Pulmonary and Orbital Bifocal IgG4 Disease: Case Report and Literature Review

Introduction: IgG4 disease is an orphan, fibro-inflammatory autoimmune disease of recent discovery whose thoracic involvement is rarer. We report a case of Pulmonary and orbital bifocal IgG4 disease with a review of the literature. Observation: This is a 71-year-old patient with a history of hypertension, dyslipidemia, smoking cessation at 15 PA, with progressive dyspnoea, weight loss of 4 kg with PS = 0 for 2 months. The thoracic CT scan revealed 3 pseudotumoral lung lesions of the LIG, LID and LM. The histology of the two CT-guided lung biopsies and the LIG wedge had objectified inflammatory lesions without signs of malignancy. The evolution was marked by the occurrence of a right orbital edema. The cerebral scanner found a voluminous right orbital inflammatory pseudotumor. Biopsy with histology found fibroinflammatory lesions with lymphoplasmacytic infiltrates and positive immunolabeling with anti-IgG4 antibodies. The PET scanner had objectified pulmonary and pleural parenchymal consolidations and moderately hypermetabolic mediastinal ADP with max SUV between 3 and 6. The patient was put on corticosteroid therapy with a favorable outcome. Conclusion: IgG4 disease is rare and difficult to diagnose despite well-defined and consensual diagnostic criteria and classification. The discovery of new biomarkers facilitates the diagnosis and monitoring of patients. Well-codified corticosteroid therapy is effective but possibility of recurrence. The current challenge remains the lack of data on the follow-up of these patients to assess the risk of neoplasia (lymphoma).

Rhupus: About Two Clinical Cases in Niger and Literature Review

Background: Rhupus is the rare association of two autoimmune diseases, systemic lupus erythematosus (SLE) and rheumatoid arthritis (RA) diagnosed according to validated ACR criteria. We present the first series of rhupus in Niger. Observation 1: This is a patient suffering from chronic polyarthritis in whom the diagnosis of active seropositive, erosive and deforming RA had been made according to the 1987 ACR criteria and SLE on the presence of 4 criteria according to the 1997 ARA criteria and putting under conventional synthetic DMARDs. The outcome was favorable with a remission achieved after 8 weeks with a disease activity score (DAS) 28 at 2.2. Observation 2: 58-year-old patient with known diabetes type 2 in whom the diagnosis of rhupus was made according to the ACR/EULAR 2010 criteria for RA with a DAS28 of 3.6 and the 2019 LES criteria at 18 points with a SLEDAI score of 6. The evolution was marked by the occurrence of a myocardial infarction with a favorable outcome under medical management. Conclusion: Rhupus is a rare entity that can be complicated by cardiovascular events with the presence of an independent cardiovascular risk factor.

Preliminary Results of Dementia Prevalence among Elderly People at Home by Cognitive Disorder Adapted Test of Niamey (Niger)

Introduction: Neurocognitive disorders are frequent with aging and are often seen at advancing stages in our context. It is really difficult to evaluate rapidly Nigerien elderly persons according to tools available on cognitive status while there is a great number of uneducated elderly people who suffer from cognitive deficiency. The purpose was to determine neurocognitive disorders prevalence in uneducated groups and Muslim elderly people by using the cognitive disorder examination (Codex) test adopted in the population living at home in Niamey (Niger). Materials and Methods: These are the results of a preliminary prospective study with simple three (3) random sampling concerning elderly people aged 60 and over living at home in whom the codex test adapted to Niamey was administered in uneducated Muslim elderly people for a total duration of four (4) minutes. This screen-adapted test should be completed by the mini-mental test of Senegal which is adapted globally to uneducated people in hospitals. The basic Codex test was developed in France for educated people, so we used this basic test in our educated people in the same study. Results: A total of 198 patients had been collected, of which 51.5% were female with an average age of 68 years with extremes of 60 to 84 years. 40 were educated. Among educated persons, 62.5% had a very low probability of dementia and 5% had a very high probability of dementia in the first step of the Codex. In the second step, 12.5% had a low probability of dementia and 20% had a high probability in the same group, while 51.3% had a very low probability of dementia and 21.3% had a very high probability of dementia in the first step of CODEX in uneducated elderly. In the second step in the same group, 12.6% had a low probability and 14.5% had a high probability of dementia. Conclusion: The adapted codex test of Niamey is simple and could be rapidly used to screen dementia among uneducated elderly Muslim prayers, and then could confirm it

