Childhood glaucoma profile in Dakahelia, Egypt: a retrospective study

AIM： To analyze childhood glaucoma regarding its demographics, presentations, different causes and surgical modalities used among patients in Dakahelia and to apply the Childhood Glaucoma Research Network（CGRN） classification retrospectively to evaluate its ***： A retrospective study in which the medical files of all glaucoma patients 〈16 years old presented to Mansoura Ophthalmic Center, Mansoura University from 2014 to 2017, were retrieved and analyzed. Collected data included： age, gender, laterality, visual acuity（VA）, refraction, intraocular pressure（IOP）, corneal diameter, cup-disc ratio, types and number of surgeries and antiglaucomatous drugs（AGD） at the first and last visit. Prevalence of different subtypes was calculated and means of clinical features were ***： A total of 305 eyes of 207 patients were included classified into 6 groups： primary congenital glaucoma（PCG）, juvenile open angle glaucoma（JOAG）, glaucoma associated with systemic disease, glaucoma associated with ocular anomalies, acquired glaucoma and glaucoma following cataract surgery. PCG was the predominant type（55.1%） followed by acquired glaucoma（29.5%）. Males represented 63.8% of the whole studied patients. Glaucoma associated with ocular anomaly group showed the youngest age at diagnosis（21.9±30.0 mo）. The shortest corneal diameter was recorded in post cataract group（10.4±0.5 mm）. Highest cup-disc ratio was found in the PCG group（P〈0.0005）. Glaucoma associated with systemic disease presented with the highest baseline IOP（34.5±5.0 mm Hg）. All the cases with PCG were treated surgically with 21.8% required more than one surgery. The majority of the patients（74.2%） in the acquired group were treated medically. Combined trabeculotomy-trabeculectomy was the most frequent operation done, accounting for 34.5% of all primary surgeries. Ahmed valve implantation comprised 87% of the secondary surgeries. We sought to evaluate immediate and delayed micro-tensile bond strength of Panavia F2.0 and Multilink Sprint resin cement to superficial, deep and cervical dentin. Thirty-six freshly extracted non-carious human molars were sectioned in the mesiodistal direction to expose three different dentin regions including superficial dentin （1 mm below the dentine-enamel junction）, deep dentin （1 mm above the highest pulp horn） and cervical dentin （0.5 mm above the cemento-enamel junction and 0.5 mm below the dentine-enamel junction）. Resin cements were applied on dentin surfaces and composite blocks were luted under constant seating pressure. Each group was divided into three subgroups according to time intervals. Specimens were sectioned to obtain sticks of 1 mm2 in diameter and subjected to microtensile bond strength testing at a cross head speed of 1 mrn/min. Both resin cements showed higher micro-tensile bond strength to superficial dentin than that to deep or cervical dentin （P 〈 0.001）. Micro-ten- sile bond strengths of Panavia F2.0 were higher than those of Multilink Sprint at different dentin regions （P 〈 0.001）. Immediate ＂micro-tensile bond strengths were higher than those of delayed micro-tensile bond strengths for both resin cements （P 〈 0.001）. It was concluded that resin cements with different chemical formulations and applications yield significantly different micro-tensile bond strengths to different dentin regions.

Launching a paradigm for first and redo-surgery in primary congenital glaucoma: institutional experience

AIM: To evaluate the outcome of the initial and the redo-surgeries for primary congenital glaucoma(PCG) correlated to its degree of severity. METHODS: A retrospective study involved patients with PCG presented between 2010 and 2018. Medical records were reviewed to assess the degree of the preoperative severity according to the intraocular pressure(IOP), corneal diameter and corneal edema. Success and failure rates were calculated for both first and redo-surgeries at 6 and 12 mo respectively then correlated to the severity of the cases. RESULTS: Complete records were retrieved for 272 eyes(153 patients) with PCG: 43 eyes were mild, 136 moderate and 93 severe. Combined trabeculotomy and trabeculectomy(CTT) had the highest success rate in moderate(96.4%) and severe cases(59.3%) while trabeculotomy had the highest success rate in mild cases(96.3%). Medical records of 88 eyes(63 patients) with recurrent PCG were analyzed, most with severe presentation(59 eyes). Ahmed glaucoma valve(AGV) was used in 67(76%) eyes and augmented trabeculectomy in 21(24%) eyes. At 12 mo, there was no statistically significant difference between both surgeries in total success rate(P=0.256). For mild cases, success rate was 100% for both surgeries. Severe cases had higher success rates following AGV(87%) than augmented trabeculectomy(20%). Preoperative severity of the disease was an independent factor affecting the failure rate in secondary trabeculectomy but not in AGV. Patients younger than 24 mo had higher probabilities of failure following both redo-surgeries with hazard ratio =1.325 and 0.37 for augmented trabeculectomy and AGV respectively. CONCLUSION: Preoperative assessment of the severity of eyes with PCG helps in the selecting the optimal primary and secondary surgery. For first surgery, trabeculotomy is more effective in mild cases whereas; CTT and augmented subscleral trabeculectomy(SST) are appropriate for moderate and severe cases. Global Software Development （GSD） is a well established field of software engineering with the benefits of a global environment. Software Project Management （SPM） plays a key role in the success of GSD. As a result, the need has arisen to study and evaluate the downsides of SPM for GSD, to thereby pave the way for the development of new methods, techniques, and tools with which to tackle them. This paper aims to identify and classify research on SPM approaches for GSD that are available in the literature, to identify their current weaknesses and strengths, and to analyze their applications in industry. We performed a Systematic Mapping Study （SMS） based on six classification criteria. Eighty-four papers were selected and analyzed. The results indicate that interest in SPM for GSD has been increasing since 2006. As a class of approaches, the most frequently reported methods （40%） are those used for coordination, planning, and monitoring, along with estimation techniques that can be used to better match a distributed project. SPM for GSD requires further investigation by researchers and practitioners, particularly with respect to cost and time estimations. These findings will help overcome the challenges that must to be considered in future SPM research for GSD, especially regarding collaboration and time-zone differences.

