Determinants of Iron Consumption among Pregnant Women in Southern Senegal

The anemia by iron deficiency is a public health problem. To palliate the multiple maternal and fetal consequences, the WHO recommends the iron supplementation during at least 90 days to all pregnant women. The goal of our study is to study the determinants of this consumption in the Kolda area (Senegal). It’s the analytical cross-sectional study referred. We use the survey by clusters with 2 levels and it’s about all of the women who gave birth in Kolda area between February 2013 and January 2014. The dependent variable was iron consumption during at least 90 days and the independents variables were grouped on personal factors, knowledge and practices. Data were collected during a personal interview face to face. We used logistic regression to identify the determinants of this consumption. The average age of women surveyed in 1442 was 25.5 years. They had knowledge of iron consumption (93%) and the number of antennal consultation (ANC) (66%). The prevalence of pregnant women who consumed iron at least for 90 days was 51%. The factors associated with consumption were schooling (ORa = 2.49 [1.54 - 4.03]), health awareness (ORa = 1.61 [1.25 to 2.07]), knowledge about number of ANC (ORa = 1.54 [1.18 - 2.00]), councils on the benefits of iron (ORa = 2.66 [1.77 - 4.00]), the household wealth index (ORa = 1.83 [1.04 to 3.19]), number of ANC (ORa = 2.05 [1.56 - 2.69]), age pregnancy on the first ANC (ORa = 2 [1.47 - 2.7]) and iron prescription (ORa = 1.64 [1.25 - 2.16]). The prevalence of iron consumption during at least 90 days is low in Kolda area (51%);however, its determinants are identified;we can solve the problem by increasing communicate more about iron supplementation and antenatal consultation.

Cancers of the lip are the leading cause of labial defects, and treatment is essentially surgical. The success of a repair is assessed by two essential criteria: The functional character of the lip (restoration of continence allowing feeding) and the aesthetic quality of the repair. However, for many patients from countries whose medical infrastructure does not allow them to undertake complex reconstructions locally, medical transfers represent their only chance of treatment. The aim of this article is to share with you an extreme clinical case of labial reconstruction, the management of which was a real challenge, using two major flaps: the free ante-brachial flap and the DUFOURMENTEL-type bi-pediculated scalp flap.

Introduction: Squamous cell carcinomas of the lip essentially pose a therapeutic management problem. In the West, their treatment is done under general anesthesia and follows the classic rules of oncology. In our context of exercise, we sometimes have a different attitude dictated by various constraints. Observation: 55-year-old patient with no notable history, seen in consultation for a very large ulcerative-budding lesion on the right 2/3 of the lower lip with mucocutaneous involvement without lymph node findings found on examination. Faced with this suspicious lesion and the patient’s financial difficulties, we decided to operate under local anesthesia to remove this tumor. The oncological excision was followed by immediate plasty using a Camille-Bernard flap with a good immediate aesthetic and functional result and postoperative day 7. The patient was unable to take his surgical specimen to anatomy-pathology and was lost to follow-up due to lack of financial resources. On anatomo-pathological analysis of the surgical specimen, the margins were healthy with confirmation of squamous cell carcinoma (anapathological reading made for scientific interest). Discussion: The particular mentality of certain patients and their difficult financial conditions push us to have an unconventional therapeutic attitude in oncology, but which nevertheless makes it possible to resolve certain problems that we often face. Local anesthesia is possible and realistic in cases like ours and the results can be acceptable and life-saving.

Introduction: Sinonasal polyposis (SNP) is a chronic inflammatory disease of the mucosa of the nasal cavities and facial sinuses. It is characterized by an oedematous, multifocal and bilateral degeneration of the nasosinus mucosa, which originates in the lateral masses of the ethmoid, where it causes the formation of smooth, gelatinous, translucent and pyriform polyp lesions. The objective of this study was to review epidemiological, clinical, paraclinical data and evaluate the results of endoscopic surgical treatment. Patients and Methods: This is a retrospective study on 60 patients followed at the ENT department of the Fann National University Hospital Center, from January 2010 to December 2015. All patients with sinonasal polyposis were included in the study. Results: The average age of our patients was 38 years and the sex ratio (M/F) was 0.8. In the patients’ histories, we found 18% asthma and 10% Widal’s disease. The average consultation time was 8.5 years. All patients had consulted for nasal obstruction;rhinorrhea was bilateral and found in 67.7% of cases, with olfactory disorders accounting for 50%. The CT scan performed in 58% of cases made it possible to specify the extent of the lesions;the involvement of the ethmoidal sinus was constant and extended to the other sinuses except in 2 cases. All patients had received medical treatment with local corticosteroids. Endoscopic surgical treatment was initiated in 43% of cases after failure of corticosteroid-based medical treatment. The evolution under treatment marked by the reappearance of symptoms that increased each month. At one month postoperatively, all clinical symptomatology had improved with the exception of olfactory disorders, which persisted in 3 patients. At 12 months we noted 12 cases of reappearance of nasal obstruction. Conclusion: SNP is a disease of little known etiology. The diagnosis is almost always clinical. The fruit of Morinda citrifolia L., commonly known as noni, has an extensive history of use as a food and traditional medicine around the world. Adding value to Morinda citrifolia L. products, particularly the fruit, could be one way of building resilience in vulnerable farming households. The aim of this study was to determine the secondary metabolite and mineral composition of noni juice obtained by fermenting the fruit of Morinda citrifolia L. Fruits were collected in August 2022 from the local field in Thiès region, West of Senegal. Extraction yields were determined and the secondary metabolites were determined using conventional analytical methods. Calcium, magnesium, iron, sodium and potassium were determined by atomic absorption spectrophotometer coupled with a CCD detector. The results show that an average fruit mass (503.2 ± 110.96 g) consists of 171.44 ± 50.01 g pulp and 34.06 ± 10.35 g seeds. The traditional extraction yield of noni juice is 16.46% after three weeks of fermentation. The contents of total polyphenols, flavonoids and tannins obtained in noni are 608.97 ± 4.53 mg EAG/100mL, 7.78 ± 0.01 mg EQ/100mL and 0.191 ± 0.01 mg EC/100mL respectively. The ethanol content of noni varies from 3.57 to 5.23 mL/100mL during extraction. Noni has a high calcium content with a concentration of 383.79 ± 33.23 mg/L. This is followed by a good concentration of magnesium, potassium and sodium, at 278.47 ± 26.30, 187.43 ± 10.7 and 155.95 ± 28.66 mg/L respectively. Noni also has an iron content of 202.15 ± 0.05 mg/L.