Introduction: Cerebral palsy is the most common motor disorder of childhood according to the Center for Disease Control and Prevention (CDC). It is much more common in premature infants. The majority of fixed organic lesions occur during the development of the central nervous system. Objective: To study the epidemiological, clinical, paraclinical and prognostic profile of children aged 0 to 5 years with Cerebral Palsy (CP) seen in consultation in the medical neurology department of the National Hospital of Niamey. Methods: This was a transverse descriptive study with prospective collection of patient records (0 - 5 years) over a 6-month period. Results: During our study, 100 children aged 0 to 5 years had to be consulted. The prevalence was 61%. 62.3% of patients were male. The mean age was 2.01 years. The majority of the patients were from the urban area (63.93%). Almost half of the mothers had no schooling. 4.92% of patients were from consanguineous marriages. Pregnancies were monitored with at least 4 prenatal visit in 86.98% of cases and 83.61% of children were born at term. In 75.41% of cases, patients were born by eutocic delivery. In 36.07% of cases, the patients were resuscitated at birth. Fetal distress was present in 29.5%. Seizures were present in 49.18% of the patients of which 50% were generalized. Tonicity was not good in 35.15% and among them 52.63% had hypertonia. 18.03% of patients had a motor deficit. For 34.43% of the patients the first nerve was affected, followed by the third nerve with 13.11%. 65.57% of the patients had fixed retardation and for 78.69% of the patients the retardation was homogeneous. Neuromalaria, meningitis and hydrocephalus were the most incriminated causes of delay in 63.51%, 19.67% and 18.03% of cases respectively. Von Willebrand’s disease was first described in 1926 by Erik Von Willebrand. It is a genetic, constitutional defect of hemostasis that is different from hemophilia. It is classified among the rare diseases whose clinical manifestations are dominated by a hemorrhagic profile, which varies from patient to patient. It is an easily diagnosed disease based on a quantitative and qualitative bioassay of VWF. Treatment is multidisciplinary and is based on well-structured prevention. We report the first case of von Willebrand disease in Niger, diagnosed in the Hematology Department of Niamey’s National Hospital.

Opinions of Students on the Contribution of the DIVA 3D Virtual Dissection Table to the Teaching of Anatomy at the Faculty of Medicine and Odontostomatology of Bamako

Objective: Study the contribution of the DIVA 3D dissection table in the teaching of anatomy at the Faculty of Medicine and Odontology of Bamako. Material and Methods: This was a qualitative study carried out from November 1 to December 30, 2023 at the clinical and morphological anatomy laboratory of the Faculty of Medicine and Odontostomatology of Bamako. Included in this study were students who participated in practical and tutorial sessions. The variables studied during this study were: the previous performance of dissection on a cadaver by the students, the opinion of the students on dissection on a cadaver, the replacement of dissection on a cadaver by virtual dissection in the absence of a body, the level student satisfaction. Results: We surveyed 130 participants. The average age was 22 ± 0.2 years with extremes of 17 and 29 years. 95.3% of participants were students. According to 66.7% of participants, virtual dissection is a good palliative in the absence of a corpse. 95.3% of participants found using the virtual dissection table easy with an average of 7.88 ± 1.4. The overall assessment was well rated by 99.3% of participants. Conclusion: According to the results of this study, the virtual dissection table should be improved by integrating commented videos. The use of the DIVA 3D virtual dissection table during practical and tutorial sessions is well appreciated by the students. We believe that the teaching of anatomy using 3D digital technology should be included in the study programs of the Faculty of Medicine and Odontostomatology.

