Evaluation of Reticulocyte Hemoglobin Content and Serum Neutrophil Gelatinase-Associated Lipocalin as Predictive Biomarkers of Anemia in Children on Hemodialysis

Background: Diagnosis of iron deficiency anemia with currently available tests is rendered difficult in hemodialysis patients. The aim: To investigate the role of reticulocyte Hemoglobin Content (CHr) in the diagnosis of iron deficiency anemia in hemodialysis children in comparison to the used traditional markers and assess the impressiveness and the utility of Neutrophil Gelatinase Associated Lipocalin (NGAL) as a novel biomarker of iron status in those patients. Methods: This study investigated CHr in addition to NGAL serum level in the same line with traditional markers for anemia, including: CBC, serum iron, ferritin, total iron-binding capacity (TIBC), and transferrin saturation (TSAT%). Results: It is more significant that CHr content in hemodialysis children is lower than their controls as they are (27.06 ± 2.90) pg and (32.86 ± 3.59) pg, respectively, p = 0.01. There is no significant difference regarding NGAL between the study groups. Significant negative correlation between CHr with ferritin, urea, creatinine, and positively correlated with iron and RBCS. CHr showed a sensitivity of 90% and specificity of 86.67% to detect iron-deficiency anemia with a cut-off value of 27 pg. Conclusion: CHr is superior to ferritin and TSAT % for the early diagnosis of iron deficiency anemia in hemodialysis children;our results do not support NGAL as a marker of anemia in hemodialysis patients.

Flavonoid chemical composition and antidiabetic potential of Brachychiton acerifolius leaves extract

Objective:To evaluate Brachychiton acerifolius leaf extracts as antidiabetic potential agent and to identify the main active constituents using bioactivity guided ***:In vitro antioxidant activity was evaluated for *** different extracts using DPPH assay and vitamin C as *** activity was then determined using STZ-induced rats treated daily with ethyl acetate and 70% ethanol leaf extracts for4 weeks at a dose of 200 g/kg body weight against gliclazide reference *** glucose,a-amylase,lipid profile,liver function enzymes and oxidative stress markers were assessed along with histopathological study for liver and pancreatic *** and structural elucidation of active compounds were made using Diaion and Sephadex followed by spectral ***:The results indicated that ethyl acetate and ethanol leaf extracts exhibited the strongest antioxidant activity compared to that of vitamin C(IC500.05,0.03 and 12 mg/m L,respectively).Both extracts showed potent anti-hyperglycemic activity evidenced by a significant decrease in serum glucose levels by 82.5% and 80.9% and a-amylase by45.2% and 53.6%,as compared with gliclazide 68% and 59.4%,*** of ethanol extract resulted in the isolation of 9 flavonoids including apigenin-7-O-arhamnosyl(1/2)-b-D-glucuronide,apigenin-7-O-b-D-glucuronide,apigenin-7-O-b-Dglucoside and ***:This study highlights the potential use of *** leaf extract enriched in flavones for the treatment of diabetes that would warrant further clinical trials investigation.

Administration of tacrolimus in 50 liver transplantation patients

Objective: To present a good tacrolimus (FK506) administration protocol in liver transplantation. Methods: Fifty liver transplantation patients were classified into FK506 high-concentration (20 pa- tients), FK506 mid-concentration (20), CsA-FK506 (5) and FK506-CsA (5) groups. Clinical manifesta- tions, FK506 side-effects and pathological changes were observed. Results: The FK506 high-concentration group show- ed complications such as infection and liver function damage. The FK506 mid-concentration group show- ed excellent therapeutic results and some complica- tions easy to deal with. Conclusion: FK506 is a highly effective immunosup- pressive agent and the mid-concentration protocol is a better one.

