Transitioning patients with inflammatory bowel disease from hospital-based to rapid home-based infliximab: A stepwise, safety and patient-orientated process towards sustainability

Variability of platelet aggregation in patients with clopidogrel treatment and hip fracture: A retrospective case-control study on 112 patients

AIm: To identify the rate of non-responders to clopidogrel treatment in hip fracture patients and study how non-responders differ from ***: In a retrospective case-control study we included 28 cases of acute proximal femoral fracture with clopidogrel treatment 2011 to 2013. Eighty-four controls from the same time period were included. Data collected included response to clopidogrel measured with multiple electrode aggregometry(mEA), intraoperative bleeding, erythrocyte transfusion, time to surgery and the incidence of adverse events up to 3 mo after surgery. RESULTS: Eight(29%) of the 28 cases were nonresponders. The median intraoperative bleeding was 300 mL(range, 0-1500), and was lower for non-responders(50 m L) but did not reach statistical significance. Erythrocyte transfusions did not differ between responders, non-responders and controls. Forty-five(40%) of 112 patients had adverse events postoperatively but the rate did not differ between patients with and without clopidogrel ***: almost one-third of patients withclopidogrel treatment and an acute proximal femoral fracture are non-responders to antiplatelet therapy and can be operated without delay.

Background: Curculigo recurvata(C. recurvata) is an enthnomedicinally important herb reported to have significant medicinal values. The present study aimed to explore the in vivo and in silico anti-nociceptive and anti-diarrh.al effects of a C. recurvate rhizome methanol extract(me-RCR).methods: The analgesic effects of me-RCR were assessed using acetic acid-induced writhing and the formalin-induced flicking test. The drugs were administered intraperitoneally(IP) at doses of 200 and 400 mg/kg body weight(bw). Anti-diarrh.al activity was evaluated by assessing intestinal motility, hypersecretion, and fecal score in mice at oral doses of 200 and 400 mg/kg·bw. Computer facilitated analyses for anti-nociceptive and anti-diarrh.al activities of three isolated compounds from C. recurvata were undertaken to identify the best-fit ***: The me-RCR showed significant(P heral anti-nociception at the highest dose. The extract inhibited both early and late phases of nociception in the formalin-induced writhing test. In the castor oil-induced diarrhoea model, the extract significantly(P he onset time of diarrhoea, inhibited percentage of diarrhoea, and decreased both the volume and weight of intestinal contents. Rates of intestinal fluid accumulation inhibition were(33.61 ± 1.00)% and(46.44 ± 0.89)% at me-RCR doses of 200 and 400 mg/kg·bw, respectively. moreover, a significant(P al motility was observed. An absorption, distribution, metabolism, excretion and/or toxicity(ADmE/T) test showed that the selected compounds yielded promising results, satisfying Lipinski's rule of five for predicting drug-like potential. Notably, of the three phytoconstituents curculigine and isocurculigine possessed the highest affinity for the COX-1 and COX-2. Isocurculigine was also identified as the most effective anti-diarrh.al compound in the computer-facilitated ***: An extract of the plant C. recurvata

Biochemical Syndrome in Adult Pyloric Stenosis, a Clue to Diagnosis in Emergency Department

Characteristic biochemical changes have been documented in patients presenting with the symptoms and signs of gastric outlet obstruction. These changes usually consist of hypochloremic, hypokalemic metabolic alkalosis and hypocalcaemia with or without renal failure. The aim of this report is to highlight these biochemical abnormalities again. moreover, we suggest conversely that the presence of these changes should prompt the clinician for further evaluation to rule out gastric outlet obstruction especially in the emergency department. Early intervention resulted in dramatic improvement in patient’s clinical and biochemical status.

Aflatoxins cause acute and chronic h.alth and production effects in humans and animals,*** study reports on the exposure of the Kenyan population to the two mycotoxins in two counties(Nandi and makueni)using children younger than 5 years as the proxy.A questionnaire was administered,which captured knowledge of the incidences of acute aflatoxin poisoning,food consumption *** of aflatoxin in samples of urine,breast milk,maize,sorghum,and millet was done using cELISA and *** and sorghum are used in the weaning formula of *** difference in consumption of maize-and sorghum-based diets in children between younger than 1 and between 1 and 5 years was significant with P=0.037 and P=0.002,respectively,in Nandi and *** children younger than 5 years,the consumption of maize ranged from 0.1 to 0.25 kg per person per day in Nandi and makueni with an aflatoxin exposure rate of 0.011 and 0.49μg per kg body weight(bwt)per day,*** exposure to aflatoxin through milk for children younger than 5 years was 4×10^(−4) and 1×10^(−4)μg per kg bwt per day in makueni and Nandi,*** exposure of nursing children through breast milk was 6×10^(−3) and 1×10^(−6)μg per kg bwt per day in makueni and Nandi,*** younger than 30 months in makueni had 1.4 times higher levels of aflatoxin m1(AFm1)in urine than those of the same age in *** stunting and severe stunting rates in makueni and Nandi were 28.7%,18.5%and 30.7%,16.5%,***,there is need for urgent mitigation measures,a constant surveillance program,strict regulations,and awareness creation directed at poor households to reduce dietary exposure to mycotoxins.