Type 1 or 2 diabetes can be complicated by numerous non-specific joint conditions. Sometimes revealing, these affections are frequent but often remain unrecognized. However, they are the cause of a significant functional handicap. The objective of this work is to describe the osteoarticular rheumatological disorders in diabetics consulting or hospitalized in the rheumatology and internal medicine departments of the National Hospital of Niamey and the Magori Polyclinic. This was a cross-sectional study at the Rheumatology Department in collaboration with the Internal Medicine Department within the National Hospital of Niamey and Polyclinic Magori over a period of 7 months (December 2018 to July 2019). One hundred (100) diabetic patients with osteoarticular diseases were collected, 75% of whom were women with a female/male sex ratio of 0.33. The average age was 56 years with extremes of 21 and 90 years. Forty-three percent (43%) of the patients had diabetes for more than 10 years, with an average age of 13 years. The average BMI was 28.08 ± 5.42 kg/m2. Poor glycemic control was found in the majority of patients (92%). Gonarthrosis was the most common disease associated with diabetes (89%). In the context of joint stiffening syndrome, the most frequent problem was a protruding finger, found in 4% of patients, followed by retractile capsulitis in 3% of the 100 cases. Infectious complications were dominated by septic arthritis in 4% of patients. The presence of one or more degenerative complications of diabetes (nephropathy, retinopathy, nephropathy) was the most frequent factor associated with osteoarticular diseases in our diabetic patients. This work underlines the great variety and frequency of osteoarticular affections during diabetes, hence the interest in optimal glycemic control of a close collaboration between rheumatologists and endocrinologists in order to avoid as much as possible the development of these rheumatological affections which are the so

Assessing the Effects of Land Use/Land Cover Change on Groundwater Recharge in a Sudano-Sahelian Zone: Case of Koda Catchment, Mali, West Africa

Groundwater is the main source of water in the studied area;therefore, it is significantly requested in all the activities of the inhabitants. These natural resources are affected by some drivers especially Land Use/Land Cover (LULC) and Climate Change. A Land Use/Land Cover (LULC) dynamics study is crucial for any global environmental change evaluation. For instance, for a given place, its change could affect considerably water cycle components. Therefore, the knowledge of the effects of LULC on groundwater recharge is then the key in water resources management system, in particular for the decision makers of the Koda Catchment where the scarcity of the water availability for agriculture is real. The spatiotemporal variation of the different units of LULC present in the catchment has been examined in this study. The Envi 4.5 Software coupled with ArcGIS using the Supervised Classification method, was applied to subset Landsat images from 1990 to 2016. Five (5) major LULC categories, cultivated land, bare land, herbaceous savannah, shrubby savannah and degraded savannah, were identified in the catchment. In a parallel direction, the groundwater recharge has been estimated through the conceptual Gardenia model for the same period 1990-2016. The results showed that the portion of cultivated land and bare land increased (14.9% and 23.5% respectively) while, the portion of savannah decreased: herbaceous savannah by 24.4%, degraded savannah by 10.32% and Shrubby Savannah by 3.6%. Savannah areas in Koda catchment is converted to agricultural land and urban area due to human activities. The decline of 8.4% in groundwater recharge might become so far obvious in the future if the current rate of deforestation continues in the Koda catchment. There is a need to closely monitor the changes in LULC for sustainable development. The results of this study could help to well understand the recharge pattern across Koda catchment under a changing LULC.

Effect of Polyethylene Terephthalate Plastic Waste on the Physico-Mechanical and Thermal Characteristics of Stabilized Laterite Bricks

The present work investigated the effect of polyethylene terephthalate (PET) plastic waste on the physico-mechanical and thermal properties of cement-stabilized laterite bricks to see the durability of the modified bricks (CSLB). Samples were formulated by mixing laterite, cement, and different percentages of PET (0%, 3%, 5%, and 7%) by volume. The bricks were produced using the M7MI Hydraform standard interlocking block and kept in the shade for a curing period of 28 days. The addition of 3% to 5% PET to the laterite stabilized with 10% cement results in a decrease in both dry and wet compressive strength, which is determined using the Controlab compression machine. However, the obtained results are in concordance with the standards. The thermal conductivity of CSLB, determined using the box method with the EI700 measurement cell, decreases as the PET content of the mixture increases. A decrease in bulk density from 1.67 to 1.58 g/cm3 was observed.
