Male papillary breast cancer treated by wide resection and latissimus dorsi flap reconstruction: A case report and review of the literature

Breast cancer(BC) in men represents between 0.5% and 1% of all BC diagnosed each year. We report a case of advanced BC in a 62-year-old male treated at our interdisciplinary Breast Cancer Center. The patient presented with a newly diagnosed large, symptomatic mass in his left breast. Clinical examination showed a not movable mass of 16 cm diameter, deforming the whole breast; the overlying skin was livid and hypervascularized. Enlarged lymph nodes were palpable in the axillary pit. He had no concomitant diseases at time of presentation. He denied any first- or second degree family medical history of cancer of any type and he never received radiotherapy. Ultrasound guided minimal-invasive 14-gauge core biopsy revealed a moderately differentiated encapsulated papillary carcinoma with high expression of estrogen and progesterone receptors(both > 80%, IRS 12) and HER2-negative. Because of the tumor size a mastectomy with axillary dissection and chest wall reconstruction using a latissimus dorsi flap was performed. Histological analysis showed invasive growth besides typical(non-invasive) papillary carcinoma and was classified as invasive solid papillary carcinoma; p T3(10 cm), p N0(0/15), M0,R0; Oncotype DX Recurrence Score indicated low risk(RS: 2). After discussion in the interdisciplinary tumor board meeting, radiation therapy and tamoxifen were recommended. The patient had an uneventful recovery and is disease-free after two years of follow-up. Male BC is typically diagnosed at an advanced stage, most likely due to a lack of awareness that men can develop BC. Therefore, in case of a large tumor, a flap-based thoracic reconstruction may be required.

AIM： Acute pancreatitis （AP） is the most common and often severe complication of endoscopic retrograde cholangiopancreatography （ERCP）. The early step in the pathogenesis of acute pancreatitis is probably the capillary endothelial injury mediated by oxygen-derived free radicals. N-acetylcysteine - a free radical scavenger may be potentially effective in preventing post-ERCP acute pancreatitis and it is also known that N-acetylcysteine （ACC） can reduce the severity of disease in experimental model of AP. METHODS： One hundred and six patients were randomly allocated to two groups. Fifty-five patients were given N-acetylcysteine （two 600 mg doses orally 24 and 12 h before ERCP and 600 mg was given iv, twice a day for two days after the ERCP）. The control group consisted of 51 patients who were given iv. isotonic saline twice a day for two days after the ERCP. Serum and urine amylase activities were measured before ERCP and 8 and 24 h after the procedure. The primary outcome parameter was post-ERCP acute pancreatitis and the secondary outcome parameters were differences between groups in serum and urine amylase activity. RESULTS： There were no significant differences in the rate of post-ERCP pancreatitis between two groups （10 patients overall, 4 in the ACC group and 6 in the controlgroup）. There were also no significant differences in baseline and post-ERCP serum and urine amylase activity between ACC group and control group. CONCLUSION： N-acetylcysteine fails to demonstrate any significant preventive effect on post-ERCP pancreatitis, as well as on serum and urine amylase activity.

The study of micromycetes infestation on flower buds, flower petals and seeds from 10 taxa of Rhododendron sp. was conducted during 2010-2012. Mycological analysis was comprised of 1,500 specimens of buds and petals and 500 seeds of Rhododendron sp.. It was shown that the necrosis and dieback of buds were caused by complex micromycetes （43 species）, with dominants Pestalotiopsis sydowiana, Alternaria alternata, Truncatella truncata and Epicoccum nigrum. Watery brown spots on the flower petals, resulting in the dieback of flowers, were caused by 38 species, including the most common P. sydowiana, A. alternata and Trichoderma viride. The seeds were contaminated by 18 species, and in addition to the above, the following species were associated： Oidiodendron tennuissimum, Davidiella macrocarpa and Phoma leveillei. The results of the mycological analysis confirmed the diversity spectrum of micromycetes species that inhabit the infested Rhododendron buds, flowers and seeds. The research revealed which taxa attracted the largest number of colonies and species of fungi.

The aim of the work was to quantify possible interactions between surfactants and preservatives, comparing surface properties, in model pharmaceutical formulations. Surface parameters of 2-component surfactant-preservative aquous mixtures were determined with a Wilhelmy plate technique, for the so-called principal surfactants (polysorbate 80, egg lecithin, phosphatidylcholine) and preservatives, which were methylparaben and benzalkonium chloride (BA-C). A generalized surface tension vs. surfactant concentration plot signatures, in the presence of preservative at a fixed amount, allowed: the critical micellar concentration (cmc) shift, additive molecules partition from the surface to the bulk, mixed micelles formation concentration, and additive surface removal concentration to be determined in reference to surface activity of the added substance. Methylparaben is a compound of lower (in comparison to BAC) surface activity, lower partitioning coefficient possessing lower energy and concentration of its removal from the surface, that makes it play effectively an antimicrobial protection role in the bulk of pharmaceutical products, as already shown by chemical tests.

