A Single Center Experience in Biological Therapy for the Treatment of Rheumatoid Arthritis in Saudi Arabia

Background: Biological therapy is indicated in the treatment of RA (Rheumatoid Arthritis) after failure of disease-modifying anti-rheumatic drugs (DMARDS) by the ACR/EULAR recommendations. The objective of the study is to describe the characteristics of Saudi patients at the initiation of biological therapy and to evaluate clinical effectiveness of this therapy measured by the disease activity score DAS 28. Methods: This was a retrospective cohort study of RA (rheumatoid arthritis) patients in King Fahad Hospital in Jeddah, Saudi Arabia from January 2005-July 2011. Data were collected from the medical records of all RA patients on biological therapy including: demographics, disease characteristics, comorbid illnesses and DAS 28 score over a period of 1 year. Results: 139 patients were studied (mean age 46 ± 13 years), of which 118 (84%) were females;mean duration of affliction with RA was 7.2 years ranging 1-45 years. Rheumatoid factor (RF) was positive in 88 patients (63.3%) and one or more comorbidities were present in 102 patients (73.3%). First choice of biological drug was ADA (Adalimumab) 44 patients (31.7%) and RTX (Rituximab) was the 2nd frequently prescribed biological drug. Mean DAS 28 activity at baseline was in ADA 41 patients (6.10 ± 1.62), ETA (Etarnercept) 29 patients (6.64 ± 1.42) and RTX 50 patients (6.7 ± 1.32). Moderate to good EULAR response was obtained in 74%, 85.7% and 53.3% at 6 months in ADA, ETA and RTX patients respectively. Moderate to good EULAR response was obtained in 61.8%, 86.6% and 72% in ADA, ETA AND RTX patients respectively at 1 year of treatment. Therapeutic effectiveness was comparable with the response rates in published observational trials. Conclusion: Our data demonstrate daily clinical practice in management of RA. The pattern of prescription is in agreement with the ACR/EULAR recommendations for initiation of biologicals in the treatment of RA.

Role of Maxillofacial Radiologist in Ballistic Wound: Case Report with Literature Review

Gunshot injuries are rather serious but uncommon type of trauma in India. Radiologists can contribute substantially in evaluation and treatment of patients with a gunshot wounds. Plain films, CT, Angiography, and sometimes MR imaging are used to localize shots. This paper describes a case report of shotgun injury to the face and neck and also attempts to illustrate the spectrum of available imaging with relevant findings pertaining to bullets and shotgun pellets in gunshot injuries. Radiologists should be aware of the associated complications and forensic implications when they take on the task of interpreting these images.

Mathematical Analysis of Nipah Virus Infections Using Optimal Control Theory

The optimal use of intervention strategies to mitigate the spread of Nipah Virus (NiV) using optimal control technique is studied in this paper. First of all we formulate a dynamic model of NiV infections with variable size population and two control strategies where creating awareness and treatment are considered as controls. We intend to find the optimal combination of these two control strategies that will minimize the cost of the two control measures and as a result the number of infectious individuals will decrease. We establish the existence for the optimal controls and Pontryagin’s maximum principle is used to characterize the optimal controls. The numerical simulation suggests that optimal control technique is much more effective to minimize the infected individuals and the corresponding cost of the two controls. It is also monitored that in the case of high contact rate, controls have to work for longer period of time to get the desired result. Numerical simulation reveals that the spread of Nipah virus can be controlled effectively if we apply control strategy at early stage.

Impact of hepatitis C virus core mutations on the response to interferon-based treatment in chronic hepatitis C

AIM To determine whether hepatitis C virus(HCV) core substitutions play a role in the response to interferon-based treatment in Caucasian patients. METHODS One hundred eight HCV chronically infected patients initiating treatment with pegylated IFN plus ribavirin for 48 wk were tested for baseline substitutions at codons 70 and 91 of the viral core protein(Big Dye Terminator vers.3.1, Applied Biosystems,) and for genetic polymorphisms in host IL28 B gene rs12979860(Custom TaqM an 5' allelic discrimination assay; Applied Biosystems).RESULTS Of the patients, all were infected with HCV genotype 1b, 44.4% had low baseline HCV viral load, and 37.9% had mild/moderate fibrosis. Only 38.9% achieved therapeutic success, defined as sustained virological response(SVR). Eighty-eight percent of the patients presented at least one substitution at core position 70(R70Q/H) or/and position 91(L91M). The favorable IL28 B CC polymorphism was detected in only 17.6% of the patients. In the univariate analysis, young age(P < 0.001), urban residence(P = 0.004), IL28 B CC genotype(P < 0.001), absence of core mutations(P = 0.005), achievement of rapid virologic response(P < 0.001) and early virological response(P < 0.001) were significantly correlated with SVR. A multivariate analysis revealed three independent predictors of therapeutic success: young age(P < 0.001), absence of core substitutions(P = 0.04) and IL28 B CC genotype(P < 0.001); the model correctly classified 75.9% of SVR cases with a positive predictive value of 80.7%. CONCLUSION HCV core mutations can help distinguish between patients who can still benefit from the affordable IFNbased therapy from those who must be treated with DAAs to prevent the evolution towards end-stage liver disease.

Objective:To investigate the elemental composition of the leaves and flowers of Catharanthus roseus(***) due to the plant's wide application in the indigenous medicinal system and its chemical constituents' ***:The atomic absorption spectrophotometer was used for quantitative analysis of various ***:Total 13 important elements were analyzed in leaves and flowers of *** indicated the presence of Na,K,Ca,Mg,Cr,Fe,Zn,Al,Cu,Ni,Pb,Cd and Mn in both leaves and *** most important finding of the work was that,leaves of *** showed high concentration of all elements except K and Zn while flowers of *** showed higher concentration of K and ***:The elemental composition in both leaves and flowers of *** were found to be ***,different parts of this medicinal plant are enriched in some micro and macro nutrients like Fe,Ca,Na,K,Zn,which are very important for biological metabolic system as well as human health.

