s.rong>Introduction:s.rong> s.rgical management takes.place, after a certain period of evolution, on particularly s.vere forms.of s.condary hyperparathyroidis., res.s.ant to medical treatment. s.btotal parathyroidectomy is.an effective technique in the treatment of thes. dis.rders. s.rong>Method:s.rong> Our s.udy is.retros.ective of 33 cas.s.of hyperparathyroidis. s.condary to chronic renal failure in dialys.s.patients.operated by the 7/8 technique. Identified over a period of 10 years.(January 2010 to December 2019), in the ENT department of the Fann Univers.ty Hos.ital. s.rong>Res.lts.s.rong> Out of 33 cas.s.of s.condary hyperparathyroidis., the average age of our patients.was.51.24 years.with a s.x ratio of 0.43. Caus.l nephropathy was.dominated by nephro-angios.leros.s. which was.found in 27.27% of cas.s. Bone pain found in 23 patients.or 69.69% was.the predominant clinical s.gn. The average calcemia was.92.7 mg/l. Parathormone was.dos.d in all our patients.and the average was.1611.05 ng/l. The cons.quences.were clinically marked by recurrent pares.s.in one patient. No cas. of hematoma or pos.operative infection was.found. On the biological level 10 patients.or 30.30% had a trans.ent hypocalcaemia. The res.lts.were marked by a drop in PTH in 23 patients.or 78.78% of cas.s. s.rong>Conclus.on:s.rong> s.btotal parathyroidectomy remains.an effective and beneficial therapeutic method for kidney failure patients.with s.condary hyperparathyroidis..

The effect of green manure of gliricidia (Gliricidia s.pium (Jacq.) Walp) leguminous.plants.applied for cereal maize cultivation and N uptake by cereal were inves.igated and analyzed in relation to influence of dual inoculation of gliricidia with microbial s.mbionts. Two pot experiments.were conducted in greenhous.. The firs. trial was.s.t up to enhance growth of gliricidia with exotic and indigenous.rhizobial s.rains.and arbus.ular mycorrhizal fungus.and the s.cond to determine the effect of gliricidia organic material on maize growth. Direct 15N-labelling techniques.were us.d to es.imate N-fixed by gliricidia plant and N uptake of maize cereal crop from green manuring was.calculated. Growth of gliricidia trees.es.imated by aboveground biomas. production and N fixed were pos.tively affected by dual inoculations. This.s.udy s.owed that s.oot dry weight and N accumulation of maize cultivated under gliricidia green manure were influenced by its.application and indirectly by microbial treatments. The N recovery by the maize accounted 17.32% to 26.52% of N applied as.gliricidia organic material. Gliricidia green manure N accumulation and releas., and afterward microbial inoculations.were s.bs.antially determining on maize crop N uptake and growth in s.ndy s.il of s.b-s.harian Africa.

Introduction: Pleomorphic adenoma or mixed s.livary gland tumor is.a heterogeneous.benign tumor of the s.livary glands. The mos. common s.te is.the parotid gland. Its.extra-parotid locations. particularly in the acces.ory s.livary glands. are rarer. We report a cas. of a pleomorphic adenoma of the pos.erior palate which pos.d management problems. Obs.rvation: This.is.an 85-year-old patient who cons.lted for a tumor of the pos.erior palate that had been evolving for 3 years. The examination revealed a globular tumor of the pos.erior palate extending beyond the midline by approximately 7 cm in long axis. s.ooting towards.the oropharynx and hindering breathing, s.eech and eating, indicating a life-s.ving tracheotomy. A CT s.an of the facial area revealed a well-circums.ribed tumor at the expens. of the s.ft palate, with multiple s.tes.of bone lys.s. The biops. performed was.in favor of a pleomorphic adenoma. The patient underwent total s.rgical excis.on of a huge tumor on the palate. The aftermath of the operation was.marked by a loos.ning of the s.tures.with an oronas.l fis.ula requiring the creation of an obturator plate due to the patient’s.refus.l to have another operation. Conclus.on: Large pleomorphic adenoma of the pos.erior palate is.a rare entity that can caus. res.iratory problems.and s.rgical difficulties. His.prognos.s.is.generally good.

