Long-term effects of lamivudine treatment in Japanese chronic hepatitis B patients

AIM： To analyze the association between the emergence of tyrosine-methionine-asparatate-asparatate （YMDD） mutants （reverse transcription; rtM204I/V） and deterioration of liver function during long-term lamivudine treatment of Japanese patients with chronic hepatitis B virus （HBV） infection. METHODS： The data of 61 consecutive Japanese pa- tients with chronic hepatitis B who underwent continu- ous lamivudine treatment for more than 24 mo and had a virological response were analyzed. Analysis of YMDD mutants was done by real-time polymerase chain reaction with LightCycler probe hybridization assay for up to 90 mo （mean, 50.8 too; range, 24-90 too）.RESULTS： A mixed mutant-type （YMDD ＋ tyrosine-iso- leucine-asparatate-asparatate： YIDD or tyrosine-valineasparatate-asparatate： YVDD） or a mutant-type （YIDD or YVDD） were found in 57.4% of 61 patients at i year, 78.7% of 61 patients at 2 years, 79.6% of 49 patients at 3 years, 70.5% of 34 patients at 4 years, 68.4% of 19 patients at 5 years, 57.1% of 14 patients at 6 years, and 33.3% of 6 patients at 7 years. Of the 61 patients, 56 （92%） had mixed mutant- or a mutant-type. Only 5 （8%） had no mutants at each observation point. Vi- rological breakthrough was found in 26 （46.4%） of 56 patients with YMDD mutants, 20 of whom had a hepa- titis flare-up： the remaining 30 （53.6%） had neither a virological breakthrough nor a flare-up. All 20 patients who developed a hepatitis flare-up had a biochemical and virological response after adefovir was added to the lamivudine treatment. CONCLUSION： Our results suggest that it is possible to continue lamivudine treatment, even after the emergence of YMDD mutants, up to the time that the patients develop a hepatitis flare-up.

The distinct characteristics of photovoltaic generators related to power and current present a complex problem in terms of opti-mizing their power *** tackle this,maximum-power-point tracking techniques such as the adaptive neuro-fuzzy inference system are frequently utilized for their swift adaptability and reduced *** addition,the backstepping controller is often selected to handle both linear and non-linear systems due to its exceptional *** purpose of this research is to propose an innovative method that merges the adaptive neuro-fuzzy inference system and backstepping controller to refine the tracking of the optimal power point and to bolster the stability of the photovoltaic system in the face of unpredictable scenarios,such as those pre-sented by the Ropp irradiance examination,which utilizes a single-ended primary inductor converter as a stage for power electronics *** conducted using MATLAB®/Simulink®demonstrate that the combination of adaptive neuro-fuzzy inference system and backstepping controller achieves an impressive efficiency of 99.6%and exhibits fast,robust,and accurate responses com-pared with other algorithms such as artificial neural networks combined with the backstepping controller and conventional perturb and observe algorithm.

High moisture content wheat kernels are subject to elevated respiration rates due to enzyme activity and mould growth that reduce the dry grain and may produce suffi-cient energy which may be harmful to wheat *** aeration provides a powerful nonchemical stored grain insect ***,aeration is a suitable and economical device to overcome this *** moisture management is vital to prevent spoilage in stored *** objective of this study is to investigate the influence of using low relative humidity(RH)aeration on the wheat moisture *** numerical investigation based on heat and mass balances is developed and used to simulate the evolution of grain temperature and moisture under various air RH in a wheat storage *** show that the dehumidification of blown air had greater potential for decreasing RH of interstitial air and wheat moisture at 30◦C temperature and RH of 40%,50%and 60%.

Unravelling lncRNA mediated gene expression as potential mechanism for regulating secondary metabolism in Citrus limon

Plant long non-coding RNAs(lncRNAs)are emerging as important regulators of gene expression through various mechanisms *** to double-stranded DNA(dsDNA),physically targeting mRNAs and also act as microRNA(miRNA)*** secondary metabolites and their content determine the market value of Citrus plant products for culinary and industrial *** lncRNAs are emerging as important molecular players that regulate secondary metabolites in plants but the roles of lncRNAs are yet unknown for the regulation of secondary metabolites in Citrus *** this study,a total of 11814 lncRNAs were identified from four RNA-seq datasets comprising of data from four different tissues of *** ***,bud,fruit and ***-expression analysis of whole set of genes with identified lncRNAs revealed a total of 632 lncRNAs correlating with 5810 *** annotation of genes correlating with lncRNAs unveiled the association of 379 genes with the biosynthesis of secondary metabolite pathways and among them,36 and 12 genes are from terpenoids and flavonoids metabolism/biosynthesis pathways *** correlation analysis unfolded the possible role of 113 lncRNAs in terpenoids metabolism and 29 lncRNAs in the flavonoids metabolic *** nine lncRNAs were predicted to take part in miRNA-mediated gene regulation,out of which 10 lncRNAs were predicted to act as endogenous target mimics(eTMs)for miRNAs and 74 lncRNAs were found to act as miRNA *** of six predicted lncRNAs and seven correlated genes involved in terpenoids and flavonoids metabolism were validated using semi-quantitative RT-PCR.