Assessment of hepatic fibrosis in pediatric cases with hepatitis C virus in Egypt

AIM: To assess hepatic fibrosis and factors associated with its progression in children with HCV infection. METHODS: At the Hepatology Unit,Cairo University Children's Hospital,a single liver biopsy was performed to 43 children with HCV infection after an informed consent between 1998-2004. Their mean age at liver biopsy was 8.67 ± 4.3 years. RESULTS: Among the 43 patients' biopsies,12 (27.9%) were having no fibrosis,20 (46.5%) mild fibrosis and 11 (25.6%) moderate to severe fibrosis. The median time for development of fibrosis was estimated to be 5.5 years. Developing fibrosis was significantly associated with shorter duration from first detected ALT elevation to biopsy (12 mo vs 1.2 mo,P = 0.015) and having higher levels of direct serum bilirubin (0.3 mg/dL vs 0.5 mg/dL,P = 0.048). No association was found between fibrosis stage and the presence of co-morbid conditions (P = 0.33). CONCLUSION: Hepatic fibrosis was present in 72.1% of children with HCV infection. The development of fibrosis was associated with higher levels of direct serum bilirubin. There was no significant association between fibrosis and age,duration of infection,risk factors,co-morbid conditions and most biochemical parameters.

Breeding Enrichment of Genetic Variation of Grain Yield and Its Attributes in Bread Wheat under Drought Stress and Well Irrigation

Drought stress(DS)is one of the most critical environmental abiotic stresses for wheat production in the arid *** of high-yielding genotypes tolerant to DS can play a significant role in mitigation the negative impacts associated with *** the present study,generation means analysis(GMA)was used to study the performance of two crosses under well irrigation(WI)and deficit irrigation[cross I(Line 44×Shandweel-1)and cross II(Line 20×Sakha 93)].Significant differences were observed for days to heading(DH),days to maturity(DM),plant height(PH),spike length(SL),number of spikes per plant(NS/P),number of grains per spike(NG/S),thousand-grain weight(TGW),grain yield per plant(GY/P),and proline content(PC)in the six populations of the two crosses within each irrigation *** II had early maturity and the highest PC,NS/P,TGW,and GY/P regardless of the irrigation *** I showed positive significant relative heterosis and heterobeltiosis for GY/P under the two irrigation *** inheritance of characters of cross I revealed additive,dominant,and epistatic effects,which varied with trait and *** genetic effects predominated in DH,SL,and PC,while non-additive were found in DM,NS/P,NG/S,and GY/***-sense heritability estimates(h^(2) n)were high for DH and PC,moderate to high for PH and SL,moderate for DM,NG/S,NS/P,and TGW,and low for GY/*** on different drought indices the populations BC_(1),BC_(2),F_(1),and P1 of cross II and BC1 of cross I were more tolerant to drought ***,PC,TGW and DH can be used as selection indicators to improve wheat for drought tolerance in early generations and other yield components traits in late *** second cross(Line 20×Sakha 93)shows promise and is of interest to a drought tolerance breeding program,where wheat breeders can use recombinant breeding strategies to construct desirable drought stress *** and path coefficient revealed that TGW and PC were the main

Evaluation of Angiopoietin-2 Serum Level as a Marker of Cardiovascular Risk in Children with Chronic Kidney Disease

