabetes control (+) (150 mg/Kg rat body weight). Role of Ischemia Modified Albumin in Diagnosis of Pulmonary Embolism

Pulmonary embolism is a common and potentially lethal condition. Clinical signs and symptoms for pulmonary embolism are nonspecific. New and simple tests are therefore needed in order to help in early diagnosis of pulmonary embolism. The aim of this work is to elucidate the role of IMA in the diagnosis of pulmonary embolism. Subjects and Methods: 75 patients with suspected pulmonary embolism and 20 control healthy subjects were included in this study. Measurement of IMA was done in all subjects. Results: The mean values of IMA were statistically significantly higher among the PE patient group (0.43 ± 0.104 ABSU) in comparison with non PE patient group (0.27 ± 0.053 ABSU) and healthy control subjects (0.21 ± 0.080 ABSU). At cut-off value of 0.305 ABSU, IMA had 97.5% sensitivity and 71.42% specificity. The area under the curve was 0.952. The positive predictive value of this cut-off value was 79.59% while the negative predictive value was 96.15%. Conclusions: IMA is a good alternative to D-dimer in the diagnosis and exclusion of PE. Larger studies are needed to augment our results.

Role of Nurse in Administrating Induction of Ovulation Medications at Assisted Reproduction Center versus at home

Background: The response of ovaries during controlled ovarian stimulation is the most important factor for evaluating the pregnancy outcome in assisted reproductive techniques. Aim: The study is to assess the role of a nurse in giving induction of ovulation medications at assisted reproduction university center versus home medications by private In Vitro Fertilization and infertility center on clinical outcomes and patient satisfaction. Subjects and Methods: Case-control research design, conducted at two centers (Banon private center for IVF and Assisted Reproductive Unit at Women health hospital, Assiut University) during the period from February 2018 to November 2018. The sample size included 150 women undergoing IVF for each group. Results: There is no statistically significant difference between women given IVF medications by the nurse at the public IVF center and women take IVF medication in the home at Banon IVF center as regards the outcome of IVF. Majority of infertile patients are satisfied with the care, they received and nearly third of them are satisfied with outcome of IVF in both groups, and there is statistically significant difference between satisfaction in public IVF center and satisfaction in private center in relation to information provided about IVF procedure, staff willingness to listen carefully and help patients;regular progress update on condition is in place with p value (0.000, 0.005, 0.003) respectively. Conclusion: IVF outcome isn’t significantly related to where induction medication was taken either at IVF center or at the home because infertile couples are keen to comply with all instructions on how to take medications with correct route, correct dose and on time. Recommendations: Empower the role of nurse as a health educator for women undergoing IVF about importance of stage of induction and take medications with correct route, correct dose and on time on outcome of IVF;other studies should also be conducted in other centers to furt

Letrozole versus Gonadotropin in Unexplained Infertile Couples Failed to Conceive with Clomiphene Citrate

Background:?Unexplained infertility represents about 15% - 20% of infertile couples. Usually, these cases need assistance. Clomiphene citrate is the most used drug for this problem but sometimes pregnancy failed to achieve it, so other options for assistance are gonadotrophin or letrezole. The objective of our study was to compare the pregnancy rate for letrezole and gonadotropin inunexplained infertile women’s who failed to conceive with clomiphene citrate. Methods: This prospective quasi-randomized trial was carried out in cytogenetic unite at obstetrics and gynecology department, Zagazig University hospital. 140 infertile females were included, induction of ovulation by letrozole for half of them and by gonadotrophin for the other half. Results: There was statistically highly significant decrease in duration of stimulation, E2 levels and endometrial thickness at day of hCG in letrezole group, no significant difference between two groups as regard number of follicles and pregnancy rate per cycle, while the cumulative pregnancy rate and the cost of stimulation are significantly higher in gonadotrophin group. Conclusion: In patient with unexplained infertility who failed to conceive with clomiphene citrate, gonadotrophins have a higher pregnancy rate than letrezole. however, pregnancy rate was high enough with lower cost with letrezole to be acceptable and justified its use in this group of patients.

Development of New Strategy for Non-Antibiotic Therapy: Bovine Lactoferrin has a Potent Antimicrobial and Immunomodulator Effects

The antibiotic therapy has many problems, such as antibiotics resistance, hypersensitivity, direct toxicity, antibiotic-induced immunosuppresion and super-infections. This is highlighting the need for a new strategy for non-antibiotic therapy through the use of novel immunomodulators as naturally released ones (Lactoferrin). The present study investigates the potential of bovine lactoferrin (bLf), isolated from bovine milk whey, to prevent Staphylococcus aureus (S. aureus), Escherichia coli (E. coli), Streptococcus agalactiae (S. agalactiae) and Pseudomonas aeruginosa (P. aerogenosa) growth and then evaluates its immunomodulator effect. First, bLf isolation was attempted from bovine milk whey using a cation exchange chromatography by SP-Sepharose. Second, the antimicrobial activity assays were trailed to study the antimicrobial activity of bLf. Finally, the immune effect of bLf was studied by lymphocyte transformation test. It was found that bLf was separated around molecular weight of 80 kDa and showed significant inhibitory effect against E. coli followed by P. aeruginosa, S. agalactiae and S. aureus. bLf increased lymphocyte transformations mean values in a dose dependant manner. The highest transformations mean value was determined at 50 μg/mL. In conclusion, these results suggest that bLf is a potent natural antimicrobials and immunomodulator agent.

and neonatal ***:This was a prospective cohort *** consecutive ERCDs,occurring at≥37 weeks of gestation between July 1,2017 and December 31,2019 in Makassed General hospital,were *** maternal and neonatal outcomes of 183 laboring women undergoing unscheduled repeat cesarean delivery(URCD)group were compared with those of 204 women undergoing cesarean delivery(CD)without spontaneous labor(ERCD)*** outcomes were“composite adverse maternal outcome”and“composite adverse neonatal outcome.”Fisher’s exact and Student’s t tests were used to assess the significance of differences in dichotomous and continuous variables,*** logistic regression models were constructed to identify risk factors with most significant influence on the rate of composite adverse maternal and neonatal ***:“Composite adverse maternal outcome”was significantly more common in women who underwent spontaneous labor((40/183)21.9%vs.(19/204)9.3%,P=0.001,relative risk(RR):2.7,95%confidence interval(CI):1.50–4.90).Similarly,“composite adverse neonatal outcome”was significantly increased in the URCD group((24/183)13.1%vs.(12/204)5.9%,P=0.014,RR:2.4,95%CI:1.18–4.98).These adverse effects persisted after adjustment for *** regression models revealed that,besides labor,CD-order impacted maternal outcome(RR:1.5,95%CI:1.02–2.30,P=0.036),while CD-order and teenage pregnancy influenced neonatal outcome(RR:2.1,95%CI:1.29–3.38,P=0.003,and RR:16.5,95%CI:2.09–129.80,P=0.008,respectively).Conclusion:In our study,spontaneous labor before ERCD,including deliveries at term,was associated with adverse maternal and neonatal outcomes,indicating that it is preferable to conduct ERCD before the onset of *** women with MRCD may identify those at increased risk for spontaneous labor for whom CD could be scheduled 1–2 weeks *** large prospective studies to assess the effects of such an approach on maternal and neonatal outco
