Introduction: The underweight at birth is recognized as a major cause of morbidity and mortality in childhood. Objective: To identify maternal and obstetric sociodemographic factors associated with low birth weight. Patients and Methods: This is a retrospective study of analytical type that examined the records of women who gave birth in the Obstetrics and Gynecology department of Ignace Deen National University Teaching Hospital from 1st December 2016 to 30th April 2017. The analysis was made with the R version 3.3.1 software. We did a univariate and multivariate analysis. Outcomes: Out of the 1633 live births of single pregnancies that occurred during the study period, 109 children were born with a low weight (<2500 g) corresponding to a rate of 6.7%. In univariate analysis, we found a significant association between low birth weight and maternal single status (p = 0.019), maternal weight less than 60 kg (p = 0.038), primary parity (p = 0.018), maternal history of abortion (p = 0.001), history of preterm birth (p < 0.001), arterial hypertension (p < 0.001), anemia (p < 0.001) and malaria (p < 0.001). In multivariate analysis, the variables associated with low birth weight were: history of preterm delivery with OR of 8.5 [1.8 - 40.1], history of abortion (OR = 4.4 [1.4 - 13.9]), malaria (OR = 23.8 [6.1 - 92.5]), anemia (OR = 11.8 [3.7 - 38.2]) and high blood pressure (OR = 5.4 [1.6 - 17.9]). Conclusion: The decrease in frequency of low birth weight in Guinea will be done by improving the quality of prenatal care with an emphasis on screening, prevention and treatment of malaria, anemia and high blood pressure during pregnancy, prevention of abortion and premature birth.

Production of New Adsorbent from Mango residues Collected in the Urban Community of Mamou

Activated coal obtained from mango residues is an alternative for the valorisation of these wastes for applications. This work focuses on the production of a new adsorbent from mango residues collected in the urban common of Mamou (Republic of Guinea) by chemical activation with potassium hydroxide. To do this, the different rates: humidity, cinders, volatile matter, fixed carbon et iodine indices before activation and after activation at 400°C and 700°C were determined which are: (6.28%;0.97%;73.80%;25.23%) respectively. The best indices were found in powdered activated coal compared to grain activated coal for the concentrations used. This study can be an alternative bio-adsorbent for the urban community of Mamou in water treatment.

Introduction: Platypnea-Orthodeoxia syndrome is characterized by dyspnea and hypoxia in the upright position, usually improving in the supine position. Two components are required: an interatrial or intrapulmonary shunt, and a functional component. Diagnosis is made by contrast ultrasonography. We report a case of Platypnea-Orthodeoxia syndrome revealed by positional dyspnea in an 87-year-old patient. The aim of this study is to describe the clinical, therapeutic and evolutionary profile of this syndrome. Case Presentation: This is an 87-year-old patient with a history of pulmonary embolism (PE) and stroke. He was seen for dyspnea and desaturation in orthostatism, revealing a patent foramen ovale (PFO). Progression was favorable after closure of the PFO. Conclusion: Platypnea-Orthodeoxia syndrome may be presented as simple exertional dyspnea. The clinician should check for improvement in symptoms and/or oxygenation during decubitus. Definitive treatment consists of percutaneous closure of the shunt.

The valorization of agricultural waste in the treatment of our rivers can be an alternative to waste management. This study deals with the use of mango kernel shells in the depollution of the Bafing River (rural commune of Tolo, Mamou prefecture) in the Republic of Guinea. Thus, the different concentrations of ions found in April (low water period) were determined and which are: a reduction of 35.64 mg/l (79.69%);a reduction of 41.53 mg/l (81.24%);and 47.53 mg/l (82.10%) and 3.83 mg/l (75.24%);8.45 mg/l (81.72%);11.67 mg/l (87.94%) at the sampling points P1, P2, and P3 respectively. In October (flood period), the concentrations found are 24.98 mg/l reductions (79.88%);29.83 mg/l reduction (81.41%) and 35.15 mg/l (85.50%) at the same sampling points P1, P2 and P3, respectively. This study can be considered as a water treatment alternative for the village community of the Tolo sub-prefecture (Mamou prefecture).

The sub-mitral annular aneurysm is a rare cardiac pathology. The etiologies are diverse, ranging from the congenital form to the idiopathic form, including the acquired form. The clinical case we report is that of a 27-year-old young man, with no particular history, admitted with a picture of global heart failure. A cardiac ultrasound diagnosed a ruptured mitral sub-annular aneurysm in the pericardium. The difficulty lies in the surgical management of this condition, due to the lack of an adequate cardiac surgery service in most of the countries in Africa south of the Sahara.

Case Report: A Severe Hypernatremia with Multiple Organ Failure, about a Clinical Observation

A hypernatremia severity is often associated with a poor prognosis, especially if it is associated with multiple organ failure. In a country with very limited resources, the prognosis may be favorable in the absence of renal replacement. We report the case of a 63 years old woman, hypertensive and diabetic, admitted to the ICU for unconsciousness. Clinically, neurological examination notes a Glasgow of 8/15 (Y2, V2, M4) and a left hemiplegia. Temperature was 39°C. Diuresis was 100 ml during the first 24 hours. Blood pressure was 90/60 mmHg, tachycardia at 133/min. Cardiac auscultation is normal. Vascular axes were weakly perceptible. Oxygen saturation was 95%. The skin examination notes a dry skin and mucous membranes, a flattening of the superficial veins, sunken eyes and a persistent skin fold. There is no hepatomegaly or splenomegaly, or jaundice. Biological point of view note natremia: 176 mmol/L;osmolarity: 390 mosmol/kg;creatinin: 300 μmol/L;glycemia > 6 g/L;transaminases 217 UI. Diagnostics: malignant hypernatremia with a high plasma osmolarity associated with an acute anuric renal failure, hydro electrolytic disorders, an abnormal liver function, a fever of central origin and a stroke. The treatment consisted of a correction of the electrolyte disorders by infusion of isotonic and hypotonic fluids with insulin. In a country with very limited resources, the severe hypernatremia prognosis associated with anuric acute renal failure may be favorable in the absence of renal replacement.

The potability of water wells, drilling and tap water is the determining factor for the quality management of health of the urban and rural population. This study has been done on the basis of monitoring aspect such as pH, content ammonium, nitrate, nitrite and chloride from a physico chemical point of view. This study allowed the chemical and bacteriological analysis of samples, based on the result obtained, and the potability of water was defined. The bacteriological analysis was carried out taking account of the total flora, coliforms and Escherichia coli. This work gave rise to analyze 36 samples of water from wells, eight samples of drilling water and nine samples of water taps from municipalities such as Mamou, Dalaba and Pita. Water wells represent 90% of water supplies in drinking water to the population. In this research, collection of samples was very important. This study shows that 16 samples from 52 are potable, therefore, it is urgent to focus on the treatment of sewage in order to ensure better health for the population from administrative region of Mamou.