Seasonal Performance of Solar Power Plants in the Sahel Region: A Study in Senegal, West Africa

The main objective of this study is to evaluate the seasonal performance of 20 MW solar power plants in Senegal. The analysis revealed notable seasonal variations in the performance of all stations. The most significant yields are recorded in spring, autumn and winter, with values ranging from 5 to 7.51 kWh/kWp/day for the reference yield and 4.02 to 7.58 kWh/kWp/day for the final yield. These fluctuations are associated with intense solar activity during the dry season and clear skies, indicating peak production. Conversely, minimum values are recorded during the rainy season from June to September, with a final yield of 3.86 kWh/kW/day due to dust, clouds and high temperatures. The performance ratio analysis shows seasonal dynamics throughout the year with rates ranging from 77.40% to 95.79%, reinforcing reliability and optimal utilization of installed capacity. The results of the capacity factor vary significantly, with March, April, May, and sometimes October standing out as periods of optimal performance, with 16% for Kahone, 16% for Bokhol, 18% for Malicounda and 23% for Sakal. Total losses from solar power plants show similar seasonal trends standing out for high loss levels from June to July, reaching up to 3.35 kWh/kWp/day in June. However, using solar trackers at Sakal has increased production by up to 25%, demonstrating the operational stability of this innovative technology compared with the plants fixed panel. Finally, comparing these results with international studies confirms the outstanding efficiency of Senegalese solar power plants, other installations around the world.

Nasopharyngeal liposarcoma is an extremely rare clinical entity. Less than ten cases have been reported in the literature. It mainly occurs in humans. The non-specificity of the clinical and radiological signs explains the difficulty of the diagnosis confirmed by anatomopathological and immunohistochemical examinations. Its pathogenesis remains obscure and often correlates with genetic abnormalities. Pleomorphic liposarcoma is its rarest subtype, remarkable for its aggressiveness and rapidity of growth. Complete surgical excision remains the treatment of choice. We report a case of liposarcoma whose diagnosis was made during histological examination coupled with immunohistochemistry. .

Introduction: Life expectancy improvement for people living with Human Immunodeficiency Virus (HIV) is coming up against the problems associated with aging and chronic diseases. Frailty is a concern affecting a growing number of patients, particularly the elderly in this population. Our study aimed to determine the prevalence of frailty and its predictors on people living with HIV aged 50 years and older followed at the Outpatient Treatment Clinic (CTA) in Dakar. Methodology: We conducted a cross-sectional study of descriptive and analytic purposes ranging from November 2022 to August 2023, in CTA, Dakar (Senegal). We included people living with HIV aged 50 years and older under antiretroviral therapy for at least 6 months (≥6 months). Frailty was considered according to Fried criteria with a score ≥3. To identify the predictors of frailty, we performed a multivariate logistic regression analysis using STATA software version 18. Results: We included 199 patients. The median age at the moment of the study was 58 years old [50 - 91] with a sex ratio (M/F) of 0.58. The most representative age group was that of [50 - 59] years (59.3%). HIV-1 profile was most common in 89.45%. The median duration under antiretroviral therapy was 180 months [6 - 284] and 94% of patients received a Tenofovir Disoproxil Fumarate (TDF)-containing regimen with 43% of them for at least 10 years. Viral load was undetectable (≤40 copies/ml) in 98% of cases. WHO Stage III was more common at inclusion and 55.78% had nadir TCD4+ Lymphocyte counts Conclusion: Our study confirms the high prevalence of frailty among older people living with HIV. Its prevention should consider the management of comorbidities and the implementation of non-pharmacological interventions such as nutrition.

The operating temperature is a critical factor affecting the performances of photovoltaic(PV)*** this work,relevant models are proposed for the prediction of this operating temperature using data(ambient temperature and solar irradiance)based on real measurements conducted in the tropical *** each weather condition(categorized according to irradiance and temperature levels),the temperatures of the PV modules obtained using the proposed approach is compared with the corresponding experimentally measured *** results show that the proposed models have a smaller Root Mean Squared Error than other models developed in the literature for all weather conditions,which confirms the reliability of the proposed framework.
