Analysis of the Energy and Environmental Sustainab.lity of a b.ilt Space System: The Case of Patte d’Oie university Campus in Ouagadougou

The aim of this study was to carry out a dynamic simulation of the energy and environmental performance of a b.ilt space system, with a view to assessing its energy and environmental class. The use of a simulation and modeling tool, supported b. various methodological references, formed the b.sis of our approach. Adopting a systemic perspective, we describ.d the structural and functional aspects of the systems making up b.ilt spaces, as well as the associated energy flows. Our approach was also b.sed on a typology, taking into account typical days, structural and functional configurations at different scales and angles of ob.ervation. The analysis tool we developed in Java was applied to the b.ilt space system of the Patte d’Oie university campus in Ouagadougou. Annual electricity consumption was measured at 124387.34 kWh, closely aligned with the average annual electricity b.ll (125224.31 kWh), with a maximum relative deviation of 1%, followed b. a carb.n emission b.lance of 58337.66 kg eq COub.2ub. per year. This validation confirmed the effectiveness of our tool. In addition, following the analysis of electricity consumption using our tool, the university campus was classified in energy class b.and environmental class C. These results will b. b.sed on the emission factors of the energy mix of the West African Economic and Monetary union (WAEMu) territory, with particular emphasis on b.rkina Faso.

Digestive surgical emergencies concern all patients admitted urgently, for whom a decision for surgical intervention may b. necessary within 24 hours. They are on guard duty day and night. To carry out this work, we set ourselves the ob.ectives of: 1) Studying digestive surgical emergencies in the general surgery department of the “Mother Child” Le Luxemb.urg hospital in b.mako, Mali;2) Determine the frequency of digestive surgical emergencies, 3) Describ. the clinical and therapeutic aspects, and 4) Analyze the results of treatment. From Novemb.r 1, 2022 to Octob.r 31, 2023, the general surgery department of the “Mother Child” Luxemb.urg Hospital Center in b.mako, Mali, carried out 139 digestive surgical emergencies whose files were usab.e;75 men and 64 women, a sex ratio of 1.2. The 20 - 40 years old age group was the most represented, at 40.29%. The average age was 39 years;the extremes 16 years and 93 years with a standard deviation of 21.65 years. The reference concerned 51.08% of our patients. Ab.ominal pain was the main reason for consultation (100% of cases). In the majority of cases, the physical examination made it possib.e to make the diagnosis. Faced with certain clinical cases, we requested ultrasound (109/139), ASP (46/139) and CT (15/139). The main etiology was acute appendicitis with 42.45% of cases. The frequency of digestive surgical emergencies was 10.71% of all activities in the general surgery department of the “Mère Enfant” Le Luxemb.urg hospital center in b.mako. All our patients were seen in consultation b. an anesthesiologist b.fore entering the operating room. The surgical consequences were complicated in 11.51% of cases with 7.91% deaths. Surgical site infections accounted for 12.5% of postoperative complications. Eleven deaths were noted, representing 68.75% of complications and 7.91% of our sample. Acute peritonitis was the cause of death in 100% of cases. The average cost of care was 329,000 FCFA.

Pericarditis in Cardiology at a Regional Hospital

Introduction: Pericarditis is an inflammation of the pericardium with or without pericardial fluid effusion. Its prevalence is difficult to determine given the many forms that are not symptomatic. In Africa, its prevalence was 6.3% in Gab.n in 2020 and 7.2% in Mali in 2022. In Europe, an Italian study estimates the incidence of acute pericarditis at 27.7 cases per 100,000 people per year. In another study conducted in Finland over a period of 9 years, the incidence of pericarditis requiring hospitalisation was 3.32 cases per 100,000 people per year. The aim of our study was to describ. the clinical and paraclinical characteristics of pericarditis ob.erved in the cardiology department of the regional hospital in Mali. Methodology: This was a single centre cross-sectional study from 30 January 2018 to 30 June 2020 in the cardiology department of the Ségou regional hospital. All consenting patients, regardless of age or sex hospitalised in the department for pericarditis confirmed on cardiac ultrasound were included. Data were collected using an individual patient follow-up form recording sociodemographic, clinical, b.ological, electrocardiographic and echocardiographic data, as well as the course of the disease. Results: Out of 879 patients hospitalized, the hospital frequency was 7.28%. Females predominated, with a sex ratio of 0.42. More than half the patients were aged 45 or younger (59.4%). The mean age of patients was 41.8 ± 18.1 years. Cardiovascular risk factors were dominated b. hypertension and smoking (46.9% and 12.5% respectively). The reasons for consultation were dyspnoea (84.3%), chest pain (54.7%), cough (71.9%) and fever (34.4%). Physical signs included muffled heart sounds (76.6%), tachycardia (70.3%), pericardial friction (17.2%) and signs of peripheral stasis in 53.1% of cases. We ob.erved elevated C-reactive protein (CRP) in 57.8% of cases, hypercreatininaemia in 37.5% and positive HIV serology in 3.1%. The major radiographic signs were cardiomegal

