Several studies show that properties of Hot Mixture Asphalt (HMA) mix design materials, aggregate gradation and volumetric properties had an influence on their resistance to rutting. However, these properties do not impact in the same way this performance. For a given aggregate type, an infinity aggregate gradation type can be observed, and for each type of HMA several types of bituminous binder can be used. This article aims to measure the evolution of resistance to rutting according to the three main classes of National Cooperative Highway Research Program (NCHRP) aggregate gradation (dense-graded, fine-graded and coarse-graded). To this end, a study was conducted on the measurement of rutting resistance for eight bituminous mixtures manufactured with two bitumen types and two types of manufacturing aggregates. The results showed that there is a priority order of these different parameters on the influence of the resistance to rutting. This highlights a competition between the properties of aggregate and type of granular skeleton. Indeed, for the same type of aggregate, asphalt binder type first impacts resistance to rutting of the HMA followed by aggregate gradation, volumetric properties of the mix and finally by the angularity of the aggregates. However, this order cannot be fixed and can depend of the intensity of each parameter.

Causes and Prognosis of Cases of Acute Obstructive Renal Failure Managed at the Donka National Hemodialysis Center

Introduction: Acute obstructive renal failure (AORF) is a frequent clinical situation, secondary to obstruction of the urinary excretory tract. Whatever the cause, urinary tract obstruction suddenly opposes glomerular filtration and is responsible for tubulointerstitial lesions. It accounts for 10% of acute renal failure (ARF). The aim of this study was to identify the causes and prognosis of cases of acute obstructive renal failure managed at the Centre National d’hémodialyse Donka. Material and Methods: This was a prospective descriptive study lasting 6 months, from September 1, 2022 to February 29, 2023. All patients undergoing haemodialysis for acute obstructive renal failure who agreed to participate in the study and whose medical records were complete were included. Results: During the course of the study, we registered 97 haemodialysis patients, including 20 cases (20.62%) of acute obstructive renal failure. The mean age of the patients was 57.8 ± 10.54 years, with a male predominance of 11 cases (55%) and a sex ratio of 1.22. The reasons for consultation were dominated by physical asthenia 11 cases (55%), lumbar pain 9 cases (50%), vomiting 6 cases (30%) and acute urine retention 6 cases (30%). Arterial hypertension 16 cases (80%) and urinary tract infection 10 cases (50%) were the most common antecedents. The etiologies of RAOI were dominated by lithiasis 10 cases (50%), neoplasia 6 cases (30%) and benign prostatic hypertrophy 3 cases (15%). mean creatinine was 1267.60 ± 710.76 μmol/l with extremes of 243 μmol/l and 2822 μmol/l, mean urea was 39.56 ± 18.36, hyperkalemia in 14 cases (70%) and hyponatremia in 8 cases (40%). After hemodialysis, 9 cases (45%) recovered renal function, 4 cases (20%) became chronic and 7 cases (35%) died. Conclusion: The frequency of AKI remains non-negligible in our department, and early detection and prompt management would considerably reduce the morbidity and mortality associated with this pathology.

Uterovaginal Prolapse in the Newborn: A Case Report from the University Hospital of Conakry

Uterovaginal prolapse is an exceptional pathology in the newborn. It is defined by the descent and protrusion of the uterus and vaginal walls to the outside via the vaginal orifice. It particularly affects newborns with neural tube defects. The diagnosis is usually made at birth. Different types of conservative or surgical treatment have been suggested for genital prolapse in neonates. We report the case of a newborn of 6 hours of life who was received for congenital utero-vaginal without neural tube closure anomaly. He was successfully treated with digital reduction of the mass associated with a cerclage of the vaginal orifice. Conclusion: Digital reduction of the prolapse associated with a cerclage of the vaginal orifice is simple, effective and avoids any recurrence.

