Otorhinolaryngology Diseases in HIV-Positive Patients: 208 Cases at the Donka National Hospital

Introduction: Otorhinolaryngology (ENT) diseases in patients living with Human Immunodeficiency Virus/Acquired Immunodeficiency Syndrome (HIV/ AIDS) are relatively common and of concern to ENT specialists and other health professionals. Aim: It was to describe the epidemiological and diagnostic aspects of ENT and cervicofacial diseases in patients living with HIV/ AIDS. Material and Methods: This was a descriptive study with prospective data collection, conducted over a period of six months (September 16, 2019 to March 16, 2020), carried out at the ENT and Infectious Diseases Departments of the Donka National Hospital. Results: Of 522 HIV-positive patients, 208 (39.8%) presented with ENT diseases. Women represented 62.5% with a sex ratio of 0.6. The mean age was 42.21 years with extremes of 16 and 64 years. Housewives were the most represented (29.8%). Married people were the most affected (76.4%). Cervical involvement was present in 7.7% of patients. Otologic involvement accounted for 43.7%. Oral cavity and pharyngolaryngeal involvement accounted for 50.9%. Nasosinus involvement accounted for 55.7%. HIV type I was the most common (99.5%). Conclusion: Otorhinolaryngology diseases were frequent in HIV positive patients. They may constitute the first reason for consultation. However, these ENT diseases remain in appendix of the opportunistic diseases of HIV/AIDS taking the front stage.

Bioclinical Study of Coronavirus Disease 2019 in the Laboratory of Virological Hemorrhagic Fever in Guinea

Introduction: This study aimed to describe the clinical and microbiological characteristics of individuals confirmed to have coronavirus disease 2019 (COVID-19) in the laboratory of virological hemorrhagic fever in Guinea. Method: This was a cross-sectional and descriptive study conducted from March 19 to June 19, 2020, in the virological hemorrhagic fever laboratory in Guinea. Samples collected from individuals living in the Conakry region, which were positive for the reverse-transcriptase polymerase chain reaction test for COVID-19, were included in this study. Results: Among the confirmed cases (N = 743), the males (n = 440, 59%) were more than the females (n = 303, 41%). The mean age of the confirmed cases was 33.31 ± 16.08 years. Almost all the confirmed cases (n = 515, 69.3%) had contact with a confirmed case of COVID-19. At the time of collection, 78.7% (481) of the cases were asymptomatic, while 35.3% (262) were symptomatic. At the time of the sample collection, the main symptoms were fever (51.53%), cough (49.24%), physical asthenia (46.18%), sore throat (33.2%), headache (28.62%), anosmia (9.92%), nausea, vomiting, and abdominal pain (5.34%), and diarrhea (1.53%). Only 2% of the cases had comorbidities, including diabetes (n = 7) and hypertension (n = 5). The viral load of the asymptomatic and symptomatic patients was similar with the cycle threshold value, all of which varied from 10 - 19, 20 - 29, and 30 - 36, respectively. In the first test, 27 samples were declared undetermined;two days later, samples were re-taken and re-tested and 18 of them were positive for COVID-19. The average length of hospitalization was 14 days for patients admitted to the hospital. On the other hand, the non- hospitalized patients isolated themselves at home for 22 days. Pronostic Factors of Cerebral Toxoplasmosis in Department of Infectious and Tropical Diseases at Donka National Hospital

Objectives: To identify predictive factors for poor prognosis during cerebral toxoplasmosis at Donka Hospital. Methods: It was a retrospective study of descriptive and analytic type lasting one year six months (18 months) from January 1st, 2016 to June 30th, 2017 which involved patients admitted and hospitalized for cerebral toxoplasmosis in HIV field. Data enter was performed by Epi data 3.1 software and SPSS 21 software for statistical analysis. The threshold of significance was p Results: We observed 87 cases of cerebral toxoplasmosis (CT). The mean age was 38.53 ± 12.16. The clinical signs were mainly infectious syndrome (100%), headache (69.0%), confusion (46.0%) and meningeal syndrome (41.4%). The lethality was 37.9%. Living with a partner (p = 0.007), CD4 at initiation of antiretroviral therapy 3 (p = 0.009), and coma (p = 0.02) were the factors associated with death. Conclusion: This study showed that cerebral toxoplasmosis is associated with very high morbidity and mortality in the Infectious Diseases Department of Donka National Hospital. Living in a relationship, CD4 counts at baseline 3 and coma were independently associated with death. Special attention to these factors associated with infectious resuscitation and primary prevention in patients with a CD4 T lymphocyte count below 200 cells/mm may improve the prognosis of this pathology.