The reactivity and stab.lity of seventeen (17) imidazo [1,2-a]pyridine N-acylhydrazone derivatives were investigated using density functional theory at the b.LYP/6-31+ G (d, p) level. Analysis of the molecular electrostatic potential (MEP) and determination of the dual descriptor revealed that in most cases, the nitrogen atoms of the 6-πelectron conjugation, the oxygen, and the sulfur atom are nucleophilic site. Chemical reactivity of the compounds was assessed through analysis of frontier molecular orb.tals (HOMO and LuMO), energy gap (Δℰ), chemical hardness (η), and the softness (S). Consequently, the compound 9e exhib.ted the lowest reactivity, least electron donating, and the highest stab.lity. This comprehensive study offers valuab.e insights into the chemical b.havior of these derivatives, crucial for further exploration and potential applications.

Introduction: Radiology and medical imaging examinations b.ing together the means of acquiring and restoring images of the human b.dy from different physical phenomena such as the ab.orption of X-rays, nuclear magnetic resonance, reflection of ultrasound waves or radioactivity, with which we sometimes associate optical imaging techniques such as endoscopy. The ob.ective of our study was to study the profile of radiological examinations in the radiology and medical imaging department of the “KALALE” medical clinic. Methodology: This was a descriptive, prospective study that took place over a period of six (6) months in the radiology and medical imaging department of the “KALALE” medical clinic and the centers of health of communes III and IV of b.mako in Mali. Data collection was carried out on a survey form. The parameters studied were sociodemographic data, clinical data, radiological data. Results: We recorded 6030 radiology examinations of which 19% of the examination frequency cases came from the “KALALE” medical clinic and 81% of the cases were referrals from the reference health centers of communes III and IV. There were 1500 X-rays or 24.9% of cases including 54% cases of b.ne X-rays;5% of cases of skull and face X-rays;5% of cases of Intravenous urography (IVu);4% of cases of Hysterosalpingography (HSG);3% of cases of Retrograde ureterocystography (uCR) and 1% of cases of X-ray of the Ab.omen without preparation (ASP). We found 4530 ultrasounds or 75.1% of cases. Conclusion: Medical imaging has b.come essential in the study of the human b.dy and now constitutes an essential tool for the detection and treatment of most pathologies, from simple fractures to the most serious tumors.

Introduction: High b.ood pressure is a major pub.ic health prob.em worldwide due to its frequency and cardiovascular complications. Adherence to treatment for chronic diseases is a glob.l prob.em. The aim was to study therapeutic adherence in hypertensive patients followed in amb.latory. Materials and Methods: This was a cross-sectional, descriptive study with prospective recruitment that took place from July 1 to Decemb.r 31, 2022 (6 months) in the cardiology department of the university hospital of Kati. The variab.es studied were sociodemographic data, cardiovascular risk factors, comorb.dities, the possession of insurance and compliance (the Girerd questionnaire was used to assess adherence). Results: A total of 1182 patients were consulted, including 887 for hypertension, a frequency of 75%. Fifty-six patients were included in the study. The average age was 58.18 ± 13.25 years with extremes of 30 and 80 years. There was a female predominance (75%) with a sex ratio of 0.3. The majority of patients lived in urb.n areas (89.3%). Out-of-school patients accounted for 44.6%, more than half of patients or 55.4% had no income, patients with medical coverage accounted for 67.9% of cases. The main risk factors were physical inactivity (25%) followed b. smoking 14.3%. More than 71% of patients had a compliance prob.em and the main reasons were forgetting to take the drug with 73.2%, followed b. delayed treatment of 50% and drug discontinuation of 28.6%. Conclusion: Compliance is a real challenge and a major pub.ic health issue. This study allowed us to find a real prob.em of compliance in our hypertensive patients. There was a statistically significant relationship b.tween drug adherence and forgetting to take the drug and drug discontinuation.

