Esophageal diverticula are rare conditions that cause esophageal symptoms, such as dysphagia, regurgitation, and chest pain. They are classified according to their location and characteristic pathophysiology into three types: epiphrenic diverticulum, Zenker's diverticulum, and Rokitansky diverticulum. The former two disorders take the form of protrusions, and symptomatic cases require interventional treatment. However, the esophageal anatomy presents distinct challenges to surgical resection of the diverticulum, particularly when it is located closer to the oral orifice. Since the condition itself is not malignant,minimally invasive endoscopic approaches have been developed with a focus on alleviation of symptoms. Several types of endoscopic devices and techniques are currently employed, including peroral endoscopic myotomy(POEM). However,the use of minimally invasive endoscopic approaches, like POEM, has allowed the development of new disorder called iatrogenic esophageal diverticula. In this paper, we review the pathophysiology of each type of diverticulum and the current state-of-the-art treatment based on our experience.

Brain activity following esophageal acid infusion using positron emission tomography

AIM: To investigate symptoms and brain activity following esophageal acid infusion. METHODS: Fifteen healthy volunteers were recruited for the study. Hydrochloric acid (pH 1 and 2) and distilled water (pH 7) were randomly and repeatedly infused into the esophagus. The brain activity was evaluated by positron emission tomography. The severity of heartburn elicited by the infusion was rated on an auditory analog scale of ***: The severity of heartburn following each infusion showed a step-wise increase with increasing acidity of the perfusate. The heartburn scores were significantly higher in the second pH 1 infusion compared with the first infusion. Acid and distilled water infusion induced activation of various brain areas such as the anterior insula, temporal gyrus, and anterior/ posterior cingulate cortex. At pH 1 or 2, in particular, activation was observed in some emotion-related brain areas such as the more anterior part of the anterior cingulate cortex, parahippocampal gyrus, or the temporal pole. Strong activation of the orbitofrontal cortex was found by subtraction analysis of the two second pH 1 infusions, with a significant increase of heartburn symptoms. CONCLUSION: Emotion-related brain areas were activated by esophageal acid stimulation. The orbitofrontal area might be involved in symptom processing, with esophageal sensitization induced by repeated acid stimulation.

A multi- pumose simulation method for the roll- formign is proposed. The forming pro- cesses of welded pipes are systematically simulated by using the method. The three-dimensional cu rved surfaces of sheet metals in forming processes are mathema tically expressed by using the shape fonction S(X) which represents flow pattern of each part of sheet metal. Through optimis- ing t e parameter n in s(X), the best approximation of the deformation of sheet metal is derived and the minimum power of deformation is obtained. A mathematical procedure of this analysis is systematically foimulated. The simulation method can be applied to the analysis and simulation for various roll- forming processes of welded pipes observed in commercial production line.

AIM To examine the role of soluble fibrin monomer complex(SFMC) in the prediction of hypercoagulable state after gastroenterological *** We collected data on the clinical risk factors and fibrin-related makers from patients who underwent gastroenterological surgery at Hiroshima University Hospital between April 1, 2014 and March 31, 2015. We investigated the clinical significance of SFMC, which is known to reflect the early plasmatic activation of coagulation, in the view of these fibrin related *** A total of 123 patients were included in the present study. There were no patients with symptomatic VTE. Thirty-five(28%) patients received postoperative anticoagulant therapy. In the multivariate analysis, a high SFMC level on POD 1 was independently associated with D-dimer elevation on POD 7(OR = 4.31, 95%CI: 1.10-18.30, P = 0.03). The cutoff SFMC level was 3.8 μg/ml(AUC = 0.78, sensitivity, 63%, specificity, 89%). The D-dimer level on POD 7 was significantly reduced in high-SFMC patients who received anticoagulant therapy in comparison to highSFMC patients who did *** The SFMC on POD 1 strongly predicted the hypercoagulable state after gastroenterological surgery than the clinical risk factors and the other fibrin related markers.