Background: The Ebola Virus Epidemic epidemic in Guinea was marked by its rapid spread in the capital, forcing the authorities to build several Ebola treatment centers including Wonkifong (Coyah), Conakry. Objective: To evaluate the case fatality rate of Ebola Virus Disease in this center and to identify associated factors. Patients and Method: There is a prospective study at wonkifong Ebola treatment center from January 1st, 2014 to April 30th, 2015. All patients diagnosed with an Ebola Virus Disease from January 1st, 2014 to April 30th, 2015 were enrolled in this study. Kaplan-Meier curves and multivariate regression model were used to analyze factors associated with death. Results: 216 patients were included, the mean age was 30.52 ± 17 years and 53.2% were female. The average time between admission to the Ebola treatment center and the onset of signs was 5.13 ± 3.8 days. The average period of stay in the Ebola Treatment Center was 12.64 ± 11.31 days and duration median of death after admission in Wonkifong ETC was 11 days. According to the source, more than half of the patients were from Forécariah, i.e. 60.2% (n = 134). The case fatality rate was 64.4%. This rate was 72.0% in Dubréka, 65.1% in Coyah and 64.6% in Forécariah. The factors independently associated with Ebola Virus Disease death were signs of diarrhea [OR = 2.22 (1.17 - 4.20)], anorexia [OR= 0.40 (0.19 - 0.81)] and period of patients stay in ETC [OR = 0.96 (0.93 - 0.99)]. Conclusion: These results show that the fatality rate linked to the Ebola virus disease remains high. The factors independently associated with death were Diarrhea, anorexia and period between beginning of illness and date of latest news. Biological monitoring and resuscitation seem interesting for the reduction of this mortality.

Clinical and Therapeutic Aspects of Temporomandibular Ankylosis at the Conakry Teaching Hospital

Introduction: Temporomandibular joint ankylosis (TMJA) is a fusion of joint surfaces by fibrous or bone tissue and the resulting limitation of mouth opening. The objectives of this study were to determine the frequency of TMJA, describe the clinical aspects and assess management. Materials and Method: It was a retrospective study that was carried out in the Department of Odontostomatology and Maxillofacial Surgery of Donka National Hospital for a period of 5 years (January 2016 to December 2020). Included were all records of inpatients and managed cases of TMJA during the study period. Socio-demographic, clinical and therapeutic variables were analyzed. Results: During the study 13 TMJA cases were collected with the frequency of 0.47%. The age group 1 - 9 was the most affected (61.54%) with extremes of 4 and 28 years. Men were the most concerned (53.87%). Restriction of mouth opening was the main reason for consultation (69.23%). The etiologies of ankylosis were dominated by infection including cellulitis of dental origin (53.85%) followed by facial trauma (30.77%). Arthroplasty was the most commonly used technique (73%), two cases of recurrence were noted (15.38%). Conclusion: TMJA affects mainly children and the etiology is dominated by cellulite of dental origin hence the interest of sensitization of the population for early management of oral diseases.

In this article we make a detailed study and a presentation of the different models of circuit’s equivalent to silicon-based photovoltaic solar cells. Starting from a real solar cell and real phenomena from the manufacture of the cell to the production of current by the cell. A comparison of the models with a real experimental method is carried out. The comparison is based on an overlay of the results. The study allowed us to choose the most suitable model. We are interested in the losses by leaks and the losses due to the development of the cell. In fact, we studied the influence of the shunt resistance on the current-voltage characteristic and the electrical power.

Epidemiology and Clinical Profile of Behcet’s Disease in a Sub-Saharan Country: About Five Observations

Behçet’s disease (BD) is a multi-systemic vasculitis characterized by a triad made up of oral aphthosis, genital aphthosis and uveitis which may or may not be associated with visceral involvement. Other times described in the ancient silk road, BD is increasingly found in countries south of the Sahara with the mixing of populations. Our objective was to describe the epidemiology and clinical characteristics of BD in an African country. Methodology: This was a five-year retrospective study in the internal medicine department of Donka National Hospital. We used international criteria finding BD. Urinary dipstick was use for finding kidney disease. Result: During the period, five cases of Behçet’s disease were collected. The average age was 35 years old. The male sex was more represented with 3 out of 5 cases. The most common clinical manifestation was oral aphthosis in all patients (100%), followed by genital aphthosis in 4 cases (80%). The other clinical manifestations observed were uveitis in 3 cases (60%), joint manifestations such as arthralgia in 3 cases (60%) and neurological manifestations such as chronic headache in one case (20%). HLA-B51 was found in two cases. The pattergic test done in two patients came back positive (100%). The patients received colchicine and oral corticosteroid therapy. Two patients were lost to sight. We deplored a death probably in a neuro-Behçet table before the rebellious headaches. Conclusion: Behçet’s disease is a multi-systemic vasculitis that must be evoked and managed quickly in a tropical environment, especially since the prognosis, vital and functional prognosis can be engaged.