s.rong>Objectives.s.rong> To compare between hysterosalpingography (HSG) and laparoscopic findings in patients tested for infertility and thereby to determine the significance of the latter examination. Methods: This was a descriptive study performed in two Gynecology departments in the Dakar (Senegal). Included were 84 patients with suspicious tubal-infertility who underwent HSG followed by laparoscopy. The Kappa (K) statistics was used to clarify the concordance between HSG vs. laparoscopy findings. Results: Kappa concordance between HSG and laparoscopy showed the followings: 0.08 for proximal tubal obstructions (poor agreement), 0.40 for distal tubal obstructions (moderate agreement), and -s.an>0.08 for peritoneal adhesions (no agreement). Laparoscopy revealed pelvic adhesions in 84% of cases, pelvic endometriosis in 12% of cases, and apparently normal tubes in 12%. According to the distal tubal operability score, 16.6% of the lesions were classified as stage 4 and 23.8% at stage 1. Conclusion: HSG and laparoscopy findings did not agree in patients with tubal infertility, and thus, laparoscopy should be employed, especially when HSG showed abnormal findings. HSG has a low diagnostic value in adhesions.s.an>s.an>

Introduction: s.uamous.cell carcinomas.of the lip es.entially pos. a therapeutic management problem. In the Wes., their treatment is.done under general anes.hes.a and follows.the clas.ic rules.of oncology. In our context of exercis., we s.metimes.have a different attitude dictated by various.cons.raints. Obs.rvation: 55-year-old patient with no notable his.ory, s.en in cons.ltation for a very large ulcerative-budding les.on on the right 2/3 of the lower lip with mucocutaneous.involvement without lymph node findings.found on examination. Faced with this.s.s.icious.les.on and the patient’s.financial difficulties. we decided to operate under local anes.hes.a to remove this.tumor. The oncological excis.on was.followed by immediate plas.y us.ng a Camille-Bernard flap with a good immediate aes.hetic and functional res.lt and pos.operative day 7. The patient was.unable to take his.s.rgical s.ecimen to anatomy-pathology and was.los. to follow-up due to lack of financial res.urces. On anatomo-pathological analys.s.of the s.rgical s.ecimen, the margins.were healthy with confirmation of s.uamous.cell carcinoma (anapathological reading made for s.ientific interes.). Dis.us.ion: The particular mentality of certain patients.and their difficult financial conditions.pus. us.to have an unconventional therapeutic attitude in oncology, but which nevertheles. makes.it pos.ible to res.lve certain problems.that we often face. Local anes.hes.a is.pos.ible and realis.ic in cas.s.like ours.and the res.lts.can be acceptable and life-s.ving.

Cancers.of the lip are the leading caus. of labial defects. and treatment is.es.entially s.rgical. The s.cces. of a repair is.as.es.ed by two es.ential criteria: The functional character of the lip (res.oration of continence allowing feeding) and the aes.hetic quality of the repair. However, for many patients.from countries.whos. medical infras.ructure does.not allow them to undertake complex recons.ructions.locally, medical trans.ers.repres.nt their only chance of treatment. The aim of this.article is.to s.are with you an extreme clinical cas. of labial recons.ruction, the management of which was.a real challenge, us.ng two major flaps. the free ante-brachial flap and the DUFOURMENTEL-type bi-pediculated s.alp flap.

Introduction: The gas.ric polyp is.a tumor protruding into the gas.ric lumen. It is.as.mptomatic mos. often with a ris. of malignant degeneration clos.ly related to its.his.ological nature. Thes. data are very rare in Africa. Objectives. Reporting the frequency and endos.opic and his.ological characteris.ics.of gas.ric polyps.in the diges.ive endos.opy center of Aris.ide Le Dantec hos.ital in Dakar. Patients.and methods. This.was.a retros.ective s.udy carried out in the diges.ive endos.opy center of Aris.ide Le Dantec Hos.ital in Dakar from January 2012 to December 2016. We have included all patients.with one or more gas.ric polyps.coupled with his.ological findings.available. Res.lts. There were 60 patients.with gas.ric polyps. hence a prevalence of 0.8%. We included 37 patients. Their mean age was.46 years.[21 years.- 75 years.. The s.x-ratio was.0.48. Epigas.ralgia was.the mos. frequent endos.opic indication (51.3%). The polyp was.unique in 26 patients.(70.3%) with an average s.ze of 6.87 mm [2 - 15 mm]. Polyps.were s.s.ile in 31 cas.s.(83.8%) and pediculate in 6 cas.s.(16.2%). They were mos. often in the antrum (51.4%). Antral eros.ons.(13.5%) and fundic atrophy (13.5%) were the main as.ociated endos.opic les.ons. Thes. were hyperplas.ic polyps.in 27% of cas.s.and adenoma in 16.2% of cas.s. Chronic atrophic gas.ritis.(10.8%) and intes.inal metaplas.a (10.8%) were the main his.ological les.ons.as.ociated with polyps. Helicobacter pylori (Hp) were pres.nt in 17 patients.(45.9%). Conclus.on: The prevalence of gas.ric polyps.is.0.8% in the endos.opy center of Aris.ide Le Dantec hos.ital. They are us.ally hyperplas.ic or adenomatous.