Pavement design tools are not universal. Indeed, in the sizing of pavements in the USA, the prediction models used in the calculation of the dynamic modulus of HMA are not adapted to the characterization of the mineral skeleton of the HMA mix designed with the French method. This article aims to assess the predictive models of the dynamic modulus used in the mechanistic-empirical design for their use in the design of bituminous pavements, and to develop new predictive models taking into account the sieve series LC and AFNOR standards. A total of six types of mixtures were subjected to the determination of complex modulus testing by direct tensile-compression on cylindrical specimens (26-700 LC) over a temperature range (5) and frequency (5) data. Dynamic modulus prediction models |E*| are studied Witczak model 1999 and model Witczak 2006. The study is carried out in imperfect contact with a concrete slab wall attached to a panel based on rice straw compressed in a dynamic frequency regime. We will propose the characterization of thermal insulation for thermal resistance of contact (x = 0.05 m). The impact of heat exchange coefficients on the front face (x = 0 m) and the rear face (x = 0.1 m) on these resistors is shown.

Asymptomatic Cardiac Manifestations in Rheumatoid Arthritis in Conakry (Guinea)

Introduction: Rheumatoid arthritis (RA) is the most common chronic inflammatory rheumatic disease. It is a systemic disease with extra-articular manifestations that can be life-threatening. In sub-Saharan Africa, there is little published information on cardiovascular manifestations in RA. Objective: The objective is to determine the asymptomatic cardiac manifestations during rheumatoid arthritis in Conakry. Patients and Methods: This was a cross-sectional study lasting 6 months from January 1, 2020 to June 30, 2020 in the rheumatology and cardiology departments of the Ignace Deen National Hospital. Patients with RA diagnosed according to the 2010 ACR/EULAR criteria, asymptomatic at the cardiovascular level, which had an ECG and echocardiography, were included in the study. Left ventricular diastolic dysfunction was considered in any patient with an E/A ratio 1, an E/E’ ratio > 10, and an LVEF = 50%, assessed by the Simpson biplane method and classified according to Redfield. Epidemiological, clinical and paraclinical data were analyzed using SPSS statistics 21.0 software. Results: Seventeen cases of rheumatoid arthritis (5.9%) were collected. There was a predominance of women with 14 cases (82.4%). The mean age of patients was 48.2 ± 11.9 years. The average duration of RA was 7.2 ± 4.2 years. Cardiovascular risk factors were dominated by overweight and obesity (58.8%) and sedentary lifestyle (35.3%). RA was predominantly high activity (DAS 28 ≥ 5.1) in 82.4% of patients. Anti-CCP antibodies were positive in 76.9% of cases. Fourteen patients (82.4%) had abnormal cardiac results. The electrocardiogram showed left atrial hypertrophy in 29.4% of cases, left ventricular hypertrophy (11.8%) and ventricular extrasystole (11.8%). The cardiac Doppler scan showed diastolic dysfunction of the left ventricle (47.1%) and moderate pericardial effusion in 11.8% of cases. How a Cancer Registry Was Set Up in Senegal: An Example to Follow for a Developing Country

Objectives: Senegal, with the support of WHO, had begun the establishment of a National Cancer Registry in 2009, although it had been at a standstill since 1969;the objective of this work was to test the reliability of data collection tools and feasibility stages necessary for the validation of media before the establishment of a national cancer registry. Methodology: We conducted a non-exhaustive preliminary study over a period of three months from the first of January to 31 March 2010 at four major hospitals in Dakar at the time. Results: Two hundred and eighty-nine cases had been identified: 44% of men (n = 127) and 56% of women (n = 162) with a sex ratio of 0.8. The ages ranged from 20 to 90 years with an average of 50 years. The main diagnostic mode was essentially histological, with 76% of cases (n = 219). The most frequent locations were: ORL (ENT meaning Ear, Nose and Throat) with 25% of cases, liver 7% and bronchopulmonary 4.5% of cases. In men, liver cancer was the most common location and women for cervical cancer with 16%. Squamous cell carcinoma was the most common histological type with 68% of cases followed by adenocarcinoma with 22% of cases. Thirty-eight percent of patients were classified as stage III and IV. A quarter of our patients had received palliative treatment. In contrast, 15 (15%) had received chemotherapy and 4% had received radiotherapy. Data collection was satisfactory. Conclusion: Cancer is a reality in Senegal but it remains underdiagnosed. The materials made it possible to make the cancer registry effective throughout the country.

