Human intestinal tract contained a diverse number of microbial communities which performed a significant role in human health. The presence of gut microbiota was affected mainly by diet. Camel milk is the source of nutrition and provides all the essential nutrients for growth. It has great significance in the treatment of liver, spleen, and anemic infections. Camel urine has also many medical advantages. In this study we examined the effect of camel milk and urine and a mixture of both (milk + urine) on the growth of Gut microbiota using an in vivo animal model. Fresh fecal samples were collected before and after administration of the tested materials. After that, the microbial analysis was conducted via culturing, denaturing gradient gel electrophoresis and metabolic analysis via high-performance liquid chromatography (HPLC). The result indicated that the numbers of bacterial groups were increased after the first dose. Coliform group have significant increase when given a mix of milk and urine compared to control group with P < 0.05. Bifidobacterium group have significant increase in their number in the Milk and Mix groups compared to control group with P < 0.05. The concentration of Short-chain fatty acids in fecal samples was increased in Milk and Mix groups compared to control group. In conclusion, drinking camal milk, urine or a mix of both increased the growth of Gut microbiota.

The Veno-venous Extra Corporeal Membrane Oxygenation (ECMO) indications and usage has strikingly progressed over the last years;especially with the COVID 19 pandemic, it has become an essential tool in the care of adults and children with severe pulmonary dysfunction refractory to conventional management. In this article, we will provide a review of ECMO development, clinical indications, patients’ management, options and cannulations techniques, complications, outcomes, and the appropriate strategy of organ management while on ECMO.

AIM： To identify the most common hepatitis B virus （HBV） genob/pe in Saudi Arabia, and correlate the prevailing genob/pes with the clinical outcome of patients. METHODS： Patients were consecutively recruited from the hepatology clinics of two tertiary care referral centers. Patients were categorized into 4 different groups： group 1, patients with hepatitis B and normal liver enzymes; group 2, patients with hepatitis B and abnormal liver enzymes but without cirrhosis; group 3, patients with hepatitis B and liver cirrhosis; group 4, patients with hepatitis B and hepatocellular carcinoma. All patients had a positive hepatitis B surface antigen （HBsAg）. Genob/ping of HBV was performed by nested PCR-mediated amplification of the target sequence and hybridization with sequence-specific oligonucleotides. RESULTS： Seventy patients were enrolled in this study. They were predominantly male （72.9%） in their midforty＇s （mean age 47 years）. Forty-nine （70%） patients were hepatitis B envelope antigen （HBeAg） negative. The majority of patients （64%） acquired HBV through unknown risk factors. Hepatitis B genob/ping revealed that 57 patients （81.4%） were genob/pe D, 1 patient （1.4%） had genob/pe A, 1 patient （1.4%） had genotype C, and 4 patients （5.7%） had genob/pe E, while 7 patients （10%） had mixed genob/pe （4 patients ADG, 1 patient DE, 1 patient DF, and 1 patient ADFG）. Based on univariate analysis of genob/pe D patients, significant predictors of advanced liver disease were age, gender, aspartate transaminase, alanine transaminase, albumin,bilirubin, and alkaline phosphatase （all P 〈 0.001）. In multivariate analysis decreased hemoglobin （r = -0.05; 95% CI： -0.08 to -0.03; P = 0.001） and albumin levels （r = -0.004; 95% CI： -0.007 to -0.001; P = 0.002） were highly significant predictors of advanced liver disease. In patients with HBV genotype D, HBeAg negativity was found to increase across advancing stages of liver disease （P = 0.024）. CONCLUSION： This study highlights tha

Project management is vital for software project success. Project Management Body of Knowledge (PMBOK), CMMI for Development (CMMI-DEV), and other software project management models and practices established to assist software organizations, each of these models offers diverse focal points. Combined effective software project management practices remain a challenge to software organizations. In striving to raise the likelihood of software project success, this paper introduces a framework for software project management effectiveness which integrated a set of software project management practices based on the PMBOK guide processes, CMMI-DEV processes, and management areas that influence a software project. The proposed framework called 4PTRB which includes people, process, product, project, technology, risk, and business management areas. The 4PTRB software project management framework provides a whole comprehensive view to help software project managers to improve their project management effectiveness. The framework is validated by conducting a web-based survey of software professional practitioners around the world. A statistical analysis of twenty software projects has been conducted as a case study thereafter, spanning small to large development projects. The consistency of the case study and survey results shows the power of the 4PTRB framework as an inclusive software project management framework.

Metformin is known to be an effective treatment for patients with type 2 diabetes mellitus. Metformin induced lactic acidosis (MALA) is a serious illness which should identify and treat urgently. In this review article, we will present our own data and review the literature to highlight the importance of recognizing MALA;causes, presentations and the proper treatment, even though there are few published studies discussing MALA treatment modalities. The two cases that we presented here were patients admitted to the ICU and undergoing hemodynamic instability, which improved after removal of Metformin and clearance of associated lactic acidosis with SLED and CVVH. A case could be made for SLED and CVVH as modalities of choice for patients with MALA and hemodynamic instability, but further research is needed in this direction.