Feto-Maternal Outcome and Complications of Emergency Caesarean Section among the Patients Admitted at a Tertiary Care Hospital in Dhaka City

Background: Caesarean Section done in emergency basis can result some adverse outcome to both the fetus & mother & complications may arise. Objective: The purpose of the present study was to see the fetal & maternal outcome or any complications among the pregnant woman who had undergone emergency caesarean section. Methodology: This descriptive type of cross sectional study was conducted in the Department of Gynaecology & Obstetrics at Dhaka Medical College Hospital during the period from July 2006 to December 2006. The patients underwent emergency caesarean section for any indication during the mentioned period of study was selected as study population. Among them, 100 cases were recruited in this study. Result: Among the cases, maternal morbidity was as high as 64% which were due to haemorrhage, post-operative infection, rise of blood pressure and other complications. Regarding fetal outcome, 92% babies were born with normal birth weight, 33% babies were developed complications which were mostly due to birth asphyxia and prematurity. Conclusion: In conclusion, emergency caesarean section has maternal morbidity as well as fetal and neonatal adverse outcome.

Interleukin-1α (IL-1α) is a proinflammatory cytokine that has also been found to act as a paracrine mediator involved in the regulation of testicular functions. The present review provides an overview of the role of IL 1α in testicular physiology. Bioactive IL-1α isolated from adult rat testis was found to consist of three distinct immunoreactive protein species with apparent sizes of 45, 24 and 19 kDa. These isoforms showed bioactivity in a thymocyte proliferation and steroidogenesis assays with different biopotencies. The background of the molecular heterogeneity and processing, secretion and regulation of the isoforms of testicular IL-1α are discussed. All three isoforms have been found to be secreted into the testis tubular lumen and interstitial space. We have provided evidence that IL-1α is a paracrine factor that may be of importance in, e.g., the regulation of Leydig cell steroidogenesis. Pathophysiologically, testicular IL-1α may contribute to testicular relapse of acute lymphocytic leukemia in boys.

Childhood Obesity in Primary School Children of Middle and Upper-Middle Income Group in the Capital City of Bangladesh

Background: No published information is available on the status of childhood obesity in Bangladeshi primary school children of urban middle and upper-middle income group. The aim of the study was to explore the proportion of childhood obesity in the primary school children of middle and upper-middle income group in capital city and to gather information on their dietary pattern and physical activity. Methods: One hundred and fifty children (age 9 ± 1 years, height (ht) 118 ± 20 cm, weight (wt) 36 ± 8 kg, wt/ht% 126 ± 24, mean ± SD) were selected purposively from the primary schools of Dhaka city. On the basis of predefined scoring, subjects were classified as underweight, normal, overweight and obese if their weight-for-height values were 120% of median values for Bangladeshi children respectively. Dietary intake and energy expenditure of the study subjects were calculated by standard techniques. Results: The proportion of childhood overweight and obesity was 52% and 20%. The proportion of overweight and obesity was higher (78%) among girls compared to boys (54%). Similarly, height, weight and other anthropometrics indices were significantly (p < 0.05) higher in girls compared to boys. The daily intake of carbohydrate and protein was significantly (p < 0.05) higher among obese group. Total energy balance was significantly correlated with weight-for-height% (r = 0.541, p = 0.0001). Conclusions: A higher proportion of overweight and obesity had been found in girls’ children than their counter parts in this study. Positive energy balance with higher energy intake from carbohydrate and fast/processed food culture habit were found among primary school children of middle and upper-middle income group in the capital city of Bangladesh.

低调陈述是一种为了满足交际需要而故意用弱化的或有节制的词语代替较直接的、唐突的言辞的修辞手段，同时它也是委婉语的一种实现手段。随着语用学的发展，低调陈述被赋予了新的活力，低调陈述成为一种被广泛使用的语用策略。它可以通过多种词汇、语法和语篇手段获得意想不到的礼貌、幽默、讽刺效果。C-R-A（合作-关联-顺应）模型是廖巧云在2005年提出的，这是一个言语交际的三维模型，它把语用学三大理论合作原则、关联理论和顺应论结合起来，旨在取长补短，优势互补，建立一个可以从哲学、认知和社会三个维度解释交际过程的模型。在这个模型中，合作是成功交际的先决条件，关联是必要条件，顺应是调节机制。通过对交际言语的阐释初步证明了它的解释力，但是仍需检验。本文采用定性分析方法在C-R-A模型下解读低调陈述言语行为，旨在通过研究初步解决四个问题：（1）基于以往研究对低调陈述进行重新定义和分类;（2）说话者和听话者在C-R-A模式简单模型下的认知过程;（3）低调陈述在C-R-A模式扩展模型下的普遍路径;（4）通过研究验证对C-R-A模式作了一定程度地改进。笔者认为，（1）低调陈述可以根据实现手段分为词汇低调陈述、语法低调陈述和语境低调陈述;（2）C-R-A模型简单模式过于简化，因此在运用此模型对低调陈述进行解读时应该将合作、关联和顺应这三个关键词深化和丰富;（3）参见图形5.9，低调陈述在C-R-A模型扩展模式下是一个合作、最佳关联和隐性顺应的成功交际过程;（4）C-R-A模型扩展模式在对复杂低调陈述言语行为进行解读时，应根据话轮的数量及交际双方是否具有共同的交际目的而片开，不应将复杂的交际过程局限在一个演示模型下。