Introduction: Obliterative arterial disease of the lower limbs is a marker of advanced atherosclerosis. It is one of the world’s most common cardiovascular diseases, present in one in five people over the age of 60, and carries an increased risk of morbidity and mortality. The aim of this study was to assess the knowledge of patients and their carers about obliterative arteriopathy of the lower limbs at Conakry University Hospital. Material and Methods: We conducted a descriptive cross-sectional study lasting 3 months, from March 12 to June 12, 2022 in the hospitals of the University Hospital of Conakry (Donka and Ignace Deen). Following free and informed consent, they were interviewed on the basis of a pre-established questionnaire. Socio-demographic data (age, gender, level of education) were collected, and knowledge of obliterative arterial disease of the lower limbs. Results: The study involved 159 people, comprising 106 patients (66.66%) and 53 accompanying persons (33.34%) in the hospitals of the University Hospital of Conakry (Donka and Ignace Deen). The 60 to 79 age group was the most represented (44.7%), with an average age of 55.4 ± 15.9 years and extremes of 20 to 84 years. Half (47.8%) had not attended school, and almost all (96.4%) of those who had attended school had a low level of education (primary and secondary). Almost all of them (91.2%) had never heard of AOMI. Only 5% identified at least one risk factor, diabetes, hypertension and smoking. Amputation was the most frequently identified complication. About 98% did not know that AOMI is associated with myocardial infarction and stroke, and 93.7% did not know of any complications. Almost all the participants (99.4%) had poor knowledge of the following complications. Conclusion: Obliterative arterial disease of the lower limbs could be prevented or delayed by screening and controlling risk factors. Creuzfelt-Jakob Disease is a rare and progressive neurodegenerative disease that results in fatal, transmissible, subacute, spongiform encephalopathy characterized by rapidly progressive dementia and movement disorder. We present a 62-year-old male with no medical history who was admitted to our hospital because of gait and balance disturbance, language impairment and progressive motor deficit of the four limbs. A neurological examination found frontal lobe syndrome signs, myoclonic movements, pyramidal and extra-pyramidal signs. Brain Magnetic Resonance Imaging detected high intensity areas in the basal ganglia. EEG showed generalized triphasic sharp-wave complexes. A Cerebro Spinal Fluid examination found protein 14-3-3. Death occurred six months after onset. This is the first known case of Creuzfelt-Jakob Disease documented in Senegal.

Background:Cholera is endemic in Guinea,having suffered consecutive outbreaks from 2004 to 2008 followed by a lull until the 2012 epidemic.Here we describe the temporal-spatial and behavioural characteristics of cholera cases in Conakry during a three-year period,including the large-scale 2012 epidemic.Methods:We used the national and African Cholera Surveillance Network(Africhol)surveillance data collected from every cholera treatment centre in Conakry city from August 2011 to December 2013.The prevalence of suspect and confirmed cholera cases,the case fatality ratio(CFR),and the factors associated with suspected cholera were described according to three periods:pre-epidemic(A),epidemic 2012(B)and post epidemic(C).Weekly attack rates and temporal-spatial clustering were calculated at municipality level for period B.Cholera was confirmed by culture at the cholera national reference laboratory.Results:A total of 4559 suspect cases were reported:66,4437,and 66 suspect cases in periods A,B and C,respectively.Among the 204 suspect cases with culture results available,6%,60%,and 70%were confirmed in periods A,B,and C,respectively.With 0.3%,the CFR was significantly lower in period B than in periods A(7.6%)and C(7.1%).The overall attack rate was 0.28%in period B,ranging from 0.17%to 0.31%across municipalities.Concomitantly,a cluster of cases was identified in two districts in the northern part of Conakry.At 14%,rice water stools were less frequent in period A than in period B and C(78%and 84%).Dehydration(31%vs 94%and 89%)and coma(0.4%vs 3.1%and 2.9%)were lower during period B than in periods A and C.The treatment of drinking water was less frequent in period A,while there were more reports of recent travel in period C.Conclusions:The epidemic dynamic and the sociological description of suspect cases before,during,and after the large-scale epidemic revealed that the Vibrio cholerae was already present before the epidemic.However,it appeared that infected individuals reacted differently in terms of disease severity as w