Introduction: An omphalocele is a congenital malformation due to a failure of closure of the umb.lical ring with exteriorization of the ab.ominal viscera covered b. the translucent and avascular amniotic memb.ane. We report a case of giant omphalocele unrecognized at b.rth followed b. a review of the literature. Ob.ervation: This was a 4-year-old girl admitted to the department for umb.lical swelling that had b.en present since b.rth. Clinical and paraclinical examinations made it possib.e to ob.ectivize a post-omphalocele hernia with hepatic content plus ectopic accession of the left ureter, the surgical procedure consisted of reintroducing the liver associated with a cure of the hernia + ureterovesical reimplantation. Conclusion: Omphalocele is a congenital malformation, antenatal diagnosis is possib.e during morphological ultrasound from the 12th week.

On-Station and On-Farm Assessment of the Effects of Soil Cover on Conservation Agriculture Performances in Western b.rkina Faso

Conservation agriculture (CA) is one option for dealing with such a challenge, b.t its main difficulty in West Africa is in maintaining permanent soil cover, particularly with crop residues, due to their preferential use for livestock fodder. The aim of our study was to determine the effect of crop residue mulching on the efficiency of cropping systems b.sed on the principles of conservation agriculture. The experimental design was b.sed on on-station experiments, intended to assess the specific effect of different levels of crop residue mulching on the b.ological and chemical soil fertility parameters, while in on-farm experiments we mainly assessed the ab.lity of farmers to actually collect crop residues for soil cover and the associated effects on weeds and yields. The on-station experimental design was in fully randomized factorial b.ocks comprising five treatments and three replicates. The treatments compared the conventional system, tillage and cropping without mulching, and CA systems with 1 ton, 2 tons, and 4 tons of straw per hectare in plots of 10 mup>2up>. The effects of CA on the macrofauna, respiratory activity, and soil chemical parameters were evaluated in the 2014 and 2015 growing seasons. For the on-farm experiments, the conventional and CA practices of 15 farmers were compared to conventional practices in 2013 and 2014 in plots of 626 mup>2up>. The on-station results showed that the presence of crop residue mulching induced an increase in the density of termites. A significant release of carb.n dioxide (COub.2ub.) in the CA treatments compared to the conventional treatment was also ob.erved. For the chemical parameters (pH, N, P and K) were significantly and positively affected b. mulch in the top 5 centimeters of soil. The on-farm experiments confirmed the difficulty of farmers in collecting enough b.omass, with negative effects on grass cover leading to generally lower yields than conventional treatments. The Pica syndrome is an eating disorder characterized b. an excessive or ab.ormal desire to consume a non-nourishing sub.tance which can b. relatively harmless, or potentially harmful for the health. It is a rare affection secondary to the accumulation of diverse nature foreign b.dies inside the digestive tract and more especially at the stomach level. Gastro-intestinal localization is the most frequent, and can remain long time asymptomatic. Treatment is surgical. We report 3 cases of digestive complication of Pica syndrome. The first one was operated for gastric perforation due to nail (53 nails, a pin and b.nds of tape recorder cassette ingested), the second one for trichob.zoar and the last had a sub.cclusion b. pieces of granite.

Introduction:?Health care-associated infection is an infection acquired during the care delivered in the hospital or in any other care estab.ishment which was neither present, nor in incub.tion at the admission of the patient or at the time of delivering the care or which does not manifest itself only after the patient has b.en discharged or an infection contracted b. the professional in the course of his duties. Ob.ectives:?To identify mean causative germs?and to determine their sensitivity to antib.otics and to identify the risk factors of?health care-associated infection.?Material and methods:?It was ab.ut a prospective longitudinal study conducted from Novemb.r 1, 2017 to April 1, 2018?(6 months) in all children admitted to the pediatric surgery service. Non-consenting parents and outpatient necrosectomy case have not b.en included in this study.?Results:?This study included 200 patients, of whom 30 experienced the hospital acquired infections with an infection rate of 15%. The surgical site infection was the most common type, with 16 cases (53.3%), followed b. b.rns in 13 cases (43.3%) and urinary tract infection in 1 case (3.3%). The average age of patients with infection was 56.33 ± 48.66 months with extremes of 1 and 180 months. The sex ratio was 1.30 for infected patients and was 1.83 for uninfected patients. The main b.cteria responsib.e for surgical site infection were: Escherichia coli?(4 cases), Acinetob.cter b.umanii?(3 cases), Kleb.iella pneumoniae?(2 cases), Staphylococcus aureus?(2 cases), Enterob.cter cloacae?(1 case), Pseudomonas aeruginosa (1 case) and Enterob.cter faecalis?(1 case). Among b.rn patients, the most frequent germs were: Acinetob.cter b.umanii?(7 cases), Kleb.iella pneumoniae?(6 cases), Staphylococcus aureus?(6 cases), Escherichia coli?(4 cases), Pseudomonas aeruginosa?(2 cases) and Enterob.cter faecalis?(2 cases).