A numerical study has been carried out to investigate the effect of aspect ratio on heat transfer by natural convection of nanofluid taking Cu nano particles and the water as based fluid. The flow is laminar, steady state, axisymmetric two-dimensional in a vertical cylindrical channel filled with porous media. Heat is generated uniformly along the center of the channel with its vertical surface remain with cooled constant wall temperature and insulated horizontal top and bottom surfaces. The governing equations which used are continuity, momentum and energy equations using Darcy law and Boussinesq＇s approximation which are transformed to dimensionless equations. The finite difference approach is used to obtain all the computational results using the MATLAB-7 program. The parameters affected on the system are Rayleigh number ranging within （10≤ Ra ≤ 103）, aspect ratio （1 ≤ As 〈 5） and the volume fraction （0 ≤0 〈 0.2）. The results obtained are presented graphically in the form of streamline and isotherm contour plots and the results show that as ~ increase from 0.01 to 0.2 the value of the mean Nusselt number increase 50.4% for Ra = 1,000.

COVID-19 Vaccine Acceptance and Hesitancy among Pregnant Women: A Knowledge, Attitude, and Perceived Risks Survey in Lusaka, Zambia

Introduction: The coronavirus disease 2019 (COVID-19) is an infectious disease of the respiratory tract caused by SARS-CoV-2. Since its emergence, there have been increased rates of transmission and spread, morbidity and mortality which led to the development of COVID-19 vaccines to address the pandemic. This study assessed acceptance, knowledge, attitude, and perceived risks regarding COVID-19 vaccines among pregnant women attending antenatal care at two First-Level Hospitals in Lusaka, Zambia. Materials and Methods: This was a cross-sectional study that was conducted among 241 pregnant women using a questionnaire from August 2023 to October 2023 in two First-Level Hospitals in Lusaka district, Zambia. The collected data were analyzed using IBM Statistical Package for Social Sciences (SPSS) version 22.0. Statistical analysis was performed using a Chi-square test. The statistical significance was set at a 95% confidence level. Results: Of the 241 participants, 107 (42.7%) were aged between 24 and 34 years. Overall, 64.3% accepted the COVID-19 vaccines, of which 122 (50.6%) were already vaccinated. Further, 203 (84.6%) of the pregnant women had good knowledge, and 199 (82.6%) had positive attitudes towards COVID-19 vaccines. However, 58.5% thought COVID-19 vaccines were not safe and could cause infertility. Alongside this, 70.1% thought that COVID-19 vaccines were harmful during pregnancy. Having good knowledge of COVID-19 vaccines was associated with age (p = 0.049), education status (p = 0.001), and employment status (p = 0.001). Having a positive attitude towards COVID-19 vaccines was associated with education status (p = 0.001) and employment status (p = 0.001). Conclusion: This study found that most pregnant women had good knowledge, and positive attitudes, and the majority accepted the COVID-19 vaccine. In the recent decades, the academic literature engages more and more with reforms in the education system. These reforms that are worldwide seek to create change in education （Adler, 2010）. If it＇s regarding the position of the school principal as designer leader has significant role in the reforms management, but, moreover the role of this article is to the change of the role of the school principal over the years. The functions of the school principal integrate managerial aspects and leadership aspects and its importance is immenseness of the operation of the school organization, both in the terms of the organization customers, the pupils, and in the terms of the organization＇s staff--teachers. Not less important is the operation and implementation of educational reforms. We have to see the role of the school principal as the most significant to the implementing of the reform in the education system.

Background: Enhanced recovery after surgery (ERAS) has been tested in a wide variety of surgeries with promising outcomes. However, there is a need for a standardized, evidence-informed approach to both the development of new ERAS Society guidelines, and the adaptation and revision of existing guidelines. Developing countries have limited resources and deserve every effort to improve economic status. Aim of the Study: to evaluate perinatal maternal outcomes of ERAS? versus routine care protocols in women undergoing elective cesarean section (CS) in a lower middle-income country with limited resources ranked as a third most country performing CS worldwide using a multidisciplinary team (MDT) management. Design: A prospective randomized Controlled Trial. Setting: Outpatient department (OPD) and labor ward at the Woman’s Health hospital, Assiut University, Egypt. Participants: Healthy pregnant women planned for elective CS (300 women) were randomly divided into a study group offered ERAS protocol and a control group offered regular care. Results: Repeat CS was the main indication of elective CS in both groups without significant difference (89 cases (59.3%) and 75 cases (50%) in both groups respectively). Other indications included cephalopelvic disproportion in 17 cases (11%) and 19 cases (12.6%), placenta previa in 28 (18.6%) and 34 (22.6%) cases, DM in 11 (7%) and 19 (12.6%) cases, and others in 5 (0.3%) and 3 (2%) cases in both groups respectively. The study group took much less time to eat and walk. It had significantly lower pain levels and postoperative problems, as well as much greater women’s satisfaction and a shorter hospital stay (p = 0.001). Conclusions: Collaboration of nursing, obstetricians and anesthesiologists is the cornerstone for a successful ERAS for CS. Significant better perinatal maternal outcomes encourage expansion of ERAS in lower middle-income countries with limited resources.