Pm2.5 filter sampling and components measurement were conducted in autumn and winter from 2014 to 2015 at a suburban site(referred herein as "LLh site") located in the southwest of *** offline aerosol mass spectrometry(offline-AmS) analysis and positive matrix factorization(PmF) were applied for measurement and source apportionment of watersoluble organic aerosol(WSOA).Organic aerosol(OA) always dominated Pm2.5 during the sampling period,especially in *** pollution was serious during the polluted period both in autumn(31.1 μg/m3) and winter(31.9 μg/m3),while WSOA accounted for 54.4%of OA during the polluted period in autumn,much more than that(21.3%) in *** oxidation degree of WSOA at LLh site was at a high level(oxygen-to-carbon ratio,O/C=0.91)and secondary organic aerosol(SOA) contributed more mass ratio of WSOA than primary organic aerosol(POA) during the whole observation *** winter,coal combustion OA(CCOA) was a stable source of OA and on average accounted for 25.1% of *** autumn,biomass burning OA(BBOA) from household combustion contributed 38.3% of WSOA during polluted *** addition to oxygenated OA(OOA),aqueous-oxygenated OA(aq-OOA) was identified as an important factor of *** heavy pollution period,the mass proportion of aq-OOA to WSOA increased significantly,implying the significant SOA formation through aqueous-phase *** result of this study highlights the concentration on controlling the residential coal and biomass burning,as well as the research needs on aqueous chemistry in OA formation.

Refractory multi-principal-element alloys(RmPEAs)exhibit high specific strength at elevated temperatures(T).however,current RmPEAs lack a balance of room-temperature(RT)ductility,high-T strength,and high-T creep *** density-functional theory methods,we scanned composition space using four criteria:(1)formation energies for operational stability:-150meV per atom;(2)higher strength found via interstitial electron density with Young’s moduli E>250 GPa;(3)inverse Pugh ratio for ductility:G/Bhigh melting points:T_(m)>2500°*** rapid bulk alloy synthesis and characterization,we validated theory and down-selected promising alloy compositions and discovered mo72.3W12.8Ta10.0Ti2.5Zr2.5 having well-balanced RT and high-T mechanical *** alloy has comparable high-T compressive strength to well-known moNbTaW but is more ductile and more creep *** is also superior to a commercial mo-based refractory alloy and a nickel-based superalloy(haynes-282)with improved high-T tensile strength and creep resistance.

Effects of shenmai and Aminophylline on Apoptosis of Small Airway Smooth muscle Cells and the Expression of Relevant Genes in Rats with Emphysema

The effects of shenmai injection (SmI) and am inophylline on apoptosis of sm all airway smooth muscle cells (SASmC) and the Fas/ Fas L expression in rats with papain- induced em physe- ma were *** emphysema model was established by a single intratrach.al instillation of *** and Fas/Fas L expression of SASmC were detected by im munohistochemistry SABC and TU NEL assay at day1,3,5 ,7,15 ,30 after modeling,and the effect of SmI and am inophylline on them were *** results indicated that the Fas/Fas L expression positive rate in SASmC was2 .31± 0 .5 5 /1.2 8± 0 .4 *** a single intratrach.al instillation of papain,the expression of Fas/Fas L positive rate in the placebo group was increased in a tim e- dependent *** could inhibit the expression of Fas/ Fas L ,but aminophylline couldn't. The positive rate of apoptosis in the control group was 0 .87± 0 .32 .After a single intratrach.al in- stillation of papain,the SASmC apoptosis positive rate in the placebo group was increased in a tim e- dependent *** SASmC apoptosis rate in all groups was declined after treatment with SmI,but the effect of am inophylline was *** dem onstrated thatin the patho- genesis of emphysem a Fas/Fas L played an important role in the regulation of SASmC apoptosis. SmI influenced the expression of Fas/ Fas L and declined SASmC apoptosis by inhibiting the releas- ing of inflamm atory m edia and played an im portant role in the therapy of em physema.