The subject of natural convection heat transfer is motivated by a wide range of applications in engineering technology. The hemispherical cavity is a part of basic geometries although it is not widely studied. The effect of inclinaison on natural convection fluid motions in the gap between two eccentric hemispheres is numerically studied. The inner hemisphere is subjected to a heat flux of a constant density and the outer one is maintened isothermal. The walls separating the two hemispheres are thermally adiabatic. Equations are formulated with vorticity and stream-functions variables. It is also assumed the fluid incompressible and obeys the approximation of Boussinesq. These equations are written by using bispherical coordinates system and solved by using a finite difference method. The results show the topology of flow is strongly dependent on the inclinaison because the flow can change from a unicellular regime to a multicellular regime by varying the inclination from 0 to π. By increasing the Rayleigh number (103Ra7), the flow intensifies. The results are shown in terms of streamlines and isotherms during their transient evolution.

Health Risk Assessment of Trace Metals in Drinking Water Consumed in Dakar, Senegal

Water contamination by chemical pollutants is increasingly associated with waterborne diseases worldwide. The objective of this study is to assess the chemical health risk associated with drinking water consumption in Dakar, Senegal. The methodology adopted in this study is based on the determination of trace metals (Cd, Pb, Fe, Cu and Mn) in human drinking water by flame atomic absorption spectrometry, the estimation of the exposure of populations of the study area on the basis of the quantity of water consumed daily and the determination of the hazard quotient which allows estimating the health risk. The results indicate unacceptable levels of Pb and Cd in the water according to World Health Organization (WHO) guidelines, while the concentrations of Fe, Cu and Mn are below the regulatory values. The hazard quotient (HQ) is in the majority of samples higher than 1 in children and infants for scenario 1 (based on WHO data), reflecting a high risk for this target. In scenario 2 based on consumption data from the populations studied, the HQ is greater than 1 only in heavy consumers. The drinking water consumed by Dakar population presents health risks related to Pb and Cd, particularly for infants, children and heavy consumers, hence the need to strengthen the water treatment system before home consumption.

Plummer-Vinson Syndrome and Esophageal Cancer in an Endoscopy Center of Dakar

Introduction: Plummer Vinson Syndrome (PVS) is a rare pathology combining dysphagia, an iron deficiency anemia and a cervical esophageal web. It is one of the risk factors of the esophageal cancer. Objectives: To report the epidemiological, clinical, paraclinical and histological characteristics of patients with Plummer-Vinson syndrome associated with esophageal cancer. Patients and Methods: This was a retrospective study carried out at the digestive endoscopy center of Aristide le Dantec hospital in Dakar from January 2008 to December 2015. We included all patients with Plummer-Vinson syndrome associated with esophageal cancer. Results: We included 7 patients. The mean age was 36 years (24 - 65 years). There were 5 women and 2 men. The mean diagnostic period was 9 months (6 months - 15 years). The main clinical manifestations were dysphagia (100%), weight loss (100%) and clinical anemia (71.4%). The biology showed a mean hemoglobin level of 8.8 g/dl (6 - 11 g/dl), serum iron at 30.8 μg/dl (11 - 52 μg/dl) and ferritinemia at 6.2 ng/dl (1.5 - 25 ng/dl). The upper digestive endoscopy revealed a stenosis web of the cervical esophagus in all patients. An endoscopic dilatation was performed in all cases. After dilation, the endoscopy showed a tumor of the middle third of the esophagus in 4 cases and of the lower third in 3 cases. It was about of a squamous cell carcinoma in the tumors of middle third and of an adenocarcinoma in those of the lower third. Conclusion: At Aristide le Dantec hospital of Dakar, PVS is sometimes associated with an epidermoid carcinoma or an adenocarcinoma of the esophagus. The improvement of the prognosis depends on the early realization of an upper gastrointestinal endoscopy (UGI) in case of any dysphagia and the monitoring of the patients.