Extended cancer-free survival after palliative chemoradiation for metastatic esophageal cancer

We report on a patient who remained cancer-free for an extended time after palliative radiotherapy(RT) and chemotherapy(nedaplatin plus 5-fluorouracil) treatment for stage Ⅳ(cT3N3M1) esophageal squamous cell carcinoma. Although multiple lymph nodes outside the RT field recurred, the local primary tumor within the RT field did not recur, even 17 mo after palliative RT of 30 Gy in 10 fractions. In this case, acute toxicity, such as myelosuppression or esophagitis, was not enhanced by increasing the fraction dose from 1.8-2.0 Gy to 3.0 Gy. Because 30 Gy in 10 fractions can be completed within a shorter time and is less expensive than 50.4 Gy in 28 fractions, we think that 30 Gy without oblique beams is a more favorable RT method for patients.

Mycotoxins are conventional and novel risk biomarkers for hepatocellular carcinoma

Hepatocellular carcinoma (HCC) is a common malignant disease with poor prognosis. To improve the clinical outcome, early diagnosis of HCC arising from nonviral agents and hepatitis virus is important. Among several etiological factors, mycotoxins defined as carcinogens by the International Agency for Research in Cancer (IARC) might be one of the critical risk factors for nonviral HCC. Aflatoxin B1 is the most well-known carcinogenic mycotoxin for HCC, but the role of the other types of mycotoxin remains unclear. Several studies have reported that a chromatographic separation technique based on high-performance liquid chromatography can successfully detect the concentration of mycotoxins in plasma. Recently, serum level of ochratoxin A (OTA), a widely distributed mycotoxin classified as Group 2B by IARC, was evaluated in HCC patients in Egypt. The results suggested that serum OTA levels might be a good biomarker for HCC. In this article, we review recent studies of OTA, and discuss its possible significance as a biomarker of HCC.

Effect of sphygmomanometer cuff pressure on the differentiation of veins from arteries on ultrasound imaging:an observational cross-sectional study

BACKGROUND:Ultrasound guidance is commonly used for accessing difficult peripheral *** successful access,a tourniquet is required for *** decrease the compressibility and increase the diameter of veins;they also obfuscate artery-vein differentiation on *** aimed to establish the upper limit of sphygmomanometer cuff pressure that facilitates artery-vein differentiation during ultrasound-guided peripheral intravenous ***:We employed the sphygmomanometer cuff as a tourniquet for venodilation and tested it on seven participants at six different levels as follows:0 mmHg,DBP/2,DBP,(DBP+SBP)/2,SBP,and SBP+20 *** used an ultrasound probe attached to a pressure-measuring instrument to record cross-sectional images of the cubital artery and *** ultrasonography,compression was applied to the blood vessels through the *** following day,we measured the wrist pulse pressure and observed the oximeter pulse wave at six different tourniquet pressure ***-measures analysis of variance(ANOVA)on ranks and Tukey’s post-hoc analysis were used for multiple ***:Arterial pulsation was maintained at tourniquet pressures between 0 mmHg and(DBP+SBP)/***,arterial pulsation decreased or disappeared when the tourniquet pressure reached or exceeded the ***,at this pressure level,the superior compressibility of veins compared to that of arteries was no longer *** of the artery to 75% and 50% of its original diameter increased arterial ***:Arterial pulsation and the superior compressibility are useful indicators for differentiating veins from arteries until the tourniquet pressure reaches(DBP+SBP)/***,these indicators are not reliable once the tourniquet pressure exceeds the SBP.

Relationship between arterial stiffness and cognitive function in outpatients with dementia and mild cognitive impairment compared with community residents without dementia

BACKGROUND It is unclear whether the dementia patients with Alzheimer’s disease(AD)and vascular dementia(VaD)and mixed dementia(MIX,including AD and VaD)would have more developed arterial stiffness as compared with local residents without *** aim of this study was to assess arterial stiffness and cognitive function in different types of dementia pa-tients[AD,VaD,MIX and mild cognitive impairment(MCI)]and community residents without *** This was a single-center,cross-sectional observational *** studied a cohort of 600 elderly outpatients with a complaint of memory loss,who were divided into four groups(AD,VaD,MIX and MCI).In addition,they were compared with 55 age-matched local residents without dementia(Controls).We assessed arterial stiffness by brachial-ankle pulse wave velocity(baPWV)and the global cognitive function by the Mini-Mental State Examination(MMSE).RESULTS The baPWV was higher in AD,VaD and MIX than in MCI and in Controls(P<0.05).The baPWV was higher in MCI than in Controls(P=0.021),while MMSE were compatible between them(P=0.119).The higher baPWV predicted the presence of AD,VaD,MIX and MCI with the odds ratio of 6.46,8.74,6.16 and 6.19,*** contrast,there were no difference in baPWV among three different types of dementia(P=0.191).The linear relationship between baPWV and MMSE was observed in the elderly with MMSE≥23(R=0.452,P=0.033),while it was not in dementia patients(MMSE<23).CONCLUSIONS The findings suggest that MCI and dementia patients have stiffer arteries as compared with age-matched local residents,although global cognitive function may be comparable between MCI and the local residents.