Objective:To evaluate Brachychiton acerifolius leaf extracts as antidiabetic potential agent and to identify the main active constituents using bioactivity guided ***:In vitro antioxidant activity was evaluated for *** different extracts using DPPH assay and vitamin C as *** activity was then determined using STZ-induced rats treated daily with ethyl acetate and 70% ethanol leaf extracts for4 weeks at a dose of 200 g/kg body weight against gliclazide reference *** glucose,a-amylase,lipid profile,liver function enzymes and oxidative stress markers were assessed along with histopathological study for liver and pancreatic *** and structural elucidation of active compounds were made using Diaion and Sephadex followed by spectral ***:The results indicated that ethyl acetate and ethanol leaf extracts exhibited the strongest antioxidant activity compared to that of vitamin C(IC500.05,0.03 and 12 mg/m L,respectively).Both extracts showed potent anti-hyperglycemic activity evidenced by a significant decrease in serum glucose levels by 82.5% and 80.9% and a-amylase by45.2% and 53.6%,as compared with gliclazide 68% and 59.4%,*** of ethanol extract resulted in the isolation of 9 flavonoids including apigenin-7-O-arhamnosyl(1/2)-b-D-glucuronide,apigenin-7-O-b-D-glucuronide,apigenin-7-O-b-Dglucoside and ***:This study highlights the potential use of *** leaf extract enriched in flavones for the treatment of diabetes that would warrant further clinical trials investigation.

Metformin is known to be an effective treatment for patients with type 2 diabetes mellitus. Metformin induced lactic acidosis (MALA) is a serious illness which should identify and treat urgently. In this review article, we will present our own data and review the literature to highlight the importance of recognizing MALA;causes, presentations and the proper treatment, even though there are few published studies discussing MALA treatment modalities. The two cases that we presented here were patients admitted to the ICU and undergoing hemodynamic instability, which improved after removal of Metformin and clearance of associated lactic acidosis with SLED and CVVH. A case could be made for SLED and CVVH as modalities of choice for patients with MALA and hemodynamic instability, but further research is needed in this direction.

Hepatocellular carcinoma (HCC) is a major cause of cancer death worldwide, accounting for over half a million deaths per year. The geographic pattern of HCC incidence is parallel to exposure to viral etiologic factors. Its incidence is increasing, ranging between 3% and 9% annually depending on the geographical location, and variability in the incidence rates correspond closely to the prevalence and pattern of the primary etiologic factors. Chronic infections with hepatitis B viruses or hepatitis C viruses have both been recognized as human liver carcinogens with a combined attributable fraction of at least 75% of all HCC cases. Multiple non-viral factors have been implicated in the development of HCC. Increased body mass index and diabetes with subsequent development of non-alcoholic steatohepatitis represent significant risk factors for HCC. Other non-viral causes of HCC include iron overload syndromes, alcohol use, tobacco, oral contraceptive, aflatoxin, pesticides exposure and betel quid chewing, a prevalent habit in the developing world. Wilson disease, α-1 antitrypsin deficiency, Porphyrias, autoimmune hepatitis, Schistosoma japonicum associated with positive hepatitis B surface antigen, and thorotrastray are also contributing hepatocellualar carcinoma. In addition, primary biliary cirrhosis, congestive liver disease and family history of liver cancer increase the risk of HCC incident. In conclusion,clarification of relevant non-viral causes of HCC will help to focus clinicians on those risk factors that are modifiable. The multilevel preventative approach will hopefully lead to a reduction in incidence of non-viral HCC, and a decrease in the patient morbidity and mortality as well as the societal economic burden associated with HCC.

The study area is located within the western part of Iraq between Latitude (32°45'0'' - 34°0'0'') North and longitude (39°30'0'' - 40°45'0'') East within Anbar Governorate. It covers an area of 4767.423 Km2 with elevation ranges from 360 to 840 m. The general slope decreases from southwest to northeast. Five old dams identified in the study area. The results of SWAT model showed high curve number (70 - 85) indicating low filtration and runoff rate (1.2 - 8.2) mm for the period (1994-2014). These dams have catchment areas ranged between (118.03 - 673.41) km2. They were to be used for agricultural purposes. The results according to the Food, Agriculture Organization (FAO) standards for their distance from agricultural areas using a GIS showed that all are not suitable except dam number (1). The distance to roads showed that dams (1, 4) are highly suitable, (2, 3) are low suitable and dam (5) is not suitable. The average annual quantity of rain water that can be harvested from the 5 dams is about 12.9 million cubic meters.
