Long-term outcomes of postgastrectomy syndrome after total laparoscopic distal gastrectomy using the augmented rectangle technique

BACKGROUND For total laparoscopic distal gastrectomies for gastric cancer,the reconstruction method is critical to the clinical outcome of the ***,which reconstruction technique is optimal remains *** originally reported the augmented rectangle technique(ART)as a reconstruction option for total laparoscopic Billroth I ***,little is known about its effect on long-term outcomes,specifically the incidence of postgastrectomy syndrome and its impact on quality of *** To analyze postgastrectomy syndrome and quality of life after ART using the Postgastrectomy Syndrome Assessment Scale-37(PGSAS-37)*** At Juntendo University,a total of 94 patients who underwent ART for Billroth I reconstruction with total laparoscopic distal gastrectomies for gastric cancer between July 2016 and March 2020 completed the PGSAS-37 *** analysis was performed,comparing those 94 ART cases from our institution(ART group)to 909 distal gastrectomy cases with a Billroth I reconstruction from other Japanese institutions who also completed the PGSAS-37 as part of a larger national database(PGSAS group).RESULTS Patients in the ART group had significantly better total symptom scores in all the symptom subscales(i.e.,esophageal reflux,abdominal pain,meal-related distress,indigestion,diarrhea,constipation,and dumping).The loss of body weight was marginally greater for those in the ART group than in the PGSAS group(-9.3%vs-7.9%,P=0.054).The ART group scored significantly lower in their dissatisfaction of ongoing symptoms,during meals,and with daily *** ART for Billroth I reconstruction provided beneficial long-term results for postgastrectomy syndrome and quality of life in patients undergoing total laparoscopic distal gastrectomies for gastric cancer.

Modelling the impact of antimalarial quality on the transmission of sulfadoxine-pyrimethamine resistance in Plasmodium falciparum

Background:The use of poor quality antimalarial medicines,including the use of nonrecommended medicines for treatment such as sulfadoxine-pyrimethamine(SP)monotherapy,undermines malaria control and elimination ***,the use of subtherapeutic doses of the active ingredient(s)can theoretically promote the emergence and transmission of drug resistant ***:We developed a deterministic compartmental model to quantify the impact of antimalarial medicine quality on the transmission of SP resistance,and validated it using sensitivity analysis and a comparison with data from Kenya collected in *** modelled human and mosquito population dynamics,incorporating two Plasmodium falciparum subtypes(SP-sensitive and SP-resistant)and both poor quality and good quality(artemether-lumefantrine)antimalarial ***:The model predicted that an increase in human malaria cases,and among these,an increase in the proportion of SP-resistant infections,resulted from an increase in poor quality SP antimalarial use,whether it was full-or half-dose SP ***:Our findings suggest that an increase in poor quality antimalarial use predicts an increase in the transmission of *** highlights the need for stricter control and regulation on the availability and use of poor quality antimalarial medicines,in order to offer safe and effective treatments,and work towards the eradication of malaria.

Utility of arthroscopic guided synovial biopsy in understanding synovial tissue pathology in health and disease states

The synovium is the soft tissue lining diarthrodial joints, tendon sheaths and bursae and is composed of intimal and subintimal layers. The intimal layer is composed of type A cells(macrophages) and type B cells(fibroblasts); in health, the subintima has few inflammatory cells. The synovium performs several homeostatic functions and is the primary target in several inflammatory arthritides. Inflammatory states are characterised by thickening of the synovial lining, macrophage recruitment and fibroblast proliferation, and an influx of inflammatory cells including lymphocytes, monocytes and plasma cells. Of the various methods employed to perform synovial biopsies arthroscopic techniques are considered the "gold standard", and have an established safety record. Synovial biopsy has been of critical importance in understanding disease pathogenesis and has provided insight into mechanisms of action of targeted therapies by way of direct evidence about eventsin the synovial tissue in various arthritides. It has been very useful as a research tool for proof of concept studies to assess efficacy and mechanisms of new therapies, provide tissue for in vitro studies, proteomics and microarrays and allow evaluation for biomarkers that may help predict response to therapy and identify new targets for drug development. It also has diagnostic value in the evaluation of neoplastic or granulomatous disease or infection when synovial fluid analysis is noncontributory.

Integrating palliative care in oncologic emergency departments:Challenges and opportunities

Although visiting the emergency departments(EDs) is considered poor quality of cancer care,there are indications these visits are ***,there is growing interest in providing palliative care(PC) to cancer patients in ***,this integration is not without major *** this article,we review the literature on why cancer patients visit EDs,the rates of hospitalization and mortality for these patients,and the models for integrating PC in *** discuss opportunities such integration will bring to the quality of cancer care,and resource utilization of *** also discuss barriers faced by this *** found that the most common reasons for ED visits by cancer patients are pain,fever,shortness of breath,and gastrointestinal *** majority of the patients are admitted to hospitals,about 13% of the admitted patients die during hospitalization,and some patients die in *** who receive PC at an ED have shorter hospitalization and lower resource *** based solely on increasing PC provision in EDs by PC specialists have had modest success,while very limited ED-based PC provision has had slightly higher ***,details of these programs are lacking,and coordination between ED based PC and hospitalwide PC is not *** some studies,the objectives were to improve care in the communities and reduce ED visits and *** conclude that as more patients receive cancer therapy late in their disease trajectory,more cancer patients will visit *** of PC with emergency medicine will require active participation of ED physicians in providing PC to cancer *** specialist should play an active role in educating ED physicians about PC,and provide timely *** impact of integrating PC in EDs on quality and cost of cancer care should be studied.

t Magnetic null points and flux ropes play important roles in the three-dimensional process of magnetic reconnection. In this study, a cluster of null points are reconstructed in the reconnection region in the magnetotail by applying a fitting-reconstruction method to measurements from the Cluster mission. The number of recon- structed null points varies rapidly, presenting a turbulentlike evolution of the magnetic structure. The electron density and the flux of the accelerated electrons were enhanced in this turbulent-like region. During this unstable reconnection process, a B-As-B null structure was formed, showing flux rope features and resembling a secondary island in the observation.

Role of endoscopic retrograde cholangiopancreatography in acute pancreatitis

Endoscopic retrograde cholangiopancreatography (ERCP) is a useful tool in the evaluation and management of acute pancreatitis. This review will focus on the role of ERCP in specific causes of acute pancreatitis, including microlithiasis and gallstone disease, pancreas divisum, Sphincter of Oddi dysfunction, tumors of the pancreaticobiliary tract, pancreatic pseudocysts, and pancreatic duct injury. Indications for endoscopic techniques such as biliary and pancreatic sphincterotomy, stenting, stricture dilation, treatment of duct leaks, drainage of fluid collections and stone extraction will also be discussed in this review. With the advent of less invasive and safer diagnostic modalities including endoscopic ultrasound (EUS) and magnetic retrograde cholangiopancreatography (MRCP), ERCP is appropriately becoming a therapeutic rather than diagnostic tool in the management of acute pancreatitis and its complications.
