ca19-9 antigen levels can distinguish between benign and malignant pancreaticobiliary disease

BACKGROUND:ca19-9 is a carbohydrate tumor-associated antigen which is frequently upregulated in pancreatobiliary ***,it may also be elevated in patients with jaundice in the absence of a tumor due to biliary obstruction,and in other non-hepato-pancreatico-biliary *** study aimed to evaluate whether ca19-9 levels could accurately differentiate between benign and malignant pancreatobiliary ***:All patients referred to a single surgeon for investigation of pancreaticobiliary disease in 2003 in whom a firm diagnosis had been established were *** malignant disease,a histological diagnosis was required but for benign disease a firm radiological diagnosis was deemed *** patients were divided into 4 categories:pancreatic adenocarcinoma(Pca);cholangiocarcinoma(Cca);chronic pancreatitis(CP)and biliary calculous disease(calc).Bilirubin and alkaline phosphatase levels corresponding to the point of assessment of ca19-9 were also ***:Final diagnoses were made of pancreatic adenocarcinoma(Pca,n=73),cholangiocarcinoma(Cca,n=19),ampullary carcinoma(Amp,n=7),neuroendocrine carcinoma(Neu,n=4),duodenal carcinoma(Duo,n=3),chronic pancreatitis(CP,n=115),and biliary calculous disease(calc,n=27).Median ca19-9 levels(U/ml)were:Pca,653;Cca,408;Duo,403;calc,27;CP,19;Neu,10.5;Amp,8(reference range:0-37).The ca19-9 levels were significantly greater for malignant than for benign disease,could differentiate Pca from Cca/Duo,and were significantly higher in unresectable than in resectable *** sensitivity,specificity,positive predictive value(PPV)and negative predictive value(NPV)for ca19-9 were 84.9%,69.7%,67.7%and 86.1%,respectively.A ROC analysis provided an area under the curve for ca19-9 of 0.871(0.820-0.922),giving an optimal ca19-9 of 70.5 U/ml for differentiating benign from malignant *** this cut-off,the sensitivity was 82.1%,while specificity,PPV and NPV improved to 85.9%,81.3%and 86.5%,*** sta

Simulation of Blast and Fragment Loading Using a Coupled Multi-Solver Approach

Simulating blast and fragment loading simultaneously in a single computation requires the combined use of multiple states of the art solvers. A pipe bomb is an example of simple improvised explosive device （IED） that consists of a piece of pipe filled with explosive material and capped at both ends. To simulate the explosion of a pipe bomb and the damage it causes, a coupled multisolver approach based upon finite element and finite volume methods is applied. The numerical calculation presented demonstrates the ability of ANSYS AUTODYN to correctly simulate the threats of IEDs and provides insight into how the most significant physical phenomena affect the results.

Synthesis of calcium Hexaluminate from Waste Slag of Aluminum and Oyster Shell

Waste aluminum slag and oyster shell were used as raw materials to synthesize calcium hexaluminate（ca6）. The effects of different source materials of caO and sintering temperature on the structures and properties of ca6 were investigated,respectively. The results show that compared to calcium oxide,oyster shell can lower the formation temperature of ca6,hence ca6 can be detected at 1300 ℃ by using oyster shell as the starting raw materials. Increasing the sintering temperature can promote the crystal growth. ca6 crystals show typical platelet shape,and its optimum sintering temperature falls in the 1450～1550 ℃ region. The bulk density is 1.54～1.83 g/cm^3,the apparent porosity is 44.1～55.2% and the flexural strength is 10.8～25.3 MPa.

In the 1960s and 1970 s, attention was focused on rapid population growth and large scale family planning programs were launched in many developing countries. However, the paradigm for understanding fertility decline assumed that exogenous socioeconomic changes were necessary to reduce family size. By the mid-1990s, the standard model of the demographic transition had been all but discarded, partly there were numerous country experiences inconsistent with the model. The 1994 International Conference on Population and Development （ICPD） emphasized women＇s broader needs, which were important but were promoted by reducing attention to population and the need for family planning. The timing of this shift was peculiar, because the cairo proponents were depending on an already discredited model of reproductive behavior. The policy shift at cairo undermined the political coalition that had supported international family planning since the 1960s. As a result of declining financial support, the health of women has deteriorated seriously in the past decade. Meantime, the countries that have been successful in lifting unprecedented numbers of people out of abject poverty were acting independently of the cairo consensus.