Background: Cardiovascular complications are a major clinical problem in uremic patients accounting for 44% of all deaths in this population. Angiopoietin cytokines are involved with controlling micro vascular permeability, vasodilatation and vasoconstriction by signaling smooth muscle cells surrounding vessels. Aim: To assess Angiopoietin-2 serum level as an early marker of cardiovascular risks in children with chronic kidney disease on regular hemodialysis and correlate with intimal medial thickness and echo data in those children. Patients and methods: The study included 40 children with CKD on regular hemodialysis (HD), and they were selected from the hemodialysis unit of Al-Zahraa Hospital, Al-Azhar University, during the period from December 2014 to April 2015. Another group of 40 apparently healthy children, matches age and sex with patients group as a controls. Angiopoietin-2 serum level, Doppler ultrasound (U/S) to assess: intima-media thickness (IMT) and the peak systolic velocity (PSV) of the main arteries including the (aorta, carotid and femoral) arteries, conventional echo and tissue Doppler imaging (TDI) of mitral and tricuspid annular velocities are obtained for both groups. Results: Children on regular HD have significantly higher (Angiopoietin-2) serum level compared to their controls, and it is (161.35 ± 38.30 ng/ml) and (9.25 ± 12.64 ng/ml) respectively (p, 0.000) and increases in the aorta, carotid and femoral (IMT) with significant increase in their mean systolic velocities in patients group compared to the controls. Significant increase in tricuspid valve late diastolic velocity (TVA vel m/s) and (E/e’ ratio) obtained by (TDI), its abnormalities threshold is detected in patients group than controls, with significant increase right ventricular systolic pulmonary pressure in patients compared to the controls. The Influence of Regular Hemodialysis on the Highly Sensitive Troponin-I Level in Children without Any Symptoms

Backgrounds: Cardiovascular diseases are still the prominent cause of death in cases of end-stage renal disease, cardiac troponin I (cTnI) can be used for detecting cardiac involvement in asymptomatic cases of end-stage renal disease on hemodialysis. Aim: Determine the direct cardiac consequence of dialysis treatments in children on hemodialysis by measuring high-sensitive troponin-I as a marker of myocardial injury. Subjects and Methods: This case-control study included thirty children with end-stage renal disease on regular hemodialysis;the study group was selected from the nephrology hemodialysis unit of Al-Zahraa Hospital, Al-Azhar University. Another group of thirty healthy children matches age and sex with the patient’s group as a control. Highly Sensitive cTnI (hsTnI) was measured pre and post hemodialysis with a sensitive assay;moreover, ECG, lipid profile including cholesterol, triglyceride, low and high-density lipoprotein (HDL) in the same line with routine investigations for those patients, we used bioimpedance for dry weight assessment in the hemodialysis (HD) group. Results: Children on (HD) have a significantly higher (hsTnI) pre-dialysis (0.250 ± 0.069 ng/ml) compared to post-dialysis (0.187 ± 0.004 ng/ml) with (p, 0.001). With no significant difference between post HD (0.187 ± 0.004 ng/ml) and the control group (0.189 ± 0.005) with (p, 0.090). cTnI is detected in (73.3%) of children pre-dialysis above the cut-off value compared to (3.31%) had a high-level post-dialysis. cTnI is positively correlated with systolic, diastolic blood pressure and heart rate with (r. 0.333, p, 0.001: r. 0.343, p, 0.001: r. 0.276, p, 0.033) respectively and (hsTnI) is negatively correlated with Hb and HDL (r. -0.333, p, 0.009: r. 0.324, p, 0.011). Serum Resistin Level and Polymorphonuclear Leukocytes Dysfunctions in Children on Regular Hemodialysis

Resistin is a secretory adipocytoine, which is expressed mainly in humans by inflammatory cells especially macrophages. Resistin serum levels are elevated in end-stage renal diseases of people having an increased risk of infections as a result of impaired polymorphonuclear leukocytes (PMNLs) functions. Objectives: To evaluate neutrophil functions (phagocytosis and oxidative burst) in children with end-stage renal disease (ESRD) on regular hemodialysis and to shed light on the contribution of resistin on neutrophil functions. Patients and Methods: The study included 40 children with ESRD on regular hemodialysis. Their ages ranged from 6 to 12 years, and they were selected from children attending the pediatric hemodialysis unit of AL-Zahraa Hospital, Al-Azher University during the period from October 2012 to December 2013. Another group of 40 apparently healthy children with matched age and sex with the patient group served as a control. Serum resistin, phagocytic index and nitro blue tetrazolium test (NBT%) were assessed in both groups. Results: There was a statistically more significant increase in resistin serum levels in cases than in controls;it was (3.25 ± 0.86 ng/ml) and (0.25 ± 0.16 ng/ml) respectively
