Prognostic Aspects of Lupus Nephritis at Aristide Le Dantec University Hospital in Dakar

Introduction: Kidney injury is common in the course of lupus and affects the functional and vital prognosis. The risk of progression to end-stage renal failure can reach 40% to 60%. Thus we carried out this work for the purpose of an evaluation of the renal and vital prognosis and to deduce the factors of poor prognosis. Patients and method: This was a retrospective, descriptive and analytical study conducted over a period of 10 years from January 1, 2007 to December 31, 2016, performed in the Nephrology Department of Aristide Le Dantec Hospital in Dakar. Patients with lupus nephritis were included. The studied parameters were epidemiological, clinical, paraclinical and progression. We had done a crossover of the patients to look for the factors of poor renal and vital prognosis. Results: Out of 93 cases of lupus patients, 64 were included, a prevalence of 69%. The mean age of the patients was 31.97 ± 10.44 years old. There were 81% women and 19% men, a sex ratio of 0.23. Class III was found in 24 cases (37.5%), Class IV in 20 cases (31.25%), Class V in 15 cases (23.4%), Class II in 4 cases (6.25%) and Class I in 1 case (1.6%). The combination of corticosteroids and immunosuppressants was used in 56.25% of cases. After a follow-up of six months, 19 patients were in complete remission, 21 had resistance and 9 had partial remission. Of the 21 patients who had resistance, 8 were in chronic renal failure. Death was observed in 5 patients and the causes were in 3 patients: pulmonary embolism, bacterial meningitis and pulmonary tuberculosis. The cause of death was unknown in 2 patients. The factors of poor renal prognosis were lymphopenia, the presence of anti-native DNA antibodies, nephrotic syndrome, microscopic hematuria, tubular atrophy and interstitial fibrosis. Risk factors affecting renal survival were the presence of native anti-DNA antibodies, microscopic hematuria, leukocyturia and the presence of a proliferative class. The management of non-communicable chronic diseases such as diabetes and hypertension remains a concern in developing countries. In Guinea, whether by choice or necessity, traditional medicine and herbal remedies are the primary recourse for the population, both in rural and urban areas. Previous investigations led to the development of two phytomedicines designed for managing essential hypertension (HTN) and type 2 diabetes (DT2), named “Guinex-HTN®” and “Sattagas.” An evaluation of their therapeutic benefits was conducted during a prevalence survey carried out in Dubréka. This survey revealed a prevalence of 73.52% (211/287) for HTN, 36.58% (105/287) for DT2, and 33.45% (96/287) for combined DT2/HTN. The therapeutic potential of Sattagas and the combination of Sattagas with Guinex-HTN were assessed in diabetic and hypertensive diabetic patients, respectively. Comparisons were made between Sattagas, Metformin, Sattagas plus Metformin, and Sattagas plus Guinex-HTN regarding their impact on blood glucose levels and/or blood pressure values. After 6 months of treatment with 2 x 2 capsules per day, the anti-diabetic potential of Sattagas was confirmed by a significant reduction in blood glucose levels (from 2.59 ± 0.43 to 1.22 ± 0.43) in 80.95% (85/105) of patients, as well as a significant decrease in blood pressure values (from 177 ± 20.77/98 ± 10.70 to 149 ± 29.02/85 ± 13.64) in 90.16% (55/61) of hypertensive diabetics. The anti-diabetic effect of Sattagas (from 2.19 ± 0.29 to 1.22 ± 0.31 g/l) was similar to that of Metformin (from 2.14 ± 0.23 to 1.28 ± 0.41 g/l), and a synergistic effect was observed when Sattagas was combined with Metformin (from 2.21 ± 0.49 to 1.10 ± 0.20 g/l). In hypertensive diabetics, the combination of Sattagas and Guinex-HTN demonstrated a beneficial effect on blood pressure values. Barriers to Utilization of Postnatal Care: A Qualitative Study in Guinea