Synthesis and manufacture of photocrosslinkable poly(caprolactone)-based three-dimensional scaffolds for tissue engineering applications

It is known that the body can efficiently repair hard tissue (bone) micro fractures by suturing the defect through the deposition of minerals resulting in an area that is stronger post-injury. Larger defects, however, generally cause more trouble since the body is incapable of repairing them. Bone defects can occur as a result of congenital abnormalities, trauma, or disease. Traditional methods for addressing these defects have involved the use of acellular cadaverous bone or autologous bone. Both contain innate prob- lems associated with them;the former method can result in disease transmission, as well as very low integration with the host due to the lack of viable cells while the latter is associated with two surgical sites and morbidity at the donor site. Alternative methods have been developed, but no method has yet provided a satisfactory solution. As a result, resear- chers and the medical community are turning toward the promising fields of biomaterial development and tissue engineering to develop new materials and me- thods of bone regeneration. In this work, a design of experiments (DOE) approach was performed to ren- der commercially available biodegradable polymers (Poly(caprolactone)-diol/triol) photocrosslinkable and resultantly manufacturable using stereolithography (SL), a rapid prototyping technology. To perform the investigations, a commercial SL system (Viper HA, 3D Systems, Valencia, ca) equipped with a solid state laser system (355 nm wavelength) was used to manu-facture synthesized poly(caprolactone) trifuma- rate (PCLtF) 3D porous constructs. Results of the work conducted produced constructs which provided pro- mising chemical and biological results for the in- tended application.

Background: Spontaneous heparin induced thrombocytopenia (HIT) is a medical phenomenon whereby patients develop clinical and laboratory features of HIT without prior exposure to heparin. Aim: We present a patient who underwent bilateral total knee replacement complicated by spontaneous HIT causing bilateral adrenal hemorrhage and deep vein thrombosis. Our patient had no prior history of heparin exposure. We reconcile our patient’s presentation with other case reports of spontaneous HIT in patients who had orthopaedic surgery. Conclusion: Spontaneous HIT is a rare but potentially devastating complication associated with total knee arthroplasty.

Clinical Analysis of the Colorectal Cohort within the Wales cancer Biobank: A Study of Outcomes and Genetic Screening

Over the last 12 years, the Wales cancer Biobank (WCB) has consented to more than 2000 patients with colorectal cancer (CRC). From these patients, clinical data has been collected and patients have been followed through their cancer journey. Clinical data from these patients have been analyzed to identify any correlation between disease grade and outcome. In a small cohort, consisting of 407 patients, WCB has performed genetic analysis on patient primary tumor samples, identifying and characterizing mutations in the KRAS, NRAS, BRAF, PIK3ca and TP53 genes. The majority of patients with CRC who were consented to WCB were male with a mean age of 69 years and received surgery as the primary treatment for their disease. Pathology and disease-free survival data confirmed worse prognoses associated with more advanced disease. Heterogeneity within the primary tumor was explored in a subgroup of patients. Analysis of the KRAS and TP53 genes confirmed that more than 40% of CRC patients who were tested, harbored a genetic mutation within these genes in their primary tumor. Due to the limited sample size tested, most mutations did not show significant differences in disease-free survival, however, mutation of the BRAF gene did show a decrease in the disease specific survival, in keeping with the published data. Analysis of the patients diagnosed with CRC within the Biobank has provided us with valuable information on the status of CRC disease and treatment within the Welsh population. Over 12 years of consenting, we have witnessed significant changes in the information that researchers are interested in when sourcing samples for translational research. The development of new drugs that are tailored to the genetics of a cancer is emerging and at WCB we are focusing our collections on samples and data that meet the needs of this ever-evolving field.