Fatal Urosepsis: A 41 Year-Old Pregnant Woman—Case Report

Introduction: Urinary tract infections are one of the most frequent infectious complications during pregnancy [1], while acute pyelonephritis is the main non-obstetric reason for hospitalisation [2] [3]and septic shock in pregnant women. Objectives and methods: The aim of our study is to analyse the diagnostics and treatment of a 41-year-old pregnant woman with the most severe form of urinary tract infection—urosepsis. Case report: A 41-year old woman in the 12th week of pregnancy hospitalised at Intensive Care Unit (ICU) after urological intervention (JJ stent). On the first day after the intervention, the patient was diagnosed as having urosepsis. Scores: SOFA 14 pts, APACHE II 26 pts, SAPS II SCORE 61 pts. Second day in ICU: spontaneous abortion. Microbiological?investigation: Escherichia coli—extended-spectrum beta-lactamase (ESBL+). Conclusions: Apregnant patient with a complicated infection of the urinary system ought to be treated in a multi-profile hospital, offering a possibility to consult doctors of various specialisations (urologist, gynaecologist, anaesthesiologist, nephrologist) as well as full access to a radiology laboratory, which will ensure the choice of appropriate and safe treatment for both the mother and the developing foetus.

Moment-Rotation Characteristics for Flexible Beam-to-Column Steel Joint Exposed to Fire

Glycine is an example of flexible molecule, with three torsional degrees of freedom leading to the eight stable local minima structures and a variety of transition state structures. For the six most stable conformers highly accurate conformational energies, bond lengths,bond angles, enthalpies and other properties have been recently reported[1]. Here we consider two remaining VIIp/tcc and VIIIn/gtc structures computing the optimal geometry parameters by CCSD(T)/(CBS+CV)[1,2] and energy by CCSD(T)/CBS+CV method[3]. Then we use different less-expensive methods and basis setsto optimize the eight minima and the 19 transition states. We found out that the Double-Hybrid Density Functional(DHDF) B2PLYP(-D3BJ)with basis sets of at least maug-cc-pVTZ quality [4] shows good results for both optimization and energy calculations, while LC-ωPBE,CAM-B3 LYP and some other DFT methods show problems when optimizing the VIIp and VIIIn structures in spite of the good results for the single-point energy calculations.

Analytical models used to describe behaviour of steel frame loadbearing structures in fully developed fire usually do not allow for reduced joint stiffness due to increased member temperature. Joints previously designed as nominally rigid tend to become flexible in fire situation, with degree of flexibility increasing during fire development. Reliable analysis of this phenomenon and its influence on the redistribution of internal forces result in the need for developing appropriate characteristics, describing relationship between bending moment applied to the joint and joint rotation. Characteristics of such type, specified for fire conditions, depend on steel temperature, in the current work, the authors propose a practical approach to develop such characteristics, based on the knowledge of analogous characteristic prepared for persistent design situation. The developed technique does not require to generalize the classical component method to the case of fire, which may be difficult in practical situations. The proposed computational algorithm has been tested on an example of a typical beam-to-column joint.

Background:The present study aimed at investigating the influence of 90%menthol-containing plant bioactive lipid compounds(PBLC,essential oils)on growth performance,blood haematological and biochemical profile,and nutrient absorption in ***-four growing Suffolk sheep were allotted into three dietary treatments:Control(without PBLC),lower dose of PBLC(PBLC-L;80 mg/d)and higher dose of PBLC(PBLC-H;160 mg/d).Sheep in all groups were fed meadow hay ad libitum plus 600 g/d of concentrate pellets for 28 ***:Average daily gain was not affected by *** of PBLC increased hay and total feed intake per kg body weight(P<0.05).Counts of total leucocytes,lymphocytes and monocytes were not different among ***,neutrophil count decreased(P<0.05)in PBLC-H with a similar trend in PBLC-L(P<0.10).Concentrations of glucose,bilirubin,triglycerides,cholesterol,urea and magnesium in serum were not different among sheep fed different doses of ***,serum calcium concentration tended to increase in PBLC-H(P<0.10)and serum concentrations of aspartate&asparagine(P<0.01)and glutamate&glutamine(P<0.05)increased linearly with increasing PBLC *** ruminal epithelia isolated from the rumen after killing,baseline conductance(Gt;P<0.05)and short-circuit current(Isc;P<0.01)increased in both PBLC *** uptakes of glucose and methionine in the presence of Na+were not affected by the dietary PBLC *** the absence of Na+,however,glucose and methionine uptakes increased(P<0.05)in *** the jejunum,Isc tended to increase in PBLC-H(P<0.10),but baseline Gtwas not *** uptakes of glucose and methionine were not influenced by PBLC in the presence or absence of Na+.Conclusion:The results suggest that menthol-rich PBLC increase feed intake,and passive ion and nutrient transport,the latter specifically in the *** also increased serum concentrations of urea precursor amino acids and tended to increase serum calcium

近年来,当谈到对学习外语的兴趣时,很容易看到趋势的动态变化。开设与东方语言相关的不同类型专业的波兰学校、大学和其他教育机构的数量逐年增加。其中最流行的语言是中文。本文旨在探讨波兰教师在汉语教学过程中所面临的问题。首先介绍了波兰汉语教学的历史背景,以及该国汉语教育的现状。其次,探讨了汉语教学中存在的问题和难点,以教学方法的选择、教材和教学大纲的选择等方面为主。通过问卷调查和面对面访谈,观察到在波兰有效教授汉语的主要障碍是学生的心态、考试制度、波兰教师的教学策略和能力以及学习环境。在文章的最后部分,提出了一些教学建议,并提出了改善波兰汉语教学状况的各种方法。其中包括以学生为中心的教学方法的指导,创造有利的汉语学习环境,并提供良好的设备和教材。