Sensible and latent heat flux at semi-arid and arid region, i.e., evapotranspiration, has been researched for long time because it serves an important role for water resource issues. However, the issues have not solved completely yet. Accordingly, by applying the Bowen ratio concept on the soil surface, the sensible and latent heat fluxes are reciprocally estimated using single height temperature (Tz) and humidity (rehz) with the net radiation (Rn) and heat flux into the ground (G). The procedure proposed by authors initially estimates the soil surface temperature (Ts) and the relative humidity (rehs) using optimization techniques. The method is remarkably effective to expand for estimating evapotranspiration at various regions. The validity of the method is confirmed by the latent heat flux (lE) and sensible heat flux (H) observed by the eddy covariance method. The hourly change of the lE, H, Ts and rehs on the soil surface, yearly change of lE and H and relationship of estimated lE and H versus observed are clarified. Yearly change of evapotranspiration is also estimated. The analysis is performed by general method (1), conventional method and general method (2). Above results are very useful for water resources issue and irrigation planning. The research is conducted using hourly data at eight globally dispersed sites using FLUXNET.

There is no word to describe the importance of evapotranspiration research for water resource utilization. We have already proposed a new method for the reciprocal estimation of the sensible (H) and latent heat fluxes (lE) by using a single height temperature (Tz) and humidity (rehz) based on the observed net radiation (Rn) and ground heat flux (G). This research is more advanced than the previous research because it uses a Ts observed by a radiometer and identifies the observed data satisfactorily heat balance relationship in every hour at nine sites. First, we confirmed that the estimated H and lE are very close reproductions of the identified H and lE. Second, by analyzing the relative ground surface temperature (Ts - T0) [Ts: ground surface temperature, T0: observed temperature near the soil surface], the hourly and seasonal changes of (Ts - T0) were clarified, resulting in a marked difference in the (Ts - T0) from previous research in arid and semi-arid regions. Next, the estimation accuracy of H, lE and rehs (the humidity of the soil surface) was determined by observing the slope of the estimated and observed relationship, resulting in the reasonable accuracy (0.85 - 1.15 times) of rehs at seven of the nine sites. Furthermore, the annual evapotranspiration was estimated by comparing the identified and estimated H and lE, resulting in a reasonable accuracy (0.85 - 1.15) at five of the nine sites in the case of the application of constraint b. Moreover, the effect of the lag-time between the net radiation Rn and both Tz and Ts for the estimation accuracy on H and lE was tested, and no remarkable difference was found because the effect was included already in the original data. The above results will contribute greatly to the advance of water resource planning and hydrometeorology. This research was conducted using FLUXNET data.

AIM To establish a hypoxic environment for promoting osteogenesis in rat marrow stromal cells(MSCs) using osteogenic matrix cell sheets(OMCSs).METHODS Rat MSCs were cultured in osteogenic media under one of four varying oxygen conditions: Normoxia(21% O_2) for 14 d(NN), normoxia for 7 d followed by hypoxia(5% O_2) for 7 d(NH), hypoxia for 7 d followed by normoxia for 7 d(HN), or hypoxia for 14 d(HH). Osteogenesis was evaluated by observing changes in cell morphology and calcium deposition, and by measuring osteocalcin secretion(ELISA) and calcium content. In vivo syngeneic transplantation using OMCSs and β-tricalcium phosphate discs, preconditioned under NN or HN conditions, was also evaluated by histology, calcium content measurements,and real-time quantitative *** In the NN and HN groups, differentiated, cuboidal-shaped cells were readily observed, along with calcium deposits. In the HN group, the levels of secreted osteocalcin increased rapidly from day 10 as compared with the other groups, and plateaued at day 12(P < 0.05). At day 14, the HN group showed the highest amount of calcium deposition. In vivo, the HN group showed histologically prominent new bone formation, increased calcium deposition, and higher collagen type Ⅰ?messenger RNA expression as compared with the NN *** The results of this study indicate that modifying oxygen tension is an effective method to enhance the osteogenic ability of MSCs used for OMCSs.