Maternal mortality remains very high in developing countries. In Guinea, it is 550 maternal deaths in 100.000 living birth. More than half of these deaths occur in the postpartum period. Hence the interests to have a better understanding of the factors that hinder the realization of post-natal care (PNC) arise. It is for this reason that the objective of this study is to identify barriers to the use of post-natal care in Guinea. Methods: A qualitative study was conducted in the prefectures of Kindia and Dubréka, in the Administrative Region of Kindia (Guinea) in April and May 2014. Fourteen focus group discussions of women and girls of the community, as well as fifteen individual in-depth interviews (9 with women and 6 with healthcare providers). Results: Most women do their PNC during their child vaccination or in case of postpartum complications. Financial constraints are the main barrier to using postnatal care identified by participants. In general, distance from the health center associated with lack of transport mean and insufficient staffs are factors related to the health system. Background: Peripheral artery disease (PAD) poses a significant health concern, particularly in Sub-Saharan Africa, where its prevalence is increasing. [1] Despite its significance, there is limited understanding of PAD among hypertensive patients in this region, highlighting a critical gap in knowledge. This study aimed to investigate the prevalence of PAD and associated factors in black hypertensive patients. Methods: A descriptive and analytical cross-sectional study was conducted over two years at a primary care center in Senegal using their ankle-brachial index (ABI) database. Data collection was strictly retrospective, and sociodemographic characteristics and clinical parameters were retrieved from the local patient database. We included all hypertensive patients who had benefited from an ABI. Statistical analysis was performed using the SPSS 18.0 software program. Results: Among the 220 hypertensive patients enrolled, PAD prevalence was 35%. Significant associations were observed between PAD occurrence and older age (>75 years, p = 0.008) and triple therapy (p = 0.015). Multivariate analysis confirmed age >75 years as a strong predictor of PAD in hypertensive patients (p = 0.01, OR = 4.6). Furthermore, PAD prevalence increased with the severity of hypertension (p = 0.03), emphasizing the need for targeted screening strategies in this population. Conclusion: Despite its limits, this study underscores the urgent need for improved access to healthcare services and tailored screening programs. The findings highlight the growing burden of PAD in Sub-Saharan Africa and the essential role of early detection and intervention, particularly in high-risk populations such as hypertensive individuals. Collaborative efforts involving healthcare providers, policymakers, and community stakeholders are crucial to implement effective interventions and reduce the impact of PAD on population health outcomes.

Estimation of Glomerular Filtration Rate: Which Formula to Apply in G-Spot Subjects Aged 1 - 17 Years?

Introduction: Morbidity and mortality in paediatric practice is dominated in Africa by malaria, diarrhoeal diseases and acute respiratory infections. Aim: In order to evaluate the Glomerular Filtration Rate (GFR) with the formulas commonly used by medical laboratories, we proposed to undertake this study. Patients and Method: This was a descriptive, prospective and cross-sectional study conducted at the PA and ka medical biology laboratory in Bamako Hamdalaye ACI 2000 in collaboration with the medical biology and hospital hygiene laboratory service at the Point G University Hospital. Results: During the study period, we recruited 360 subjects, 189 of whom were male (52.5%) and 171 female (47.5%). The mean age was 8.75 ± 4.8 years with extremes of 1 and 17 years. Subjects aged 15 to 17 years were the most numerous. The reference means GFR of the subjects according to age was with the SCHWARTZ formula 179.16 ± 50.47 with extremes of 173.93 and 184.39;with CKD-Epi 37.63 ± 11.25 with extremes of 36.46 and 38.79;with MAYO 107.87 ± 18.11 with extremes of 105.99 and 109.75 and MDRD 350.83 ± 251.15 with extremes of 324.79 and 376.86. Conclusion: The Mayo formula showed a better distribution around the mean than other formulas for estimating the glomerular filtration rate.

Effect of Cardiac Rehabilitation on Physical Capacity of Heart Failure Patients in Senegal

Background: Heart failure is a chronic and severe condition that often results from various heart diseases. Cardiac rehabilitation (CR) is currently a crucial component in managing this condition. The aim was to assess the effects of cardiac rehabilitation on physical capacity of heart failure patients. Methods: This was a cross-sectional study conducted from February 1, 2021, to June 30, 2023. We included all patients with heart failure who underwent cardiac rehabilitation. Data analysis was performed using SPSS software version 24.0, with a significance level set at p Results: The study included 87 heart failure patients, with a male-to-female ratio of 1.8. Mean age was 57.10 years (±11.75). Coronary artery disease was the primary cause of heart failure, accounting for 75.9% of cases. Atrial fibrillation was present in 4.7% of cases. Following cardiac rehabilitation, Left Ventricular Ejection Fraction increased from 40.15% to 49.48% (p = 0.001). Resting heart rate decreased significantly from 81.4 bpm to 68.3 bpm (p = 0.000), and the number of METS increased from 4.3 to 6.57 (+56.8%;p = 0.000). The mean distance covered in the 6-minute walk test significantly increased from 337.8 meters to 522.7 meters (p = 0.000), reflecting a gain of 183.5 meters. Moreover, the increase in the number of METS was more pronounced in females (p = 0.001), non-obese individuals (p = 0.000), non-diabetics (p = 0.001), non-sedentary individuals (p = 0.000), and non-smokers (p = 0.000). The study reported a low readmissions rate of 2.2% and a mortality rate of 1.1%. Conclusion: Our study demonstrates that cardiac rehabilitation is beneficial for black African heart failure patients, resulting in significant improvements in symptoms, physical and capacity.
